Objective To investigate the clinical treatment of congenital heart disease complicated with infective endocarditis effective programs, joint analysis of the clinical efficacy of the use of antibiotics in surgery on the basis of providing a basis for clinical treatment.MethodsAnalysis of January 2009 to December 2016 in our hospital with congenital heart disease complicated with infective endocarditis clinical data of 78 cases of patients were randomly divided into observation group and control group of 39 cases, The control group was given appropriate antimicrobial treatment, patients in the observation group according to the samples of culture and drug sensitivity test choose appropriate broad-spectrum antibiotics for treatment, observation after treatment and the clinical efficacy of the two groups were analyzed.According to the medical staff were given psychological psychological counseling for patients to explain the importance of disease related knowledge and related treatment, timely eliminate / alleviate the negative emotion of patients with, to ensure that patients with a good attitude to face treatment.Results78 cases of congenital heart disease complicated with infective endocarditis clinical infection germs mainly Staphylococcus aureus and Streptococcus viridans, in addition to a small number of patients infected with Enterococcus faecalis, Staphylococcus epidermidis, gram-negative bacteria and mixed bacteria.Two groups of patients after treatment statistical clinical efficacy results show 56.41% cure rate in patients in the observation group, the effective rate was 97.43% clinical cure rate was 35.90% in the control group of patients, clinical effective rate was 79.49%, a statistically significant difference between groups (P<0.05).Statistical quality of life of patients after two scores, statistics of daily living, psychological feelings, general health, drug dependence and pain and discomfort five indicators, pain and discomfort, and two indicators of the overall health of patients in the observation group was significantly better than the control group (P<0.05), no significant difference.ConclusionThe clinical administration congenital heart infection endocarditis surgical treatment of broad-spectrum antibiotics combined with substantial clinical efficacy, safety, reliability, patients were recovering well, can be clinical.

Objective To find out the weak points of students through analyzing the scores of medical licensing examination, to explore the causes and to put forwards some advices. Methods Excel 2007 was used to statistically analyze the average scores, average passing rate and the mastering rate of 3 060 candidates who took the exam from 2006 to 2011. Literatures were reviewed to find out reforming measures taken by other schools. Results The average scores changed lightly. The master-ing rate changed differently. The average passing rate declined from 83.6% in 2006 to 71.65% in 2011. Conclusions Score of medical licensing examination is an important index of the teaching quality of medical universities, which should be emphasized on. Some practicable methods are put forward taken into all related factors:adjusting course setting, strengthening teaching of practical skills, reforming teaching methods and examination systems, placing more emphasis on examination work.

Objective To evaluate the advantages of nasal septum deviation with three lines reduce tension septorhinoplasty under endoscopy.Methods Three lines reduce tension septorhinoplasty of 48 cases were compared with nasal septum submucous resection(Killian) of 43 cases.Results Followed-up for 3 months to 1 year postoperation,study group were remedied and no clinical symptoms,no complications,control group had septal perforation of 1 case,nasal synechia of 2 cases,saddle nose of 1 case.The incidence of two groups postoperative complications had statistically significant difference.Conclusion Compared with Killian,three lines reduce tension septorhinoplasty was easy to stuty and have the advantages of micro-invasion,safety and efficiency,and was favorable recovery nasal cavitysinuses physiological functions.

Objective To evaluate the clinical significance of the serum β2-microglobulin (β 2-MG) and lactic dehydrogenase (LDH) in multiple myeloma (MM) patients.Methods The serum levels of β2-MG and LDH in 63 MM patients (MM group) and 20 healthy person (control group) were measured by biochemical assay.Results The serum levels of β 2-MG and LDH in MM group were higher than those in control group[(3.81 ± 0.62) mg/L vs.(2.43 ± 0.91) mg/L and (296.4 ± 34.7) U/L vs.(145.5 ±17.8) U/L,P ＜ 0.05].Furthermore,the serum levels of β 2-MG and LDH increased gradually with MM clinical staging (P ＜0.05).After VAD chemotherapy,the serum levels of β 2-MG and LDH in clinical response MM patients(54 cases) were significantly decreased [(2.51 ± 1.36) mg/L vs.(3.57 ± 0.82) mg/L and (159.1 ± 35.2) U/L vs.(285.3 ± 87.6) U/L,P ＜ 0.05],while those in no response patients (9 cases)were unchanged (P＞ 0.05).Conclusion The serum levels of β 2-MG and LDH can be taken as a clinical index to classify the clinical phase,prognosis and effectiveness of chemotherapy in MM patients.

