1.The Diagnostic Value of Multiplanar Reconstruction(MPR) of Spiral CT inDegenerative Disorders of Lumbar Spine

Objective To explore the diagnostic value of multiplanar reconstruction(MPR)of spiral CT in the disorders of lumbar intervertebral discs.Methods Fourty-one cases with the disorders of lumbar intervertebral discs were examined with conventional CT scans and MPR of spiral CT.CT findings of intervertebral discs at L 3/4 ,L 4/5 and L 5/S 1 were compared and observed.Results The conventional scans showed bulging and /or herniation of intervertebral discs in 35 cases,and normal in 6 cases which compression of nerveroots by intervertebral discs or osseous narrow of intervertebral discs were showed on MPR.The MPR showed compression of nerve roots,compression of dural sac or osseous narrow of intervertebral foramens in 36 cases.Conclusion MPR possesses important value in diagnosing the disorders of lumbar intervertebral discs,showing the causes of lumbar vertebral diseases and distinguishing the various diseases of lumbar vertebrae.
2.MR-guided Neurolytic Celiac Plexus Block for Treatment of Upper Abdominal Cancer Pain

Objective To evaluate the value of MR-guided neurolytic celiac plexus block(NCPB)for treatment of upper abdominal cancer pain.Methods 13 neurolytic celiac plexus blocks were carried out in 12 patients with severe upper abdominal pain caused by malignant tumors.The pain-relieving effect of the block was both evaluated using visual analogue scale(VAS) and analyzed statistically in all cases.Results The placement of the needle MR-guided was easy and accurate,the successful rate of the puncture was 92%.There were no severe complications.The pain before and after the procedure had obvious difference and the pain relief could last for a long time.Conclusion MR-guided NCPB is a simple and effective technique for treatment of upper abdominal cancer pain.
3.DRGs-based medical performance evaluation of tertiary hospitals in Yunnan

Objective To identify objective methods for medical service performance evaluation,and means to improve the efficiency of medical services.Methods By means of collection of medical records from 10 tertiary hospitals in Yunnan in 2012,and Diagnosis-related groups of Beijing version(BJDRGs)as a risk-adjustment tool,medical service performance was made to such hospitals.Results The capacity,efficiency and medical safety of hospital of the hospitals were subject to a comprehensive evaluation of their medical service capabilities.In terms of discharged inpatients and total weight,hospital A were found to be the highest(77 458 cases,104 227.5 respectively),and hospital I the lowest(25 450 cases,38 162.28)among these hospitals in 2012; compared with other hospitals,DRG groups of hospitals A,b and D were found higher,while the case mix index(CMI)of hospitals e and I were higher.The expenditure index of hospital C is the lowest(0.9359,0.8514); the mortality in cases of low risk of hospitals B,c and D were the lowest,while hospital F was the highest.Conclusion DRGs as a risk adjustment tool,can significantly improve the reliability,science and continuity of medical service performance evaluation,providing reliable basic data for hospitals accreditation in Yunnan.
4.To assess the value of dynamic contrast-enhanced MRI for diagnosis and difteretial diagonis of prostatic cancer

Objective To assess the role of dynamic contrast-enhanced MRI for diagnosis and differential diagnosis of prostatic cancer. Methods Six volunteers, 32 patients with benign prostatic hyperplasia and 13 patients with biopsy-proven prostatic cancer underwent MR imaging. Dynamic MR with Gd-DTPA (gadolinium-diethylene triamine pentaacetic acid) bolus enhancement was performed followed by post-contrast T 1-weighed imaging. The signal intensive value in dynamic MRI was measured and calculated to draw the time-signal intensity curve of normal peripheral zone, prostatic cancer and benign hyperplasia. Results In dynamic MRI, the normal peripheral zone were enhanced mildly and slowly. The lesion enhancement of benign prostatic hyperplasia in 32 patients were obvious in early phase (60 s) and strengthened gradually, and then went to decrease in late phase (240 s) after peak value. The lesions in 9 of 13 cases with prostate cancer were enhanced obviously in early phase (60 s) and washed out rapidly in late phase, and the peak value was located on early phase. Conclusions In dynamic MRI, the enhancement of normal peripheral zone, prostate cancer and benign hyperplasia were different significantly. Dynamic MRI was very useful in the diagnosis and differentiation of prostate cance.
5.MR Study on the Mesorectum and Mesorectal Fascia

