1.Clinical features and pathogens analysis of complicated and non-complicated urinary tract infections in infants

Objective To summarize the clinical and pathogenic characteristics of urinary tract infection(UTI)in infants,and to provide reference for clinical diagnosis and treatment.Methods One hundred and eighty-eight cases of hospitalized infants with UTI diagnosis standard in Department of Pediatric Renal Rheumatism Immunology,Shengjing Hospital Affiliated to China Medical University from January 2012 to January 2015 were collected,and on the basis of the imaging they were divided into complex group(complicated UTIs) and non-complex group(non-complicated UTIs),the differences between 2 groups in the general condition,clinical manifestations,serological examination,urine culture and distribution of pathogenic bacteria and drug sensitivity results were analyzed.Results Among 188 UTI infants,148 were male and 40 were female,50 cases were in complex group (26.6%),and 138 cases were in non-complex group (73.4%).In complex group,the prevalence of prenatal ultrasound abnormalities (34.0%),the incidence of recurrent infections(24.0%),the number of fever incidence(78.0%),and peripheral WBC count [(16.4 ± 4.3) × 109/L] were higher than those in non-complex group[1.5%,10.9%,58.7%,(14.6 ± 3.5) × 109/L] (all P < 0.05),and the naked eye hematuria incidence was lower than that of the non-complex group (14.0% vs 34.8%,P =0.006).In complex group,pneumonia klebsiella bacteria positive rate was higher than that of non-complex group (22.0% vs 2.9%,P =0.000),and the positive rate of Escherichia coli was lower than that of non-complex group(26.0% vs 46.4%,P =0.000).There was no difference in the drug sensitivity and resistance of the pathogenic bacteria in the urine culture of 2 groups.Conclusions For fever,increased peripheral WBC,repeated infection and urine culture for Klebsiella pneumoniae UTI infants,in particular,should be alert to the prese-nce of urinary tract abnormalities,timely improve the urinary system ultrasound and urinary tract contrast is very necessary.

3.Simvastatin in combination with aspirinon for diabetic renal microangipathy in patients with inflammation factor

Objective To investigate the clinical significance of serum P-selection,TNF-α,hs-CRP,IL-1β, ICAM in patients with diabetic renal microangipathy, and the therapeutic mechanism of Simvastatin combined with aspirinon in treating renal microangipathy. Methods The serum levels of P-selection,TNF-α, hs-CRP, IL-1β, ICAM were determined by ELISA in all groups. Patients with diabetic renal microangipathy treated with simvastatin and as-pirinon. Results ① The serum levels of P-selection,TNF-α,hs-CRP,IL-1β,ICAM in DM groups were higher than those of normal controls(P<0.01). ②After treatment with simvastatin and aspirinon for 8 weeks in patients with di-abetic nephropathy,the serum levels of P-selection, TNF-α, hs-CRP, IL-1β, ICAM were all decreased significantly compared with those before treatment respectively (P<0.01). After treatment for 16 weeks the above indices re-mained decreased until treatment for 21 weeks (P<0.01). Conclusion ①The plasma levels of Poselection,TNF-α and hs-CRP, IL-1β, ICAM are correlated to the occurance and development of diabetic renal microangiopathy, becom-ing predicted factors in early stage of diabetic renal microangiopathy. ②The therapeutic mechanism of simvastatin and aspirinon in patients with diabetic nephropathy lies probably in the inhibition of these factors expression, thus di-rectly or indirectly affecting the prolffer-ation of mesangial cells and mesangial matrix.
4.Analysis of surveillance results of iodine deficiency disorders in 2013 in Hainan Province

Objective To analyze the coverage of iodized salt and the focus population iodine nutrition after implementing new standard of iodized salt in Hainan Province.Methods Eighteen cities (counties,districts) were divided into eastern coastal,western coastal and inland areas according to coastline and terrain of Hainan Province in 2013.One township (town,street) was sampled from each city (county,district) in the east,the west,the south,the north and the center of these areas.Four villages (committees) and 15 households in each village (committee),and 40 children aged 8-10 in one primary school,20 pregnant women and lactating women were sampled from each township (town,street).Iodized salt samples collected from household were monitored by the method of direct titration;random urine samples of children aged 8-10,pregnant woman and lactating woman were tested by arsenic cerium catalytic spectrophotometry.Results Totally 6 310 edible salt samples were tested,and the median of salt iodine was 25.9 mg/kg.The coverage of iodized salt was 97.92% (6 179/6 310).Residents consumption rate of qualified iodized salt reached 95.47% (6 024/6 310),and the rate of non-iodized salt was 2.08% (131/6 310).About 4 444 urine samples of children aged 8-10 were examined,and the median of urinary iodine was 165.4 μg/L,in which suitable urinary iodine (100-199 μg/L) accounted 49.3% (2 194/4 444);about 1 286 urine samples of lactating woman were examined,the median of urinary iodine was 122.3 μg/L,≥ 100 μg/L was 60.6% (779/1 286);about 813 urine samples of pregnant woman were examined,the median of urinary iodine was 126.6 μg/L,59.6% (485/ 813) of this group were urinary iodine deficiency (< 150 μg/L).There were significant differences between the three groups in the east,the middle and the west areas of Hainan Province (children:176.7,147.7,158.0 μg/L,lactating women:148.7,108.7,106.7 μg/L,pregnant women:146.7,108.0,147.1 μg/L,H =34.70,26.76,35.95,all P < 0.01).Conclusion The results have showed that pregnant woman are iodine deficiency,they should be given more iodine.

