1.Problems and Countermeasures of Public Participation in the Food and Drug Safety Administration in Chi-na

OBJECTIVE:To perfect public participation system of food and drug safety in China and establish“society multi-governance”of food and drug safety. METHODS:The problems of public participation in food and drug safety administration were analyzed in China. Referring to the experience of USA,Japan,South Korea and other developed countries in stage of food and drug safety legislative decision-making,regulation and enforcement and dispute settlement,the countermeasures were put forward to perfect public participation system of food and drug safety. RESULTS & CONCLUSIONS:The public is in a passive position and shows weak right of speech in legislative decision-making stage;the public is not active and shows weak awareness of partici-pating in management in regulation and enforcement stage;public interest litigation system in dispute settlement stage has not been established. Those problems impede the generation of“society multi-governance”of food and drug safety in China. In United States,Japan,South Korea and other developed countries,the position and right of the public participation can be guaranteed com-pletely through establishing complete legal system,strengthening government information transparency,improving complaints sys-tem and public interest litigation system,etc. Our country can perfect the system of public participation in the food and drug safety and promote the generation of“society multi-governance”of food and drug safety by detailing the regulations in legislative deci-sion-making,emphasizing the information opening and feedback,strengthening the propaganda of public responsibility conscious-ness,setting up scientific food and drug safety complaint and reparting system and establishing public interest litigation system.
2.Immunohistochemical Detection of Ras p21 in Prostatic Carcinoma and Prostatic Dysplasia

An expression of ras oncogene protein (p21) was assessed with immunohistochemistry in normal, dysplasia, benign hyperplasia, chronic prostatitis and prostatic carcinomatous tissues. The results showed that the positivity rate of p21 was higher in prostatic carcinoma than that in other prostatic lesions and was markedly correlated to histologic tumor grade. The expression level of p21 was more intentive in prostatic dysplasia surrounding carcinoma than that in simple prostatic dysplasia. It is suggested that overexpression of ras oncogene p21 product may play an important role in progressing from dysplasia to cancer and may be used as a new tumor marker for assessment of biological behavior of prostatic carcinoma.
3.The Study of Peanut Aggiutinin Receptor in Human Pancreatic Cancer

This histochemical binding of horseradish peroxidase labeled peanut agglutinin (HRP?PNA) to paraffin sections of 18 cases of pancreatic cancer was investigated with 5 case of normal pancreas and 8 pancreatitis as the control. The PAN receptor was not found in the normal pancreas, but it appeared in 15 cases of pancreatic cancer (82%). Three cases of pancreatitis presented positive stainings of PNA binding (37%). There were 3 kinds of PNA binding forms: (1) linear distribution along the gland of carcinoma cells; (2) round staining with the ductal epithelium; and (3) diffuse scattering with an increased dot staining. There was a good relationship between the staining degree and cell differentiation. The positive staining increased when the cell was transformed from inflammation to carcinogenesis, and decreased from well differentiation to poorly differentiation.
4.A study on heart rate variability in elderly patients with sleep apnea/ hypopnea syndrome

Objective To study the changes of heart rate variability (HRV) and the effects of treatment with nasal continuous positive airway pressure (nCPAP) in elderly patients with sleep apnea/ hypopnea syndrome (SAHS). Methods SAHS patients and controls were examined by polysomnography(PSG) and dynamic cardiograph(DCG). Mean RR interval(Mean RRi), standard deviation of RR interval (SDRRi), low frequency(LF), high frequency(HF) and their ratio(LF/HF) were calculated. Results When compared with controls(847.9?113.8)ms vs (45.7?16.2) ms, there was significantly decrease in the Mean RRi and SDRRi in moderate, severe elderly patients and non-elderly SAHS patients(764.3?131.0)ms, (709.4?101.8)ms, (759.5?80.0)ms and (37.5?12.2)ms, (31.5?9.6)ms, (41.4?10.6)ms. 5.Clinic Analysis of 35 Cases of Hyperthyroidism Featuring in Vomit Mainly

Objective To disuss the clinic characteristics,the etiologies and the misdiagnosis cause of hyperthyroidism featuring in vomit mainly.Methods The clinic data of 35 hyperthyroids featuring in vomit mainly were reviewed and analyzed.Results The hyperthyroids in this group were middle-aged female mainly whose symptoms showd that they vomit frequently with abdominal distention,thin,inappetence,chest congestion,palpitation;tachycardia;hypohepatia and electrolyte disturbance.Furthermore,their thyroid gland function assay is in concordance with the hyperthyroidism diagnosis.Treatment for antihyperthyroidism can get remarkable effects.Conclusion The patient showing vomit of unknown origin may be taken into account as hyperthyroid.The key points to prevent misdiagnosis are to detect thyroid gland function in time and to improve the understanding for un-typical hyperthyroidism.
6.Effect of Enalapril on lipoprotein(a) and oxygen-derived free radicals in patients with acute myocardial infarction

