1.Prediction of plasma myeloperoxidase on cardiovascular event of patients with stable angina pectoris

Objective To explore the independent prognostic value of plasma myeloperoxidase (MPO) in cardiovascular event of patient with stable angina pectoris.Methods Two hundred and forty-seven patients with stable angina pectoris (defined as 50% stenosis in any coronary vessel) were selected as our subjects and they all were followed up for 2 years.Plasma MPO and hsCRP concentrations were measured.The major adverse cardiovascular events (MACEs),including death,myocardial infarction and stroke,occurred in patients were recorded.Results All patients were divided into high MPO group (84 cases and plasma MPO concentrations over 320 mg/L) and low MPO group (163 cases,plasma MPO concentrations less than 320 mg/L).Compared with patients in low MPO group,the risk of occurring MACEs in high MPO group was higher(HR =1.67,95% CI =1.32-2.57,P < 0.05).There was positive correlation between plasma MPO and hsCRP concentrations (r =0.132,P < 0.05).Conclusion MPO level may serve as a better prediction index for adverse cardiovascular events of patients with Stable angina pectorisindependently.MPO has relationship with hsCRP concentrations.
2.Changes of serum nitric oxide and nitric oxide synthase in acute cerebral hemorrhage with multiple organ dysfunction syndrome caused by systemic inflammatory response syndrome

Objective To explore the possible mechanism of acute cerebral hemorrhage with systemic inflammatory response syndrome (SIRS) which caused multiple organ dysfunction syndrome (MODS), and to investigate the significance of serum nitric oxide (NO) and nitric oxide synthase (NOS) in the development of acute cerebral hemorrhage from SIRS to MODS.Methods The incidence rate of acute cerebral hemorrhage leading to SIRS and MODS was investigated. 4.Comparative Study of Thymosin α1 Combining Anti-coagulation Medication and Simple Anti-coagulation Medication for Treating the Patients With Deep Venous Thrombosis Formation

Objective: To compare the therapeutic effect of thymosin α1 combining anti-coagulation medication and simple anti-coagulation mediation in treating the patients with deep venous thrombosis (DVT) formation. Methods: A total of 92 patients with lower extremity vascular ultrasound conifrmed diagnosis of DVT were studied. The patients were randomly divided into 2 groups for treatment: Combination group, the patients received thymosin α1 combining anti-coagulation medication,n=45 and Simple group, the patients received only anti-coagulation medication,n=47. The changes of their deep venous thrombosis condition after treatment were compared between 2 groups. Results: By 4 weeks treatment, the proportion of completely dissolved thrombus in Combination group and in Simple group were 67 branches (49.26%) and 54 branches (37.24%); the thrombus progression and recurrence condition were 6 branches (4.41%) and 16 branches (11.03%), allP<0.05. Conclusion: Thymosin α1 combining anti-coagulation medication has the better effect than simple anti-coagulation medication for treating DVT patients.

Objective:To analyze the structure of supply and demand, the distribution status of essential drugs and low-cost drugs and the influencing factors in the distribution, and provide strategies and suggestions. Methods:The literature research, Delphi method, sampling survey and the other methods were used to analyze the effects of distribution pattern on the drug distribution in Henan, and then optimization measures were studied systematically. Results:The distribution mode of centralized purchasing drugs was the major influen-cing factor in the drug distribution rate and enthusiasm. Conclusion:The distribution of centralized purchasing drugs should stick to the principles of essential drugs priority and reasonable use in order to enhance the distribution enthusiasm of manufacturers. The distribution should base on the two-way choice distribution mode and combine with the mode of centralized distribution and independent choice, mean-while, the information technology should be improved to optimize dynamic management of distribution and promote the effectiveness of drug delivery.

