1.The community prevention,therapy and rehabilitation of psychiatric patients undergoing "unlock program"

Objective To discuss the effect of community prevention,therapy and rehabilitation for psychiatric patients undergoing "unlock program" and free from captivity. Methods We collected and compared the result for 71 pschiatric patients before and after free from captivity by the Brief Psychiatric Rating Scale(BPRS)and Global Assessment Scale(GAS)according to the work model of the psychiatric prevention,therapy and rehabilitation nursing program which complied with "socialization,comprehension and accessibility". Results After the unlock program the total score of BPRS and GAS greatly dropped to (22.57±3.13)and (80.67±15.57)respectively 2 years later,which were statistically different from those before the attendance of the unlock program (P<0.01). Conclusions The unlock program proved to be a novel and effective nursing program for community prevention,therapy and rehabilitation of psychiatric patients free from captivity.
2.The diagnostic value of high density lipoprotein cholesterol, serum albumin and C reactive protein in patients with severe acute pancreatitis

Objective To investigate the diagnostic value of serum high density lipoprotein cholesterol (HDL-C), serum albumin (SA) and C reactive protein ( CRP) levels combined detection of severe acute pancreatitis.Methods 180 cases of acute pancreatitis were selected from January 2012 to January 2015 in Tianjin fifth central hospital,including 87 severe acute pancreatitis ( SAP) and 93 mild acute pancreatitis ( MAP) , and 100 cases of healthy people were selected as control group.The serum levels of HDL-C, SA and CRP were compared among three groups.Results HDL-C and SA in SAP group and MAP group were significantly lower than control group (P<0.05), the level of CRP was significantly higher than that of control group ( P<0.05); HDL-C, SA levels in SAP group were significantly lower than those of MAP group, the CRP level was significantly higher than that of MAP group (P<0.05).Compared to HDL-C and SA, the sensitivity(89.4%), specificity (95%) and accuracy (92%) of CRP were significantly higher (P<0.05).The sensitivity(93.6%)and specificity (95%) and accuracy (94.3%) of combined detection of three markers were significantly higher than the detection results of the three indexes either,the differences were statistically significant (P <0.05).Conclusion The combined detection of serum HDL-C, SA and CRP levels can effectively improve the early diagnosis rate of SAP, and provide reference for clinical diagnosis.

Objective To invest the effect of minimal inflammation on patients with maintenance hemodialysis(MHD) and the intervention effect of ftuvastatin.Methods Blood indicators such as PA,ALB and BUN,Cr,TC,TG,HDL-C,LDL-C,Lp(a),SF,hs-CRP,IL-6 and TNF-α were detected at the first day of the study and six months later in all patients.Patients were divided into inflammation group(CRP≥8 mg/L) and non-inflammation group(CRP<8 mg/L) according to CRP levels.Only inflammation group used fluvastatin.Two groups were observed for six months.Results The level of hs-CRP,IL-6,TNF-α,TG,Lp(a),SF in MHD patients was obviously higher than the normal control group.The level of ALB was obviously lower than the normal control group.The angiocardiopathy incidence of inflammation group was obviously higher than that of the non-inflammation.The more obvious of minimal inflammation,the more significant of lipid metabolism disturbance and malnutrition.Hs-CRP,IL-6,TG,TC,LDL-C and Lp(a) of inflammation group decreased obviously(P<0.01 or P<0.05).ALB、HDL-C increased obviously(P<0.05).Indicators of the non-inflammation had no statistical difference after therapy.Conclusions Patients with MHD generally have minimal inflammation and malnutrition.Minimal inflammation has important effect on the angiocardiopathyincidence,lipid metabolism disturbance and malnutrition.Fluvastatin not only could regulate lipid metabolism but also improve the minimal inflammation of patients with MHD.Early detection and therapy of minimal inflammation has important significance on improving prognosis of patients with MHD.
6.The significance of renal tubular dysfunction in patients with refractoriness nephritic syndrome and interference study with Valsartan

