1.The new resuscitation strategy for hemorrhagic shock

For uncontrolled hemorrhagic shock the traditional method and target of resuscitation may be harmful. The existing data supported a strategy of less aggressive interventions to those shock with uncontrolled hemorrhage until a decisive surgery begin. The choice of liquor for resuscitation remains in controversy. A combined application of crystalloid and colloid liquor seems to be reasonable and may be accepted by most physicians. It should be noted that the new resuscitation strategy may not be suitable to some kinds of trauma, such as traumatic brain injury, and its long-term effects on to the prognosis should be further evaluated.
2.Endoscopic treatment of small osteoma of nasal sinuses manifested as nasal and facial pain.

OBJECTIVE:
To discuss the clinical features, diagnosis and endoscopic surgical intervention for small steoma of nasal sinuses causing nasal and facial pain.
METHOD:
A retrospective review was performed on 21 patients with nasal and facial pain caused by small osteoma of nasal sinuses, and nasal endoscopic surgery was included in the treatment of all cases.
RESULT:
The nasal and facial pain of all the patients was relieved. Except for one ase exhibiting periorbital bruise after operation, the other patients showed no postoperative complications.
CONCLUSION
Nasal and facial pain caused by small osteoma of nasal sinuses was clinically rare, mostly due to the neuropathic pain of nose and face caused by local compression resulting from the expansion of osteoma. Early diagnosis and operative treatment can significantly relieve nasal and facial pain.

3.Display Principle and Clinical Application of PET-CT

Objective To introduce the display principle and clinical application of PET-CT. Methods The display principle of PET-CT was analyzed. Results The status of tissue or organ were showed by using PET in the form of dissection images and the relative physiological parameters. Advanced CT technology exactly locates and indicates anatomic and functional information of target tissue or organ. Conclusion In combination of PET and CT, PET-CT show high value in clinical application.
4.Development of Optometer Controlled by Micro-computer

Objective To develop a new optometer controlled by micro-computer.Methods With AT89C51 single-chip as hardware and programmed by C language,the instrument applied,the optometer adopted LCD and international standard optotype.The optotype had 14 lines of object-finders,and only one line of random characters was displayed on the screen every time.The optometry distance was 3m.All procedures were controlled by singlechip.Results The case of memorizing eye-chart was avoided and the optometry was every simple.Conclusion The instrument eliminates mirror used for optometry short distance,and the optometry results can be stored and printed.It also has the merits of small volume,precise manufacture,simple operation,low cost and AC/DC power supply.
5.A study of impact of stent implantation in distal common bile duct on duodenal-biliary reflux

Objective This study aimed to investigate the incidence and the cause for duodenalbiliary reflux and reflux cholangitis after metallic stent placement in distal common bile duct Methods After percutaneous transhepatic bile duct puncture and biliary outside drainage was performed, 16 cases with malignant distal biliary stricture underwent metallic stent placement in distal common bile duct Before stent placement, the routine laboratory studies including leukocyte, neutrophil percentage and the levels of total bilirubin and direct bilirubin in blood were performed for all patients. Two to five days [ an average of (3.3 ±0. 9) days ] after stent implantation, the above indexes were tested again, and 1 ml of water containing 185 MBq of 99Tcm-DTPA was given orally before extubation, then 99Tcm radioactivity in the bile was detected 2 hours later. For the measurement data obtained from the experiment, t test or Wilcoxon signed rank test was adopted to compare them, and P < 0. 05 was considered to be statistically different Results In 14 cases, radioactivity was successfully detected in the bile 2-5 days after stent implantation. Twelve of them was detected to have radioactivity in the bile 2 hours before extubation with duodenal-biliary reflux. The technetium count in the bile accounted for 1.82% of the total intake dose. There was no radioactivity in the bile in 2 cases. In 14 patients, there were no symptoms of cholangitis such as high fever, chills, increased jaundice, and so on after stent implantation. The mean of white blood cell count was (7.59 t2. 62) × 109/L, and the median of neutrophil percentage was 0. 74. Compared with those before stent implantation, the difference did not reach statistical significance ( t = 0. 423, Z = 1. 036, P > 0. 05 ).After stent implantation, the median of total bilirubin and direct bilirubin were significantly lower, which were 92. 2 and 74. 3 μmol/L. Compared with those before stenting,the difference was statistically significant (Z= -3. 170, -3. 170, P <0.05). Conclusions There is a high incidence of duodenal-biliary reflux after stent implantation in distal common bile duct in the early stage. However, there is no simultaneous cholangitis caused by duodenal-biliary reflux.
6.The differential diagnosis research between autoimmune pancreatitis and pancreatic cancer on imageological features