This article was inspired by the paper that Clinical Research of Traditional Chinese Medicine NeedsA Breakthrough, which was written by Shang Hongcai. This article took the 11th Five-year Plan of the National Science and Technology Support Project and the National Public Welfare Projects of Chinese Medicine, which were undertaken by the Heilongjiang Gynecology , which is one of the National Trial Bases of Chinese Medicine as an example to illustrate the Chinese medicine clinical trial management , achievement producing , and realization of achievement transformation . The emphasis should be made on the optimization of Chinese medicine clinical trial from the topic selection , operation mode and quality control . A comprehensive and detailed management system should be worked out to ensure the smooth conduction of clinical trials. The clinical research results should be clarified to achieve high quantity and quality outputs . The article was aimed to provide references for the development and breakthrough of Chinese medicine clinical trials .

To review the research of block copolymer micelles as delivery system for poorly soluble(antineoplastic) carrier over the last 10 years,including the composition,preparation methods and factors which influence the loading efficiency,physicochemical properties,and targeting characteristics of block copolymer micelles.Block copolymer micelles are self-assembled structures formed from amphiphilic block copolymers by dispersing in aqueous media.The hydrophilic blocks of the copolymer form the outer shell of the micelle,while the hydrophobic blocks form the inner core,and the proper coreshell micellar architecture was constituted.Block copolymer micelles have a whole set of unique properties,such as small sizes,narrow particle size distribution,high drug loading capacities,and available disposition characteristics in the body.Block copolymer micelles have been found as promising drug carriers due to making poorly soluble antineoplastic lysis,toxicities and side effects decrease,bioavailability increase,and targeting the drugs to specific sites in a passive way or attaching ligands in an active way,which can be specifically(recognized) by receptors onto the surface of copolymers.
9.Antioxidative effect of sodium ozagrel on myocardial ischemia in rats

Objective To disccuss the antioxidative effects of sodium ozagrel(OZG) in myocardial ischemia in rats and illuminate its effect of decreasing oxidative damage.Methods Forty healthy Wistar rats were randomly devided into five groups:control,model,pretreatment,treatment and therapeutic alliance groups.OZG was used for 2 weeks in pretreatment group,then isoprenaline hydrochloride (ISO) was applied to make myocardial ischemia model.5 min after the establishment of model,OZG and OZG combined with glyceryl trinitrate(GT) were used in treatment group and therapeutic alliance group,respectively,ECG was recorded at the same time.1 h later,the activities of serum myocardial creatases(AST,LDH,CK),malonaldehyde(MDA) level and superoxide dismutase(SOD) activity were detected.Results ECG:When the model was successfully set up,ST segment in every group all raised≥0.1 mV,the minimum value of raise was in pretreatment group.When the model was succeeded 5 min later,ST segment in model group slightly decreased but still ≥0.1 mV and it was significant in pretreatment group.In treatment and therapeutic alliance groups,compared with model succeeding'ST segment,ST segment decreased significantly at 5 min later(P
10.Ultrastructural changes and significance of endometriotic rat model with HCG treatment

AIM:Toinvestigatewhetherandhowhumanchorionicgonadotropin(HCG)treatmentameliorates endometriosis in the endometriotic rat model .METHODS:The rat model of endometriosis was established and the model rats were divided into 4 groups.The rats in HCG groups were treated with 19.4, 25.8 and 51.6 IU/100 g of HCG every day (low-dose HCG, medium-dose HCG and high-dose HCG, respectively).The rats in control group were treated with 0.9%NaCl.After 15 days (3 estrous cycles), the ectopic lesion volume and ultrastructural characteristics in eutopic and ectopic endometria were investigated .RESULTS: After HCG treatment , the volume of endometriotic lesions was signifi-cantly smaller than that before treatment .Numerous and mitochondrial , endoplasmic reticulum and ribosomes were ob-served in the cytoplasm of eutopic and ectopic endometrium before treatment .After treatment , some cell structures were not clear , and mitochondrial cristae decreased or disappeared partly .Some cells were densed and shrinkage , autophagosome in cytoplasm increased , and mitochondria and endoplasmic reticulum swelt .CONCLUSION:HCG therapy appears to be an effective treatment for endometriosis in rats attributed to its influence on cell metabolism dysfunction of eutopic and ectopic endometria .
