1.Correlation between white matter alterations and cognitive function decline in early Alzheimer's disease

ObjectiveTo investigate the effects of early stage Alzheimer's disease(AD)on white matter(WM)integrity using diffusion tensor imaging(DTI)and its relationship with cognitive function decline.MethodsDTI was performed in 32 subjects,including 14 early AD patients and 18 elder controls(ON)with a 1.5 T MR scanner. Fractional anisotropy(FA)and mean diffusivity(-D)values were computed and compared for 9 regions of interest(ROI).Eight standard neuropsychological tests were performed and compared between AD and ON to evaluate basic cognitive capacities of AD.Correlation analysis was applied between FA,(-D) values and scores of neuropsychological tests for all subjects.ResultsFA significantly decreased in splenium of the corpus callosum and the posterior parietal-temporal region(S2),and D significantly increased in the splenium in AD patients(P<0.05).AD patients showed lower scores compared with ON in all neuropsychological tests(P<0.05).FA of the splenium and S2 positively correlated with several tests scores,while (-D) of multiple ROIs negatively correlated with several tests scores (P<0.05).ConclusionsIn the early stage of AD,neuropathology has effect not only on cognitive function,but also on white matter structure,and they have strong relationship.AD patients show white matter changes in specific regions,which reflect loss in cortico-cortical connections.
2.Overexpression of N-terminal amino acids of p55γregulatory subunit of PI3K inhibits cell adhesion of human gastric carcinoma MGC803 cells

AIM:To investigate the effect of the N-terminal 24-amino acid (N24) overexpression in p55γre-gulatory subunit of phosphatidylinositiol 3-kinase ( PI3K) on the cell adhesion of human gastric carcinoma cell MGC 803. METHODS:MGC803 cells, which stably expressed GFP-N24 fusion protein and GFP alone , were generated by transfec-tion with pEGFPN-24 plasmid and control plasmid pEGFP-C1, respectively.The morphological change of the cells was ob-served under inverted microscope .The expression of GFP-N24 fusion protein was detected by Western blot .The adhesion of the cells was determined by cell adhesion assay .The effects of GFP-N24 fusion protein on the expression of E-cadherin andβ-catenin were analyzed by Western blot .The expression and secretion of matrix metalloproteinase 9 (MMP9) and u-rokinase-type plasminogen activator ( uPA ) in the MGC803 cells were detected by gelatin zymography .RESULTS:MGC803/GFP-N24 cell line steadily expressed GFP-N24 fusion protein and MGC803/GFP cell line steadily expressing GFP were successfully established , but the expression of fusion protein GFP-N24 was lower than that of the control protein GFP.The morphological changes of the transfected cells from paving stone to fibroblast cell form after gene transfection , and the cytoplasm secretory granules were increased significantly .The cell adhesion to fibronectin and collagen decreased after GFP-N24 transfection .GFP-N24 fusion protein increased the expression of cell adhesion molecule E-cadherin and de-creased the wnt signal pathway molecule β-catenin in the MGC803 cells.However, it did not affect the expression and se-cretion of tumor metastasis-related proteins MMP9 and uPA.CONCLUSION:Overexpression of N24p55γinhibits cell ad-hesion by influencing the expression of E-cadherin and β-catenin in the MGC803 cells.
3.The application of sliding interleaved Ky(SLINKY) technique in MRA of head and neck

Objective To study the application of SLINKY technique in MRA of head and neck by comparison of four TOF MRA methods. Methods We obtained images on 21 patients and 5 volunteers with SLINKY on all the 26 cases, MOTSA on 18 cases, single volume on 16 cases, and 2D on 12 cases. Three experienced radiologists evaluated the images, unknowing which method the images were. The evaluations included clearness of vessel branches, smoothness of vascular wall, and vascular continuity. Signal-to-noise (SNR), contrast-to-noise (C/Ns), and scan time were measured. 4.A retrospective study of the application value of 18 F-FDG PET/CT in patients with fever of unknown origin

Objective To evaluate the application value of 18 F‐FDG PET/CT in patients with fever of unknown origin (FUO) to improve the etiological diagnosis accuracy of PET/CT in patients with FUO and provide better service for clinical prac‐tice .Methods Clinical data and imaging results of 144 cases of FUO patients diagnosed through 18 F‐FDG PET/CT examination in Wuhan union hospital from December 2009 to December 2011 were retrospectively analyzed .The final diagnoses were based on sur‐gical pathology ,aspiration cytology ,or discharge diagnosis combined with more than 12 months of follow‐up results .All the PET/CT images were analyzed and the clinical application value of PET/CT in FUO patients were evaluated through the diagnose rate , diagnose efficiency and clinical staging of malignant tumor .Results Among the 144 patients ,there were 130 cases ultimately had clear diagnosis ,which including 41 cases of malignant tumor ,89 cases of benign lesions and 14 cases the cause of unknown .PET/CT detected abnormal lesions for 97 .6% (40/41) of patients with malignant lesions and 88 .7% (79/89) of patients with benign le‐sions ,and the diagnostic sensitivity ,specificity ,accuracy ,positive predictive value ,and negative predictive value were :97 .6% (40/41) ,86 .5% (77/89) ,90% (117/130) ,76 .9% (40/52) and 98 .7% (77/78) .PET/CT detected the primary lesions for the first time in 46 .3% (19/41) patients with malignant tumor ,and detected more lesions than the traditional imaging methods in 92 .7% (38/41) of these patients .Conclusion 18 F‐FDG PET/CT imaging can provided important reference value for the detection of primary lesions ,the qualitative diagnosis of benign and malignant lesions and clinical staging of malignant lesions .
5.Effects of community nursing interventions on rehabilitation of stroke patients with attention disorders

