1.Effects of very-low-carbohydrate diet on body and function index in overweight or obese subjects

Objective To assess the effects of very-low-carbohydrate diet (VLCD) on body function and body composition in obese or overweight subjects.Methods Thirty five overweight or obese subjects were enrolled in the study,including 10 males and 25 females with a mean age of (43 ± 10) years.All subjects received VLCD intervention and 30 min moderate-intensity exercise daily for 3 months.The height,weight,blood pressure,heart rate,blood lipids and body composition were measured before and 1,3 months after intervention.ANOVA or nonparametric test was used for statistical analysis.Results After 3-month intervention,compared to baseline,the average body weight was decreased by 17.2 and 5.9 kg,waist circumference was decreased by 15.1 and 4.5 cm,BMI index decreased by 4.3 and 3.4 kg/cm2,waist to hip ratio was decreased by 0.1 and 0.1,for males and females,respectively (F =11.65 and 5.28,8.36 and 3.73,9.28 and 12.06,3.32 and 19.43,all P < 0.05).Compared with the baseline,the systolic pressure was decreased by 12.2 and 9.2 mmHg(1 mmHg =0.133 kPa),triglycerides were decreased by 0.25 and 0.42 mmol/L,fasting blood glucose was decreased by 0.13 and 0.43 mmol/L after 1 month and 3 months of intervention,respectively.The proportion of patients with fatty liver was decreased from 69% (24/35) at baseline to 49% (17/35) at 1 month after intervention and further to 6% (2/32) at 3 months after intervention (x2 =27.52,P < 0.05).Conclusion The results indicate that VLCD can effectively control the body weight,and significantly improve the state of body function disorder.
2.Association between Helicobacter pylori infection and prevalence of type 2 diabetes

To investigate the association between Helicobacter pylori ( Hp ) infection and type 2 diabetes.Total 4 129 health check up subjects aged (49.1 ±11.3) years (18-92 years), including 2 649 males and 1 480 females were enrolled in this cross-section study.Hp infection was confirmed by 13 C-urea breath test;type 2 diabetes mellitus was diagnosed according to WHO criteria (1999).Single factor analysis and multiple regression analysis were used to evaluate the association between Hp affection and the prevalence of type 2 diabetes.Hp infection rate in diabetic group was significantly higher than that in non-diabetic group (45.6%vs.39.0%, P<0.05).Hp infection rate in subjects with newly diagnosed diabetes was highest than those with previously diagnosed diabetes and non-diabetic subjects ( P<0.05).The glycosylated hemoglobin level in Hp infection group was higher than that in non infection group [ ( 5.88 ±0.64 )% vs.( 5.62 ± 0.88)%, P<0.05];the fasting plasma glucose and postprandial blood glucose levels in Hp infection group were also higher than those in non-infection group ( P<0.05 ).Logistic regression analysis showed that Hp infection was one of the independent risk factors of type 2 diabetes (OR=2.002, 95%CI:1.564-2.563, P<0.05).The study indicates that Hp infection is closely associated with type 2 diabetes mellitus.
3.Expressions of krüppel-like factor 4 in human glioma tissues and its effect on activity of tumor cells

