1.Preliminary study on serial observation of rabbit carotid arteriosclerosis model before and after carotid endarterectomy with high resolution MRI

Objective To serially observe carotid arteriosclerosis in rabbit model before and after CEA using high-resolution MRI. Methods Using New Zealand rabbits, lesions were created by air-drying an isolated segment of common carotid artery (120ml/min for 15min) and feeding the rabbits with a diet of 2% cholesterol, 6% peanut oil mixed with normal diet for 2 months. High-resolution MRI examinations were performed for 1 normal rabbit, 4 rabbits with replicated arteriosclerosis before CEA, and 2 rabbits with arteriosclerosis 10 days after CEA, and the results were compared with pathological findings. Results High-resolution MRI revealed the contour and shape of the type II to type IV lesions of rabbit carotid arteriosclerosis model. The MRI images of the lesions corresponded well with the pathological findings. The fibrous plaques were showed as low signals on TOF film, gray images on T 1, T 2, and PDW film, and were obviously enhanced by Gd-DTPA. The MRI images obtained 10 days after CEA also corresponded well with the neo-intima. Conclusion High-resolution MRI can be used as an effective non-invasive method to serially observe the arteriosclerosis lesion of rabbit carotid artery before and after CEA.
2.Correlation between brain natriuretic peptide and cerebral vasospasm following subarachnoid hemorrhage and hyponatremia

Objective To explore the correlation between brain natriuretic peptide (BNP) and cerebral vasospasm (CVS) following subarachnoid hemorrhage (SAH) and hyponatremia. Methods Plasma BNP levels and serum natrium were assessed at 4 different time periods (1 to 3 days, 4 to 6 days, 7 to 9 days, and 10 to 12 days after SAH) in 30 patients with SAH. Correlation analysis was carried out between plasma BNP and blood sodium level. Plasma BNP levels were compared according to patients with or without CVS, and with or without symptom. 3.Bibliometric analysis of proliferative vitreoretinopathy based on Web of Science database

Objective:To study the distribution of the literature and the trend of international research of proliferative vitreoretinopathy (PVR) in 10 years from 2009 to 2018.Methods:Using Web of Science database of the American Institute of Scientific Information as the data source to collect literature on proliferative vitreoretinopathy published from 2009 to 2018 and a statistical analysis on the distribution of the publication time, nation and funding agency was made to figure out the institution, periodical, author with highest citations respectively among the collected literature.Meanwhile, the bibliometric analysis software Citespace and VOSviewer were employed to carry out the distribution analysis and cluster analysis of keywords as well as the co-citation cluster analysis.Results:A total of 905 articles related to PVR were included, mainly in English, most of which were written by American authors, and the number of literature published by Chinese authors was ranked the second.Among all the article funding institutions, the National Natural Science Foundation of China ranked NO.1 with the most funded articles.Citations increased year by year since 2009 and increased obviously since 2013.The journal with the highest citations was Investigative Ophthalmology & Visual Science, and the research institution with the highest citations was Harvard University.The statistical analysis of the citation quantity also showed that the top five authors with the highest citations were all from Massachusetts Eye and Ear, Department of Ophthalmology, Harvard Medical School.The results of cluster analysis showed that there were 41 high-frequency subject terms, and they were mainly grouped in 4 categories, which were etiology, risk factors, molecular mechanism, treatment and management of PVR.Keywords network visualization and overlay visualization both showed that etiology and risk factors were the focus of current research, while epithelial mesenchymal transition, transforming growth factor-β and fibrosis were new research fields in recent years.The results of citation cluster analysis showed there were 12 co-citation clusters. Conclusions:The literature of PVR shows an upward trend, and the researches focus on the molecular mechanism, prevention, and neuroprotection of PVR, which may be the future PVR research direction.
4.Promoter hypermethylation of the RUNX3 gene in colorectal carcinogenesis and its prognostic significance