Objective To improve the differential diagnosis between autoimmune pancreatitis (AIP) and pancreatic cancer (PC) by a contrast analysis of imageological features.Methods Thirty-six patients who had postoperative pathological with Asian AIP standards and 95 patients who had postoperative pathological consistent with PC.The imageological results of these AIP and PC patients were analyzed.Results AIP was significantly less than PC in the enhanced CT of a mass or enlargement of the pancreatic head,enlargement of the lymph nodes around the pancreas,dilation and interrupt in pancreatic and bile duct,peripheral vascular and organ involvement (11/27 vs.28/40,2/27 vs.17/40,13/27 vs.32/40,1/27 vs.10/40,8/27 vs.26/40,2/27 vs.15/40,0/27 vs.15/40,0/27 vs.10/40,P < 0.05).AIP was significantly more than PC in the enhanced CT of a diffusely enlarged pancreas,calcification or pancreatic calculus,capsule-like rim or the vague peripancreatic fat interval (4/27 vs.0/40,7/27 vs.0/40,10/27 vs.6/40,P < 0.05).AIP was significantly less than PC in the three-dimensional ultrasonography of dilation diameter of pancreatic duct and dilation of common bile duct [(0.421 ± 0.270) cm vs.(0.594 ± 0.270) cm,1/18 vs.16/26,P< 0.05].AIP was significantly less than PC in the magnetic resonance cholangiopancreatography of dilation of common bile duct and interrupt in pancreatic duct (7/13 vs.16/18,1/13 vs.10/18,P < 0.05).Conclusion AIP as a unique type of chronic pancreatitis can be distinguished from PC on distinctive imageological features
7.Clinical application of percutanously interventional therapeusis in patients with hepatocellular carcinoma accompanied with bile duct thrombosis

Objective To explore the method and value of percutanously interventional therapeusis for treatment of obstructive jaundice caused by hepatocellular carcinoma accompanied with bile duct thrombosis.Methods Sixteen cases with bile duct thrombosis proved by pathology and imaging examinations were retrospectively analyzed.According to the clinical symptoms, all the patients received percutaneous transhepatic biliary drainage (PTBD) including permanent external drainage, temporary internal drainage and implantation of covered stents.Serum total bilirubin (TBIL) after the interventional therapeusis were measured and compared with that before the treatments by t test to evaluate the efficacy of these treatments.The relief of clinical symptoms was also reviewed to evaluate the efficacy of these treatments.The patients were followed up within 2 years.Results The PTBD was successfully performed in 16 cases.Permanent external drainage, temporary internal drainage and implantation of covered stents were performed in 2 patients, 7 patients and 7 patients respectively.TBIL after the interventional therapy decreased significantly (t=7.366, P<0.01) to (161.2±80.5) μmol/L averagely from (261.9±77.2)μmol/L before the treatments.All the patients died before the end of followed-up.The average survival time was 204 days (30 to 391 d)and the median survival time was 200 days.Bleeding and infection were the main complications, which could be controlled successfully by routine treatments.Conclusion With high achievement ratio and good efficacy, percutanously interventional therapeusis are good choices for the treatments of obstructive jaundice due to bile duct thrombosis.
8.An Analysis of the Common Deafness Gene Mutation from 356 Non-Syndromic Deafness Patients in Guizhou Province

