1.Comparation of clinical value between subcutaneous insulin pump and multiple subcutaneous insulin intensive treatment on type 2 diabetes combined with stroke

Objective To explore the clinical value of the insulin pump(CSⅡ) and multiple subcutaneous insulin intensive treat-ment(MDI) for patients with type 2 diabetes and stroke .Methods A total of 80 patients with type 2 diabetes and stroke in Weifang Community Health Service Center from April 2011 to May 2014 were selected as subjects and divided into experimental group and control group according to random number table method ,with 40 cases in each group .Patients in the experimental group were trea-ted with CSⅡ ,patients in the control group were treated with MDI .Glucose level ,therapeutic time ,the number of low blood sugar , insulin dosage ,National Institute of Health Stroke Scale(NIHSS) score ,insulin secretion index(IS) ,insulin resistance index(IR) , hospital costs were compared before and after treatment in the two groups .Results The glucose levels ,therapeutic time ,the num-ber of low blood sugar ,insulin dosage ,NIHSS score ,IS ,IR in the two groups before and after breakfast ,before and after lunch ,be-fore dinner ,after dinner 2 hours before treatment had no significant differences(P>0 .05) ,the glucose levels ,therapeutic time ,the number of low blood sugar ,insulin dosage ,NIHSS score ,IS ,IR after treatment in both groups were significantly decreased at these time points ,but those indicators in the experimental group decreased more significantly ,and had significant differences with those of the control group(P<0 .05) .The cost of hospitalization of patients in the experimental group was significant lower than that in the control group(P<0 .05) .Conclusion CSⅡ treatment for type 2 diabetes and stroke patients ,not only control glucose level well ,re-duce the number of low blood glucose and insulin dosage ,significantly improved IS and IR ,reduce hospital costs ,and also could sig-nificantly improve the neurological disorders of consciousness ,which is an ideal treatment method compared with MDI .
2.Clinical analysis of pulmonary tuberculosis combined with Stenotrophomonas maltophilia infection

Objective To explore the characteristics and drug resistance of pulmonary tuberculosis combined with Stenotrophomonas maltophilia infection.Methods The clinical data of 65 patients with Stenotrophomonas maltophilia infection were analyzed retrospectively.Results It showed that 65 strains of Stenotrophomonas maltophilia mainly came from sputum sample [75.4%(49/65)] and were highly resistant to many kinds of antimicrobial agents,of which the resistance rate to carbapenems,β-lactames was above 60% and the sensitive rate to moxifloxacin,ethoprin-sulfamethoxazole and ticarcillin/clavulanic was above 70%.Conclusion Clinical practices should pay more attention to the high resistance of Stenotrophomonas maltophilia infection,which might result in pulmonary tuberculosis,and prescribe antibiotics rationally according to the drug sensitive test.
3.Aplication effect of fiber bronchoscope examination in the treatment of respiratory internal medicine emer-gency and its effect on the quality of life of patients

Objective To study the aplication effect of fiber bronchoscope examination in the treatment of respiratory internal medicine emergency and its effect on the quality of life of patients.Methods 156 patients with emergency were randomly divided into the observation group and control group,each group in 78 cases.The control group was given routine treatment plan according to the primary disease type which was the respiratory treatment of the corresponding diseases,the observation group in the control group was added fiberoptic bronchoscopy based on the treatment.Compared blood clinical curative effect,heart rate and quality of life score changes of the two groups of patients after treatment.Results The total effective rate of the observation group was 94.87%,which was significantly higher than 81.01% of the control group(χ2 =6.28,P <0.05).The pH,PaO2 and SaO2 in the observation group after treatment were(7.35 ±0.04),(73.23 ±9.11)mmHg and(92.33 ±4.32)%,which were significantly higher than the corresponding index in the control group(t =3.97,6.08,4.96,all P <0.05);while PaCO2 and HR were (51.53 ±9.24)mmHg,(91.69 ±12.42)times/min,which were significantly lower than the corresponding index in the control group(t =6.73,5.13,all P <0.05);The quality of life score in the observation group was(54.04 ± 10.94)points,which was significantly lower than(77.23 ±12.32)points of the control group(t =5.86,P <0.05). Conclusion Fiberoptic bronchoscopy for respiratory internal medicine emergency treatment can significantly improve the clinical curative effect of treating disease and patient quality of life.
4.Dynamical measurement system of respirator based on data collection card

5.Epidemiological Survey on Echinococcus Infection in Animals in Qinghai Province

