Age-related white matter changes are considered as a manifestation of arteriolosclerotic small vessel disease and are associated with advanced age and vascular risk factors.White matter changes have been recognized as one of the manifestations of cerebral small vessel disease.They are the pathological basis of cognitive impairment and functional loss in the elderly.Studies have shown that when white matter changes develop to a certain extent,there will be many clinical symptoms,including cognitive impairment,dementia,depression,gait disturbances,and urinary incontinence,and they are associated with the increased risks of stroke and death.
3.Effect of mindfulness training intervention on cancer related fatigue after chemotherapy in patients with breast cancer

Objective To investigate the effects of mindfulness training on cancer related fatigue (CRF) in patients with breast cancer .Methods Two hundreds young and middle‐aged patients with breast cancer receiving chemotherapy in our hospital from January to June 2015 were divided into the observation group and control group ,100 cases in each group .The control group was given the routine nursing ,while on this basis the observation group was given the mindfulness training .The cancer related fa‐tigue status was evaluated according to the CFS scale .The CFS various dimensions and total fatigue scores before and after inter‐vention were compared between the two groups .Results The fatigue status existed in most of patients with breast cancer after chemotherapy ,the occurrence rate of CRF was 97 .0% ,which was dominated by moderate fatigue ,followed by mild fatigue ,severe fatigue was minimal ;the body fatigue ,cognitive fatigue ,emotional fatigue and total fatigue scores in the second time of chemothera‐py had no statistically significant differences between the two groups (P>0 .05);in the fourth and sixth time of chemotherapy ,the body fatigue ,cognitive fatigue ,emotional fatigue and total fatigue scores in the observation group were significantly lower than those in the control group ,the differences were statistically significant (P<0 .05) .Conclusion The mindfulness training interven‐tion can relieve the fatigue status and improve the living quality of the patient .
4.Expression of cellular fibronectin mRNA in adult periodontitis and peri-implantitis: a real-time polymerase chain reaction study.

Cellular fibronectin (cFn) is a type of bioactive non-collagen glycoprotein regarded as the main substance used to maintain periodontal attachment. The content of cFn in some specific sites can reflect the progress of periodontitis or peri-implantitis. This study aims to evaluate the expression of cFn messenger RNA (mRNA) in tissues of adult periodontitis and peri-implantitis by real-time fluorescent quantitative polymerase chain reaction (PCR) and to determine its clinical significance. A total of 30 patients were divided into three groups of 10: healthy, adult periodontitis and peri-implantitis. Periodontal tissue biopsies (1 mm×1 mm×1 mm) from each patient were frozen in liquid nitrogen. Total RNA was extracted from these tissues, and the content, purity and integrity were detected. Specific primers were designed according to the sequence, and the mRNA expression levels of cellular fibronectin were detected by real-time PCR. The purity and integrity of the extracted total RNA were both high, and the specificity of amplified genes was very high with no other pollution. The mRNA expression of cFn in the adult periodontitis group (1.526±0.441) was lower than that in the healthy group (3.253±0.736). However, the mRNA expression of cFn in the peri-implantitis group (3.965±0.537) was significantly higher than that in the healthy group. The difference revealed that although both processes were destructive inflammatory reactions in the periodontium, the pathomechanisms were different and the variation started from the transcription level of the cFn gene.

5.Effects of electroacupuncture at "Yishu" (EX-B 3) on the relative hormones of HPA axis in rats with type-2 diabetes mellitus.

OBJECTIVETo observe the intervention effects of electroacupuncture at "Yishu" (EX-B 3) on rats with type-2 diabetes mellitus (T2DM), so as to provide experiment references for acupuncture to treat T2DM.
METHODSAmong seventy male Wistar clean-grade rats, 8 rats were randomly selected into a control group; the rest rats were made T2DM model. Fifty-two rats which were successfully made T2DM model, according to randomized block method, were divided into a model group (10 rats), a medication group (10 rats), an electroacupuncture at "Shenshu" (BL 23) group (11 rats), an electroacupuncture at "Pishu" (BL 20) group (10 rats) and an electroacupuncture at "Yishu" (EX-B 3) group (11 rats). Seven days after successful establishment of model, the rats in the model group were fixed in the self-made rat bag without receiving any treatment; the rats in the medication group, according to body mass (10 mL/kg), were treated with intragastric administration of glimepiride; the rats in all the electroacupuncture groups were treated with electroacupuncture at "Shenshu" (BL 23), "Pishu" (BL 20) and "Yishu" (EX-B 3), respectively. The continuous wave was selected with a frequency of 15 Hz and a current intensity of 4 to 6 mA. The treatment was given 20 min per treatment, once a day, 5 treatments per week for continuous 4 weeks. Before the establishment of model and continuous 4 weeks after the intervention, blood samples were collected from rats' caudal vein, and fasting blood glucose (FBG) was measured with FBG device each week. After the last intervention, the rats were killed and hypothalamus, pituitary and adrenal gland were collected. The colorimetric method was applied to measure the contents of triglyceride (TG), cholesterol (TC), high-density lipoprotein (HDL-C) and low-density lipoprotein (LDL-C); radioimmunoassay was used to test the contents of glycated serum protein (GSP), fasting insulin (FINS), corticotropin-releasing hormone (CRH), adrenocorticotropic hormone (ACTH) and cortin (CORT).
RESULTSFour weeks after the intervention, except that the rat's body mass in the normal group continued to increase, body mass in the model group, medication group and each electroacupuncture group were significantly reduced (P<0.01, P<0.05). Compared with the model group, the FBG in the electroacupuncture at "Pishu" (BL 20) group and electroacupuncture at "Yishu" (EX-B 3) group were obviously reduced (P<0.05, P<0.01); FBG in the electroacupuncture at "Yishu" (EX-B 3) group was lower than that in the medication group and electroacupuncture at "Shenshu" (BL 23) group (both P<0.05). The contents of TG, HDL-C and LDL-C in the electroacupuncture at "Yishu" (EX-B 3) group were reduced (P<0.05, P<0.01), the content of TG was significantly lower than that in the medication group and electroacupuncture at "Shenshu" (BL 23) group (both P<0.05), the content of LDL-C was significantly lower than that in electroacupuncture at "Shenshu" (BL 23) group (P<0.05). Insulin sensitivity index (ISI) in the medication group, electroacupuncture at "Pishu" (BL 20) group and electroacupuncture at "Yishu (EX-B 3)" group were evidently increased (P<0.05, P<0.01); ISI in the medication group was lower than that in the electroacupuncture at "Yishu" (EX-B 3) group (P<0.05). The content of CRH in the electroacupuncture at "Yishu" (EX-B 3) group was lower than that in the medication group and electroacupuncture at "Shenshu" (BL 23) group (P<0.05, P<0.01); the content of CORT in the electroacupuncture at "Yishu" (EX-B 3) group was lower than that in the medication group and electroacupuncture at "Pishu" (BL 20) group (both P<0.05).
CONCLUSIONElectroacupuncture at "Yishu" (EX-B 3) could reduce the level of CORT to improve the insulin resistance in rats with T2DM, improve insulin sensitivity index, regulate blood lipid metabolism and relieve the hyperactivity of the HPA axis.

