1.Study on HLA-DRB1 shared epitope in patients with IgA mesangial proliferative glomerulonephritis

Objective:To explore the clinical correlation of HLA-DRB1 shared epitope with IgA mesangial proliferative glomer-ulonephritis( IgA MsPGN).Methods:The renal function were examined routinely,and HLA-DRB1 shared epitope were determined by PCR in a total of 164 patients with IgA MsPGN and 164 healthy subjects.The renal function and HLA-DRB1 shared epitope were compared in patients with IgA MsPGN and healthy subjects,and clinical correlation of HLA-DRB1 shared epitope and renal function were analyzed.Results:(1)24 h proteinuria,serum creatinine and serum urea nitrogen were significantly different in patients with IgA MsPGN and healthy subjects(P<0.01);(2)The high-frequency gene of HLA-DRB1 in patients with IgA MsPGN were DRB1*04, DRB1*07,DRB1*09,DRB1*11,DRB1*14 and DRB1*15;(3) The distribution frequency of DRB1*09 and DRB1*11 in patients with IgA MsPGN increased significantly(P<0.01 or P<0.05);(4)24 h proteinuria,serum creatinine and serum urea nitrogen in patients with IgA MsPGN with DRB1*09 and DRB1*11 were significantly different compared with those in patients with IgA MsPGN with other HLA-DRB1 shared epitope(P<0.01).Conclusion:HLA-DRB1 shared epitope is related to IgA MsPGN,DRB1*09 and DRB1*11 can increase the risk of IgA MsPGN,and related to the severity of glomerulonephritis.
3. Effects of transfection of hGM-CSF gene into HepG2 cells mediated by hydroxyapatite nanoparticles on the growth of HepG2 cells

Objective: To observe the impact of transfection of human granulocyte macrophage colony stimulating factor (hGMCSF) mediated by hydroxyapatite(HA) nanoparticles on the growth of HepG2 cells in order to lay the foundation for further studying gene-modified tumor vaccines of hepatoma in clinic. Methods: The proliferation of HepG2 cells was measured by MTT assay. The plasmids containing hGM-CSF gene were transfected into HepG2 cells mediated by HA nanoparticles. Subsequently, resistant cells were selected by G418 screening. The monoclonal cells were selected by limiting dilution method. RT-PCR was used to identify the integration of hGM-CSF into HepG2 cells and transcription of hGM-CSF. ELISA was performed to detect the level of secreted hGM-CSF in cultured medium and the lasting time. The effect of hGM-CSF on cell cycle and apoptosis were assessed by flow cytometry (FCM) analysis. Results: MTT assay demonstrated that Nano-HA suspension liquid had no significant effects on the growth of HepG2 cells at the concentration below 80 μg/mL. RT-PCR demonstrated the hGM-CSF gene was successfully integrated and steadily expressed in the transfected HepG2 cells. ELISA indicated stable secretion of hGM-CSF from the stable transfected HepG2 cells [mean (216.22 ± 45.78) ng/106 cells per 24 h]. FCM analysis showed that hGM-CSF had no significant effects on the cell cycle and apoptosis of HepG2 cells. Conclusion: hGM-CSF gene could be safely transfected and stably expressed in HepG2 cells mediated by HA nanoparticles. Transfection of hGM-CSF gene mediated by HA nanoparticles has no effects on the growth of HepG2 cells. This study may lay a foundation for the further study of gene-modified hepatoma vaccines.
4.Expression of renal integrin-linked kinase and lesion of podocyte in a diet-induced hyperlipidemic model of rats

Objective To investigate the lesion of podocyte and the expression of renal integrin-hnked kinase (ILK) in a diet-induced hyperlipidemic model of rats. Methods Thirty-six 6-8 week-old female Wistar rats were randomly assigned into three groups, high-fat diet group, rats in each group were sacrificed at the 4th and 10th week respectively. The levels of serum cholesterol and triglyceride were determined by enzymic method. The morphology of podocyte was observed and photographed with electron microscope. The expression of ILK mRNA was determined by RT-PCR. The expressions of ILK and desmin protein were determined by Western blot analysis. The distribution of ILK in renal tissue was examined by immunohistochemieal staining. Results Levels of serum cholesterol and triglyceride, the expression of desmin, renal ILK mRNA and protein, as well as the foot process effacement were significantly up-regulated in both high-fat diet group and simvastatin group as compared with control group. However, all of the above parameters were ameliorated in simvastatin group as compared with high-fat diet group (P<0.01). ILK was mainly expressed in glomendar podocytes and renal tubular cells by immunohistochemical staining, and its change was similar to the results detected by Western blot analysis in each group. A positive correlation was found between ILK protein expression and desmin expression in renal tissue(r=0.93107, R2=0.8669, P<0.01). Conclusions Podocyte lesion can be induced by high-fat diet, which is correlated with over-expression of renal ILK. Simvastatin may play an important role in protecting against podocyte injury induced by hyperlipoidemia, properly through down-regulating ILK expression in renal tissue.
5.Assessment of quality of life and its influence factors in maintenance hemodialysis patients