Objective To study the effects of community nursing interventions on stroke patients with attention disorders.Methods A total of 60 stroke patients fulfilling eligibility were divided randomly into two groups.Their data were collected between December 2010 and February 2012.The intervention group was treated by community nurses while the control group trained by family members.Attention was assessed by simple reaction time (SRT) and number cancellation test (NCT).Modified Barthel index was used to assess the activities of daily living (ADL).Both groups were evaluated before and after a 2-month intervention.Results Before treatment,the control group had(0.71 ± 0.25),(662.8 ± 152.7) and(31 ±8)scores at SRT,NCT and MBI respectively.And the intervention group yielded(0.69 ± 0.19),(652.7 ±131.0) and (30 ± 8)scores at SRT,NCT and MBI respectively.There were no significant differences in average value of SRT,NCT and MBI between training and control groups at pre-treatment (P > 0.05).After 2 months,the control group had (0.68 ± 0.20),(637.4 ± 151.7) and (33 ± 8) scores at SRT,NCT and MBI respectively.And the intervention group yielded (0.56 ± 0.16),(540.3 ± 125.9) and(37 ± 8) scores at SRT,NCT and MBI respectively.Very significant difference existed in attention scale scores at Month 2 (P < 0.01) and the difference in ADL scale scores was also significant (P < 0.05).Conclusion For stroke patients with attention disorders,strengthening community nursing interventions may help to improve attention and boost activity of daily living.
6.Rehabilitation effect research of electro-acupuncture combined with computer aided training on brain injury patients with cognitive deficits

A total of 60 brain injury patients with cognitive impairment were randomly divided into electro-acupuncture,computer and combination groups (n =20 each).The electro-acupuncture group received electroacupuncture,computer group cognitive rehabilitation training with specialized equipment and combination group both regimens.The treatment lasted for 2 months.Before training,Loewenstein occupational therapy cognitive assessment (LOTCA) test was not statistically significant among all groups (P 8 0.05).After 1 month,the combination group scored higher than pre-training (P < 0.05).After 2 months,the scores of all groups were higher than pre-training(P < 0.05).After 1 and 2-month training,the combination group scored higher than the other two groups in aspects of consciousness,optomotor tissue,attention,thinking and aggregate score (P < 0.05).However,the orientation scores of all groups changed insignificantly after 1-month training (P 8 0.05),the combination group scored higher than the other two groups in improved orientation after 2 months (P < 0.05).

8.Expectant treatment of fresh cervical spinal cord injury in children with non-fracture dislocation

BACKGROUND: Cervical spinal cord injury without radiographic abnormality (SCIWORA) is a special type of spinal cord injury. Because of the particularity of children in the period of growth and development on the aspects of anatomy and biomechanics, their injured mechanism and therapeutic method differ from those of adults.OBJECTIVE: To explore clinical feature and therapy of first-episode cervical SCIWORA in children.DESIGN: Retrospective analysis and self pre-and post-control observation.SETTING: Department of Spine Surgery, Second Affiliated Hospital,Harbin Medical University.PARTICIPANTS: Totally 14 patients with cervical SCIWORA, who were treated at the Departmentof Spine Surgery, Second Affiliated Hospital,Harbin Medical University between June 1997 and June 2003. Inclusive criteria: ①patients who had dysfunction of sensation, exercise and sphincter to different degree in clinic, ②patients who had no fracture dislocation after cervical vertebra X-ray plain film and MRI examination. The MRI examination showed the sign of spinal cord injury. T1WI spinal cord became thick. T2WI distributed at strip-shape high-signal region along spinal prosenchyma and spread upward and downward. There were 3 patients with complete spinal cord injury and 11 patients with incomplete spinal cord injury, among the patients with incomplete injury, there were 6 patients with central cord syndrome (CCS), 3 with Brown-Sequard syndrome and 2withfrontal spinal cord injury syndrome.METHODS: Retrospective analysis was performed in 14 children patients with cervical SCIWORA. Thirteen patients were not treated with operation and one patient was treated with cervical posterior atlantoaxial fusion. MAIN OUTCOME MEASURES: ASIA grading before and after treatment in patients.RESULTS: There were 1 dead patient and 13 cases with 3-year follow up averagely. Three patients were with insignificant amelioration, and others had recovery of limb function to different degree. According to ASIA grading standard, before treatment there were 3 cases in A grade, 5 cases in B grade, 5 cases in C grade and 1 case in D grade, and after treatment there were 1 case recovering to B grade, 2 cases to C grade, 5 cases to D grade and 2 cases to E grade.CONCLUSION: Restoration of neurofunction of children with cervical SCIWORA has closely correlation with degree of primary injury of spinal cord. Most of the patients were not treated with operation, and those with obviously instable cervical vertebra can be treated with operation.

10.The effect observation of visible light therapeutic equipment accompanied with electrolyzed-oxiding water on pressure ulcer above the second phase

Objective The purpose of this study was to discuss the effect of visible light therapeutic equipment accompanied with electrolyzed-oxiding water (EOW) on pressure ulcer above the second phase. Methods We divided 44 patients with pressure ulcer above the second phase into the test group (23 cases, 28 spots) and the control group (21 cases, 25 spots). The test group received local decompression, debridement with EOW, exposure to visible light for 20 minutes, 10 times per day. The control group adopted local decompression, debridement with normal saline and local application of burn cream, 3 times per day. Results The effective rate of the test group was apparently higher than that of the control group (P<0.05). The healing time was also shorter than that of the control group (P<0.01). Conclusion Treatment of pressure ulcer above the second phase with visible light therapeutic equipment accompanied with electrolyzed-oxiding water proved to be an effective treatment method.