Parasitological and dissection methods were used to confirm Echinococcus infection and echinococcosis in animals,and to understand the epidemiological situation in final and intermediate animal hosts in Qinghai Province. Three Echinococcus species were found in the Province including E. granulosus,E. multilocularis and E. shiquicus. The prevalence of hydatid disease showed a tendency of increase with an average rate of over 50% in yaks and sheep. Higher prevalence in wild animals as intermediate host was found in the southern plateau,while that in final hosts such as dogs and wolves,the prevalence was higher in the area of Qilian mountain. There is a complex chain of life cycle of the parasites among animals,which poses a threat to the health of human population. Effective measures to control the disease in animals are urgently needed.
6.Chronic Severe-degree Hepatitis B Complicated by Aspergillosis:Analysis of Clinical Characteristics

OBJECTIVE To evaluate the clinical characteristics of chronic severe-degree hepatitis B complicated by aspergillosis and its susceptive factors and prevention and treatment.METHODS The clinical data of 3 patients with chronic severe-degree hepatitis B complicated by aspergillosis were analyzed retrospectively.RESULTS From 23 patients with chronic severe-degree hepatitis B complicated by fungus infection,there were 3 patients complicated by aspergillosis and died.The susceptive factors included poor immunity,incorrect use of antibiotis and invasive operation.CONCLUSIONS Patients with chronic severe-degree hepatitis B complicated by aspergillosis have poor progress and prognosis. The effective preventive methods are treating underlying disease actively,using antibiotis correctly,reducing or avoiding invasive operation and disinfecting air regularly.
7.Experience on Purchase of Medical Equipment in Grass Root Hospitals

Advanced hospitals and high-level hospitals are different from grass-root hospitals,since they not only treat difficult diseases but also have the obligation of scientific studies and teaching.Grass-root hospitals mainly focus on treating common diseases,and only conduct a small amount of teaching,thus resulting in the difference between grass-root hospitals and large-scale hospitals in the aspect of purchasing medical equipment.Because the money for grass-root hospitals may only be used as designated and some even comes from loan or collecting,we have to pinch pennies and make the best use of everything so as to recover the cost rapidly and bring the equipment into full play.

AIM:To optimize the formulation of Fuyankang Dispersible Tablets and study the in vitro release characteristics of them. METHODS: The orthogonal design was used to obtain the optimal formulation with the disintegration,hardness and weight of tablets as markers,and then dispersible uniform and the in vitro release characteristics of the optimal formulation were studied with dispersed experiment and PR-HPLC,respectively.(RESULTS:)The proportion of each adjuvant in the optimal formulation consisted of 10% MCC,4% L-HPC,12% PVPP.The optimized dispersible tablets disintegrated in 1 min,the hardness was 5.24 kg,the average weight was 0.407 g;the dispersible uniform was excellent,and the settling velocity equation was logF=-0.028-1.001?10~(-3)t;The released rate parameters of Fuyankang Dispersible Tablets were T_(50)=3.0 min and T_d=4.0 min,which were remarkly less than that of the control group,T_(50)=14.28 min and T_d=16.62 min(P
9.Effect of Qianlieantong Tablets on NE-induced Contraction of Rabbit Trigone Muscle

Objective To observe spasmolytic effect of Qianlieantong tablets. Methods Twelve rabbits were randomly divided into 2 groups. One group was to prepare ring muscles, another was to prepare longitudinal muscles. Contraction of trigone muscle was induced by NE. Effects of 6 groups drug were observed, which were high, middle and low concentration Qianlieantong group, Hytrine group, blank group, Tween 80 group (TW-80). Results Qianlieantong tablets can significantly inhibit contraction of NE-inducted rabbit trigone muscle (including ring and longitudinal muscles) in vitro, and the inhibition was positively correlated with concentration, which was less than Hytrine. Conclusions Qianlieantong tablets had spasmolytic effect of alleviating BPH-induced badder outlet obstruction and reducing dynamic factors caused by BPH.
10.Selective heterologous communication among human lung epithelial cells, fibroblasts, and lung cancer PG cells

Objective: To further understand the roles of gap junctional intercellular communication in carcinogenesis and progression of human lung cancer. Methods: The heterologous communication was characterized among human lung epithelial cells HLEC, human lung fibroblasts HLF, human lung giant carcinoma PG cells and its Cx43 transfectants PG/C4 by using a preloading assay. Cx43 immunofluorescent staining and Northern blot were performed to examine Cx43 gene expression. Results: Although both human lung firoblasts HLF and epithelial cells HLEC expressed Cx43 and had very strong homologous communication, HLEC cells were unable to communicate with HLF cells. The human lung carcinoma cell line PG was defective of both homologous communication, and heterologous communication with its epithelial origin HLEC. Transfection of Cx43 into PG cells, which rescued PG cells' homologous communication and enhanced heterologous communication with HLF cells, could restore heterologous coupling with HLEC cells. Conclusion: Selective heterologous communication exists among different kinds of human lung cells. when human lung carcinoma cells lost coupling with its epithelial origin, Cx43 gene expression is not enough to establish heterologous communication between them.