Objective:To investigate the expressions of krüppel-like factor 4 (KLF4) in human glioma tissues and its effect on the activity of tumor cells.Methods:The glioma tissues specimens of 74 patients with primary malignant gliomas who were admitted to Nanyang Second General Hospital of Henan Province from March 2018 to May 2019 were collected. During the same period, 50 cases of benign meningioma tissues and 31 cases of normal brain tissues receiving surgery because of head injury were also collected. Real-time fluorescence quantitative polymerase chain reaction (RT-PCR) was used to detect the expression of KLF4 mRNA in tissues. Glioma U-87MG cells were selected and the glioma cell models with low-expression of KLF4 were constructed and were divided into the blank control group, KLF4-NC group and KLF4-siRNA group. The proliferation ability of cells was detected by using MTS cell proliferation detection kit, and the expression levels of E-cadherin, vimentin and zonula occludens protein 1 (ZO-1) were detected by using Western blot.Results:The relative expression level of KLF4 mRNA in low-grade glioma, benign meningioma, and normal brain tissues was 0.26±0.04, 0.13±0.02, 0.11±0.02, respectively, which were lower than that in high-grade glioma(0.34±0.06), and the difference was statistically significant ( t = 8.381, 15.720, 15.984, all P<0.05). The relative expression level of KLF4 mRNA in benign meningiomas and normal brain tissues was lower than that in low-grade gliomas, and the difference was statistically significant ( t = 13.771, 14.239, all P<0.05). At each time point of cell culture, the proliferation ability of U-87MG cells in KLF4-siRNA group was lower than that of the blank control group and KLF4-NC group, and the difference was statistically significant (all P < 0.05). There was no significant difference in U-87MG cell proliferation ability between the blank control group and KLF4-NC group ( P > 0.05). The relative expression level of E-cadherin and ZO-1 protein in KLF4-siRNA group was 0.82±0.10, 0.79±0.11, respectively, which were higher than that in the blank control group (0.24±0.08, 0.39±0.05) and KLF4-NC group (0.26±0.05, 0.42±0.09), and the difference was statistically significant (all P < 0.01); and the relative expression level of vimentin in KLF4-siRNA group (0.31±0.08) was lower than that in the blank control group (0.90±0.08) and KLF4-NC group (0.92±0.05), and the difference was statistically significant (all P < 0.01). There was no statistically significant difference in the expression level of E-cadherin, vimentin and ZO-1 between the blank control group and KLF4-NC group (all P > 0.05). Conclusion:The expression level of KLF4 is increased in human glioma tissues, especially in high-grade glioma. Down-regulating the expression of KLF4 may inhibit glioma cell proliferation and epithelial-mesenchymal transition, and reduce cell activity.
4.Establishment of nomogram prediction model for neurological complications after interventional treatment of ruptured intracranial aneurysms

Objective:To explore the risk factors of neurological complications after interventional treatment of ruptured intracranial aneurysms(RIAS), and to establish a predictive model of nomogram.Methods:The clinical data of 89 patients with RIAS who underwent endovascular treatment in Nanyang Second General Hospital Affiliated to Xingxiang Medical University from January 2016 to January 2019 were retrospectively studied.The clinical imaging data were collected and followed up for 6 months.The patients were divided into two groups: no neurological complications group (61 cases) and neurological complications group (28 cases). To analyze the clinical indicators and the possible related factors of neurological complications after RIAS interventional therapy.A nomogram was established to score the influencing factors, and a scoring prediction model was constructed; the clinical calibration of the model was evaluated by consistency index (C-index) and calibration curve, and the clinical differentiation of the model was evaluated by nomogram relying on ROC curve.Results:Multivariate logistic regression analysis showed that Hunt-Hess classification ( OR=4.927, 95% CI: 1.189-20.426, P=0.028), Fisher classification ( OR=4.633, 95% CI: 1.012-21.208, P=0.048 ), aneurysm cyst xiaofu ( OR=5.918, 95% CI: 1.104-24.948, P=0.015), wide carotid aneurysm ( OR=4.381, 95% CI: 1.029-18.645, P=0.046) and treatment Strategy ( OR=4.887, 95% CI: 1.235-19.329, P=0.024) is an independent risk factor for nerve-related complications after RIAs interventional therapy.The predictive model of nomogram showed that Hunt-Hess classification (grade IV, V) was 100, aneurysm bleb (with) 98, treatment strategy (stent implantation) 95, wide-necked aneurysm (yes) 92 and Fisher grade (grade III, IV) 81; the C-index of the predictive model was 0.871; the nomogram relied on ROC curve AUC 0.871, and the treatment strategy (stent implantation) was 95; the Fisher grade (grade III, IV) was 81; the C-index of the predictive model was 0.871.The sensitivity and specificity were 85.71%(24/28) and 77.05%(47/61) respectively. Conclusion:Hunt Hess classification, Fisher classification, aneurysmal sac caruncle, wide necked aneurysms and treatment strategies will affect the occurrence of neurological complications after RIAS interventional therapy.The nomogram established by this method can provide intuitive and reliable reference for clinical practice.
5.Comparative Study on Evaluating the Bladder Volume between BladderScan BVI9400 and Ultrasound System iU22.

