1.Dexmedetomidine combined with remifentanil used for sedation and analgesia in ICU patients with hypertensive cerebral hemorrhage

Objective To explore the administration of Dexmedetomidine combined with remifentanil for sedation and analgesia of ICU patients with hypertensive cerebral hemorrhage after operation.Methods A total of 60 patients with hypertensive cerebral hemorrhage treated with hematoma removal under craniotomy were selected from May 2013 to June 2015.The patients were randomly (random number) divided into the Dexmedetomidine combined with remifentanil group (D + R, n =30), and Midazolam combined with remifentanil group (M + R, n =30).The blood pressure, respiration rate, oxygen saturation, heart rate, ICP (intracranial pressure), Ramsay sedation scores, and IL-1, and TNF-α levels were recorded after sedation and analgesia in ICU, and 6 h, 24 h, 48 h after operation (T0-T3).Results Compared with M +R group, the MAP, RR, HR, ICP, IL-1β, TNF-α, rate of reoperation for check bleeding, and mortality were significantly decreased in D + R group (P < 0.05), and Ramsay sedation score was significantly increased at the same time (P < 0.05) without excessive sedation and analgesia noticed.Conclusions Dexmedetomidine combined with remifentanil exhibits significant benefit in many respects including control of great fluctuations of blood pressure and intracranial pressure after craniotomy, reduce the production and release of inflammatory mediators, reduce the occurrence of rebleeding after operation.It shows good controllability and safety, it is an optimal method producing sedation and analgesia in ICU patients with hypertensive cerebral hemorrhage after operation.

Objective To study the utility of brainstem auditory evoked potentials in the evaluation of children with cerebral palsy. 3.The protective effects of bovine colostrum truncated IGF-1 on kidney of diabetic rats and the study of mechanism

Objective To observe the effect of BCtIGF-1 on diabetic nephropathy and explore its mechanism. Methods Techniques including biochemistry, radioimmunoassay, fluoroassays, molecular biology(HT-PCR) were used in this study. Results (1) Serum FBG, TG and FMN concentrations in BCtIGF-1 groups were significantly lower than those of diabetic control group; (2) 24h UAER, 24h urinary volume in BCtIGF-1 groups were significantly lower than those of diabetic control group; (3) BCtIGF-1 had protective effect on diabetic nephropathy by observing the kidney tissues with electron microscope; (4) Kidney IGF-1mRNA in diabetic control group was significantly lower than that of normal control group, no differences were found between diabetic control group and BCtIGF-1 groups; (5) Collagen and AGEs of kidney tissue, urinary TXB2/6-keto-PGF1a in BCtIGF-1 groups were significantly lower than those of diabetic control group; Conclusions (1) BCtIGF-1 has hypoglybemic and hypolipidic effects; (2) BCtIGF-1 does not increase the level of IGF-1mRNA in kidney tissue; (3) BCtIGF-1 might have protective effect on diabetic nephropathy. The protective mechanism may be associated with its hypoglycemic and hypolipidic effects; in addition, it might relate to the improvement of PGI2-TXA2 synthetic imbalance and the reduction of the collagen and AGEs levels in kidney tissue.

5.Relationship between oxyhemoglobin saturation in internal jugular vein (cerebral extraction of oxygen) and prognosis of head injury

Objective To observe blood gas analysis of internal carotid artery and internal jugular vein to calculate the cerebral extraction of oxygen, and to investigate the relationship between oxyhemoglobin in internal jugular vein, cerebral extraction of oxygen, and the prognosis of patients with head injury. Method Seventy patients with acute severe head injury in ICU of Taizhou People Hospital were studied, and another 80 patients with mild head injury were enrolled as controls. Twenty-four hours after first aid such as keeping airway open and circulatory and ventilation support, and emergency craniotomy, the blood samples from internal carotid artery and internal jugular vein were collected for blood gas analysis including SaO2, PaO2, SjvO2, PJVO2 > PaCO2, PJVCO2, SaO2-SjvO2, Pa-jvCCO2, CaO2-CjvO2 and Ca-jvO2/CaCO2 (CEO2, cerebral oxygen extraction). Results There were significant differences in SjvO2, PjvO2, Sa-jvO2, Pa-jvO2 Ca-jvO2 and CEO2 between two groups. Conclusions The SjvO2 and CEO2 represent the cerebral oxygen uptake and oxygen consumption precisely, and they can be used to predict the outcome of patients with severe craniocerebral trauma commendabiy.
6.A prospective clinical study of concurrent chemoradiotherapy with nedaplatin for patients with middle-advanced stage uterine cervical carcinoma

