1.Clinical research of restructuring human brain natriuretic peptide treatment of in heart failure associated with severe acute myocarditis

Objective To explore the clinical effect of restructuring human brain natriuretic peptide in treatment of heart failure associated with severe acute myocarditis.Methods Fifty cases of heart failure associated with severe acute myocarditis were divided into experimental group and control group according random number table method,each group 25 cases.The patients in control group were given conventional treatment,and the patients in experimental group were given restructuring human brain natriuretic peptide on the basis of conventional treatment.The clinical effects of two groups were observed.Results The level of N terminal brain natriuretic peptide,left ventricular ejection fraction,urine volume,creatinine in two groups had no significant difference before treatment (P > 0.05).After treatment,these index in experimental group were better than those in control group [(7 785 ± 432) ng/L vs.(10 022 ± 447) ng/L,(45.28 ± 2.67)% vs.(41.34 ±3.11)%,(1 833 ±49) ml/24 h vs.(1 456 ±47) ml/24 h,(109.34 ± 10.77) μmol/L vs.(125.44 ± 11.00) μ mol/L] (P < 0.05).The rate of clinical symptoms improving in experimental group was significantly higher than that in control group [71.4% (55/77) vs.26.0% (20/77)] (P <0.01).Conclusion Restructuring human brain natriuretic peptide with conventional treatment can effectively improve heart failure associated with severe clinical symptoms of acute myocarditis,improve the clinical therapeutic effect.

METHODS: Serum levels of interleukin2(IL-2), interleukin12(IL-12), interleukin4(IL-4), tumor necrosis factor-alpha (TNF-?) and transforming growth factor beta (TGF-?1) were measured in 26 patients with depression(depression group) before and after treatment and 28 healthy volunteers (control group) by enzyme linked immunosorbent assay (ELISA). Objective To establish hyperoxia retinopathy in premature rat model with SpragueDawley (SD),observe the pathological changes of angiogenesis and expression of vascular endothelial growth factor (VEGF),and compare the effect of bevacizumab periocular injection and intravitreal injection of retinopathy of premature rats.Methods Ninety preterm rat retinopathy models were randomly divided into 3 groups:high oxygen group,retrobalbar injection group,and vitreous cavity group (n =30 per group);36 healthy rats were chosen as the normal control group.Each group were randomly selected 10 rats 'right eyes to make retinal flat mount to observe the right eye retina vascular morphology;10 resections of right eyes were embedded in paraffin,and stained with hematoxylin eosin (HE);the number of right eye of retinal neovascularization was observed;the right eye RNA was extracted,and reverse-transcription polymerase chain reaction (RT-PCR) was used to detect the expression of VEGF.Results The normal group showed retinal vessels were around a reflection of distribution,good branch,shape rules,and clear structure.Intravitreous neovascularization occurred in the high oxygen group and peribulbar injection group.Retinal vascular endothelial cells in the high oxygen group were increased significantly compared to the normal group (P < 0.05).The visible clusters of red blood cells in the periocular injection group and vitreous group were decreased compared to the high oxygen group (P < 0.05),but no significant difference between the periocular injection group and vitreous group (P > 0.05).The expressions of VEGF mRNA and HE staining had a consistent trend.Conclusions Bevacizumab inhibited the expression of VEGF that was increased in premature rats in hyperoxia retinopathy.The effects of bevacizumab periocular injection and intravitreal injection were similar,but the former was more secure than the latter.
4.Relationship Between Plasma Level of Asymmetric Dimethylarginine and Coronary Artery Ectasia in Relevant Patients