Objective To investigate the significance of renal tubular dysfunction in patients with refractoriness nephrotic syndrome(RNS)and the effect of interference treatment of Valsartan (VAL).Methods 79 cases of RNS and 68 healthy controls were recruited into the study. The 79 patients of RNS were divided randomly into the VAL group and the dipyridam group. On the basis of routine therapy, the VAL group was given VAL(80mg/d),and the dipyridam group taken dipyridam (150mg/d)orally for 12 weeks The glomerular tubular function(u-RBP,α1-MG,β2-MG,mAlb,NAG)were detected and the pathologic changes of tubukinterstitium were observed by using the methods of ELISA, biochemistry and scoring of the pathologic damage of tubulointerstitium before and after 12 weeks of VAL treatment in all of the cases. Results Urinary RBP,α1-MG,β2-MG,mAlb and NAG in all patients with RNS was evidently higher than that in healthy controls(P<0.01).Those had positive correlations with damages of tubulointerstitium(r=0.436,0.626,0.499,0.668,0.657,P<0.01).The interference outcome displayed that the excretion rates of urinary series of protein after oral use VAL in treatment group were markedly lower, while the control group had no distinct change. Conclusion There were various injury of tubulointerstitium and the disfiguration of renal tubular function in all cases with RNS. Damages of renal tubular function had positive correlation with tubulointemtitium injury and renal globular injury. Interference treatment with ARB in patients with RNS could improve renal tubular function, which is of great significance in delaying the progress of RNS.
7.Dexamethasone Protects Mice against Acute Inflammatory Liver Injury by Regulating T Cell Immune Response

Background:Dexamethasone can protect mice against the acute inflammatory liver injury by inhibiting innate immune cell function. However,the roles of T cell in this protective effect remain unknown. Aims:To investigate the regulatory effect of dexamethasone on T cell immune response in acute inflammatory liver injury. Methods:Six male C57BL/ 6J mice were randomly divided into 2 groups. One hour before induction of acute inflammatory liver injury by lipopolysaccharide, dexamethasone 5 mg/ kg and PBS were given intraperitoneally in experimental group and model group,respectively. All the mice were sacrificed 12 hours after model construction. The clinical score and liver function parameters were assessed;splenic mononuclear cells were isolated for measurements of T cell activation,as well as cytokine expression,secretion, and transcriptional factor expression for different T-cell subsets. Results:Clinical score and serum levels of transaminase were significantly lower in experimental group when compared with the model group. Meanwhile,percentage of CD44 +CD62L - T cells(i. e. activated or memory T cells)from spleen was significantly decreased in experimental group. Among splenic T cell population,expression and secretion of IFN-γ,a Th1-type cytokine,was decreased;expression and secretion of IL-4,a Th2-type cytokine,percentage of regulatory T cells(Treg cells),and ratios of Th2 / Th1 and Treg/ Th1 were increased;transcriptional factor specific for Th1 cells was down-regulated,and those for Th2 and Treg cells were up-regulated. Conclusions:Dexamethasone inhibits T cell activation and directs the reciprocal T cell lineage differentiation (repressing Th1 cell differentiation,promoting Th2 and Treg cell differentiation),which may contribute to the protection against acute inflammatory liver injury.

Background:
Enteritis is one of the most frequently reported symptoms in piglets infected with porcine circovirus type 2 (PCV2), but the immunopathogenesis has not been reported.
Objectives:
This study examined the effect of a PCV2 infection on the intestinal mucosal immune function through morphological observations and immune-related molecular detection.
Methods:
Morphological changes within the ileum of piglets during a PCV2 infection were observed. The expression of the related-molecules was analyzed using a gene chip. The immunocyte subsets were analyzed by flow cytometry. The secretory immunoglobulin A (SIgA) content was analyzed by enzyme-linked immunosorbent assay.
Results:
The PCV2 infection caused ileal villus damage, intestinal epithelial cells exfoliation, and an increase in lymphocytes in the lamina propria at 21 days post-infection.Differentially expressed genes occurred in the defense response, inflammatory response, and the complement and coagulation cascade reactions. Most of them were downregulated significantly at the induction site and upregulated at the effector site. The genes associated with SIgA production were downregulated significantly at the induction site. In contrast, the expression of the Toll-like receptor-related genes was upregulated significantly at the effector site. The frequencies of dendritic cells, B cells, and CD8 + T cells were upregulated at the 2 sites. The SIgA content decreased significantly in the ileal mucosa.
Conclusions
PCV2 infections can cause damage to the ileum that is associated with changes in immune-related gene expression, immune-related cell subsets, and SIgA production.These findings elucidated the molecular changes in the ileum after a PCV2 infection from the perspective of intestinal mucosal immunity, which provides insights into a further study for PCV2-induced enteritis.
9.Preparation and evaluation of inactivated vaccine against sheep Mannheimia hae-molytica type A1

