1.The changes of insulin resistance before and after weight loss treatment

Objective To study the changes of insulin resistance due to severe short-term fluctuations of body mass in simple obese patients. Methods 94 women with simple obesity, were treated by low caloric and high protein diet for two months. A series of examinations were taken before and after the treatment. Results After two months, weight, body mass index (BMI), waist, hip, fasting serum insulin and HOMA-IR reduced significantly ( P <0. 01 or P < 0.05 ). But after 6 months follow-up,these data began to regain. After 12 months ,these data were not significantly different. Fasting serum insulin and HOMA-IR increased ( P < 0. 05). Conclusions The low caloric and high protein diet combined acupuncture or cupping could reduce weight. But 44% patients weight rebound, and HOMA-IR increased.
2.Effects of Vitamin D Combined with Azithromycin on Pneumonia in Patients with Type 2 Diabetes

OBJECTIVE:To investigate clinical efficacy and safety of vitamin D combined with azithromycin in the treatment of pneumonia in patients with type 2 diabetes and its effects on serum inflammation indexes.METHODS:A total of 68 patients with type 2 diabetes complicated with pneumonia selected from our hospital during May 2014-Sept.2015 were divided into control group and trial group according to random number table,with 34 cases in each group.Both groups received insulin intensive hypoglycemic therapy.Control group was given Azithromycin for injection 10 mg/kg+0.9% Sodium chloride injection 100 mL,ivgtt,qd,for consecutive 5 d,and then used Azithromycin dry suspension instead 10 mg/kg,po,qd,consecutive 3 d treatment and 4 d interval as a treatment course,for 2 cycles.Trial group was additionally given Vitamin D drops 800 U,po,qd,for consecutive 4 weeks,on the basis of control group.The clinical efficacies of 2 groups were observed,and serum inflammation indexes [interleukin-6 (IL-6),C-reactive protein (CRP),immunoglobulin G protein (IgG)] levels were also observed before and after treatment.The occurrence of ADR was recorded.RESULTS:Total response rate of trial group (91.18%) was significantly higher than that of control group (70.59%),with statistical significance (P<0.05).Before treatment,there was no statistical significance in serum inflammation indexes as IL-6,CRP,IgG between 2 group (P>0.05).After treatment,IL-6 and CRP of 2 groups were decreased significantly,and the trial group was significantly lower than the control group;IgG of 2 groups were increased significantly,and the trial group was significantly higher than the control group,with statistical significance (P<0.05).There was no statistical significance in the incidence of ADR between 2 groups (P>0.05).CONCLUSIONS:Vitamin D combine with azithromycin in the treatment of pneumonia in patients with type 2 diabetes has a good clinical curative effect,helps to improve the levels of IL-6,CRP and IgG with good safety.
3.Breast bracket combined with polyurethane foam improves the accuracy of immobilization in breast cancer radiotherapy

Objective:To compare the difference between breast bracket combined with polyurethane foam and single polyurethane foam in the accuracy of immobilization, providing a better immobilization for breast cancer radiotherapy.Methods:Fifty breast cancer patients who received radiotherapy in Sun Yat-sen University Cancer Center from March 2021 to July 2021 were selected. Among them, 25 patients were immobilized with polyurethane foam (foam group), and the other 25 patients were immobilized with polyurethane foam combined with breast bracket (combination group). All patients were scanned by CBCT once a week to obtain setup errors in the SI, LR and AP directions for t-test. The formula M PTV=2.5 Σ+0.7 σ was used to calculate the margin of the planning target volume(M PTV). Results:The setup errors in the foam group were SI (2.0±3.26) mm, LR (0.88±2.76) mm, AP (1.22±3.55) mm, Rtn -0.24°±0.85°, Pitch 0.16°±1.11°, Roll -0.32°±1.05°, and the M PTV were 6.75 mm, 8.46 mm and 8.73 mm, respectively. The setup errors in the combination group were SI (1.0±3.01) mm, LR (0.62±2.74) mm, AP (1.82±3.21) mm, Rtn 0.64°±0.59°, Pitch 0.71°±1.22°, Roll 0.29°±0.73°, and the M PTV were 6.35 mm, 7.47 mm, and 7.61 mm, respectively. After comparing the setup errors in the three-dimensional directions between two groups, the t value of LR, SI, AP and Rtn, Pitch, Roll was -4.304, -2.681, 1.384, and -9.457, -3.683, -5.323, respectively. And the differences in the LR, SI, Rtn, Pitch and Roll directions were statistically significant (all P<0.05). Conclusions:The immobilization effect of polyurethane foam combined with breast bracket is better and the M PTV is also smaller than those of polyurethane foam alone. Therefore, it is recommended to use polyurethane foam combined with breast bracket for immobilization in breast cancer radiotherapy.