Objective: To investigate the relationship between plasma level of asymmetric dimethylarginine (ADMA) and coronary artery ectasia in relevant patients. Methods: A total of 72 patients received coronary angiography (CAG) in our hospital were studied and the patients were divided into 3 groups: Coronary ectasia group, Coronary stenosis group and Normal coronary group.n=24 in each group. Plasma levels of ADMA, symmetric dimethylarginine (SDMA) and L-arginine (Arg) were measured by HPLC-MS/MS methods. The relationship between ADMA and CAD was examined by Logistic regression analysis. Results: Plasma level of ADMA in Coronary ectasia group (0.437 ± 0.098) μmol/L and Coronary stenosis group (0.456 ± 0.088) μmol/L were higher than that in Normal coronary group (0.381 ± 0.057) μmol/L,P<0.05. The ratio of Arg/ADMA in Coronary ectasia group (208.54 ± 61.52) and Coronary stenosis group (220.00 ± 104.82) were lower than that in Normal coronary group (254.26 ± 76.22),P<0.05. Logistic regression analysis presented that with adjusted age, gender, smoking, family history of CAD and LDL-C level, and plasma ADMA was still related with CAD (Partial regression coefifcient 9.469, P=0.011). Conclusion: Plasma levels of ADMA were higher in patients with coronary artery ectasia/stenosis than those with normal coronary artery; while ADMA levels were similar between the patients with coronary ectasia and stenosis. Plasma ADMA level was the independent risk factor of CAD.
5.The correlation of plasma homocysteine and intima-media thickness with incipient cerebral infarction

Objective To explore the relationship between the level of plasma homocysteine(HCY)and intima-media thickness(IMT)with incipient cerebral infarction.Methods According to the clinical diagnostic criteria,112 patients with incipient cerebral infarction were randomly selected as the cerebral infarction group,and 102 healthy people were randomly selected as the control group.All carotid arteries of both groups were checked by carotid artery ultrasound and IMT was tested.The levels of plasma HCY and plasma folate as well as vitamin B12 were measured in both groups by chemiluminescence immunoassay.Results The plasma HCY level in the cerebral infarction group [(24.89 ± 1.96) μmol/L] was significantly higher than (8.58 ± 2.34) μmol/L in the control group (P < 0.01).The levels of plasma folate [(4.79 ± 2.35) μg/L] and vitamin B12 [(228.67 ± 114.75) ng/L] in the cerebral infarction group were significantly lower than [(8.65 ± 2.64) μg/L and (320.53 ± 154.78) ng/L] of the control group (P < 0.01).The IMT of the cerebral infarction group [respectively the left common carotid artery fork (1.12 ± 0.25) mm,the right common carotid artery fork(1.09 ± 0.28) mm,the left internal carotid artery(1.18 ± 0.23) mm,the right internal carotid artery(1.03 ± 0.24)mm] were obviously higher than those of the control group[respectively the left common carotid artery fork (0.65 ± 0.18) mm,the right common carotid artery fork (0.61 ± 0.24) mm,the left internal carotid artery(0.58 ±0.22)mm,the right internal carotid artery(0.61 ±0.19)mm] (P <0.05).Significant positive correlation between IMT and HCY was found in patients with cerebral infarction(r =0.834,P < 0.01),but no corre lation was found in the control group(r =0.081,P > 0.05).Conclusion The increase of plasma HCY and the thickening of IMT are closely related with incipient cerebral infarction.Examination of plasma HCY and IMT has important clinical significance for the secondary prevention of incipient cerebral infarction.
6.Research of Narcotrend monitor for the prevention of awareness during general anesthesia

ObjectiveTo study the clinical efficacy and safety of Narcotrend (NT) monitor for the prevention of awareness during general anesthesia.MethodsFour hundred and thirty-two patients with elective extubation and general anesthesia (ASA Ⅰ-Ⅱ) were divided into observation group and control group with 216 cases each by random digits table.All patients were treated with total intravenous anesthesia.The patients were given anesthetics by NT monitor in observation group and detemined by the clinical experience of anesthesiologists in control group.The mean arterial pressure (MAP),heart rate (HR),respiratory rate(RR),pulse oxygen saturation(SpO2),NT stages (NTS),NT index (NTI),total sedatives,awaking time,extubation time,Ramsay score was recorded.All patients in the postoperative on the first day and the fourth day were followed-up two times to understand the perception and memory in patients undergoing the situation.ResultsThere was no know in operation in observation group,there was 1 case with suspectedknow and 1 case with definite know in control group.Compared with control group,the awaking time and extubation time was decreased in observation group [ ( 6.0 ± 2.8 ) min vs.( 10.0 ± 4.9 ) min,( 12.0 ± 5.5 ) min vs.( 19.0 ± 6.9) min] (P < 0.05 ); MAP and HR was even more stable.In addition to midasolam,the remaining amount of the drug was less (P < 0.05 ).Ramsay score at awaking,extubation and out of the operating room in observation group was lower than that in control group (P < 0.05).ConclusionNT monitor for total intravenous anesthesia increases the safety of general anesthesia,and it can be reduced to some extent,intraoperative awareness.
7.The association between hepatic steatosis and HBsAg and HBcAg in chronic hepatitis B patients