BACKGROUND: Many studies have suggested that angiotensin-converting enzyme inhibitors(ACEI) protect blood vessels through anti-atherosclerosis independent of lowering blood pressure, but its mechanism is still unclear.OBJECTIVE: To investigate the anti-atherosclerotic mechanism of ACEI by observing the effects of Enalapril on lipoprotein(a) and oxygen free radicals in patients with acute myocardial infarction (AMI) .DESIGN: A controlled study based on the observation of the patients with AMI.SETTING: Second Department of the South Building, General Hospital of Chinese PLAPARTICIPANTS: Thirty-five inpatients with AMI(19 males and 16 females, aged 42 -75 years old, and averaged (62 ± 9) years old and hospitalized at the Department of Cardiology of Tianjin Harbor Hospital from April 2001 to August 2002 were chosen. These patients were randomly divided into 2 groups: the therapeutic group(20 cases) and the control group(15cases). Inclusion criteria: the diagnosis of patients with AMI was confirmed by WHO criteria. Exclusion criteria: patients with renal dysfunction, shock,hypotension, a history of allergy to ACEI, and a history of severe cough induced by ACEI. All patients had not taken ACEI in the past 2 weeks and agreed to participate in this study.METHODS: On early morning of the third day after AMI, patients in the treatment group took 5 mg of Enalapril one time. If they had no first-dose reaction of hypotension, on the fourth day after AMI, the patients of the treatment group were given a dose of 5 mg twice per day for the following 2 weeks. Then, they were given the drug at a dose of 10 mg twice per day for 2 weeks. The patients in the control group were not given Enalapril. Blood samples were taken respectvely prior to the administration and 2 weeks and 4 weeks after the administration in the two groups. Serum content of lipoprotein(a), oxygen free radicals, triglyceride, total cholesterol, high density lipoprotein(HDL) cholesterol, Apo(a) were measured.MAIN OUTCOME MEASURES: We compared the level of serum of lipoprotein (a), oxygen free radicals (OFR), triglyceride, total cholesterol,HDL cholesterol, Apo(a) pre-treatment and post-treatment respectively in patients of the two groups.RESULTS: Serum levels of OFR were significantly lowered in the treatment group, which were(1 423.14±216.23), (1 076.62±287.12) and (566.57 ± 138.02) U/mL respectively 2 weeks and 4 weeks before and after the treatment(t =2. 937, 3. 571, P <0. 01), but there were no significant changes in serum concentrations of lipoprotein(a) and lipids( P > 0.05) .CONCLUSION: Enalapril improved the prognosis of patients with AMI by antioxidation, but not by lowering the serum levels of lipoprotein(a) and lipids. The study can serve as a theoretical reference that the mechanism of Enalapril might inhibit atherosclerosis in patients with AMI.
7.Transcription factors in spinal dorsal root ganglion in rats recurrently infected with respiratory syncytial virus

Objective To elucidate intracellular transcription factor activation of C_7~T_5 dorsal root ganglia in rats recurrently infected with respiratory syncytial virus (RSV). Methods Eighty 1~2 weeks old Sprague-Dawley rats were randomly divided into 2 groups: a control group and a RSV-infection group. The rats in the RSV-infection group were infected with 5 ×10~5 U/mL RSV once a week and the rats in the control group were treated with culture medium without RSV. Airway response was measured after 8 weeks. Lung tissue was submitted for HE staining and in situ hybridization. The C_7~T_5 dorsal root ganglia were obtained for the preliminary screening of the intracellular transcription factors by TranSignal~(TM) protein/DNA combo array. Nuclear protein of C_7~T_5 dorsal root ganglia were extracted and submitted to Western blot. Results Airway response in the RSV-infection group was higher than that in the control group (P<0.05). HE staining showed inflammatory cell infiltration, and in situ hybridization demonstrated positive RSV RNA in the RSV-infection rat lung which was not present in the control group, thus validating the efficacy of our model. TranSignalTM protein/DNA combo array screening showed that 55 transcription factors increased by at least 2 folds in the C_7~T_5 DRG cells of the RSV-infection group. The transcription factors Smad and interferon regulatory factor (1 or 2) were the 2 most upregulated transcription factors identified by combo array screening (59 and 43 fold increase compared with the control, respectively). Western blot confirmed Smad(1/2/3) and IRF-1 upregulate while IRF-2 remained unchanged. Conclusion Respiratory syncytial virus infection results in airway hyperresponsiveness and transcription factor activation in C7~T5 spinal adorsal root ganglia in rats, which may contribute to airway nerve network dysfunction and airway hyperresponsiveness.

9.Study on the removal method of electrogastrogram baseline wander based on wavelet transformation.

This paper presents a removal method of electrogastrogram (EGG) baseline wander based on wavelet transformation. The basic idea of this method is using the low-frequency signal which is obtained through multi-scale decomposition of EGG signals to approximate the baseline wander of EGG, so the component of baseline wander is filtered out from the sampling EGG signals. The method was applied successfully to process the experimental data of dog EGG in our laboratory. The experimental data and analysis of results showed that this method could filter out the baseline wander of EGG, and this method would not affect the gastric spike and slow wave bandwidth signals, which could be shown from the characteristics of bandwidth filter of wavelet transformation.

10.The application of wavelet transform and wavelet lifting in signal processing of EGG.

This paper introduced the definition, structure of basis wavelet functions and wavelet lifting. Wavelet transform and wavelet lifting were applied in signal processing of EGG in our study. The characteristics of signal energy, information entropy and joint entropy were analyzed to introduce general selective method of wavelet basis functions after the signals have been filtered. In order to verify the rationality of evaluation criteria, signals of electrogastrogram (EGG) were processed and filtered with different wavelet basis functions in the experiments. Signals of EGG were filtered by wavelet transform and wavelet lifting, slow wave and spike wave of EGG were filtered. And the rationality of wavelet transform, wavelet lifting and effectiveness of EGG filter algorithm were proved by the experimental data. It provided an effective solution for the diagnosis and measurement of gastric diseases.
