1.Formulation of Criterion for Clinical Application of Shuxuetong Injection by Clinical Pharmacists in Our Hospital Based on Evidence Quality Evaluation Method

OBJECTIVE: To provide reference for strengthening clinical application of key monitoring drugs and promoting rational drug use in clinic. METHODS: Based on evidence-based medicine, taking key monitoring drugs Shuxuetong injection as example, clinical evidence of domestic and foreign clinical studies were collected. The included literatures were graded according to the quality of GRADE evidence and recommended strength system. Evidence-based medicine evidence for the indications of Shuxuetong injection were evaluated, and criterion for clinical use of Shuxuetong injection was formulated in Huaihua First People’s Hospital (our hospital). RESULTS: The main content of criterion for clinical application of Shuxuetong injection formulated by our hospital was that there was A-level evidence support for acute ischemic cerebral infarction, but it was weakly recommended and only used for adjuvant therapy; there was B-level evidence support for anticoagulation (for preventing DVT), diabetic peripheral nerve lesion, but it was weakly recommended; there was only C-level or D-level evidence support for other indications, it was strongly recommendation against use. CONCLUSIONS: Clinical pharmacists formulate the criterion for clinical application of Shuxuetong injection by evidence quality evaluation method, provide reference for clinical application management of key monitoring drug and play an important effect on rational drug use in clinic.
2.Clinical study of protective technique for intercostal nerve in the application of esophageal cancer surgery

Objective To investigate the clinical effect of protective technique for intercostal nerve in the application of esophageal cancer surgery.Methods Seventy cases of esophageal and cardia cancer patients with thoracic operation were randomly divided into observation group and control group,and each group was 35 cases.Patients in the observation group were treated with improved protective technique of intercostal nerve during operation,and patients in the control group using the traditional methods of operation for treatment.All patients were treated with intramuscular injection of morphine analgesia drugs after operation.Results The pain score of every time point analgesia patients in the observation group (VAS score) was significant difference from that in the control group (t =3.041,2.854,1.827,2.156,1.965,2.706,P< 0.05).The incidence of adverse reactions of analgesia,the incidence of painless patients of postoperative 30 d and 60 d,morphine dosage,chest tube indwelling time and the average hospitalization time in observation group were 0,42.86%,68.57%,(10.6±5.2) mg (4.0±1.6) d,(17.6±4.5) d,and the control group were 17.14%,20.0%,34.29%,(22.8±7.6) mg,(4.8±1.8) d,(20.4±5.8) d (x2=4.56,4.24,8.24,t=7.838,1.965,2.257,P<0.05).Conclusion The improvement protective technique for intercostal nerve in esophageal cancer surgery,not only can reduce thoracotomy acute postoperative pain,but also reduce the happening of the CPP.Meanwhile,the analgesic effect is good.It is proved with a simple operation,less adverse reaction,conducive to disease recovery after esophageal cancer surgery,indicating that it has clinical popularization value.

4.Deviation index of eye and mouth on peripheral facial paralysis.

Differences of some points, levels and angles of the healthy and affected sides of patients with peripheral facial paralysis were picked out according to photographs. Through analysis of the index between the healthy and affected side of the patients and the difference between healthy people and patients, it is approved that those special points, levels and angles, which are called as deviation index of eye and mouth, can evaluate peripheral facial paralysis objectively and judge the degree of deviation. Therefore, it provides references for the diagnosis of facial paralysis and its degree judgement.

5.p53 gene intron 7 polymorphism and its association with oral neoplasms.

OBJECTIVETo investigate the association between oral neoplasm genetic susceptibility and genetic polymorphism of p53 intron 7.
METHODSThe intron 7 ApaI polymorphism of p53 was analyzed in 95 oral neoplasm patients and 105 healthy individuals by utilizing polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) genotyping assay technique, and direct sequencing was performed in 30 cases which were selected from the patients and controls by random sampling.
RESULTSIn oral neoplasms cases, haplotype combinations were T-G 43.2%, C-T 56.8%, and frequencies of genotype were T-G/T-G 15.8%, C-T/T-G 54.7%, C-T/C-T 29.5%, while in controls they were T-G 30.9%, C-T 69.1% and T-G/T-G 10.5%, C-T/T-G 41.0%, C-T/C-T 48.5%. There was a significant difference in the allelic frequency and the genotypical distributions between the oral neoplasm patients and the controls. The individuals with the T-G allele had a slight increasing neoplasm risk than individuals with C-T allele; the OR for T-G versus C-T was 1.69 (95% CI, 1.12 - 2.51). The risk of suffering from oral neoplasms was higher in the individuals of T-G/T-G genotype and of T-G/C-T genotype than in individuals of C-T/CT genotype with odds ratio of 2.48 versus 2.20.
CONCLUSIONSThere are two polymorphic points in the 7th intron of human p53 gene, which could be associated with genetic susceptibility of oral neoplasms. T-G allele may be the risk factor of oral neoplasms.