ObjectiveTo explore the association between hepatic steatosis and the liver tissue expression of HBsAg and HBcAg in chronic hepatitis B (CHB) patients.MethodsFrom January 2005 to June 2008,a total of 147 CHB patients with hepatic steatosis diagnosed by liver biopsy and other 149 CHB patients without hepatic steatosis but with similar HBV DNA titer were enrolled.The differences of HBsAg and HBcAg immunostaining and liver injury in these two groups were compared.The data were analysed using t test and chi square test.ResultsCompared with non-steatosis group,the average age and body weight index of hepatic steatosis group were higher (t values were -3.31and -6.57,both P<0.01).The percentage of moderate to severe hepatic inflammation in liver,obvious hepatic fibrosis and the strong positive HBsAg staining was lower (30.6% vs 15.4% ; 26.5%vs 12.8%; 23.1 % vs 6.7 %; x2=9.63,8.92,15.76; all P<0.01),and the percentage of strong positive HBcAg staining was also in downtrend.Compared with degree F1 and F2 of liver steatosis,the percentage of HBsAg and HBcAg strong positive staining in liver tissues of degree F3 and F4 was in downtrend.ConclusionsHepatic steatosis affected the expression of HBsAg and HBcAg in liver tissue of CHB patients.As hepatic steatosis appeared and became more severe,both expression of HBsAg and HBcAg and the degree of liver injury were in downtrend.
8.The clinical application of homemade airway exchange catheter on extubation of patients with difficult tracheal intubation in intensive care unit

Objective To discuss the clinical application of homemade airway exchange catheter on the extubation of patients with difficult tracheal intubation in intensive care unit(ICU). Methods Sixty-two patients with difficult tracheal intubation who failed their initial extubation trial were randomly divided into conventional group (31 cases)and observation group(31 cases). The patients in the conventional group received routine extubation process,while the patients in the observation group were extubated under the guidance over a homemade airway exchange catheter. The changes in heart rate(HR),blood pressure,respiratory rate(RR)and pulse blood oxygen saturation(SpO2)were compared at 12 hours after extubation,so as the re-intubation rate,intubation success rate at first attempt and re-intubation time in two groups,and the tolerance and complications after extubation were observed. Results After extubation,the HR,blood pressure and RR were increased significantly(all P<0.05), and the SpO2 was much lower in conventional group(P<0.05),while those parameters were changed little and basically in the normal ranges in the observation group. At 12 hours after extubation,the re-intubation rate was much lower(6.45%vs. 25.81%,P<0.05)in the observation group,with shorter re-intubation time(seconds:27±14 vs. 49±28,P<0.01),higher intubation success rate at first attempt(90.32%vs. 54.84%,P<0.01)and better tolerance (77.4% vs. 61.3%,P<0.05)compared with those in the conventional group. There was no severe complication in the observation group,and there were 1 cases of glottic edema with cricothyroid membrane puncture,2 cases of broncheal mucous membrane bleeding and 2 cases of bucking in the conventional group. Conclusion Compared with conventional extubation process,the extubation over homemade airway exchange catheter can increase the rate of extubation,reduce re-intubation rate and the re-intubation time,with favorable tolerance and no occurrence of serious complications,and is one of the safe and effective extubation strategies in patients with difficult tracheal intubation in ICU.
9.Embryo Toxicity and Teratogenic Effects of Microula Seed Oil on Rat

