Objective To investigate whether cystatin C-based prediction equations for GFR estimation are superior to SCr-based prediction equations.Methods One hundred and ninety-eight consecutive patients (85 males,113 females,average age 66.5 years) who underwent GFR measurement with 99TcmDTPA and serum cystatin C and SCr tests were included in this retrospective study.GFR,serum cystatin C and SCr concentrations were determined by the Gates method (measured GFR),the particle-enhanced turbidimetric immunoassay,and the Jaffe method,respectively.Eight different equations (6 equations based on the serum cystatin C,and the other 2 based on SCr) were used to estimate GFR values,and the results were compared with that of the Gates method.Patients were divided into different groups according to the measured GFR (normalized to body surface area,1.73 m-2):normal renal function,mild,moderate or severe renal impairment groups.One-way analysis of variance and the least significant difference t test were used to compare the estimated GFR,andx2 test was used to compare the diagnostic efficiencies of different GFR estimation equations.Results Among 198 patients,159 cases were with renal impairment (78 mild,58 moderate,23 severe),and the other 39 cases were with normal renal function.For patients with moderate or severe renal impairment,the estimated GFR calculated by the Tan formula was not different from the measured GFR (severe:(20.7±7.4) ml · min-1 vs (19.9±8.2) ml · min-1; F=6.75,t＜1.05; moderate:(42.1±14.4) ml· min-1 vs (46.8±9.2) ml· min-1; F=10.49,t＜1.63; both P＞0.05),and it had the least error compared with the measured GFR (severe:(12.3±7.0) ％ ; moderate:(17.9± 13.0) ％).For the patients with mild renal impairment and normal renal function,the estimated GFR calculated by the Tan formula was not valuable.For the diagnosis of renal impairment,the sensitivity and accuracy of the modification of diet in renal disease (MDRD) formula were 66.0％(105/159) and 71.2％(141/198),respectively,and those of the chronic kidney disease-epidemiology collaboration (CKD-EPI) formula were 70.4％ (112/ 159) and 73.7％(146/198),respectively.The sensitivities and accuracies of the cystatin C-based formulas (≥83.6％ (133/159) and ≥79.3％(157/198),respectively) were higher than those of MDRD formula and CKD-EPI formula (x2 ≥23.50,all P＜0.01).For the diagnosis of chronic kidney disease (including 81 patients with moderate and severe renal impairment),the sensitivities of cystatin C-based prediction equations (≥ 86.4％ (70/81)) were higher than those of the MDRD formula and the CKD-EPI formula (76.5％ (62/81),79.0％ (64/81)),but the accuracies were slightly lower (Tan formula:80.3％ (159/198),x2≥ 56.42,all P＜0.05).Conclusion The Tan formula may be more suitable for the GFR estimation than the MDRD formula and CKD-EPI formula in the patients with severe or moderate renal impairment (serum cystatin C≥ 1.55 mg/L),but it may not be reliable for the patients with mild renal impairment and normal renal function.

Objective To know the prevalent trend of goiter in Shaanxi province and to provide the basic data for making the control strategy. Methods In 2002, with the population proportionate sampling, the investigation was conducted according to China National Criteria for Diagnosis and Grade Division of Endemic Goiter GB16004-1995. Results The prevalence rate of goiter was 6.52%, the number of patients with visible goiter was 317 000 in Shaanxi province. Conclusion It was not proper to evaluate the population iodine nutrition by the goiter rate of children aged 8-10 years, it may aggrandize the serious degree of IDD. Up to now, the number of patients with visible goiter is decreased by 659 000 and no new Cretinism patient has been found in Shaanxi.

Objectives To investigate and evaluate the effect of Nutrison-5Fu on immune function of perioperative patients with progressive gastrointestinal cancer,and simultaneously we also recommended the methods of application of Nutrison-5Fu.Methods Seventy eight cases were equally divided at random into three groups,26 cases in each group.⑴Nutrison-5Fu group:the patients were treated with Nutrison-5Fu.⑵Chemotherapy group:the patients only received 5-Fu.⑶Control group:there were not any chemotherapy and immunetherapy during the perioperation.The values of CD 3,CD 4,CD 8 and NKCC were determined in all cases.Results Values of CD 3,CD 4,CD/CD 8 ratio and NKCC in most patients were distinctly low,but the condition rapid improved by giving them Nutrison-5Fu.On the contrary,the values declined further after chemotherapy alone.Conclusion The treatment of Nutrison-5Fu not only can repress the development of the tumor,but also can improve the dystrophic status and immune function of the patients with cancer.