Objective The study was to investigate the spectrum and incidence of the hot-spot deafness gene mutations from non-syndromic hearing loss(NSHL)in Guizhou province.Methods Genomic DNAs of 356 patients of NSHI in Guizhou were obtained from peripheral blood and a deafness gene test chip was used to screen nine hot spot mutation in the gene GJB2,SLC26A4,GJB3 and mitochondria 12SrRNA.Results Eighty-eight (24.72%)out of 356 patients were found carrying at least one pathogenic gene mutation.Among them,40 patients with the GJB2 mutations were found(11.24%),including 19 cases(5.34%)of homozygous matutions,5 cases(1.40%)of com-pound heterozygous mutations,and 15 cases(4.21%)of single heterozygous mutations.Twenty-nine patients with the SLC26A4 mutations were found (8.15%),including 9 cases(2.53%)of homozygous mutations and 19 cases (5.34%)of single heterozygous mutations.Homogenic mitochondrial 12SrRNA 1555A>G mutations were in 10 patients(2.81%)and 1494C>T mutations in 10 patients (0.56%),and heterogeneous mitochondrial 12SrRNA 1555A>G mutations in 7 patients(1.97%).GJB3 gene c.538C>T heterozygous mutations was found in 1 patients. Conclusion The mutations of GJB2 and SLC26A4 gene are two major pathogenic genes for patients with NSHL in Guizhou province.Our study provides a theoretical basis for the early diagnosis,genetic counseling and treatment of deafness.
9.Correlation between serum interleukin-6 and MRI dynamic changes in patients with acute cerebral infarction

Objective To investigate the function of serum Interleukin-6 (IL-6) in the formation of cerebral edema after acute cerebral infarction through dynamic changes of serum IL-6 and MRI indicators.Methods One hundred and twenty-eight patients with cerebral edema after acute cerebral infarction and 30 healthy were selected as our subjects.The serum IL-6 were measured in at 1,5 and 14 d after acute cerebral infarction.MRI scans were performed in the corresponding time,and then processed synthesis of apparent diffusion coefficients (ADC) graph.Cerebral infarction volume,signal intensity ratio (SIR) of each sequence and relative apparent diffusion coefficient(rADC) were measured and calculated.Results The serum IL-6 in patients with acute cerebral infarction at 1 d after cerebral infarction was significantly higher than that in normal control group and reached the peak level at 5th D after cerebral infarction,significantly decreased at 14th d.The serum IL-6 of each time point were significantly higher than that of normal control group(P =0.000).There were liner positive correlation between the serum IL-6 and the volume of cerebral infarction and SIR of T2 weighted image and fluid attenuation inversion recovery (r =0.750,0.621,0.691 ; P =0.000).The serum of IL-6 and SIR of T1 weighted image was showed negative correlation (r =-0.404,P =0.000).The serum of IL-6 and SIR of diffusion-weighted imaging sequence at 1,5 d after cerebral infarction showed a positive correlation (r =0.678,P =0.000).There was liner negative correlation between the serum IL-6 and rADC at 1 d after cerebral infarction (r =-0.826,P =0.000).Conclusion The rise of serum IL-6 may promote the formation and development of ischemic cerebral edema.
10.The influence of the life quality by the cognitive,behavioral,psychological intervention to the spouses of breast cancer patients receiving chemotherapy

Objective To explore the influence of life quality for the breast cancer patients receiving chemotherapy after the cog-nitive ,behavioral and psychological intervention to their spouse .Methods 120 breast cancer patients received standardized chemo-therapy and their spouses ,and divided into control and intervention groups .The intervention group receive the routine care and health guidance .Before and after chemotherapy ,the life quality of patients was investigated .The data was analyzed statistically .Re-sults The result by the breast cancer patients Quality of Life Questionnaire in Chinese (FACT-B) show that ,the scores of the con-trol and intervention groups in the physiological status ,social/family status ,emotional status ,functional status ,additional attention andtotalscorewere(18.77±4.18,16.48±4.60,17.35±4.41,16.04±4.80,20.81±6.02,89.45±6.34 ;22.46±3.57,19.03± 4 .83 ,18 .58 ± 3 .96 ,18 .59 ± 4 .48 ,24 .73 ± 5 .63 ,103 .39 ± 8 .91) .The scores of intervention groups was increased significantly than the control group .The data was analyzed statistically .Conclusion The quality of life of patients was improved by the cognitive ,be-havioral and psychological guidance and intervention to the spouse of breast cancer chemotherapy patients.