Mannheimia haemolytica is an important pathogen causing respiratory diseases in sheep,and the development of an inactivated vaccine of sheep origin against Mannheimia haemolytica is an effective means of prevention and control of this pathogen.In this study,Mannheimia haemo-lytica isolated from the diseased material collected from a sheep farm was screened for the most suitable vaccine strain by mouse pathogenicity test,and the inactivated vaccine was prepared by combining four antigenic amounts(106,107,108,109 CFU/mL)with three adjuvants(ISA 201 ad-juvant,ISA 206 adjuvant,and aluminum adjuvant),which were used in immunization and attack protection tests,and then screened for immunity and attack protection tests.Immunization and at-tack protection tests were conducted to screen the optimal antigen amount and adjuvants for inacti-vated vaccine preparation.Finally,the vaccine was prepared under the optimal conditions and the dynamic changes of antibody levels in immunized sheep were observed.The results showed that a total of 24 strains of Mannheimia haemolytica of sheep origin were isolated,among which 12 strains of serotype A1 were the dominant serotypes for the infection in this sheep farm;one strain of Mannheimia haemolytica of sheep origin of type A1(Mh1)was screened and obtained as the strain for vaccine preparation.Its LD50 for mice was 3.16 × 107 CFU/mL,with strong pathogenici-ty,and could cause pathological damage such as inflammatory cell infiltration of varying degrees in heart,liver,spleen,lung and kidney tissues;the results of the immunization and attack protection test showed that the optimal antigenic amount of the inactivated vaccine was 109CFU/mL,and the most suitable adjuvant was ISA 201,which stimulated the production of the highest antibody level and provided 100%protection for the mice.The protection rate of mice reached 100%,and good humoral immunity could be induced after immunizing sheep.The inactivated Mannheimia haemo-lytica type A1 vaccine prepared in this study has good immunoprotection effect,and lays a good foundation for the further development of commercial vaccine against Mannheimia haemolytica of sheep origin.
10.The effect of microRNA-155 on the proliferation,invasion,migration and apoptosis of hepatocellular carcinoma cells

Objective To discuss the effect of PI3K-AKT signaling pathway regulated by microRNA-155(miRNA-155)targeted protein tyrosine phosphatase non-receptor type 21(PTPN21)on the proliferation,migration and invasion of hepatocellular carcinoma(HCC)cells.Methods Lentivirus transfection was used to silence the expression of miRNA-155 in human Huh7 HCC cells,and real-time fluorescent quantitative polymerase chain reaction(RT-qPCR)was used to detect the silencing effect of miR-155.After obtaining stable cell lines,the cell lines were randomly divided into Blank group(normal Huh7 cells),shNC group(Huh7 cells+empty miR-155 vector),sh-miR-155(Huh7 cells+miR-155 silencing),sh-miR-155+Recilisib group(Huh7 cells+miR-155 silencing+PI3K-AKT agonist),shNC+Recilisib group(Huh7 cells+empty miR-155 vector+PI3K-AKT agonist).Dual luciferase assay was used to determine whether PTPN21 was the downstream of miR-155.The cell proliferation ability of cells in each group was detected by MTT assay.The apoptosis level of each group was tested by flow cytometry.The invasion and migration ability of cells was assessed by Transwell assay.Western blot analysis was used to observe the differences in protein expression of PTPN21,PI3K,P-PI3K,AKT,P-AKT,and apoptosis-related proteins including BAX,BCL-2 and caspase-3 in all groups.Results The expression level of miR-155 in sh-miR-155 group was lower than that in Blank group and shNC group(P<0.000 1),and the difference in miR-155 expression level between Blank group and shNC group was not statistically significant(P>0.05).MTT results showed that A values of Huh7 cells at 2,3,4 and 5 day in sh-miR-155 group were lower than those in Blank group and shNC group(P<0.000 1),while these differences between Blank group and shNC group were not statistically significant(P>0.05).In sh-miR-155 group the A values at 2,3,4 and 5 day were lower than those in sh-miR-155+Recilisib group and shNC+Recilisib group(P=0.0052 and P<0.0001,respectively),while the A values at 2,3,4 and 5 day in sh-miR-155+Recilisib were lower than those in shNC+Recilisib group(P<0.000 1).There was no significant differences in cell migration and number of invasion cells between the Blank group and shNC group(P>0.05).After activation of PI3K-AKT signaling pathway,the migration and invasion capacity of HCC cells in the shNC+Recilisib group were significantly enhanced when compared with the Blank group(P<0.000 1).In contrast,the number of migrated and invaded Huh7 cells after miR-155 silencing was significantly lower than that in the Blank group and shNC group(P<0.000 1)and this phenomenon became reversed by PI3K agonist.Compared with the sh-miR-155 group,in the sh-miR-155+Recilisib group the migration and invasion ability of HCC cells was enhanced(P=0.000 2).Lentiviral transfection of Huh7 human HCC cells to silence miR-155 and downregulate miR-155 inhibiting PTPN21 regulation of the PI3K-AKT signaling pathway,thus inhibiting the invasion,migration and proliferation ability of HCC cells and promoting the apoptosis of HCC cells.Conclusion miR-155 inhibits the migration,invasion and proliferation of HCC cells through targeting PTPN21 regulation of PI3K-AKT signaling pathway.The miR-155 may be a potential therapeutic target for HCC in the future.(J Intervent Radiol,2024,32:44-51)
