Journal of Chinese Physician 2001;0(04):-
Objective To compare the effect of acute hypervolemic hemodilution (AHH) and normovolemic hemodilution (ANH) as means of perioperative blood conservation in the patients undergoing hepatic tumor operation. Methods 45 ASA I-II patients with hepatic tumor were randomly divided into three groups, control, AHH and ANH, each group containing 15 cases. After induction of anesthesia, the patients of AHH group were rapidly infused 20 ml/kg fluid intravenously, the patients of ANH group were withdrawn 15ml/kg intravenous blood and simultaneously infused the same volumes of 6% hydroxyethyl starch. BP, HR, ECG, SaO2, urine output, fluid input and blood loss were monitored during operation. Blood samples were collected before and after hemodilution, at end of operation and the first day of postoperation to measure Hb, Hct, Plt, blood biochemical indices, PT and APTT. Results After hypervolemic and normovolemic hemodilution Hct reduced to 30.9?3.3% and 29.6?2.8% from 39.8?3.4% and 40.2?3.5% respectively, and PT and APTT were significantly prolonged in both the AHH and ANH patients (P

Journal of Chinese Physician 2000;0(12):-
Objective To observe the changes of hemorheology after acute hypervolemic hemodilution (AHH), and provide theoretic basis for clinical rationally using artificial plasma substitutes. Methods 90 patients undergoing hip joint operation were randomly divided into 3 groups (each group containing 30 patients), which received intravenous infusion of 6% hydroxyethyl starch (HES), 4% gelofusine or ringer's solution at 20ml/kg, respectively before operation. The whole blood viscosity, plasma viscosity, Hct, index of RBC aggregation and index of RBC deformation were measured before and after hemodilution. Resutls Hemodynamics was stable after AHH, and the amounts of perioperatively blood transfusion in the patients received colloid solution were obviously less than those in the patients received crystalloid solution. The whole blood viscosity, Hct and index of RBC aggregation significantly decreased after AHH in all the patients, and index of RBC deformation markedly increased in the patients after AHH with 6% HES. Conclusion AHH could effectively maintain hemodynamics stable, improve hemorheological condition and perfusion of microcirculation, enhance the tolerance of blood loss, and reduce homologous blood transfusion. Colloid solution was better than crystalloid solution for AHH.

Objective:To investigate the predictive value of P-selectin and D-dimer in the early diagnosis of portal vein thrombosis(PVT) in patients submitted to portal hypertension operation. Method:Eighty-two patients were assigned into two groups(PVT group and non-PVT group)after the operative treatment of portal hypertension. The levels of P-selectin and D-dimer were detected dynamically in the patients of two groups. Results:The preoperative levels of P-selectin and D-dimer in the PVT group were higher than those in the non-PVT group respectively(P

Objective To observe the protective and regeneration-promoting effects of Ruyi Zhenbao Pill (RZP) on nerve injury in zebrafish. Methods The zebrafish model of central nervous injury was induced by mycophenolate mofetil, the model of peripheral motor nerve and axonal injury was induced by ethanol, and the model of myelin damage was induced by ethidium bromide. The variations of central nerve, axon and myelin sheath fluorescence intensity, and peripheral motor nerve length in zebrafish, which exposed to the different concentration (10.0,33.3,and 100.0 μ.g/mL) of RZP,were observed with fluorescence microscope. The effective protection rates of RZP on zebrafish central nerve and axone, and the regeneration-promoting effect on peripheral motor nerve and myelin sheath were analyzed and calculated with the image processing software NIDS-Element's.Results In 10.0,33.3,and 100.0 μg/mL RZP groups, the zebrafish central nervous injury protective rates were 2%,24%,and 50% (P < 0.001),respectively,the peripheral nerve regeneration promoting rates were 44% (P < 0.05),49% (P < 0.01),and 93 % (P < 0.001),the axonal injury recovery rates were 3%,29% and 48% (P < 0.05),and the myelin sheath regeneration promoting rates were 36% (P < 0.01),37% (P < 0.001),and 41% (P < 0.001),compared with model group. Conclusions RZP could not only protect the central nervous and axonal injury, but also promote the regeneration of peripheral nerve and myelin sheath in zebrafish.

In this article, the Chinese Traditional Medicine and Materia Medica Subject Headings, Standards of the People's Republic of China - Classification and Codes of Diseases and Zheng of Traditional Chinese Medicine and the Chinese Terms in Traditional Chinese Medicine and Pharmacy were compared. Three standards were compared from the terminology quantity, content and classification. Each standard has its special feature. The compatibility and consistency are not strong in these standards. More authoritative traditional Chinese medicine terminology standards need to be established for the application in the clinical practice and scientific research.

This article was aimed to study the semantic relationship of clinical terminology of traditional Chinese medicine (TCM) from 3 000 clinical medical records and outpatient medical records from books. The aim was to verify the coverage of semantic relationships of UMLS and SNOMED in medical language system with real medical records. The extraction was from the practical aspect in order to improve the semantic relationships involved in TCM clinical practice processes.

In China, the treatment of AIDS With Chinese herbal medicine compound has been widely studied and practiced. Clinical Studies showed that the treatment of AIDS with Chinese medicine can significantly relieve AIDS patients' clinical symptoms, improve life quality with rising immunity and alleviate toxic and side effects in antiviral drug. This study took 28 Clinical Studies on the treatment of AIDS with Chinese herbal medicine compound published in Chinese periodicals as the research object, and designed Tree Analysis Algorithm was used to analyze prescription compatibility in the treatment of AIDS with Chinese herbal medicine compound. Results of data analysis revealed that clinical therapy on AIDS could be classified into three types: predominent de-toxication, predominent supplementing Qi, and simultaneous de-toxication and supplementing Qi due to different understanding the pathogenesis of excessive evil and deficient essence-qi. But in practical medication, there has not formed a reasonable and effective combination or basic prescription up to now, except that Astragalus was wildly used.

Curriculum integration and problem based learning (PBL) is the development trend of medical education all over the world, and it is an effective mode to train high-quality medical talents to meet the needs of the times and national medical and health services. However,PBL is difficult to popularize in an all-round way at present due to the large enrollment of clinical medicine major in medical colleges and universities in China.Aiming at integrating knowledge and focusing on ability,Nanjing Medical University implemented a problem-based two-way integrated case teaching after top-level design, careful trial and steady promotion and has achieved initial results and improved the clinical competency of medical students.

Chinese Journal of Digestive Surgery 2021;20(6):643-647
At present, the surgical treatment of upper gastric cancer, including esophago-gastric junction cancer, mainly includes the total gastrectomy and proximal gastrectomy. After total gastrectomy, the reconstruction of digestive tract is completed through the anastomosis of esophagus and jejunum. Patients undergoing total gastrectomy often face the risk of poor eating effect and malnutrition. Compared with total gastrectomy, the proximal gastrectomy can preserve part of the gastric tissue, but due to the loss of the normal physiological structure of the cardia, patients have a higher risk of postoperative reflux. In order to solve the problem of reflux, there are many improved operation methods of digestive tract reconstruction after proximal gastrectomy. At present, the choice of total gastrectomy or proximal gastrectomy and the operation methods of digestive tract reconstruction is still controversial. Because of the lack of sufficient theoretical research support. The authors comb the research progress and consider the concept and method of digestive tract reconstruction after total gastrectomy and proximal gastrectomy, in order to provide theoretical basis for clinical work.
