Objective To investigate the clinical significance of serum P-selection,TNF-α,hs-CRP,IL-1β, ICAM in patients with diabetic renal microangipathy, and the therapeutic mechanism of Simvastatin combined with aspirinon in treating renal microangipathy. Methods The serum levels of P-selection,TNF-α, hs-CRP, IL-1β, ICAM were determined by ELISA in all groups. Patients with diabetic renal microangipathy treated with simvastatin and as-pirinon. Results ① The serum levels of P-selection,TNF-α,hs-CRP,IL-1β,ICAM in DM groups were higher than those of normal controls(P<0.01). ②After treatment with simvastatin and aspirinon for 8 weeks in patients with di-abetic nephropathy,the serum levels of P-selection, TNF-α, hs-CRP, IL-1β, ICAM were all decreased significantly compared with those before treatment respectively (P<0.01). After treatment for 16 weeks the above indices re-mained decreased until treatment for 21 weeks (P<0.01). Conclusion ①The plasma levels of Poselection,TNF-α and hs-CRP, IL-1β, ICAM are correlated to the occurance and development of diabetic renal microangiopathy, becom-ing predicted factors in early stage of diabetic renal microangiopathy. ②The therapeutic mechanism of simvastatin and aspirinon in patients with diabetic nephropathy lies probably in the inhibition of these factors expression, thus di-rectly or indirectly affecting the prolffer-ation of mesangial cells and mesangial matrix.

Objective To study the influence of interleukin(IL)-1β,intercellular adhesion molecule(ICAM)on diahetic renal microangiopathy.Methods Sixty-two patients with type 2 diabetic mellitus were divided into three groups:normal albumin urine group(20 patients),microamount albumin urine group(22 patients),and dominancy albumin urine group(20 patients)according to their urinary albumin excretion rate(UAER).IL-1β,ICAM was measured by enzyme-linkedimmuno sorbent assay(ELISA)method in the type 2 diabetic mellitus and the healthy persons.Results The concentration of IL-1β,ICAM in the three groups with type 2 diabetic mellitus was higher than that of the twenty healthy persons(P＜0.05),higher in the dominancy albumin urine group than that in the microamount albumin urine and normal albumin urine groups(P＜0.01).The concentration of IL-1β in the microamount albumin urine group was higher than that in the normal albumin urine group(P＜0.05),the concentration of ICAM in the microamount albumin urine and normal albumin urine groups was equal(P＞0.05).Higher serum IL-1β and ICAM was associated with higher UAER(P＜0.05).The concentrations of IL-1βwas positively correlated with ICAM.Conclusion ICAM,IL-1β is probably correlated to UAER of diabetic renal microangiopathy,80 it has very important meaning and clinic value to measure the concentration of IL-1β,ICAM in serum earlier.

Objective To evaluate the effects of static magnetic fields (SMFs) of different intensity and exposure duration on the proliferation and apoptosis of human umbilical cord endothelial cells (HUVECs),and their release of nitric oxide (NO),6-keto-prostacyclin 1α (6-keto-PGF1α) and endothelin (ET-1).Methods Cultured HUVECs were exposed to a SMF at 5,22,86 or 135 mT for 8,12 or 24 hours.Their proliferation and apoptosis were monitored by flow cytometry (FCM).The medium was collected to test its NO content by optical density.ET-1 and 6-keto-PGF1α were measured by radioimmunization.Results ( 1 ) The proliferation of HUVECs increased when the cells were exposed to a SMF at 5 mT for 8 h,but a SMF at 135 mT for 12 h or 24 h inhibited the proliferation of HUVECs.(2)An SMF had no effect on apoptosis of HUVECs.(3)An SMF at 5 mT for 8 h increased the release of NO and 6-keto-PGF1 a,but the release of NO and 6-keto-PGF1 a decreased when the SMF intensity was 135 mT or the cells were exposed to an SMF for 12 h or 24 h.(4) An SMF at 5 mT or 22 mT for 8 h did not effect the release of ET-1.An SMF at 86 mT or 135 mT increased the release of ET-1.Compared with a control group,an SMF at 5 mT for 12 or 24 h did not affect the release of ET1,but at 22,80 or 135 mT,the release of ET-1 decreased significantly.Conclusions Exposure to a low intensity SMF for a short duration could improve the proliferation of HUVECs and increase the release of vasoactive factors,but if HUVECs are exposed to a strong SMF or exposed for a long duration,the proliferation and the release of vasoactive factors is decreased.

