2.Efficacy and safety observation after implantation of TICL in the treatment of high myopia with astigmatism

AIM:To study the efficacy and safety of Toric implantable contact lens (TICL) implantation in the treatment of patients with high myopia and astigmatism.METHODS: Totally 90 patients (178 eyes) diagnosed as this disease were selected in our hospital during September 2012 to September 2016 by the method of random.The UCVA, BCVA, refraction, astigmatism coefficient, corneal endothelial cell, intraocular pressure, manifest refraction examination were accessed and compared before and after 3 and 9mo of surgery.RESULTS: After 3 and 9mo of the surgery, the UCVA and BCVA were much higher than those before treatment, which the difference was statistically significant (P<0.05).We compared the refraction at 3mo (-0.52±0.23D) and 9mo (-0.54±0.16D), the astigmatism coefficient at 3mo (-0.39±0.23D) and 9mo (-0.33±0.56D) after treatment, and we found that the differences were not statistically significant (P>0.05).The corneal endothelial cells at 3 and 9mo after operation were compared with those of before treatment, and we found that the differences were statistically significant (P<0.05).The intraocular pressure of 3 and 9mo after operation was compared with that before operation, and we found that the differences were not statistically significant (P>0.05).CONCLUSION: TICL implantation in the treatment of patients with high myopia and myopic astigmatism has a more obvious clinical efficacy and safety and reliability.
4.Interaction of Risk Factors for Esophageal Cancer in Dongping County

Objective To explore the risk factors of esophageal cancer and these factors' interaction Methods 472 new diagnosed cases of esophageal cancer in 2004—2005 and 472 controls were investigated by case-control study. Etiological fractions and interactions between family history of esophageal cancer and other risk factors of esophageal cancer were analyzed. Results The significant independent risk factors included taking hot-meal, preferring pickled-vegetables, preferring dry-rigidity-food, being irritable, drinking polluted-water perennially, family history of esophageal cancer and drinking alcohol a great deal. The etiologic fractions attributable to interaction were 0.855, 0.758, 0.683, 0.464, 0.459, 0.343 and 0.108 respectively. The synergy indexes of family history of esophageal cancer with taking hot-meal and with preferring dry-rigidity-food were both 0.86. Concludes The interactions between family history of esophageal cancer and ill life-behavior-style or drinking polluted-water perennially were particularly significant in the development of esophageal cancer in Dongping County.
5.Research advances on late intraocular lens dislocation after phacoemulsification

Late intraocular lens dislocation is one of the most severe late complications after phacoemulsification.It often occurs 3mo after the surgery.Different from early intraocular lens dislocation, late intraocular lens dislocation is caused by zonular dehiscence and contraction of the capsular bag many years after phacoemulsification.In recent years, the incidence of late intraocular lens dislocation gradually increases, having a risk of 0.1% after 10a and 1.7% after 25a.In the long-term follow-up patients who underwent cataract surgery, 90% had zonular insufficiency and capsular contraction.Among the multiple factors which may contribute to zonular weakness and capsular contraction, pseudoexfoliation is the most common cause, accounting for 50% of all the cases.Other risk factors include aging, high myopia, uveitis, trauma, previous vitreoretinal surgery, retinitis pigmentosa, diabetes mellitus, atopic dermatitis, previous acute angle-closure glaucoma attack, and connective tissue disorders.The understanding of these predisposing factors will suggest necessary preventions for high-risk patients in the future.

7.The Study on Synthesis on 20(S)-9-nitrocamptothecin

20(S)-9-nitrocamptothecin was synthesized from camptothecin by nitration reaction using mixed nitrate salts in sulfuric acid medium.The yield was raised from 31% to 40% after chromatographic purification treatment.The best reaction conditions was as follows:0.5 g of camptothecin;30ml of sulfuric acid;0.007mol of nitrate salts(NH4NO3/CaNO3=1/1).The reaction was completed in 24h at 20℃.The advantage of this method is high yield and easily purification.
8.Interventional therapy for pancreatic carcinoma

Target of the treatment for advanced pancreatic carcinoma is always palliative without prolongation of survival time almost, therefore the aims of treatment should be concentrated on improving the quality of life and alleviateing the disease-related symptoms. We do not call for the treatment and medicines that can incur obvious adverse reaction with limited effectiveness. Interventional therapy provides superexcellent short term efficacy with alleviating the disease-related symptoms significantly, and without severe side effects and thus coincides to the principle of the treatment for advanced pancreatic carcinoma. The surpassing advantages provide popular recommendation for this very kind of therapy in the recent years.
9. Resistin expression in adipose tissues of diet-induced obese mouse model and its influence on skeletal muscle glucose uptake

Objective: To investigate the expression of resistin (Retn) gene in the adipose tissues of obese mouse and its influence on the skeletal muscle glucose uptake, in an effort to understand the correlation between obese, resistin, and insulin resistance. Methods: The obese and insulin resistance model was induced with high-fat diet in C57BL/6J mice. Twenty-two weeks later the Lee's index (BMI) and the concentrations of blood glucose and plasma insulin were determined. Glucose tolerance test was carried out to verify the appearance of insulin resistance and the impair to glucose tolerance, with normal mice taken as control. Retn mRNA expression in the adipose tissues of model mice (n=10) and control mice (n=5) was detected by Real-time RT-PCR. The resultant resistin protein was co-cultured with mouse skeletal muscle to assess its influence on glucose uptake. Results: An obese and insulin resistance mouse model was successfully induced with high fat diet. The Retn gene expression in adipose tissues was significantly higher in the obese mice than that in normal control mice (P<0.01). The resultant resistin protein had a significant inhibitory effect on the glucose uptake by skeletal muscle with or without insulin (P<0. 05). Conclusion: It is suggested that the overexpression of Retn gene might be one of the reasons responsible for the decrease of glucose uptake by skeletal muscle and the subsequent insulin resistance in the diet-induced obese mice.
10. Influence of Retn gene expression on glucose uptake by 3T3-L1 cells and the related mechanism

Objective: To investigate the influence of high expression of Retn gene on glucose uptake by 3T3-L1 cells and to study its mechanism in inducing insulin resistance. Methods: (1) Radioimmunoassay was used to determine the glucose uptake by 3T3-L1 cells with low-, normal- and high-level Retn expression under basal and insulin-stimulated states. (2) RT-PCR and real-time RT-PCR were used to determine the mRNA levels of several glucose transport proteins in 3T3-L1 cells with different expression of Retn, including insulin receptor substrate-1(IRS-1), phosphatidylinositol 3-kinase (PI-3K), AKT-2, glucose transporter-4 (GLUT-4), p38 mitogen-activated protein kinase (p38MAPK), and glycogen synthase kinase-3β(GSK-3β). Results: The uptake of glucose decreased with the increase of Retn expression under basal and insulin-stimulated conditions. The mRNA expression of 2 signal protein PI-3K and AKT-2 decreased with the increase of Retn expression; and the expression of GSK-3β and p38MAPK increased with the increase of Retn expression. Conclusion: Resistin protein can induce insulin resistance in adipocytes, which might be related to the expression changes of some proteins in PI-3K and Ras pathways.