Objective To study MR imaging manifestations of the mesorectum and mesorectal fascia. Methods 100 cases were divided into five groups according to the age. All MR images were retrospectively observed and analyzed by two radiologists.The observing contents included: ①exhibition of the mesorectal fascia, ②the signal intensity of the mesorectum and the pelvic fat and ③the thickness ratio of the mesorectum and the subcutaneous fat in different ages. All data were dealed with SPSS 10.0 software. Results The showing rate of the anterior, posterior, left and right mesorectal fascia respectively were 77, 100, 91 and 93 by MR imaging. The signal intensity of the mesorectum(550.8843) was significantly higher than that(469.8693) of pelvic fat in all five groups(P﹤0.05) and the signal difference between the mesorectum and the pelvic fat could be detected by naked eye. The thickness of the mesorectum and the subcutaneous fat had no association with ages.Conclusion MRI can display the mesorectum and mesorectal fascia clearly. The signal intensity of the mesorectum is significantly higher than that of pelvic fat in five groups. The thickness of the mesorectum and the subcutaneous fat have no association with ages.
6.The value of MR perfusion weighted imaging in normal and abnormal kidneys

Objective To explore the characteristics and the clinical application of MR perfusion weighted imaging(PWI)in the normal kidneys and the renal diseases.Methods Thirty-one subjects including 9 cases without urinary diseases,14 cases with renal carcinoma,6 cases with renal cyst and 2 cases witll renal tuberculosis who had been examined with T1WI,T2WI and PWI were analyzed retrospectively.All the data were processed by a workstation to obtain time-signal intensity curves,color perfusion maps and relative perfusion value.The relative renal blood volume(RBV),relative renal blood flow(RBF),mean transition time(MTT)and the time to peak(TTP)in the normal renal cortex and medulla and the renal lesions were calculated.Comparisons between the right and the left normal kidneys,and between the cortex and the medulla of the normal kidneys were performed using t test,and comparisons between the normal and the abnormal kidneys were performed using q test.Results Relative RBV and relative RBF of the cortex were 1.33±0.08 and 1.44±0.09 respectively,and for medulla were 0.58±0.05 and 0.78±0.13 respectively(t=9.2241 and 5.0336,P<0.01);MTT(1.11±0.08)and TTP (1.04±0.06)of the cortex has no difference with that of the medulla(0.97±0.04 and 0.94±0.03)in the normal kidneys(t=2.2551 and 2.2613.P>0.05).The values of relative RBF of the renal carcinoma (1.35±0.34)were significantly higher than that of the normal tissues(1.02±0.06)(q=3.0882,P<0.01).Conclusion PWI is able to demonstrate the hemodynamic change of the normal renal tissues and the renal lesions,and it maybe an ideal method for showing the functional changes of the kidney and for differentiating the renal diseases.

Objective ERP research has always been the hot spot of the n~ve electrophysiology research.A gTowing body of research suggests that ERP can effectively reflect the cognitive function in patients with depression,evaluate the risk of suicide.This paper reviews the ERP related components with depression and related research.On the basis of the application of ERP in the depression research discusses limitations and prospects on the later development.Methods The articles were retrievaled from databases,such as P UBMED,Foreign medical information resources retrieval platform,CNKI,Wanfang database,ect,from 2013-06 to 2015-01.Index strategy TI:TI:[depression OR affective disorder] AND [suicide OR suicidal] All field:event related potential.Reading and narrowing the scope of the articles,we should understand the experimental design,such as Sample company have suicidal ideation and (or) behavior of patients with depression.Potential induced way,ERP related component detection method,etc.The articles included 43 in English and 15 in Chinese.Read the full text and retain nearly five years about depression and related literature.Results Eventually,the article includes 34 articles and 3 of in Chinese.Conclusion Associated with suicidal ideation and (or) behavior of the existing cognitive dysfunction in patients with depression,ERP related components such as:abnormal P300 and CNV,MMN.N400 have not found abnormal.This article only from the Angle of depression and related research are reviewed in this paper the characteristics of ERP,ERP of other mental illnesses among related research,and to be perfect in the future.