Objective To investigate toxicity and teratogenic effect of Microula seed oil on embryo of rat.Methods 150 mature Wistar rats (100 females, 50 males) were selected with female∶ male = 2∶1 cage match. During the daily morning examination the sperm was discovered in the vagina as the zero day for conception.100 pregnant rats were randomly divided into 5 groups (n=20 in each group): treatment group (included 3 groups: to give Microula seed oil 2.5 g /kg, 5.0 g /kg, 10.0 g /kg body weight, ig, respectively), cyclophosphamide group(7 mg/ kg body weight, sc) and the edible oil group (to give dose, such as volume as Microula seed oil, ig). From the 7th day of pregnancy, the treatment group, the edible oil group were given intervention once a day for 10 days. From the 11th day of pregnancy cyclophosphamide group was given cyclophosphamide as intervention once a day, for 3 days. On the 20th day the pregnant rats were killed.Results The body weight of pregnant rats and the rate of live births were significantly higher in the Microula seed oil dose group than in the cyclophosphamide group (P<0.01), stillbirth rate and birth rate of absorption was significantly lower in the Microula seed oil dose group than in the cyclophosphamide group (P<0.01), and no significant difference from the edible oil group (P> 0.05); the fetal rat, body weight, body length, tail length in all groups of Microula seed oil was no significant difference from the edible oil group (P> 0.05). There was no malformation in appearance, viscera, bones in the treatment group and the edible oil group while there were 112 fetal rats with deformity in 140 in the cyclophosphamide group.Conclusion Microula seed oil at doses of 2.5 g /kg, 5.0 g /kg, 10.0 g /kg body weight had no significant role in the toxicity and teratogenicity on embryos of pregnant rats.
10.Safety and efficacy of policosanol in elder patients with high on-treatment platelet reactivity after ;drug-eluting stent implantation:a subgroup analysis of the SPIRIT study

Objective To explore the safety and efficacy of policosanol in elder patients with high on-treatment platelet reactivity ( HPR) after drug-eluting stent ( DES) implantation. Methods This study was a prespecified subgroup analysis of the multicenter, randomized SPIRIT trial,in which there were a total of 169 elder patients (≥60 years old) with HPR. Among these patients, 30 patients were in group A ( given clopidogrel 75 mg/d for one year) , 75 patients in group B ( given clopidogrel 150 mg/d for 30 days followed by 75 mg/d until one year ) and 64 patients in group C ( given policosanol 40 mg/d for 6 month and clopidgrel 75 mg/d for one year ) . All patients were treated with aspirin at the same time. The primary endpoint was the reversion rate of HPR at 30 days (reversion was defined as platelet aggregation ﹤65%). The secondary endpoint was 2-year major adverse cardiac events ( MACE ) rate, which included cardiac death, non-fatal myocardial infarction and ischemic symptoms driven target vessel revascularization. The safety endpoint was any bleeding as defined by the Bleeding Academic Research Consortium ( BARC ) definition. Results At 30 days, the reversion rate of HPR in group C was numerically higher as compared with group A ( 42. 9% vs. 23. 3. 0%, P=0. 068 ) , and similar with group B ( 42. 9% vs. 49. 3%, P=0.447). MACE occurred in 4 (13.3%), 5(6.7%) and 3(4.7%) patients in group A, B and C respictively ( P=0. 352). Bleeding events in group A and group C were both markedly lower in comparison to group B (3. 3% vs. 17. 3% vs. 1. 6%, P=0. 001). At the 24-month follow-up, the MACE-free survival rates were not significantly different (95. 3% vs. 93. 3% vs. 86. 7%, P=0. 146). Conclusions For elder patients with HPR, policosanol reduced platelet reactivity to a similar extent in comparison of high maintenance dose of clopidogrel without increasing bleeding risk.