6.The Outcomes Measures of the Cochlear Implantation in Patients with Waardenburg Syndrome

Objective To measure the outcomes of the cochlear implantation for patients with Warrensburg syndrome in comparison with non-syndromic patients with sensorineural hearing loss. Methods 16 of 1 300 Waardenburg syndrome patients who received cochlear implants at PUMC hospital were reviewed in this papers. The authors will discuss the effects between Warrensburg syndrome and non- syndromic patients with sensorineural hearing loss by means of the Parents Evaluation of Aural-Oral Performance of Children (PEACH). Results There were no statistical difference in quiet, in noise, in overall results and in conversation on the phone between Warrensburg syndrome and non-syndromic patients. There was statistical difference between in quiet and in noise in every group. Conclusion The auditory perception and speech recognition between Warrensburg syndrome and non-syndromic patients with sensorineural hearing loss is same, and the results in quiet is better than that in noise.
7.Application of Quality Metrics in Food Inspection Laboratory Management

Because of the particularity and the complexity of integrated management of food, government regulators must perform the whole process of quality control and trace to evaluate and judge the merits of food instead of only depending on product standards.Quality metrics is a new concept for the food industry, and can be regarded as the comprehensive evaluation and measurement for product quality and the standard for judging the quality of food products.Request for Quality Metrics Guidance for Industry was issued, which not only established a new model for the quality control of pharmaceutical industry, but also provided a new way of quality management for the inspection organization.By introducing the contents of Request for Quality Metrics Guidance for Industry issued by the US FDA, on the basis of summarization and thought, the quality management and evaluation of pharmaceutical industry was compared, and the feasibility and applicability of quality metrics management in inspection and testing field, especially in food testing field was evaluated.In this paper, the concept of quality metrics was extended to the level of food inspection laboratory management with hope to provide ideas for the future development of food quality management model.
8.Effects of laparoscopic gastric bypass surgery on lipid metabolism in obese patients

Objective To assess the lipid profiles of obese patients undergoing laparoscopic gastric by-pass surgery.Methods A retrospective observational study was performed , involving 25 patients(BMI>28 kg/m2 ) who consecutively underwent gastric bypass surgery in the First Affiliated Hospital of Nanjing Medical Univer -sity from Oct.2010 to Dec.2012.For the surgery, a small gastric pouch about 30 ml was performed with 100 cm biliopancreatic limb and 100-200 cm alimentary limb.Total cholesterol(TC),triglycerides(TG),high density lip-oprotein( HDL) and low density lipoprotein ( LDL) at baseline and 1, 3, 6, 9 and 12 months after surgery were measured.Related complications were recorded .Results All surgeries were performed laparoscopically and no serious complications occurred .TC, TG, and LDL significantly reduced after the surgery (P<0.05 for all).In-versely, HDL disclosed a significant rise(P<0.05).Of the lipid profiles at baseline, only HDL concentration displayed a correlation with BMI .Percentage of excess weight loss ( PEWL ) displayed significant correlation with the variety of TG、LDL、HDL after surgery(P<0.05 for all).Conclusions Laparoscopic gastric bypass is a safe operation which improves the lipid metabolism of obese patients .The operation can decrease the concentration of TC, TG, LDL and increase HDL level .
9.Effect of Processing on Content Difference between Crude Materials and Decoction Pieces of Ginseng Radix et Rhizoma

Objective:To establish a content determination method for the decoction pieces of ginseng radix et Rhizoma and investi-gate the quality of the slices by measuring the content of ginsenoside in commercially available crude materials and the decoction pieces. Methods:An HPLC method was used to determine the content of ginsenoside Rb1, Re and Rg1 in the crude materials and decoction pieces of ginseng radix et Rhizoma. Results:Significant difference in ginsenoside content between the medicinal materials and decoction pieces was found, and saponin content in the slices was decreased. Conclusion:The quality of the slices of ginseng radix et Rhizoma is changed significantly, and it is urgent to develop a agreed standard for the quality control of ginseng radix et Rhizoma during the process-ing.