OBJECTIVETo evaluate the accuracy of the latest BladderScan BVI9400 on measuring bladder volume.
METHODSTwo bladder phantoms were selected for investigating the accuracy of BVI9400. 341 patients with the iU22 ultrasound examinations were followed by BVI 9400. The difference and correlation between BVI9400 and iU22 were contrastively analyzed.
RESULTSThe relative difference between results from BVI9400 and phantom volume was 2.5% and 1.36%. There was a strong correlation for patients between BVI9400 and iU22 (R = 0.96, P < 0.001). The relative difference between BVI9400 and iU22 decreased with the increasing of bladder volume and had no significant difference with patient's gender (P > 0.1).
CONCLUSIONBladderScan BVI9400 had the ability of high accuracy and good stability of measured data. In view of quick and conveniences, BVI9400 could be as auxiliary equipment on pelvic tumor to evaluate whether the bladder volume during fractional radiotherapy was consistency with that during CT positioning.

6.Expression and significance of KL-6 mucin in the tissue and serum of hepatoma

Objective To detect the expression of KL-6 mucin in the tissue and serum of hepatoma in different hepatoma patients,and to investigate the value of KL-6 mucin as a tumor marker in the diagnosis of hepatoma.Methods The expression of KL-6 mucin in the hepatoma tissues of 81 patients with hepatocellular carcinoma(HCC),21 patients with cholangiocarcinoma(CC),12 patients with combined hepatocellular and cholangiocarcinoma(HCC-CC)and 56 patients with metastatic liver cancer(MLC)was detected by immunohistochemical analysis.The expression of KL-6 mucin in the serums of 34 HCC patients,8 CC patients,30 MLC patients and 19 healthy individuals was detected by enzyme-linked immunosorbent assay,and all the data were analyzed by t test.Results Expression of KL-6 mucin was detected in the cholangiocarcinoma tissues in all CCand HCC-CC patients.In several hepatoma cells and partial hepatoma tissues of patients with MLC,the expression of KL-6 mucin was detected.No expression of KL-6 mucin was detected in the hepatocellular carcinoma tissuesand non-cancerous tissues of patients with HCC or HCC-CC.The serum levels of KL-6 mucin expression in CC patients were signifcantly higher than those in healthy individuals,HCC and MLC patients(t=5.58,5.34,4.00.P<0.01).The difference of the serum levels of KL-6 mucin expression between MLC patients and healthy individuals had statistical significance(t=2.77,P<0.01).However,no significant difference in serum levels of KL-6 mucin expression was found between HCC patients and healthy individuals and between HCC patients and MLC patients(t=2.03,1.89,P>0.05).Conclusion The expression of KL-6 inucin in CC patients is significantly higher than in patients with other types of hepatoma in both tissue and serum levels.Thus,KL-6 may be a usetul new tmnor marker for distinguishing CC from other types of hepatoma.
7.Comparisons of several laboratory tests in the diagnosis of neurosyphilis