Objective To investigate the efficacy and toxicity of concurrent chemoradiotherapy with nedaplatin versus cisplatin for patients with middle-advanced stage uterine cervical carcinoma.Methods 180 patients with middle-advanced stage uterine cervical carcinoma were randomized into concurrent chemoradiotherapy with nedaplatin group (nedaplatin group) and concurrent chemoradiotherapy with cisplatin group (cisplatin group).The short-term efficacy and the toxicity were observed.Results In the nedaplatin group,the short-term response rate、the one-year relapse-free surviva l、one-yea metastasis-free survival、the two-year relapse-free survival、the two-year metastasis-free survival were 98.85％、89.66、86.21％ 、85.06％ 和 80.46％,But in the cisplatin group,the short-term response rate 、the one-year relapse-free surviva l、one-yea metastasis-free survival、the twoyear relapse-free survival、the two-year metastasis-free survival were 97.60％ (x2 =3.07,P ＞ 0.05) 、81.93％(x2 =3.07,P ＞0.05) 、83.13％ (x2 =0.31,P ＞0.05) 、78.31％ (x2 =1.30,P ＞0.05) 和 80.72％ (x2 =0.00,P ＞ 0.05),so there was no significant difference.The incidences of nausea and vomiting in the cisplatin group were 52.27％ (grade Ⅰ ～ Ⅳ toxicities),12.50％ (grade Ⅲ ～ Ⅳ toxicities),which were higher than those in the nedaplatin group 27.27％ (grade Ⅱ ～ Ⅳ toxicities),6.82％ (grade Ⅲ ～ Ⅳ toxicities) (P ＜ 0.05),while there were no significant difference in the other toxicities such as anemia,granulocytopenia,thrombocytopenia,diarrhoea between the two groups (x2 =12.18,P ＞ 0.05).Conclusion The efficacy of concurrent chemoradiotherapy with nedaplatin is the same as that of concurrent chemoradiotherapy with cisplatin,and its toxicity is well-tolerated.

AIM:To explore the characteristics of hepatitis B virus S gene mutation in the vertical transmission after active and passive vaccination .METHODS:Fifteen cases of immunoprophylaxis failure were enrolled in the study . HBV S gene (including pres-S and S) from the mothers, newborns before active and passive vaccination and 7-month-old infants with immunoprophylaxis failure were detected by PCR amplification .The characteristics of HBV S gene mutation were compared among the 3 groups.RESULTS: The genotype of HBV in the newborns and the infants was the same as that in the mothers .The frequencies of mutation in the 2 fragments of the HBV S gene had no significant difference between the 3 groups.The homology tree model based on HBV S gene was analyzed in the 3 groups, in which every group had their own cluster.There were 15 different mutation sites between 7 pairs of mothers and newborns .There were 3 different muta-tion sites between 3 pairs of newborns and infants (nt273A→A/G, nt512C→C/T and nt1139C→A), among which the first 2 were located in the S gene region but not in the “a” determinant , and the latter was located in the overlap region of S and X genes .There were 25 different mutation sites between 9 pairs of mothers and infants , but only 1 case had a differ-ent mutation site between the mother , newborn and infant .CONCLUSION: The HBV species in newborns and infants with immunoprophylaxis failure were transmitted from the mothers .The mutations in the HBV S gene with immunoprophy-laxis failure happened before and after active and passive vaccination , mainly before vaccination .The relationship between HBV S gene mutations and immunoprophylaxis failure should be further explored .