7.Discussion on the academic system of "theory, method, recipe and degree" in s infantile Chinese massage of the western part of Hunan province.

The theory and technique of s infantile Chinese massage of the western part of Hunan province are reviewed in clinical diagnosis and treatment and the core theoretical system is refined. Based on clinical experiences and in consideration of the analysis on relevant literature, the academic system of s infantile Chinese massage of the western part of Hunan province is summarized as four aspects, named "theory, method, recipe and degree". "Theory" refers to the four diagnostic methods and eight principle in terms of diagnosis and treatment and the therapeutic emphasis on the main disorders of organs in children. "Method" refers to promotion and restriction of five elements, meridian tropism in treatment. "Recipe" refers to the selection of acupoints, optimization of therapeutic techniques and the key in the composition of the herbs with different roles considered, named monarch herb, minister herb, assistant herb and guiding herb. "Degree" refers to the strength and intensity of manipulation to ensure the optimal effect. All of the aforementioned reviews provide the reference to theoretic study on this Chinese massage school.

8.Remote Ischemic Postconditioning for Ischemic Stroke: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BackgroundRemote ischemic postconditioning (RIPostC) appears to protect distant organs from ischemia-reperfusion injury (IRI). However, cerebral protection results have remained inconclusive. In the present study, a meta-analysis was performed to compare stroke patients with and without RIPostC.
MethodsCNKI, WanFang, VIP, CBM, PubMed, and Cochrane Library databases were searched up to July 2016. Data were analyzed using both fixed-effects and random-effects models by Review Manager. For each outcome, risk ratio (RR) and mean difference (MD) with 95% confidence interval (CI) were calculated.
ResultsA total of 13 randomized controlled trials that enrolled a total of 794 study participants who suffered from or are at risk for brain IRI were selected. Compared with controls, RIPostC significantly reduced the recurrence of stroke or transient ischemic attacks (RR = 0.37; 95% CI: 0.26-0.55; P < 0.00001). Moreover, it can reduce the levels of the National Institutes of Health Stroke Scale score (MD: 1.96; 95% CI: 2.18-1.75; P < 0.00001), modified Rankin Scale score (MD: 0.73; 95% CI: 1.20-0.25; P = 0.00300), and high-sensitivity C-reactive protein (MD: 4.17; 95% CI: 4.71-3.62; P < 0.00001) between the two groups. There was no side effect of RIPostC using tourniquet cuff around the limb on ischemic stroke treating based on different intervention duration.
ConclusionThe present meta-analysis suggests that RIPostC might offer cerebral protection for stroke patients suffering from or are at risk of brain IRI.

9.Casticin Attenuates Stemness in Cervical Cancer Stem-Like Cells by Regulating Activity and Expression of DNMT1.

OBJECTIVE:
To explore whether casticin (CAS) suppresses stemness in cancer stem-like cells (CSLCs) obtained from human cervical cancer (CCSLCs) and the underlying mechanism.
METHODS:
Spheres from HeLa and CaSki cells were used as CCSLCs. DNA methyltransferase 1 (DNMT1) activity and mRNA levels, self-renewal capability (Nanog and Sox2), and cancer stem cell markers (CD133 and CD44), were detected by a colorimetric DNMT activity/inhibition assay kit, quantitative real-time reverse transcription-polymerase chain reaction, sphere and colony formation assays, and immunoblot, respectively. Knockdown and overexpression of DNMT1 by transfection with shRNA and cDNA, respectively, were performed to explore the mechanism for action of CAS (0, 10, 30, and 100 nmol/L).
RESULTS:
DNMT1 activity was increased in CCSLCs compared with HeLa and CaSki cells (P<0.05). In addition, HeLa-derived CCSLCs transfected with DNMT1 shRNA showed reduced sphere and colony formation abilities, and lower CD133, CD44, Nanog and Sox2 protein expressions (P<0.05). Conversely, overexpression of DNMT1 in HeLa cells exhibited the oppositive effects. Furthermore, CAS significantly reduced DNMT1 activity and transcription levels as well as stemness in HeLa-derived CCSLCs (P<0.05). Interestingly, DNMT1 knockdown enhanced the inhibitory effect of CAS on stemness. As expected, DNMT1 overexpression reversed the inhibitory effect of CAS on stemness in HeLa cells.
CONCLUSION
CAS effectively inhibits stemness in CCSLCs through suppression of DNMT1 activation, suggesting that CAS acts as a promising preventive and therapeutic candidate in cervical cancer.

In ancient literatures, there are many records about the efficacy of Chinese herbal medicine in treating tumor, and modern pharmacological researches have shown that more and more active ingredients of traditional Chinese medicine(TCM) have gradually highlighted their inhibitory effect on various types of tumor.