ObjectiveToinvestigateRUNX3genepromoter methylationincolorectal carcinogenesis and its prognostic significance. MethodsThe protein expression of RUNX3 was detected by immunohistochemistry and the methylation status of the RUNX3 was determined by methylation-specific PCR (MSP) in colorectal normal mucosa and cancer tissue from 65 patients,and adenoma from 28 patients.5-year overall survival rate was analysed according to RUNX3 methylation status from cancer patients.Kruskal-Wallis rank sum test,x2 or Fisher's exact test,Log-rank test and multivariable Cox regression analysis were used for statistical analysis.ResultsThe protein expression of RUNX3 gene in adenoma and cancer was 85% (24/28) and 52% (34/65),significantly lower than that in normal mucosa 94% (61/65)( x2 =4.328,P =0.037 ; x2 =16.675,P =0.000),the difference between adenoma and cancer tissue was no statistic significance (x2 =3.266,P =0.071 ).No case showed RUNX3 methylation in normal mucosa,methylation rates in adenoma and cancer tissue were 21% (6/28) and 35% (23/65),significantly higher than in normal mucosa (P =0.000 ),but there was no statistic significance between adenoma and cancer tissue (x2 =1.766,P =0.183).The protein expression rate with RUNX3 methylation was 67% (4/6) in adenoma,unmethylation 91% (20/22) (P =0.191 ).The protein expression rate with RUNX3 methylation was 26% (6/23) in cancer,unmethylation was 88% (28/32).The presence of RUNX3 methylation was related to loss of protein ( x2 =9.810,P =0.002 ).5-year total survival rate with methylation in cancer was significantly lower with unmethylation ( x2 =5.87,P =0.016 ).Multivariate analysis showed RUNX3 methylayion wasanindependentprognosticfactoramongthefactorsanalyzed(P=0.033 ).ConclusionsRUNX3 methylation is important genetic event in colorectal carcinogenesis,possibly related to protein downregulation,and an independent prognostic factor for colorectal carcinoma.
5.Effect of intermittent use of low molecular heparin on microemboli detected by transcranial Doppler monitoring in patients with ischemic stroke

Objective To investigate the effect of intermittent use of low molecular heparin (LMWH) on microembofic signal (MES) in patients with ischemic stroke on the basis of anti-platelet aggregation and statin therapy.Methods Ninety MES-positive patients with acute ischemic stroke detected by transcranial Doppler were randomly divided into a non-LMWH group (n =44) and a LMWH group (n =46).The non-LMWH group was treated conventionally with enteric-coated aspirin and atorvastatin.On the basis of conventional therapy,the LMWH group was treated with LMWH,subcutaneous injection of LMWH calcium 4 100 AXaIU every 3 months,twice a day,and one week as a course of treatment (a total of 3 courses).The number of MES,MES-positive rate and incidence of ischemic cerebrovascular events at 3 and 6 month were compared in both groups.Results There was no significant difference in the MES-positive rates at 3 month after treatment between the non-LMWH group and the LMWH group (70.45% vs.61.36% ;x2 =1.357,P =0.244),but the number of MES in the non-LMWH group was higher than that in the LMWH group (12.07 ± 10.16 vs.8.09± 8.13; t =1.470,P =0.043); the MES-positive rate at 6 month after treatment in the non-LMWH group was significantly higher than that in the LMWH group (36.96% vs.19.57%;x2=3.982,P=0.046),and the number of MES in the non-LMWH group was also significantly higher than that in the LMWH group (10.32 ±9.93 vs. 1.46 ± 3.27; t =5.450,P =0.000).There was no significant difference in incidence of ischemic cerebrovascular events at 3 month (2.17%vs. 9.09%,P =0.198 ),but the incidence of ischemic cerebrovascular events at 6 month in the LMWH group was significantly lower than that in the non-LMWH group (4.35% vs.20.45%,P =0.025).Conclusions On the basis of anti-platelet aggregation and statin therapy,the intermittent use of LMWH may decrease the MES-positive rate and the incidence of ischemic cerebrovascular events in the MES-positive patients with ischemic stroke at 6 month.
6.Application of phase angle in estimating malnutrition and clinical practice

Phase angle (PA), derived by bioelectrical impedance analysis, has been used to evaluate nutritional status.It has a unique advantage in diagnosing early malnutrition related to diseases by the detection of human body cell membrane integrity and fluid composition change, and being associated with bad disease prognosis in part of clinical research.Because of its non-invasive, convenient, accurate and affordable technique, it has a wide prospect of clinical application.However, lack of uniform PA reference value somehow limited its application.We summarize the latest clinical research and application situation of PA, and explore the characteristics and the influence factors of PA in different disease group, in order to provide theoretical basis for the Chinese PA reference range.
7.Research advances in the mechanism of subthreshold micropulse laser in diabetic macular edema

In recent years, the subthreshold micropulse laser is a kind of laser mode which is characterized by long intermittence. It achieves effective therapeutic effect while minimizes the damage to tissues. At present, it has been used to treat diabetic macular edema. Early studies suggested that the laser selectively acts on retinal pigment epithelial cells to reduce macular edema by regulating the expression of inflammatory biomarkers, growth factors, heat shock proteins and other substances. In recent years, with the development of research, more and more emphasis has been placed on the role of retinal glial cells. Müller cells are also considered as one of the target cells affected by micropulse laser, but there is no evidence of direct or indirect effects of micropulse laser on Müller cells. In the near future, it is expected that we will have more clinical evidence to confirm the target cells of the micropulse laser, which may be further confirmed by in vitro experiments through Müller cells or Müller cells co-cultured with retina pigment epithelium cells, so as to make a more detailed statement on the mechanism of it.
8.Perihematomal edema in acute intracerebral hemorrhage treated with hyperbaric oxygen

