Objective To analyze the effects of Ileus tube (IT) along with somatostatin (SS) used for treatment of patients with severe acute pancreatitis (SAP).Methods Under conventional treatment,75 patients with SAP were divided into three groups as per different additional treatments, namely group A (IT and SS),group B (nasogastric decompression tube and SS),and group C (IT alone),and the therapeutic efficacies of those treatments were evaluated in respects of improvement of physical signs and symptems, dynamic changes in decompression drainage and prognosis. Results Therapeutic efficacy was 100% in group A,84% in group B and 80% in group C.The difference between group A and B was x2 =8.26 (P <0.01) ; group B vs.C was x2 =0.38 (P > 0.05).The physical signs and symptoms in group A were improved more rapid than those in group B and C (P < 0.05),but there was no significant differences in those signs and symptoms between group B and group C (P > 0.05).In comparison of decompression drainage, the t value of group A vs.group B was 2.14,group B vs.group C was 3.83,and group A vs.group C was 2.23 (P < 0.05).As cure rate of patients with SAP in hospital on the 14th day, rate of transferring to surgical treatment as a last resort and mortality in group A were compared with group B and C,the differences were statistically significant (P < 0.05),while group B vs.C,the difference was not statistically significant (P > 0.05).Conclusions Application of IT combined with SS can significantly improve the condition of patients with SAP,thereby reducing the operation rate, shortening hospital stay, lowering mortality and improving the outcome, and it is worthy of clinical popularization.

Objective To investigate the inhibiting effects of shRNA(short hairpin RNA,shRNA) on COX-2 gene expression, the cell cycle and growth of gastric carcinoma SGC-7901 cells. Methods Specific shRNA plasmid to COX-2 were constructed, and then transfected into SGC-7901 cells by lipofectamine methods.Tests were divided into three groups: untransfected gastric carcinoma SGC-7901 cells group, negative control HK group and pshRNA-COX-2 group.Gastric carcinoma SGC-7901 cells were transfected with LipofectamineTM 2000.Expression of COX-2mRNA and protein were detected with reverse transcriptional polymerase chain reaction(RT-PCR) and Western blot respectively.Cell cycle analysis and cell growth chart were detected with flow cytometry and cell count respectively. Results Compared with negative control HK group, recombinant expression vector pshRNA-COX-2 resulted in the reduction of COX-2mRNA and protein expresion by 70.1% and 43.2% respectively;cells in G0-G1 phase increased from 61.5%to 70.2%,cells in S phase decreased from 27.3% to 21.7%,and the growth of SGC-7901 cells cells was slowed significantly. Conclusions Recombinant expression vector pshRNA-COX-2 can significantly inhibit the expression of COX-2 gene, result in the increase of cells in G0-G1 phase and decrease of cells in S phase, and suppress proliferation of gastric carcinoma SGC-7901 cells.

Objective To investigate the application effect of electromagnetic navigation bedside nasoenteral intubation technology in elderly bedridden patients. Methods A total of 92 elderly patients with tube feeding in bed in a tertiary A hospital in Beijing from June 2021 to December 2022 were selected by convenience sampling method, and they were divided into an experimental group and a control group with 46 patients in each group by random number table method. The experimental group adopted the electromagnetic navigation bedside nasoenteral catheterization technique and the control group adopted the blind catheterization method. The total success rate of catheterization, the success rate of the first catheterization, the time taken for catheterization, the proportion of catheter tip reaching jejunum, and the incidence of catheter-related complications were compared between the 2 groups. Results There were no shedding cases in both groups. The success rate of total catheterization was 95.7%in the experimental group and 80.4%in the control group. The success rate of first catheterization was 95.7%in the experimental group and 71.7%in the control group. The time taken for catheterization was(12.37±4.19)min in the experimental group and(19.22±5.48)min in the control group. The proportion of catheter tip reaching the jejunum was 60.9%in the experimental group and 28.3%in the control group. The above data were compared between the 2 groups, and the differences were statistically significant(P<0.05).In terms of the incidence of catheter-related complications, the incidence of epistaxis in the experimental group was 6.5%and it was 21.7%in the control group, and the difference was statistically significant(P=0.036).The positive rate of fecal occult blood test was 2.2%in the experimental group and 17.4%in the control group, and the difference was statistically significant(P=0.030).There was no significant difference in the incidence of nausea, vomiting and abdominal distension between the 2 groups(P>0.05).No serious complications such as catheter ectopic placement, pneumothorax and perforation occurred in the 2 groups. Conclusion The application of electromagnetic navigation bedside nasoenteral intubation technology for elderly bedridden patients has a high success rate and takes a short time, which can improve the proportion of catheter tip reaching the jejunum and reduce the incidence of epistaxis and fecal occult blood test.