Background It is essential to measure and assess the parameters of ocular anterior segment for refractive surgery in myopic eyes.Some different imaging devices can be used for biometric measurement of ocular anterior segment,but which is more accurate and convenient is still under investigation.Objective This study was to compare the anterior segment parameters in myopic eyes measured by anterior segment optical coherence tomography (AS-OCT),Orbscan topography and ultrasonic pachymetry (US).Methods One hundred and forty eyes of 70 myopic subjects with the diopter of-0.75 to-10.25 D,who intended to receive corneal refractive surgery in the Second Affiliated Hospital of Zhejiang University School of Medicine from November 2011 to May 2012,were retrospectively analyzed.Central corneal thickness (CCT) was measured using AS-OCT,Orbscan Ⅱ and US,respectively,and anterior chamber depth (ACD) was measured by AS-OCT and Orbscan Ⅱ,and the angle to angle (ATA) distance and corneal white-to-white corneal distance (WTW) were measured by AS-OCT and Orbscan Ⅱ,respectively.The parameters from different apparatuses were statistically compared.Results The mean CCT were (516.57±30.25) μm in AS-OCT,(523.68±31.87) μm in US and (514.69±38.40) μm in Orbscan Ⅱ,without significant difference among them (F =2.775,P =0.063).Then the patients were divided into three groups based on the US measurement of CCT (＜500 μm group,500-569 μm group,and ≥ 570 μm group).In the ＜500 μm group,there was a significant difference in the CCT among the three methods (F =22.236,P =0.000),significant differences were found between AS-OCT and Orbscan Ⅱ,or Orbscan Ⅱ and US(both at P＜0.05).In the 500-569 μm group,there was no significant difference in the CCT among the three methods (F =3.011,P =0.051).In the ≥ 570 μm group,there was a significant difference in the CCT among the three methods (F =4.133,P =0.021),a significant difference was found between AS-OCT and US(P＜0.05),but there was no significant difference between AS-OCT and Orbscan Ⅱ (P＞0.05).The ACD values measured by AS-OCT was (3.83±0.21) mm,which was higher than (3.75 ± 0.21) mm by Orbscan Ⅱ,with a significant difference between them (t =-8.520,P =0.000).In addition,the ATA value by AS-OCT (12.43 mm±0.74 mm) was higher than the WTW value (11.42 mm±0.33 mm) by OrbscanⅡ,with a significant difference between them(t=-18.088,P=0.000).Conclusions AS-OCT,US and Orbscan Ⅱ can offer accurate CCT value,and they can provide references to one another before refractive surgery.However,the ACD,ATA and WTW values by AS-OCT and Orbscan]Ⅱ have large differences.

Objective To investigate the correlation between the clinical pathological factors and the expression of EGFR, HER2 and HER3 in non-small cell lung cancer tissues. Methods The expression of EGFR, HER2 and HER3 in non-small cell lung cancer tissues was detected by MaxVision immunohistochemical. The relationship between the protein expression and the clinical pathologic characteristics was studied. The correlation between the protein expression and the survival was also retrospectively analyzed. Results Increased expression of EGFR, HER2 and HER3 was detected in the NSCLC tissues and their rates were 45.9 % (73/156), 30.8 % (49/156) and 21.4 % (37/156), respectively. There were significant associations between the expression of these proteins and lymph node metastasis, tumor differentiation degree and TNM staging. Conclusions Overexpression of EGFR, HER2, HER3 or all three proteins is a predictor of poor prognosis. Moreover, combined detection of all 3 markers may work better than the individual detection in the prediction of the prognosis to guide the clinical treatment.

Based on the conservation of nucleotides at splice sites, the characteristics of base composition and base correlation in the adjacent segment sequences, the distance between alternative donor or acceptor splice sites and the content of GC and TC near splice sites, the donor and acceptor splice sites for alternative and constitutive introns are predicted by use of the method of diversity measure combined with quadratic discriminant analysis. For alternative splice sites the total prediction accuracies are 87.9% and 89.9% for donors and acceptors respectively (with the chosen threshold -2). For constitutive splice sites the total accuracy are 92.8% and 94.3% for donors and acceptors respectively (with the chosen threshold -1).

Objective To investigate the effect of mycophenolic acid on the proliferation of smooth muscle cells (SMCs) in rats' pulmonary arterial at cellular level. Methods Growth curve, MTT tests, and flow cytometry were used to detect the number of proliferative cells, the A-value of living cells and the DNA content of the control and drugs groups respectively and the number of cells in G1-phase, S-phase, G2M- phase and the proliferation index were calculated. Results Compared with the control group, the number of living cells in the mycopbenolic acid groups (with the concentration of 1, 10, 100 μmol/L) decreased, except the lowest dosage group (100 nmol/L). The difference was statistically significant (P<0.01). The living cells measured by MTT dose-dependently reduced in the mycophenolic acid groups. In the mycophenolic acid groups, the fraction of living cells in the S-phase and G2M-phase decreased respectively while that in G1- phase increased, and the proliferation index decreased. All these responses presented with a dose-dependent manner. Conclusion Mycophenolic acid can effectively inhibit the proliferation of rats' pulmonary arterial SMCs. This process happens mainly in ONA synthesis phase, and is dose dependent. In addition, these effective concentrations are all in the available range for clinical application.
