1.Therapeutic effect of trimetazidine on patients with ischemic cardiomyopathy heart failure and significance of plasma level of N-terminal pro-B type natriuretic peptide

Objective To observe the therapeutic effect of trimetazidine on patients with ischemic cardiomyopathy heart failure and the changes of plasma level of N-terminal pro-B type natriuretic peptide(NT-proBNP).Methods 62 patients with ischemic cardiomyopathy heart failure were randomly assigned to regular treatment group(3 1)and trimetazidine treatment group(31).All of them were given either regular medicine treatment or trimetazidine for 4 weeks.Concentration of NT-proBNP and echocardiography were detected before and after treatment and the correlation between NT-pmBNP and ultra-sound values was studied.Results There was no statistic difference in the NT-proBNP level between two groups before treatment,but after 4-week treatment the NT-proBNP level in both groups were deCreased(P<0.05),the decline in the trimetazidine group was more significant.The NT-proBNP level was significantly correlated to LVEF and LVEDD(γ=-0.472、0.45 1;P<0.05).Conclusions Trimetazidine has a positive effect on patients with ischemic cardiomyopathy heart failure,and decreases the plasma NT-proBNP level.NT-proBNP Can be used as a follow-up index.
2.Relationship between placental growth factor and acute coronary syndrome

Objecitve To investigate the relationship between placental growth factor and acute coronary syndrome(ACS). Methods 70 cases of patients with acute chest pain were enrolled. 36 cases of patients who is posi-tive on cardiac troponin T(cTnT) as treatment group, 24 patients who is negative on cTnT as control group, blood of all patients were detected about PLGF and cTnT in hospital,and coromary angiography examination, Gemini score is used to evaluate coronary artery lesion. Result PLGF and cTnT were elevated in ACS, it had significant difference between two groups(P <0. 01) ,PLGF and cTnT are significantly positive correlated(r= 1.2,P <0. 01), PLGF and cornorary artery lesion severity are significantly positive correlated(r = 1 ,P <0. 01). Conclusion PLGF is helpful to predict ACS attack to comorary heart disease,higher PLGF predicts unstable to coronary artery lesion in coronary heart disease.
3.Therapeutic effect of combined medication of amlodipine and captopril on patients with hypertension complicated with coronary heart disease

Objectlve To study the therapeutic efficacy of the combined medication of ShiHuiDa and capto pril on patients with by Opertension and coronary heart disease.Methods 80 patients witll hypertension complicated with coronary disease were included in this study.They were randomly dirided in to treatment group and control group,every group is forty patients.Treatment group treated by ShiHuiDa and captopril,control group treated by hydrocblorothiazide nitrendipine and dinitrosorbide.Blood pressure angina pectoris relief and electrocardlogram were observed after eight weeks therapy.Results Systolic pressure and diastolic pressure in treatment group is less than control group,it is obviously better than control group about depressurization effect,it has significant difference by statistical test.Angina pectoris relief rate and electrocardiogram improve rate in treatment group is better than control group,it has significant difference.Conclusion The combined medication of ShiHuiDa and captopril is not only effective in lowering blood pressure and stronger and persistence,but also in relieving angna pectoris attack.
4.Radiofrequency catheter ablation in frequently single premature ventricular beats

Objective To discuss curative effect about radio frequently catheter ablation(RFCA) in frequently single originating premature ventricular beats.Methods 19 patients with frequently single original premature ventricular beats.The site of orgin was determined by activation mapping for the earliest endocardial activation,and by pace mapping for the exact QRS pacing site.Results 18 patients operation obtained immediate success,1 patient was failure,but no complication happened.After RFA in 2 weeks 24 hour Hoter ECG was used to examine arrhythmia condition,and all cases were follow up for 6~24 months after RFA,there were 17 patients who had no recurrence or phenomenon of premature beats augument,1 patient showed other type premature ventricular bearts.Conclusion It is safe and effective about RFA in frequently single origin premature ventricular beats of outflow lane.
5.Superiority of thread-dragging therapy in treatment of refractory sinus tracts or fistulae

The thread-dragging therapy, as a new external treatment of traditional Chinese medicine, could eliminate pathogenic factors and restore the vital qi by promoting qi and activating blood circulation to advance tissue repair in treating refractory sinus tracts or fistulae. This article reviewed its origin and development, and introduced its use in treating refractory sinus tracts or fistulae.
6.Influence of Metformin on Metabolic Syndrome in Patients With Coronary Artery Disease

Objective To investigate the influence of metformin on metabolic syndrome in patients with coronary artery disease.Methods 52 patients with coronary artery disease and metabolic syndrome were randomly assigned into 2 groups.the control group with 20 cases and the study group with 32 cases.All the patients were treated with standard coronary artery disease therapy and meanwhile the study group was administered with metformin in addition to the basic therapy above.The follow-up time is one year.FPG,2h postprandial glucose,FINS,TG,TC,HDL-C,insulin resistance index and other standards were compared before and after the therapy,and between the 2 groups when the observation was finished.The rate of cardiovascular events was also compared between the 2 groups.Results Compared with the control group,the study group had lower levels of FPG,2h postprandial glucose,FINS,TG and insulin resistance index when the therapy was completed(P<0.05),and the rate of cardiovascular events was also lower(P<0.05).Conclusion Metformin could lower the levels of glucose,FINS,BMI,TG and insulin resistance index in patients with coronary artery disease and metabolic syndrome.Thus,it could reduce and undermine cardlovascular risk factors,protect the cardiovascular system,reduce the rate of cardiovascular events,which fully benefits the patients.
8.Discussion of relationship between quality of life and clinical effect assessment of malignant tumor treated with traditional Chinese medicine

Traditional Chinese Medicine (TCM) therapy is effective in treating malignant tumor. TCM therapy is a multi-way, multi-layer and multi-target integrated treatment characterized by "survival with tumor" and improvement of life quality. An ideal result of the TCM therapy for cancer should be a positive response of the patient with satisfactory quality of life (QOL) and longer survival time. So it is necessary to introduce the QOL, quantification of symptoms and signs and standardization of TCM syndromes to the objective, quantitative and standardized effect assessment of TCM therapy for cancer. A criterion for evaluating the effect of TCM therapy on tumors should be established, which reflects the characteristics and advantages of the TCM therapy, and meets the requirement of the developing effect assessment of modern medicine in cancer treatment. It is suggested that the treatment effect should be the sum of quality adjusted life years (QALYs) (equal to QOL multiplied by time of survival), syndrome remission rate and tumor remission rate. But the proportion of each of the three is variable, which has close relationships with the different tumor stages and corresponding treatments.