Objective To investigate the quality of life (QOL) of maintenance hemodialysis (MHD) patients and its influencing factors.Methods A total of 257 MHD patients in our hospital were recruited in this study.Clinical data of the patients were collected,and the QOL was assessed by MOS 36 item short form health survey(SF-36).Nutritional status of patients was evaluated by modified quantitative subjective global assessment (MQSGA).Univariate analysis of variance,pearson correlation analysis and multiple linear stepwise regression analysis were performed to determine the effect of related factors on QOL scores.Results The scores of all scales of SF-36 evaluation in MHD patients were relatively lower than that of general population as reported before.Their physiological component summary (PCS) score decreased gradually as age grew,nevertheless,the mental component summary (MCS) score was highest in the group aged 41-60.The score was lower in patients with moderate to severe malnutrition or diabetic nephropathy when compared with other patients.Univariate analysis of variance also revealed that high SF-36 scores associated with higher education or income.Multivariate analysis indicated that PCS score and total SF-36 score of MHD patients were positively correlated with body mass index (BMI) and cholesterol,but negatively correlated with diabetic nephropathy,pulmonary artery systolic pressure and MQSGA score (all P < 0.05).There was positive correlation between MCS score and income,yet negative correlation between MCS score and MQSGA score (all P < 0.05).Conclusion MHD patients had relatively poor QOL.Primary diseases and nutritional status were probably the main influencing factors.Age,educated level,family income and pulmonary artery systolic pressure might also have effects on their QOL.
6.Effects of 1,25-dihydroxyvitamin D3 on the expression of urinary nephrin in diabetic nephropathy patients

Objective To investigate the effects of 1 ,25-dihydroxyvitamin D3 on the expression of urinary Nephrin in patients with diabetic nephropathy .Methods According to albumin excretion rate ,40 type-2 diabetic nephropathy patients were divided into 2 groups ,10 healthy volunteers served as control .The treatment groups received 1 ,25-dihydroxyvitamin D3 0 .25 μg/d for 12 weeks .The levels of blood calcium ,phosphorus ,serum creatinine ,24-hour urinary protein excretion were detected .Enzyme-linked immunosorbent assay was used to detect urinary Nephrin in different groups .Results The expressions of urinary Nephrin were sta-tistically different among NC group ,DN1 group and DN2 group .1 ,25-dihydroxyvitamin D3 treatment significantly decreased the ex-pression of urinary Nephrin in DN1 patients ,whereas no statistically changes were observed in DN2 group .Conclusion In diabetic nephropathy patients ,urinary Nephrin is supposed to be effective an marker of disease progression .1 ,25-dihydroxyvitamin D3 may be effective to prevent podocyte injury in early-stage diabetic nephropathy .
7.The relationship between epithelial-mesenchymal transition and clinic pathology of IgA nephropathy

Objective To analyze the correlation between the epithelial‐mesenchymal transition and the clinicopathologic features of IgA nephropathy .Methods A total of 168 patients diagnosed as IgA nephropathy by renal biopsy in Xinqiao hospital from Janu‐ary 2011 to December 2013 were divided into high expression of dual‐positive Snail and a‐SMA group (Group A ,117 cases) and low expression of dual‐positive Snail and a‐SMA group (Group B ,51 cases) according to results of immunohistochemical method .The clinical parameters (age ,gender ,course of disease ,BMI and chemical indicators) and renal pathology grade were compared by statis‐tical analysis .Results There were difference between group A and group B in the course of disease and BMI(P<0 .05) .There were differences between group A and group B in the incidences of creatinine ,blood urea nitrogen ,serum triglycerides and 24‐hour urine protein amount (P<0 .05) .The percentage of Lee′s grade Ⅰ ,Ⅱ ,Ⅲ in group B was significant ,while percentage of Lee′s gradeⅣ + Ⅴin group A was significant .The expression of Snail and a‐SMA in grade Ⅳ + Ⅴ was more than that in grade Ⅰ + Ⅱ (40 .2%vs .9 .4% ) ,and the difference between two groups was statistically significant (P< 0 .05) .Conclusion The expression of Snail and‐SMA were related with 24‐hour urine protein amount and kidney function in IgA nephropathy ;and Lee′s grade was severe in patients with high expression of Snail and‐SMA .
8.Continuous veno-venous hemofiltration in treatment of acute respiratory distress syndrome

Objective In this study,we cooperated CVVH(continuous veno-venous hemofiltration)in treatment of Acute Respiratory Distress Syndrome(ARDS)and investigated the efficacy of CVVH in treatment of ARDS.Methods From May 2002 to Nov.2005,76 ARDS patients were involved.CVVH was carried out continuously for at least 24 hours.During CVVH,the patients' conditions were observed.Blood gas analysis and serum concentrations of endotoxin were detected.Results After CVVH,the patients' symptoms such as panting and cyanosis were remission.APACHE Ⅱscore decreased significantly.After six hours of CVVH,serum concentration of endotoxin was decreased significantly.Conclusion CVVH is effective in redressing the hypoxemia and relieving the patients' condition.
9.The expression of TLR4 on peripheral blood monocytes from uremic patients with diabetic nephropathy and its relation with plasma MCP-1 concentration

Objective:To explore the expression of TLR4 on peripheral blood monocytes in uremic patients with diabetic nephropathy and the relation between TLR4 and diabetic nephropathy.Methods:RT-PCR analysis and flow cytometry were undertaken to measure the expression of TLR4 mRNA and protein.The plasma concentration of MCP-1 was measured with ELISA.Results:The gene and protein expression of TLR4 on peripheral blood monocytes in patients with diabetic nephropathy was significantly increased compared with that of the health control,and TLR4 expression was positively related with the the plasma concentration of MCP-1.Conclusion:TLR4 expression is upregulated on peripheral blood monocytes in patients with diabetic nephropathy and the upregulation is related with the pathogenesis of diabetic nephropathy.