Objective To compare the sensitivity and specificity of venereal disease research laboratory (VDRL) test versus several other laboratory tests in the diagnosis of neurosyphilis. Methods Lumber puncture was conducted to obtain cerebrospinal fluid (CSF) from untreated outpatients with latent syphilis (LS) or serofast outpatients with LS. Then, VDRL test, rapid plasma regain (RPR) test, Treponema pallidum particle agglutination (TPPA) assay, fluorescent treponemal antibody-absorption (FTA-ABS) test and protein quantification were performed on these CSF samples. The sensitivity, specificity, positive predictive value and negative predictive value were compared between VDRL test and four other laboratory tests in the diagnosis of neurosyphilis. Results Totally, 61 cases of latent syphilis were included in this study. The sensitivity, specificity,positive predictive value and negative predictive value were 93.44% (57/61), 99.32%(293/295), 96.61%(57/59), 98.65% (293/297)for CSF-RPR, respectively, 91.80% (56/61), 82.71% (244/295), 52.34% (56/107),97.99 (244/249) for CSF-TPPA, respectively, 93.44% (57/61), 82.71% (244/295), 52.78%(57/108), 98.39%(244/248) for CSF-FTA-ABS, respectively, and 49.18%(30/61), 97.29% (287/295), 78.95% (30/38),90.25% (287/318) for CSF protein quantification, respectively. Conclusions CSF-VDRL cannot be replaced by CSF-RPR, -TPPA, -FTA-ABS, or CSF protein quantification in the diagnosis of neurosyphilis. CSF-RPR shows a high sensitivity and specificity in the diagnosis of neurosyphilis, with an increased diagnostic capability (area under the receiver operating characteristic curve) compared with CSF-TPPA, CSF-FTA-ABS or CSF protein quantification.

9.Expression of octamer binding transcription factor 4 in glioma tissues and its relationship with prognosis of patients

Objective:To investigate the expression of octamer-binding transcription factor 4(OCT4) in glioma tissues and its relationship with prognosis of patients.Methods:From March 2015 to June 2016, 168 patients with glioma and 72 normal brain tissues in the Second People's Hospital of Nanyang were collected.The expression levels of OCT4A and OCT4B were detected by reverse transcription-polymerase chain reaction(RT-PCR), and the relationship between clinicopathological characteristics and prognosis of glioma patients was analyzed.Results:The relative expressions of OCT4A(2.28±0.85) and OCT4B(2.84±1.29) in 168 glioma tissues were significantly higher than those in normal tissues [(1.05±0.41), (1.18±0.46)] ( t=11.649, 14.798, all P<0.01), and there was a positive correlation between OCT4A and OCT4B in glioma tissues( r=0.682, P=0.001). The expression levels of OCT4A and OCT4B in glioma patients with different pathological stages, pathological types, degree of differentiation and depth of invasion had statistically significant differences( t=14.695, 11.309, 16.038, 13.721, 17.216, 15.083, 14.871, 15.417, all P<0.01). Taking the median value of OCT4A and OCT4B mRNA expression as the cutoff value, the patients were divided into low expression group and high expression group.The overall survival of patients with low expression of OCT4A and OCT4B was (24.17±3.41)months and (25.30±4.16)months, which were significantly longer than (15.80±2.93)months and (15.94±3.07)months of high expression patients, the differences were statistically significant( t=15.639, 16.143, all P=0.000). Cox regression model analysis showed that the expression levels of OCT4A and OCT4B were independent factors affecting the prognosis of glioma patients.The hazard ratio (95% CI) was 1.731(0.804-3.181) and 1.605(0.795-2.942), respectively. Conclusion:The expression levels of OCT4A and OCT4B in glioma tissues are abnormally elevated, which is closely related to the severity of the disease.The patients with high expression of OCT4A and OCT4B have a shorter overall survival period, which has a certain predictive value for the treatment effect and prognosis of glioma.
10.Prenatal diagnosis for a fetus with 5p deletion syndrome.

OBJECTIVE:
To explore the genetic basis for a fetus with club foot detected upon mid-pregnancy ultrasonography.
METHODS:
Amniotic fluid of the fetus and peripheral blood samples of its parents were collected and subjected to G-banding karyotype analysis and copy number variation sequencing (CNV-seq). The result was verified by fluorescence in situ hybridization (FISH).
RESULTS:
The fetus and its parents all had a normal karyotype. CNV-seq analysis revealed that the fetus has harbored a 23.12 Mb on chromosome 5 and a 21.46 Mb duplication on chromosome 7. FISH assay has verified that its mother has carried a cryptic t(5;7)(p14.3;q33) translocation.
CONCLUSION
CNV-seq combined with FISH can effectively detect cryptic chromosome aberrations, and can help to reduce severe birth defects and provide a basis for prenatal genetic counseling.