Objective To explore the effect of transcription factor E2F1 on the apoptosis of small cell cancer line H446. Methods Plamid vector- mediated E2F1 small hairpin RNA (shRNA) was used to silence E2F1 in H446 cell. RT-PCR and western-blot assay were used to detect the expressions of E2F1 and Bcl-2. The apoptosis rate in H446 cell line was detected by flow cytometry assay. Result E2F1 protein was suppressed in shRNA1-modified H446 cell. Sgnificant difference of the apoptosis was shown between E2F1 shRNA1 group and the other two groups. Additionaly, the expression of Bcl-2 protein increased in E2F1 shRNA1-modified cell line. Conclusions E2F1 is highly expressed in H446 small cell lung cancer cell line. E2F1 promotes apoptosis of H446 through upregulating Bcl-2 expression.

Objective To study the efficacy of voice therapy combined with Jinsangsanjie pill in patients with vocal nodule .Methods A total of 146 patients with vocal nodes were randomly divided into three groups :45 cases in group A(single Jinsangsanjie pill therapy) ,47 cases in group B(single voice therapy) and 54 cases in group C(voice therapy combined with Jinsangsanjie pill therapy) ,30 healthy adults were as a normal control group .The treatment lasted 1 month .The results were evaluated by voice handicap index ,voice acoustic analysis and electronic laryngo‐scope which were collected before and after 1 month treatments .Results The VHI ,jitter ,shimmer and NNE of all patients were reduced while the MPT was increased after the treatment .The differences were significant (P<0 .05) .VHI 3 .64 ± 2 .12 ,jitter 0 .30% ± 0 .08% ,shimmer 1 .41% ± 0 .31% ,NNE -16 .83 ± 5 .84 dB of group C were significantly lower while MPT 18 .87 ± 3 .38 s and cure rate(93 .5% ) were significantly higher than those of in groupA(7.39±2.56,0.38% ±0.12% ,1.78% ±0.41% ,-13.38±4.42dB,16.38±3.11s,80.5% )andgroupB (23.6±12.5,0.48% ±0.18% ,1.98% ±0.42% ,-9.42±3.82dB,14.98±3.28s,52.4% )(P< 0.05).The VHI ,,jitter shimmer ,NNE of group A were significantly lower .MPT and cure rate were significantly higher than those of in group B .There were no statistical differences in each vocal indicators between group C and the normal control group (P>0 .05) .Conclusion The voice therapy combined with Jinsangsanjie pill in the treatment of vocal nodules is more efficiently ,and can improve patient’s pronunciation features .