8.Cause and clinical analysis of lumbar reoperation

[Objective] To investigate the causes,indications,operative methods and clinical results of lumbar reoperation.[Methods]Twenty-eight failure lumbar procedure cases were received reoperation from May 2003 to May 2005.There were 16 males and 12 females.These cases were from 31 to 70 years old(mean 47).It took a mean 1 year and 8 month(from 1 month to 8 year)from last operation to this registry.The causes of reoperation included rehemiated disc nucleus pulpous,degeneration of adjacent segment and instability and insufficiency of canal decompression.Some cases of discogenic back pain were operated by mistaken procedure.Some cases involved severe complications including infection in disc space or vertebral itself,pedicle screws misposition,aneurysm formation and CSF leakage.The operative methods included rediscectomy,lumbar canal decompression,and lumbar reconstruction by implant and fusion.[Results]The operative time and blood lose were from 50 to 350 minutes(mean 213)and from 100 to 3 000 ml(mean 760 ml)respectively.One patient died because of severe abdominal cavity infection.The other cases were recovered and clinical complaint became less.The VAS and Oswestry Disability Index decreased from(7.9?1.6)and(52.4?7.2)to(2.8?1.0)and(8.3?3.6)respectively(the decrease rate:64.3% and 66.5%)after 24 to 48 months follow-up(mean 32.8 months).[Conclusion]The causes of failure lumbar procedure must be analyzed as soon as possible.The risks and benefits of reoperation must be balanced although it may make patients get more chance to recovery.It is very difficult for these procedures and the risks are also high.The dura sac rupture is the most common complication.It is the most important to deal with the adhesion and bleeding control in canal.The use of the magnification and lighting equipment can reduce the complications.

Objective To investigate the effects of tissue inhibitor-3 of matrix metalloproteinases(TIMP-3)Gene-transfected vascular smooth muscle cells(VSMCs)transplantation on heart function after acute myocardial infarction(AMI)in rats and to explore the potential mechanisms.Methods Wistar rats were produced AMI models by ligating the descending left coronary artery.Rats were survived and divided into 3 groups randomly(n=18):0.5mlDMEM containing 1?106 TIMP-3 gene-transfected VSMCs(group A),1?106 VSMCs(group B)or 0.5 ml DMEM without cell(group C)were injected into the ischemic myocardium immediately.Ischemic myocardium samples were harvested at 3 day after operation.The heart function was observed through the tissue functional examination.The activity of TIMP-3 gene-transfected VSMCs were measured by immunohistochemical method.mRNA of TIMP-3 and matrix metalloproteinase 9(MMP-9)were determined by RT-PCR.Results VSMCs were cultivated and had a high purity(98%).TIMP-3 gene was transfected into VSMCs successfully.Three day after operation in group A the average percentage of LVIDd、LVIDs、EDV and ESV were significantly higher than group normal(P
10.Effects of cryopreserved neural stem cell transplantation on rat axonal regeneration after spinal cord injury:Retrograde tracing observation

AIM:There are some reports about the treatment of spinal cord injury with neural stem cells(NSCs) transplantation,but the ideas about the transplantation time and patterns and detection indexes are still different.In this study,the effects of cryopreserved NSC transplantation on the axonal regeneration after the spinal cord injury in rats were observed.METHODS:The experiment was carried out in the experimental animal center of China Medical University from June 2005 to June 2006.①Thirty-six adult Wistar rats,either male or female and 250-300 g,were provided by the experimental animal department of China Medical University.Neural stem cells were isolated from 10 neonate rats and cultured.All treatments for animals were accorded with the animal ethical criteria.②NSCs at logarithmic phase were cryopreserved at-70 ℃ for 2 weeks and labeled with 5-bromodeoxyuridine(Brdu) after rewarming.After the models of spinal cord injury were established,the NSCs were transplanted into the injured site immediately.Thirty-six rats were randomly divided into NSC transplantation group,DMEM solution group,and control group.③NSC survival and migration were detected by immunohistochemistry,and the reconstruction of spinal cord were detected by horseradish peroxidase(HRP) staining.RESULTS:Of the 36 spinal injured model rats,4 died of overdose of anesthesia,and 5 died of infection,but all were supplemented.Brdu-labeled positive NSCs were detected in the injured spinal cord after transplantation on the 7th day,and increased on the 14th day,then gradually decreased since the 28th day till disappeared.The number of HRP positive cells in transplantation group was significantly higher than DMEM solution.CONCLUSION:Cryopreserved NSCs can survive in the injured site after transplantation and promote the reconstruction of axoplasm pathway of injured spinal cord.