Objective To explore the characteristics of executive function and the changes of executive function in schizophrenics with and without homicide behaviors during follow-up.Methods Twenty-two schizophrenics with homicide behaviors (homicide group)and twenty-one schizophrenics without homicide behaviors (without-homicide group) were examined by Wisconsin Card Sorting Test(WCST),and followed up 3 months.All patients'clinical symptoms were evaluated by positive and negative symptom scale(PANSS)at the same time.Re-suits (1)At the baseline,homicide group showed higher score in complete lst category(R1st)than without-homicide group((28.64±32.73)vs(11.71±8.10),P=0.027)and no difference on other index.(2)No difference on WCST in homicide group between at the baseline and at three-month treatment (P>0.05).Without-homicide group at the baseline showed higher score in conceptual level responses than that of at three-menth treatment((46.80±27.04)vs.(65.02±21.32),P=0.048).(3)Homicide group at three-month treatment showed lower scores in positive scale,general psychopathology scale,PANSS total,thought disorder,activation,paranoid,aggressive than that of at the baseline(P<0.05).Without-hemicide group at three-month treatment showed lower scores in positive scale,negative scale,general psychopathology scale,PANSS total,thought disorder,activation,aggressive than that of at the haseline(P<0.01).(4)At the baseline,there was no correlation between WCST and PANSS in homicide group,only R1st score correlated with activation positively in without-homicide group(P<0.05).At three-month treatment, homicide group showed the score of failure to maintain set negatively correlated with negative scale,PANSS total, and aggressive(P<0.05).Conclusions Schizophrenics with homicide and without-homicide behaviors show executive function deficits.Homicide group shows more deficits in abstraction capacity than without-homicide group.

Objective:To prepare anti-human PSA(Prostate Specific Antigen)monoclonal antibodys(McAb)and identify their distinct biological function.Methods:Prostate Specific Antigen(PSA)was purified by salting out,ion exchange and gel filtration method from human sperm.The purity and contents of the objective protein was detected by SPS-PAGE and f-PSA EiAsy.BALB/c mice were immunized with the purified PSA purified.Then the immunized splenocytes were isolated and fused with a hypoxanthine-aminopterin-thymidine-sensitive mouse myeloma cell line SP2/0.Positive clones were screened by limiting dilution method.After acquisition of the hybidomas secreting anti-PSA monoclonal antibody,investigation of their sub-class isotype,binding affinity and specificity were followed.Results:Fifteen hybridoma cell lines that could continuously secret anti-PSA monoclonal antibody were obtained.An Ig subclass was classified as IgG2a and the other were IgG1.Conclusion:This McAb is propitious to establish PSA-ELISA diagnose reagent.

Objective To investigate the auditory sensory gating P50 in schizophrenia patients with homicide during three months of follow up. Methods Twenty-five patients with schizophrenia patients with homicide and 27 healthy adults matched with age and education level were recruited in the present study. P50 was recorded in twenty-seven normal controls and twenty-five schizophrenia patients at the baseline. P50 was repeatedly recorded in eleven patients who completed the follow-up at three months following antipsychotics treatment. The psychotic symptoms of patients were assessed with Positive and Negative Syndrome Scale (PANSS) at the same time. Results ①In comparison with normal controls, schizophrenia patients with homicide showed increased S2-P50 amplitude (P<0.01), increased S2/S1 ratio (P<0.01), decreased S1-S2(P<0.05), and 100(1-S2/S1) (P<0.01) at the baseline and after three months treatment. The differences in P50 amplitude, latency, and P50 suppression index between the baseline and after three-month treatment in schizophrenia patients with homicide were not significant (P>0.05). ② Schizophrenia patients with homicide after three-month treatment showed decreased PANSS total scale, positive scale, general psychopathology scale, and six symptoms scales such as lack of response, thought disorder, activation, paranoid, depression, and aggression (P<0.05). ③ No significant correlations were found between the S2/S1 ratio, S2-S1, and 100(1-S2/S1) with disease duration, PANSS scores, and six symptoms scales at either baseline or after three-month treatment (P>0.05). Conclusions Schizophrenia with homicide has sensory gating deficits and P50 suppression index might be a stable trait maker for schizophrenia with homicide.

