CONCLUSION: The results show the TCM drug SWS is effective and softy on treating the patients with primary hyperlipemia.
2.Analysis on drug treatment protocols of out-patients with hypertension

The antihypertensive treatment protocols of 4102 patients from the database of Changbai Community Health Service Center of Shanghai Yangpu District were analyzed.Patients with irregular drug treatment were excluded in the study.In 4102 hypertensive patients,45.6% took calcium channel blockers (CCB),16.0% took angiotensin converting enzyme inhibitors ( ACEI),3.6% took AT1-receptor blocker (ARB),9.0% took β-adrenergic receptor blockers,4.7% took diuretics and 21.1% took compound preparation.68% patients were in single-drug protocol,29.3% in two-drug,2.6% in three-drug and 0.1% in four-drug.In patients with two-drug protocol,75.8% was based on CCB; three-drug or four-drug protocols were also based on the combination of CCB.The results indicate that the treatment protocol for hypertensive patients in our community health service center basically meets the requirements of International Society of Hypertension Treatment Guidelines,but the proportion of the single drug use seems to be high,more combination of antihypertensive drug use should by proposed.

4.Insight Into Folding,Binding and Stability of Insulin by NMR

Insulin is one of the most important hormonal regulators of metabolism. Since the diabetes patients increase dramatically, the chemical properties, biological and physiological effects of insulin had been extensively studied. In last decade the development of NMR technique allowed us to determine the solution structures of insulin and its variety mutants in various conditions, so that the knowledge of folding, binding and stability of insulin in solution have been largely increased. The solution structure of insulin monomers is essentially identical to those of insulin monomers within the dimer and hexamer as determined by X-ray diffraction. The studies of insulin mutants at the putative residues for receptor binding explored the possible conformational change and fitting between insulin and its receptor. The systematical studies of disulfide paring coupled insulin folding intermediates revealed that in spite of the conformational variety of the intermediates, one structural feature is always remained: a "native-like B chain super-secondary structure", which consists of B9-B19 helix with adjoining B23-B26 segment folded back against the central segment of B chain, an internal cystine A20-B19 disulfide bridge and a short α-helix at C-terminal of A chain linked. The "super-secondary structure" might be the "folding nucleus" in insulin folding mechanism. Cystine A20-B19 is the most important one among three disulfides to stabilize the nascent polypeptide in early stage of the folding. The NMR structure of C.elegans insulin-like peptide resembles that of human insulin and the peptide interacts with human insulin receptor. Other members of insulin super-family adopt the "insulin fold" mostly. The structural study of insulin-insulin receptor complex, that of C.elegans and other invertebrate insulin-like peptide, insulin fibril study and protein disulfide isomerase (PDI) assistant proinsulin folding study will be new topics in future to get insight into folding, binding, stability, evolution and fibrillation of insulin in detail.

7.The clinical observation on 52 cholesteatoma otitis media patients by the surgery of mastoid muscle periosteum valvuloplasty after modified radical mastoidectomy

Objective To investigate the clinical effects of the surgery of mastoid muscle periosteum valvuloplasty after modified radical mastoidectomy treatment of cholesteatoma otitis media.Methods 52 cases (52 ears)cholesteatoma otitis media patients for the observation group were collected.The patients in the observation group were treated by mastoid muscle periosteum valvuloplasty after modified radical mastoidectomy.49 cases (49 ears) for the control group were treated by concomitant mastoidectomy and the classic conchaplasty cavity angioplasty.Results Patients were followed up for 1 year.38 ears were dry in control group(77.6%),8 ears were recurrenced(16.3%),regular clean-up of surgical cavity at least 3 months postoperative.50 ears were dry in observation group (95.2%),dry ear rate was significantly higher (x2 =12.374,P =0.000) ;1 ears was recurrenced(2.4%),the recurrence rate was significantly reduced (x2 =11.966,P =0.000) ; Operative cavity after complete epithelialization no longer need to be cleaned regularly; No postoperative complications (such as periosteal flap or pinna cartilage of the mastoid muscle necrosis,facial paralysis,dizziness)were occurred,the prognosis was good.Conclusion The surgery of mastoid muscle periosteum valvuloplasty after modified radical mastoidectomy treatment of otitis media with cholesteatoma is safe and effective,which is worthy of promotion.

The lack of pulmonary surfactant(PS) and the immature of lung structure can cause neonatal respiratory distress syndrome (NRDS).Pulmonary surfactant,noninvasive ventilation,such as nasal continuous positive airway pressure(nCPAP) and the InSurE technique(intubation-surfactant-extubation) are the important methods for the management of NRDS.In this paper,the application and the research progress of the three strategies for NRDS,especially the clinical advantages and progress of InSurE technique,are summarized as follows.
9.Fetal inflammatory response syndrome

The fetal inflammatory response syndrome (FIRS) is a state of activation of the innate immune system in the fetus body and is a kind of neonatal inflammation for 72 hours after birth.It is defined as the following items > 2:(1) breathing too fast (> 60 times/min),accompanied by breathing difficulties or with oxygen desaturation; (2) temperature instability > 37.9 ℃ or < 36 ℃ ; (3) capillary filling time > 3 s;(4) the white blood cell count >34 × 109/L,or <4 × 109/L; (5) CRP > 10 mg/L; (6) IL-6 or IL-8 >70 g/ml;(7) gene testing of 16S rRNA for PCR is positive.FIRS can be induced by infectious and non-infectious factors.In FIRS,fetal immune system is over-activated to the external violation,which causes uncontrolled release of inflammatory mediators and cytokines.A variety of inflammatory mediators and cytokines directly or indirectly activate the coagulation system and interfere with the body's anticoagulation system,which induces the coagulation disorders.Multiple organ systems involved in the inflammatory response throughout this process.FIRS can lead to premature delivery,perinatal death,cerebral white matter damage,necrotizing enterocolitis,and affect fetal lung maturity and multi-organ damage.In order to reduce fetal injury,it is necessary for appropriate treatment and prediction of the FIRS.
10.Analysis of clinical features of 426 patients with acute myocardial infarction

Objective To analyze clinical characteristics, hospital treatment, complications and mortality differences of the patients with acute myocardial infarction (AMI) by conventional treatment. Methods Retrospective analysis of 426 cases of AMI patients, aged 20-103 years [mean age(67. 82 ±0. 56) years] ,male 261 cases,female 165 cases, the general condition of patients were recorded, history features, diagnosis, treatment, complications and prognosis. Comparison of elderly(≥70 years) and non-elderly(<70 years) ,male and female patients with clinical features, treatment, complications and mortality differences. Results Compared with the non-elderly patients with AMI,elderly patients had significantly lower smoking history(P<0.05) ,female ratio,history of old myocardial infarction rate, cardiac function≥Killip Ⅲ level was significantly higher(all P<0.05). During hospitalization in patients of different ages diuretics,β blockers, digitalis, clopidogrel and other drugs, application of a significant difference (all P<0.05). Elderly AMI patients with complete left bundle branch block,atrioventricular block,atrial fibrillation,the incidence was higher than non-elderly patients (all P<0.05) ,but no difference in mortality between the two groups (P>0.05). Female patients than male patients with age of onset was significantly (P<0.05) , and the smoking rate is low,but clearly the high rate of angina died(P<0.05) ,sub-poor cardiac function after myocardial infarction(P<0.05). Application of cardiac drugs in addition to nutrition than men,female patients, the rate of other conventional therapy,no difference in both complications and prognosis(all P>0.05). Conclusion AMI was the major diseases threatening human life,different age had its own clinical characteristics of patients,and it could not be impacted by gender.
