Objective To understand hot-spot of preventive medicine research at the beginning of the 21st century.Methods To analyze all the key words in the articles from four highly influential comprehensive preventive medical journals from 2001 to 2006 through word frequency analysis.Results In research population,children have always been the focus of attention,and more attention was paid to young people and college students in 2006.In research methods,the epidemiological and the molecular biological methods have been the main preventive medicine research ones.As regards research content,risk factors have been mainly studied;hypertension and obesity has been focused on,and diabetes and AIDS have been paid more attention to since 2003;Lead on trace elements has received more attention gradually.Conclusions Close attention should be given to the research of preventive medicine in the rural areas.
2.Expression of TGF β1 in the Left Ventricular Tissue and its Relations to Left Ventricular Hypertrophy and Myocardial Interstitial Fibrosis in Renovasc ular Hypertensive Rats

Objective To investigate the roles of T GFβ1 and TGFβ1 typeⅠrecepter(TβRⅠ) in development of myocardial hypert rophy and myo cardial interstitium remodelling in renovascular hypertensive rat. Methods TGFβ1 and TβRⅠwere evaluated by qualitative and semi -qu antitative immunohistochemical staining to explore their distribution and ex peression in left ventricular tissue. Van Gieson staining were used in the stud y of total collagen in left ventricular interstitial tissue. Results Compared with sham-operated rat(SOR),the expression of T GFβ1 andTβRⅠ in left ventricular tissue were significantly increased in RHR(P <0.01 all). In RHR the antibody to TGFβ1 reacted primarily with cytoplasma a nd myo cardial interstitium while antibody to TβRⅠreacted mainly with myocyte membran es with weak labeling in myocardial interstitium. In SOR,very mild labeling for TGFβ1 and TβRⅠin myocardiun was found with no labeli ng in myocardial interstitium. Correlative analysis revealed that TGFβ1 or T βRⅠ was positively correlated with left ventricular mass index(LVMI),negativel y correlated with grey level of myocardial total collagen. Conclusions TGFβ1 and TβRⅠmay play important roles in develo pment of left ventricular hypertrophy and myocardial interstitial fibrosis.
3.Expression of miR-483-5p in epithelial ovarian cancer and its effects on cisplatin resistance in epithelial ovarian cancer cells

Background and purpose:Although cisplatin-based chemotherapies are used as the first-line treatment for ovarian cancers, the majority of patients eventually progress with platinum-resistant disease. miR-483-5p is overexpressed in lung cancer. However, the research on miR-483-5p in epithelial ovarian cancer (EOC) is still unclear. This study aimed to investigate the expression of miR-483-5p in EOC and its effects on cisplatin resistance in EOC cells.Methods:This study analyzed the expression of the miR-483-5p by real-time lfuorescent quantitative polymerase chain reaction (RTFQ-PCR) in EOC tissues, normal ovarian tissues, and EOC cells. The role of miR-483-5p in EOC was evaluatedin vitro by lentivirus-mediated knockdown of miR-483-5p or overexpression of miR-483-5p in EOC cell lines. Drug sensitivity assay was carried out by CCK-8 kit.Results:miR-483-5p was upregulated in EOC tissues as compared with normal tissues (P<0.01). Furthermore, miR-483-5p expression in advanced stage (Ⅲ–Ⅳ) EOC was significantly higher than that in early stage (Ⅰ–Ⅱ) EOC (P<0.05). Interestingly, miR-483-5p expression was higher in cisplatin-resistant A2780/CP cells than other cells. Increased miR-483-5p expression caused EOC cell resistance to cisplatin and downregulated the expression of p21 and Bcl-2, whereas reduced miR-483-5p expression induced its sensitivity and upregulated the expression of p21 and Bcl-2.Conclusion:The results suggest that miR-483-5p is highly expressed in EOC and contributes to cisplatin resistance. Thus, miR-483-5p is a potential therapeutic target for ovarian cancer.