Objective To investigate the efficacy and toxicity of concurrent chemoradiotherapy with nedaplatin versus cisplatin for patients with middle-advanced stage uterine cervical carcinoma.Methods 180 patients with middle-advanced stage uterine cervical carcinoma were randomized into concurrent chemoradiotherapy with nedaplatin group (nedaplatin group) and concurrent chemoradiotherapy with cisplatin group (cisplatin group).The short-term efficacy and the toxicity were observed.Results In the nedaplatin group,the short-term response rate、the one-year relapse-free surviva l、one-yea metastasis-free survival、the two-year relapse-free survival、the two-year metastasis-free survival were 98.85%、89.66、86.21% 、85.06% 和 80.46%,But in the cisplatin group,the short-term response rate 、the one-year relapse-free surviva l、one-yea metastasis-free survival、the twoyear relapse-free survival、the two-year metastasis-free survival were 97.60% (x2 =3.07,P > 0.05) 、81.93%(x2 =3.07,P >0.05) 、83.13% (x2 =0.31,P >0.05) 、78.31% (x2 =1.30,P >0.05) 和 80.72% (x2 =0.00,P > 0.05),so there was no significant difference.The incidences of nausea and vomiting in the cisplatin group were 52.27% (grade Ⅰ ~ Ⅳ toxicities),12.50% (grade Ⅲ ~ Ⅳ toxicities),which were higher than those in the nedaplatin group 27.27% (grade Ⅱ ~ Ⅳ toxicities),6.82% (grade Ⅲ ~ Ⅳ toxicities) (P < 0.05),while there were no significant difference in the other toxicities such as anemia,granulocytopenia,thrombocytopenia,diarrhoea between the two groups (x2 =12.18,P > 0.05).Conclusion The efficacy of concurrent chemoradiotherapy with nedaplatin is the same as that of concurrent chemoradiotherapy with cisplatin,and its toxicity is well-tolerated.

With the development of modern radiotherapy techniques,radiotherapy has been widely used in the multimodality therapy for various malignant tumors,including head and neck cancers such as nasopharyngeal cancer and laryngeal cancer.A combination of surgery and radiochemotherapy significantly improves patients' cure rate and survival time;however,with the increase in survival time,some patients receiving radiotherapy develop marked cognitive impairment.Ionizing radiation-induced cognitive impairment mainly nanifests as hippocampus-dependent cognitive impairment,which is associated with inhibited hippocampal neurogenesis due to ionizing radiation.Therefore,it is necessary to investigate the mechanisms of the inhibition of hippocampal neurogenesis by ionizing radiation.This article reviews the molecular mechanism of neurogenesis disorders induced by ionizing radiation.
9.Patient preference for Shexiang Baoxin Pill versus extended release Isosorbide 5-Mononitrate in elderly diabetic patients with angina pectoris

AIM: To gain an insight into preference for elderly diabetic patients with angina pectoris between Shexiang Baoxin Pill (SXBXP) (Moschus, Radix Ginseng, Calculus Bovis, etc.) and Extended Release Isosorbide 5-Mononitrate (ERIM). METHODS: Followed a two-week run-in, 56 elderly patients were randomized to take SXBXP 2 pills tid or ERIM 60 mg qd for 4 weeks; then followed a two-week wash-out, patients crossed over to the complementary therapy for another 4 weeks. Daily diary cards were used to record the side effects, the frequency of angina pectoris attacks and use of rescue medication. At the end of the study, participants were asked to complete a 3-item scale questionary to assess their angina pectoris treatment in terms of convenience, side effects and overall preference. RESULTS: No treatment difference in the frequency of angina pectoris attacks and use of rescue medication was found; 71.4% of patients thought use of SXBXP was the same convenient as that of ERIM; patients felt SXBXP had side effects fewer than ERIM and overall 75% of patients preferred SXBXP. CONCLUSION: Elderly diabetic patients with angina pectoris prefer SXBXP to ERIM.
10.Effect of Gasoline on Barrier Function of Skin

PurposeTo investigate the effect of gasoline on the skin barrier function.MethodsA rat skin model in vitro was used in this study. The amount of 3H-water penetrated throught the skin was applied as the index of evaluating the barrier function.ResultsThe results showed that the amount of 3H-water was not obviously increased in those exposed to gasoline for 0.5,1 and 2 h(P > 0.05), but the 3H-water penetration amount through the skin increased linearly with the time. In the group exposed to gasoline 4 h, the 3H-water penetration amount through the skin was significantly higher than that in the control group (P 0.05). The skin barrier function of the group (expopsed to gasoline 4 h) was disrupted.Conclusions Gasoline may disrupt the skin barrier function. Gasoline would remove the lipids within the intercellular domains of the stratum corneum and then lead to damage the skin.
