Retrospective study on the situation of over loading at Viet Duc Hospital, Saint Paul Hospital, Hanoi Gynaecology Hospital and Gia Lam District Hospital was carried out from 1999 to 2002. 400 inpatient medical records at internal medicine, surgical, gynecological, and pediatric departments; 100 specialistic medical records were studied; Patients were asked to answer Questionnaire; and officers of Hanoi Health Service and leaders of 4 hospitals were organized group discussions. Results: through 4 years all these hospitals have always been over loaded with both out patient and in patient. On average, 1 doctor has examined 25.5-33.5 patients a day. Over loading occurred frequently in some departments and in epidemic seasons. The causes might be both in hospital and out hospital leading to overloading. Authors also proposed some solutions to resolve this problem

This paper introduced the causes of overload of hospital, including no implementation of compulsory management of the health and consultation as administrative and geographical division, bad implementation of technical distribution, improper policies of hospital bursar and disadvantages of market economy. It was difficult to prevent the overload of hospitals. Therefore, it should be strengthened by practicable solutions in which the health sector plays an important role.

There were some urgent problems in hospitals of Viet nam, especially the lack of budget and the poor materials, equipment and infrastucture. This study aimed to review some precondition and introduce some necessary solutions which include 1, change of hospitals with the subcidize mechanism to the health services 2, the hospital should be considered as an economy facility 3, change of National hospitals to the joint stock company for the health services.

Aiming at the management efficiency problems in the traditional medical low-value consumables management mode of the nursing unit, based on the perspective of supply chain management, this study uses lean management method to construct the lean management mode of low-value consumables with a whole cycle and whole process information monitoring, and analyzes the application effect of this mode. The results show that after the application of lean management mode, the low-value consumables of the nursing unit can achieve "consumables in use=priced consumables + un-priced consumables", the settlement cost is significantly reduced and its stability is high, and the efficiency of "supply-inventor-distribution" link is significantly enhanced. This model effectively improves the management efficiency of low-value consumables in the hospital, and also provides a reference for other hospitals to improve the management level of low-value consumables.

The process of clinical nutrition diagnosis and treatment can be standardized and the systematic management of clinical nutrition diagnosis and treatment is realized by using information technology. The system is based on B/S architecture, developed in Java language, and uses Web Service to interact with the clinical business system through the integrated platform, which meets the business needs of the department. The system is deeply integrated with the clinical business system through online and offline forms to realize the functions of nutrition assessment, nutrition risk early warning, nutrition consultation, mobile ward round, intelligent nutrition consultation and so on. The system can improve doctors' work efficiency, realize the whole process of nutrition diagnosis and treatment, fine management, and realize the interconnection between systems.

The demographic transition occurs all over the world, including Indonesia. The Indonesian Ministry of Health announced that Indonesia has been dominated by an aging society since 2017. The proportion of elderly has reached 9.03%. This commentary study aimed to see the general conditions of healthcare providers (primary healthcare centers and hospitals) in facing the aging society. Results showed that the primary healthcare centers and public hospitals are not ready yet to face the aging society. In fact, primary health care is not friendly to the elderly in terms of the facilities and the limited services of home visits. This study recommended the government and stakeholders to provide the optimal quality and quantity of health care services to the elderly who have dominated the population.