Objective To study the effect of hyperbaric oxygen (HBO) therapy on perihematomal edema in acute spontaneous intracerebral hemorrhage. Methods Sixty-three consecutive hospitalized patients with supraten-torial intracerebral hemorrhage were allocated to an HBO group (n = 30) or a control group (n = 33) at random. Routine therapies were used with both groups. The treatment group received in addition twenty consecutive daily ses-sions of HBO therapy beginning 3~5 d after onset. MRI brain scans were performed on the 5~7th and 25th day. Absolute edema volumes and relative edema volumes were measured from T2-weighted images. Apparent diffusion co-efficients (ADCs) of the edematous regions were calculated on diffusion-weighted images (DWI). Results There were no statistical differences between the two groups in terms of age, sex, blood pressure, NIHSS, hematoma posi-tion or volume. At the 5th~7th d, both absolute and relative edema volumes in the HBO group were smaller than in the controls (P≤0.05). Brain edema was still prominent at the 25th d. Absolute edema volumes, relative edema volumes and ADC values were all smaller in the HBO group at the 25th day compared with the controls (P≤0.05). Conclusion HBO therapy soon after intracerebral hemorrhage can lessen the severity and range of brain edema. E-dema persists after the onset of the disease, and HBO can reduce such delayed brain edema. HBO may benefit func-tional recovery from intracerebral hemorrhage by reducing perihematomal edema.
9.The influence of insulin therapy on the short-term prognosis of patients with acute ST-elevation myocardial infarction complicated with hyperglycaemia

Objective To observe the influence of insulin therapy on the short-term prognosis of patients with acute ST-elevation myocardial infarction (ASTEMI) complicated with hyperglycaemia.Methods A total of 128 patients with ASTEMI complicated with hyperglycaemia were randomized into 2 groups:treatment group and control group.There were 64 patients(50.0% )in the treatment group and 64 patients(50.0% ) in the control group.All of the patients were given anticoagulants,antiplatelet drugs,statins,beta-receptor blockers,nitrates and angiotensin-converting enzyme inhibitors (ACEI)or angiotensin receptor blocker (ARB) Moreover,the patients in the treatment group were given extra 50 U insulin ( Novolin R) in 50 ml saline by venous infusion.In the treatment group,Venous blood samples were obtained before and 7 days after treatment to determine the fasting blood glucose and hs-CRP in serum; the levels of fasting blood glucose and hs-CRP and short-term prognosis were compared between the 2 groups.ResultsThe levels of fasting blood glucose in the treatment group were decreased significantly after treatment ( [ 5.4 ± 0.8 ] mmol/L vs [ 11.6 ± 2.3 ] mmol/L,P <0.01),but there was no significant change in the control group( [ 10.7 ± 2.1 ] mmol/L vs [ 11.2 ± 2.7]mmol/L,P > 0.05 ).The levels of hs-CRP in both groups were decreased significantly after treatment (P <0.05),but it was much more obvious in the treatment group( [ 6.2 ± 1.5 ] mg/L vs [ 8.7 ± 1.8 ] mg/L,P <0.05).The incidence of major adverse cardiac events(MACE) during a 3 months′ follow-up in the treatment group was significantly lower than that in the control group ( 12.50% vs 34.38%,P < 0.05 ).Conclusion Insulin therapy can improve the short-term prognosis in patients with ASTEMI complicated with hyperglycaemia.
10.Experimental research of shielding the patients during electron beam radiotherapy

Objective To explore the shielding effects of 1-4 layers of lead aprons (LPs) and body shielding devices (BSDs) against stray radiation (SR) outside the electron beam field of 6-15 MeV.Methods JR-115B LiF TLDs were used to measure the stray radiation doses (SRDs) to the patient undergoing treatment,before and after being shielding,for different distances,different energies,different applicators,variable layers of LPs,and different thickness of body shielding devices (BSDs),respectively,along long calculating and comparing the shielding ratios of LPs and BSDs against SR.Results When the applicator (10 cm × 10 cm) is unchanged,the shielding ratio increased with the increased distance from measuring point (r =0.717,P < 0.05) and decreased with the increased electron energy (r =-0.678,P < 0.05);when the energy was constant,there was no correlation between the shielding ratio and the size of applicator (P > 0.05);For lower energy electron beam of 6 and 9 MeV,the shielding ratio for 1 mm Pb-BSD was slightly higher than that for 2 layers of LA (t =2.519,2 662,P < 0.05),ranging from 81.5% to 95.3% and 55.4% to 84.6%,respectively.For 12 and 15 MeV higher energy electron beam,the shielding ratio for 2 mm Pb-BSD was slightly higher than that for 4 layers of LA (t=3.768,7.934,P<0.05),ranging from 64.6% to 93.4% and 51.1% to 92.4%,respectively.Conclusions LAs or BSDs are availavle for effectively reducing the doses from stray radiation,and may help reduce the secondary risks from stray radiation.BSDs have more obvious advantages than LPs with regard to shielding effect.