Background The establishment of chronic ocular hypertension is a basis for the research of glaucoma.Previous laser photocoagulation method to establish ocular hypertension model showed obvious fluctuation of intraocular pressure (IOP) and complications and need repeatedly photocoagulation.Improvement of modeling method is of important significance for glaucoma.Objective This study was to establish chronic ocular hypertension rat models by transgoniscope laser photocoagulation to trabecular meshwork and to compare this method with previous translimbal laser photocoagulation.Methods Thirty-six 8 to 12-week-old clean grade Fischer344 rats were collected and divided into normal control group,translimbal laser photocoagulation group and transgoniscope laser photocoagulation group,12 rats for each group.Five hundred and thirty-two nm YAG laser was used to photocoagulate trabecular meshwork translimbally in the right eyes of rats in the translimbal laser photocoagulation group,with the laser power 440-500 mW and spots 40-60,and the photocoagulation was perfored transgoniscopely in the right eyes of rats in the transgoniscope laser photocoagulation group,with the laser power 800-850 mW and spots 100-120.IOP was measured by using Tonolab tonometer in all the rats after modeling.The rats were sacrificed 3 weeks after modeling and retinas were isolated,the Tuj-1 positive retinal ganglion cells (RGCs) were counted by immunofluorescence technology.The use and care of the animals followed the Statement of ARVO.Results The successful rate of establishement of models was 75％ in the translimbal laser photocoagulation group and 100％ in the transgoniscope laser photocoagulation group.The mean IOP was (11.0±1.3),(23.4±12.6) and (25.3± 4.9) mmHg,and the peak IOP was (12.3 ± 1.0),(50.5 ± 7.3) and (44.3 ± 12.3) mmHg in the normal control group,transgoniscope laser photocoagulation group and translimbal laser photocoagulation group,respectively,with significant differences among the groups (F=25.496,80.762,both at P＜0.001),and the mean IOP was significantly higher in the transgoniscope laser photocoagulation group and translimbal laser photocoagulation group than that in the normal control group (all at P＜0.001),and no significant differences in the mean and peak IOP between transgoniscope laser photocoagulation group and translimbal laser photocoagulation group (P=1.000,P=0.195).The numbers of Tuj-1 positive RGCs in the retinas were (2 048.2± 148.5),(645.2 ± 177.1) and (1 223.7 ± 148.6)/mm2 in the normal control group,transgoniscope laser photocoagulation group and translimbal laser photocoagulation group,showing a significant difference among the groups (F=98.767,P＜0.001).The number of Tuj-1 positive RGCs was considerably reduced in the transgoniscope laser photocoagulation group and translimbal laser photocoagulation group compared with the normal control group and the number of Tuj-1 postive RGCs was low in the translimbal laser photocoagulation group compared with the transgoniscope laser photocoagulation group (all at P＜0.001).Conclusions Transgoniscope laser photocoagulation targeting trabecular meshwork can induce chronic ocular hypertension and RGCs losing.However,its pattern is different from translimbal laser photocoagulation.Transgoniscope laser photocoagulation has a higher successful rate of chronic ocular hypertention than that of translimbal laser photocoagulation.

AIM: To characterize the gene expression of sortilin on adipogenic and osteogenic differentiation in mesenchymal stem cells (MSCs) in vitro and explore its significance.METHODS: MSCs derived from human bone marrow were isolated and cultured in vitro ,then were stimulated in osteogenic medium and adipogenic medium,respectively. Osteopontin and lipoprotein lipase were detected by RT-PCR. Sortilin expression was analyzed by semiquantitative RT-PCR. RESULTS: 1.MSCs displayed the potential of differentiation into osteoblast and adipocyte. 2.Sortilin was upregulated one day after osteogenic induction and remained upregulated for a week. The expression of sortilin was significant increased on day 3( P 0.05).CONCLUSION: Sortilin may be useful to modulate the osteogenic differentiation and may not be necessary for adipocyte commitment in MSCs. The regulation of sortilin expression may provide new protocal and strategy for the treatment of osteoporosis and osteopenic disease.

AIM: To investigate the effects of transforming growth factor ?1 (TGF-?1) on murine-derived dendritic cells (DC). METHODS: Murine bone marrow cells were cultured with GM-CSF and TGF-?1 to develop TGF ?-DC. Then they were stimulated by lipopolysaccharide (LPS). Their phenotypes were assessed by flow cytometry (FCM). The allogeneic stimulating capacity of DC was measured by mixed lymphocyte reaction (MLR) using BrdU ELISA method. IL-12 p70 protein was detected by ELISA and the expressions of Toll like receptor 4 (TLR4) on DCs were measured by semi-quantitative RT-PCR and FCM. RESULTS: Compared to immature DC (imDC) cultured with GM-CSF alone, the expressions of CD_80, CD_86, I-A~b and CD_40 in TGF ?-DC were lower. The TGF ?-DC was resistant to maturation by LPS. Maturation resistance was evident from a failure to up-regulate CMs, to stimulate larger T cell proliferation and to increase secretion of IL-12 p70. Down-regulation of TLR4 expression on TGF ?-DC was also found. CONCLUSION: TGF-?1 inhibits the expression of co-stimulatory molecules on DC. It is resistant to maturation stimulus (LPS) and might be linked with TLR4 down-regulation.
