1.Role of myosin regulatory light chain phosphorylation in the pathogenesis of chronic heart failure

In the embryonic period, myosin regulatory light chain (MYL2) plays a pivotal role in the development and function of the heart. A large number of studies have previously confirmed that the mutation of MYL2 gene, also known as MLC2, confers intimate associations with hypertrophic cardiomyopathy. MYL2 gene mutation impacts the structure and function of myosin, thereby leading to the occurrence and progression of hypertrophic and dilated cardiomyopathy as well as the following chronic heart failure. Importantly, MYL2 phosphorylation renders crucial effects in the processes of cardiac contraction, ventricular torsion and cardiac function.
2.Treatment of osteochondritis lesion in the knee with reduction and fixation under arthroscopy.

OBJECTIVETo study clinical results of reduction and fixation under arthroscopy for the treatment of osteochondrotical lesion in the knee.
METHODSFrom December 2012 to December 2013, 4 patients with the knee joint osteochondritis lesion and free bone fragments were treated with arthroscopy to detect the stripped cartilage, and then the cartilages were reduced with small incision and fixed by cartilage nail. There were 3 males and 1 female, with age of 15, 15, 20 and 27 years old. The durations of the disease were 1 d, 5 d, 1 month and 1 year. All the patients had swelling and effusion in the knee joint, and the floating patellar test was positive. CT examination showed bone defects and loose bodies. The Lysholm, VAS, and Tegner scale were used to evaluate the knee joint functions before and after operation.
RESULTSAll the patients underwent successful surgery. Postoperative CT showed good location of the cartilage without loss of reduced bone and cartilage. All the patients were followed up, and the duration ranged from 10 to 13 months, with a mean of 12 months. In one patient, arthroscopy examination was conducted for a second time to examine the connection of the fracture part to the surrounding cartilage, and it showed that the internal fixator was not absorbed. The Lysholm, VAS and Tegner scale of all patients were better than those before operation.
CONCLUSIONTreatment of osteochondritis lesion with the fixation of absorbable cartilage nails may reconstruct the integrity of articular surface and recover the stability of joints, and it is an effective treatment method.

3.Modified dacrocystorhinostomy with lacrimal duct intubation for chronic dacryocystitis and its complications

AlM: To discuss the efficacy of modified dacrocystorhinostomy with intubation in the treatment of chronic dacryocystitis combined with lacrimal duct narrow or obstruction.
METHODS: Retrospective analysis of 56 patients who underwent modified dacrocystorhinostomy with intubation in our hospital from July 2010 to September 2011.
RESULTS: All patients underwent the extubation of lacrimal and catheters irrigation, the patency rate was 95% at 2a after operation.
CONCLUSlON: Modified dacryocystorhinostomy combined with intubation is a effective method for chronic dacwocystitis combined with lacrimal duct obstruction, which has simple operative procedure, high success rate.
4.Analysis of risk factors for allograft survival by COX model after renal transplantation

Objective To analyze the factors influencing the long-term allograft survival in cadaver- ic renal transplantation.Methods Parameter data were obtained from 249 patients who received immuno- suppressive agents after renal transplantation.Health resources were evaluated for 1-5 years;and 14 relevant factors,including age,sex,therapeutic regimen choice and complications,etc were analyzed.Life table method and COX regression model were used to analyze the risk factors influencing the outcomes and to calculate the survival rates.Results Following renal transplantation,the survival rates of recipients who lived for 1,2 and 3 years were 72.6%,56.0% and 40.8%,respectively;and the rates of those who lived for 4 and 5 years both were 22.5%.The median survival time was 34.9 month.With the therapy prolonged,the survival benefit in MMF group was superior to that in AZA group,with the median survival time being 38.9 months 30.6 months,respectively.COX regression model showed that the main predictive factors were treatment regi- men(P=0.000),follow-up period(P=0.000),patient's compliance(P=0.000),acute rejection episode (P=0.020),sex(P=0.001)and hospitalization period(P=0.040).Conclusions Life table and COX regression model are useful methods for evaluating long-term outcome and influencing factors in renal trans- plant patients.
5.Design and application of testing system for visual-feedback-based posture adjusting ability

Objective To develop a new visual feedback postural adjusting ability assessing system, and to study its application value. Methods Pressure sensor was used to determine the changes of the body weight center by tracing the pressure variation of the soles and heels, then eight target points in different directions were added into the software interface to enable the subject move his body weight center to the target point intentionally, finally the system was evaluated by loading on back.Results Program design was performed for the testing system. It's proved that posture adjusting ability might decrease with 20% load on back or the body leaning backwards. Conclusion The newly developed visual feedback postural adjusting ability testing system can be used to research standing balance function.
6.Therapeutic effects of early open decompression drainage on acute pulpitis

Objective To investigate the clinical effects of open decompression drainage on acute pulpitis.Methods Totally 180 acute pulpitis patients hospitalized during January 2012 to September 2014 in our department were selected and randomly divided into control group and observation group (n=80 each).The control group was treated with pulp inactivating agent and the observation group was given open decompression drainage.The clinical effects,analgesia efficiencies at 7d after treatment,treatment course and the complications were compared between the 2 groups.Results There was significant difference in the effective rate between control and observation groups after treatment [61.3% (49/80 cases) vs.86.3% (69/80 cases),P<0.05].The analgesia efficiencies at 7d after treatment was 46.3% (37/80 cases) in control group and 73.8% (59/80 cases)in observation group (P<0.05).There was no significant difference in treatment course between two groups [(7.7±1.4) times vvs.(6.1± 1.2) times,P>0.05]].6 patients developed periapical periodontitis in control group,and only 4 cases in the observation group (P>0.05).Conclusions Early open decompression drainage has good clinical effects and analgesia efficiencies in treatment of acute pulpitis,which is worthy of clinical application.

[Objective] To investigate professor SHAN Zhaowei's experience of diagnosing and treating bile reflux gastritis(BRG). [Method] The experience of Professor SHAN Zhaowei on recognition of etiology and pathogenesis, treatment principle, characteristics of prescription selection and administration of BRG is summarized and studied. Besides his academic ideas and clinical medication experience are shown with typical cases. [Result] In his view, emotional disorder is the most common etiology, and liver-stomach disharmony is the basic pathogenesis. He puts forward that we should pay attention to dispersing the stagnated liver Qi, regulating the stomach to lower the adverse flow of Qi when we treat it, and emphasizes the combination of disease differentiation and syndrome differentiation. Meanwhile, herbs of protecting gastric mucosa are compatibly used to improve the curative effect. And the case in this article has obtained good treatment results. [Conclusion] Pro. 9.Research advances on late intraocular lens dislocation after phacoemulsification

Late intraocular lens dislocation is one of the most severe late complications after phacoemulsification.It often occurs 3mo after the surgery.Different from early intraocular lens dislocation, late intraocular lens dislocation is caused by zonular dehiscence and contraction of the capsular bag many years after phacoemulsification.In recent years, the incidence of late intraocular lens dislocation gradually increases, having a risk of 0.1% after 10a and 1.7% after 25a.In the long-term follow-up patients who underwent cataract surgery, 90% had zonular insufficiency and capsular contraction.Among the multiple factors which may contribute to zonular weakness and capsular contraction, pseudoexfoliation is the most common cause, accounting for 50% of all the cases.Other risk factors include aging, high myopia, uveitis, trauma, previous vitreoretinal surgery, retinitis pigmentosa, diabetes mellitus, atopic dermatitis, previous acute angle-closure glaucoma attack, and connective tissue disorders.The understanding of these predisposing factors will suggest necessary preventions for high-risk patients in the future.