5.Bolton analysis with malocclusions in Shanghai Fengxian district

Objective To measure Bolton index with malocclusions in Shanghai Fengxian district,and ana-lyze the distinction among different malocclusions patients in respect of gengder,age,and different Angle classifica-tion.Methods Measure Bolton index of plaster models of 348 eligible malocclusions patients who were selected ran-domly from Shanghai Fengxian District,and divided them into different groups by gender,age and Angle classification. Then,Bolton overall ratio and Bolton anterior ratio among these groups were analyzed and compared.Results The results showed that there were no significant differences of Bolton analysis in respect of gender or age(all P >0.05). It exsisted obvious differences in anterior ratios among the three Angle malocclusion class(P <0.05).Bolton anterior ratios were (79.88 ±3.15)%,(78.91 ±2.86)% and (80.59 ±1.55)%,mean of Class I and mean of ClassⅢwere bigger than that of Class Ⅱ.Bolton overall ratios of Angle class Ⅰ,classⅡ and class Ⅲ were (79.88 ± 3.15)%,(79.88 ±3.15)% and (79.88 ±3.15)%,and there were no obvious differences in overall ratios among them.Conclusion Bolton index with malocclusions for Shanghai Fengxian District accord with normol Bolton index of our country.Gender and age are not the influencing factors of Bolton index in Fengxian District.Bloton index anomalies are the important fator in malocclusions.We should pay more attention to the Bolton analysis of class Ⅱ malocclusions in clinical practice,and draw up a treatment plan combined with specific classification.
6.Clinical application of percutanously interventional therapeusis in patients with hepatocellular carcinoma accompanied with bile duct thrombosis

Objective To explore the method and value of percutanously interventional therapeusis for treatment of obstructive jaundice caused by hepatocellular carcinoma accompanied with bile duct thrombosis.Methods Sixteen cases with bile duct thrombosis proved by pathology and imaging examinations were retrospectively analyzed.According to the clinical symptoms, all the patients received percutaneous transhepatic biliary drainage (PTBD) including permanent external drainage, temporary internal drainage and implantation of covered stents.Serum total bilirubin (TBIL) after the interventional therapeusis were measured and compared with that before the treatments by t test to evaluate the efficacy of these treatments.The relief of clinical symptoms was also reviewed to evaluate the efficacy of these treatments.The patients were followed up within 2 years.Results The PTBD was successfully performed in 16 cases.Permanent external drainage, temporary internal drainage and implantation of covered stents were performed in 2 patients, 7 patients and 7 patients respectively.TBIL after the interventional therapy decreased significantly (t=7.366, P<0.01) to (161.2±80.5) μmol/L averagely from (261.9±77.2)μmol/L before the treatments.All the patients died before the end of followed-up.The average survival time was 204 days (30 to 391 d)and the median survival time was 200 days.Bleeding and infection were the main complications, which could be controlled successfully by routine treatments.Conclusion With high achievement ratio and good efficacy, percutanously interventional therapeusis are good choices for the treatments of obstructive jaundice due to bile duct thrombosis.
7.Clinical outcome of the combined nutritional support for colorectal cancer patients at nutritional risk: a retrospective study of 60 cases

ObjectiveTo explore the clinical outcome of the combined nutrition support for colorectal cancer patients.MethodsTwo research arms were obtained using a cohort sampling method.Ann A ( the study group): from 2009 to 2010,30 colorectal cancer patients were enrolled.They received nutritional risk screening (NRS) 2002 after admission.Nutritional risk was defined as NRS 2002 score ≥3 three days before operation.Patients with nutritional risk received enteral nutrition (EN) for bowel preparation without laxative drug and enema.After operation,they received EN combined with parenteral nutrition (PN) supports provided.Arm B (control group): 30 cases with historically confirmed colorectal cancer were enrolled from 2007 to 2008.They received routine bowel preparation (diet control,laxative drug,and enema) and PN supports after operation.Nutritional parameters,the rate of infectious complications,the rate of systemic inflammatory response syndrome,and the duration of hospital stay were analyzed.ResultsThere were no significant difference in body weight and plasma albumin between these two arms ( P > 0.05 ). The incidence of systemic inflammatory response syndrome (13.3 % ),infectious complications (10.0% ),and the duration of hospital stay [ (12.3 ± 6.5 ) d ] in arm A were significantly lowerthan those in arm B [33%,30%,and (15.0 ±7.2) d,respectively] (P =0.038,P =0.042,P =0.045).Conclusion For the colorectal cancer patients,nutritional risk screening on admission,bowel preparation with eneral nutrition before operation,and combined nutritional support after operation can improve the clinical outcome.
8.The clinical value of " Push-pull traction-relax homing-Repeatedly confirmed" in the prevention of bile duct injury in LC