Objective Recent years,epigenetics research mainly focused on DNA methylation.A growing body of research indicated that DNA methylation may play vital role in etiology of affective disorder.With brief introduction of DNA methylation molecular mechanisms and relevance of DNA methylation to affective disorder,this review focuses on some supporting evidence which are recently emerged in major depressive disorder (MDD) and bipolar disorder related gene DNA methylation.Methods We retrievaled related articles from databases,such as PUBMED,Foreign medical information resources retrieval platform,CNKI,Wanfang database,ect,from 2013-06 to 2015-01.Index strategy TI:[depression OR bipolar disorder OR affective disorder] AND All field:DNA methylation.Reading and narrowing the scope of the articles,the experimental design was understanded,such as DNA from either blood cell or brain parenchyma of sample,methods of methylation detection,gene detected and methylation position,etc.The articles included 168 in English and 32 in Chinese.The whole and retained the articles were red about MDD and bipolar disorder related gene DNA methylation in recent five years.Results Eventually,the article includes 29 articles and two of in Chinese.Most of them are receptor gene epigenetics research associated with disease.Conclusion MDD and bipolar disorder are associated with the higher of DNA methylation levels in multiple gene promoter region,such as GR,BDNF,5-HTR,SLC6A4 and the lower of COMT.This article only from the perspective of affective disorder reviewed the relationship between DNA methylation and diseases.None of other diseases epigenetics research,there is to be perfected in future.
9.The impact of pain, anxiety and depression on sleep quality in Chinese patients with neuromyelitis optica spectrum disorders

Background & Objective: Sleep quality in neuromyelitis optica spectrum disorders (NMOSD) were
investigated in two recent studies. However, factors affecting sleep quality have not been studied in
NMOSD. This study aimed to investigate the prevalence of sleep disorders in Chinese outpatient clinics
with NMOSD and its clinical correlates. Methods: We administered Chinese validated self-questionnaires
on HRQOL (MSQOL-54), sleep (PSQI), pain (SF-MPQ-2), anxiety (HARS) and depression (HDRS)
to 42 patients followed up in our outpatient department. We assessed the relationships between sleep
quality with pain, anxiety, depression, gender, age, disability, disease duration, NMO-antibody status
and explored the determinants of poor sleep quality. Results: Sixty four percent of NMOSD patients
were poor sleepers. Significant correlations were found between duration, disability, pain, anxiety,
depression and sleep quality. Disability, depression and the domain of affective descriptors of pain
were the three main predictors of poor sleep in NMOSD.
Conclusion: This study reveals that poor sleep in NMOSD is common and it decreases physical function
of quality of life. It is worthwhile considering exploring adjuvant strategies aimed at controlling pain
associated affect, and treatment of depression may help to improve sleep quality in NMOSD.

10.Effect of Jin's Three-needle Acupuncture Combined with Occupational Therapy on Function of Upper Limbs for Stroke Patients with Hemiplegia

Objective To explore the effects of Jin's Three-needle Acupuncture combined with occupational therapy on function of upper limbs for stroke patients with hemiplegia. Methods 90 patients with stroke were randomly divided into control group (n=30), observation group (n=30) and experiment group (n=30). The control group received routine rehabilitation, the observation group received occupational therapy in addition, and the experiment group received Jin's Three-needle Acupuncture and occupational therapy in addition. They were assessed with Fugl-Meyer Assessment (FMA) of upper limbs and Barthel index (BI) before and 4 weeks after treatment. Results The scores of FMA and BI improved in all the groups after treatment (P<0.001). The difference of scores of FMA and BI before and after treatment were more in the experiment group than in the observation group (P<0.001), observation group than the control group (P<0.001). Conclusion Jin's Three-needle Acupuncture combined with occupational therapy can further improve the motor function of upper limbs and activities of daily living for stroke patients with hemiplegia.