Objective: To investigate the expression of HSP-27,-60 and -90 in gastric cancer and its clinical significance. Methods:66 cases of gastric carcinoma was detected by immunohistochemistry HSP-27,60 and 90 of the expression and clinical significance of combined with clinical and pathological characteristics, tumor cell proliferation and survival analysis of three kinds of heat shock protein expression. Results: HSP-27,-60 and -90 were highly expressed in gastric cancer tissues. HSP-27 expression and tumor size (pT,P=0. 026),organ metastasis (pM,P=0. 046) and pathological staging (P=0. 041),HSP-27 staining intensity and lymph node status were significantly correlated ( pN, P=0. 042 ) . HSP-60 expression was associated with gender ( P=0. 011),and HSP-60 staining intensity was associated with age (P=0. 027) and tumor grade (P=0. 031). There was no correlation between HSP-90 expression and the clinical pathological parameters of this study; however, the intensity of HSP-90 staining was significantly correlated with tumor size (P=0. 020,pT). Single factor analysis showed that HSP-90 was significantly associated with longer survival (P=0. 033). Multivariate analysis demonstrated that HSP-90 was highly expressed as an independent prognostic factor for gastric cancer (P=0. 026). Conclusion: the HSP-27,-60 and -90 and some clinical pathological parameters. These parameters is very important for the treatment of patients with gastric cancer. The high expression of HSP-90 in patients with gastric cancer were inde-pendent prognostic indicators.

Objective To evaluate the effect of bone mesenchymal stem cells (BMSCs) on lidocaine-induced apoptosis in dorsal root ganglion cells (DRGCs) of rats in vitro.Methods DRGCs in the logarithmic phase were incubated in culture plates at the density of 2 × 104 cells/cm2 (27 wells in total).DRGCs were randomly divided into 3 groups (n =9 each) using a random number table:control group (group C),lidocaine treatment group (group L) and BMSC treatment group (group B).The DRGCs in group C were incubated routinely without lidocaine,while the DRGCs were incubated for 2 h with lidocaine with the final concentration of 50 mmol/L in L and B groups.The DRGCs were then incubated normally in group L.The DRGCs were then co-cultured with the BMSCs which were incubated in Transwell chambers with the density of 2 × 104 cells/cm2 in group B.DRGCs were collected at 48 h of incubation for detection of apoptosis by flow cytometry.Apoptosis rate was calculated.Results Compared with group C,the apoptosis rate was significantly increased in L and B groups (P ＜ 0.05).The apoptosis rate was significantly lower in group B than in group L (P ＜ 0.05).Conclusion BMSCs can reduce lidocaine-induced apoptosis in DRGCs of rats in vitro,indicating that BMSCs may reduce local anesthetics-produced toxicity to the peripheral nerve.

OBJECTIVE: To investigate the effects of scalp-acupuncture on intelligence in patients with vascular dementia (VaD). METHODS: A total of 108 VaD patients were randomly divided into two groups: scalp-acupuncture group and Western medicine group. The scores of Mini-Mental State Examination (MMSE), Blesse Dementia Scale (BDS), Hasegawa Dementia Scale (HDS), Abilities of Daily Life (ADL) before and after treatment in the two groups were detected. RESULTS: A total of 92 patients completed the treatment. The scores of MMSE, BDS and HDS in the two groups were significantly increased, and the ADL scores significantly declined (P<0.05). There was no difference in the scores of MMSE, BDS, HDS and ADL between the two groups before and after the treatment. There was no difference in the total effectiveness between the two groups (P>0.05). CONCLUSION: Scalp-acupuncture therapy could improve the clinical intelligence level of VaD patients.

Totally 177 individuals (87 males and 90 females), who took abdominal computer tomography were enrolled. Anthropometric surrogates such as abdominal height( AH), waist circumference( WC), body mass index(BMI), and waist to height ratio(WHtR) were measured. Correlations between AH, WC, BMI, and WHtR with visceral abdominal fat were analyzed, and receiver operating characteristic curves ( ROC) were prepared for detecting abdominal adiposity at or beyond 80cm2 using lumbar vertebra 4-5 cross-section visceral fat areas as the screening cut-point within gender separately. Youden′s index was used to determine cut-off values of AH, WC, BMI, and WHtR by which to classify excess abdominal fat. 70(80. 5% ) males and 59(65. 9% ) females had abdominal obesity. Compared to other anthropometic indicators, AH presented the highest correlation with visceral adipose fat. The pearson correlation efficiencies were 0. 825 and 0. 732, the cut-off points were 20 cm and 20. 3 cm in male and female respectively. After controlling for age, receiver operating characteristic analysis showed that AH, followed by WHtR, outperformed WC and BMI in identifying central obesity participants. Optimal cut-off values of the physical anthropometric indices to indentify abdominal adiposity were 20 cm(AH), and 91 cm(WC) 25 kg/ m2 (BMI), and 0. 52(WHtR) for men, and 20. 3 cm(AH), 86. 5 cm(WC), 25 kg/ m2(BMI), and 0. 54(WHtR) for women. AH is effective for predicting visceral adiposity and might be considered the best predictor of abdominal obesity at a cut-point of 20cm in Chinese adults.