Objective To study the value of the technique of "Push-pull traction-relax homingrepeatedly confirmed" in the prevention of bile duct injury in LC. Methods From March 2001-August 2009, we applied this technique in 4800 cases of LC. The technique of "Push-pull traction" showed the structures of in the Calot's triangle. The technique of "relax homing" was to restore the cystic duct,hepatic duct and common bile duct to their original anatomical positions. The technique of "repeatedly confirmed" repeatedly identified the positions of the cystic duct, the common hepatic duct and the common bile duct. Results There was no bile duct injury. Conversion to open surgery happened in 118patients due to difficulties in identifying the Calot's triangle structures, bile duct stones, gallbladder cancer, and gallbladder-duodenal fistula. Conclusions The "Push-pull traction-relax homing-repeatedly confirmed" technique could effectively prevent bile duct injury in LC. The method is simple, easy to master and worthy of promotion.
9.Retroperitoneoscopic adrenalectomy in semilateral supine position

Objective To discuss the semilateral supine position for retroperitoneoscopic adrenalectomy. Methods From Jan. 2006 to Dec. 2008, 36 patients (20 males and 16 females with mean age of 43 years) underwent retroperitoneoscopic adrenalectomy in 60° -70° semilateral supine position. There were adrenal cortex adenomas in 18 cases, pheochromocytoma in 6 cases, adrenal cysts in 3 cases, myelolipoma in 2 cases, gangliocytoma in 1 case, lymphangioma in 1 case, metastatic tumor in 1 case and corticohyporplasia in 4 cases. The mean diameter of the tumors was 2.6 cm( 0.5 - 7.7 cm ). The tumors were superior to the renal pole in 5 cases, anteromedial in 10 cases and superomedial in 17 cases. The three ports that were usually used in lateral position and were placed anteriorly to create retroperitoneal place: the first port was placed 2 -4 cm superior to the iliac crest along the anterior axillary line, the other two were placed just below the costal margin along the midaxillary line and at the same level along the midclavicular line, and dissected along the anterior surface of kidney to its superomedial aspect, so as to avoid the hampering of the kidney in the exposing of the diseased adrenal gland. Results The procedure was completed successfully in all of the cases with the operating time of 37 - 145 min ( mean 69 min) and intraoperative blood loss of 30 - 100 ml (mean 48 ml). Six cases had rupture of peritoneum, which were sutured and the procedure was continued to completion. The postoperative hospital stay was 3 -8 d (mean 5 d ). Thirty-five patients were available for follow-up of 3 - 28 months ( mean 14 months). The case of metastatic tumor died of the primary diseases in the 12th month postoperatively. No other complication was found. Conclusion With this alternative position and ports' location, the procedure of retroperitoneoscopic adrenalectomy could be easier and safer than the conventional position.
10.Mediastinal Emphysema, Giant Bulla, and PneumothoraxDeveloped during the Course of COVID-19 Pneumonia

The coronavirus disease 2019 (COVID-19) pneumonia is a recent outbreak in mainland China and has rapidly spread to multiplecountries worldwide. Pulmonary parenchymal opacities are often observed during chest radiography. Currently, few caseshave reported the complications of severe COVID-19 pneumonia. We report a case where serial follow-up chest computedtomography revealed progression of pulmonary lesions into confluent bilateral consolidation with lower lung predominance,thereby confirming COVID-19 pneumonia. Furthermore, complications such as mediastinal emphysema, giant bulla, andpneumothorax were also observed during the course of the disease.
