1.Effect of glutaraldehyde cross-linking on the properties of chitosan/hydroxyapatite-gentamicin delayed materials

BACKGROUND:Cross-linking is a common method for the modification of bone tissue engineering materials, but there are few studies about the effect of cross-linking on properties of drug-loaded artificial bone. OBJECTIVE:To study the effect of glutaraldehyde cross-linking on mechanical strength, degradation rate and in vitro release behavior of chitosan/hydroxyapatite-gentamicin delayed materials. METHODS:The chitosan/hydroxyapatite-gentamicin drug-loaded artificial bone and the cross-linked chitosan/hydroxyapatite-gentamicin drug-loaded artificial bone were prepared at 10%, 20%and 30%chitosan mass ratio. The mechanical strength, absorption rate, degradation rate and in vitro release behavior of materials in each group were determined. RESULTS AND CONCLUSION:The compressive strength of 10%, 20%, 30%chitosan/hydroxyapatite-gentamicin drug-loaded artificial bone was (10.16±1.17) MPa, (28.4±0.64) MPa and (23.28±1.30) MPa, respectively. After cross-linking, the strength was increased to (36.3±1.20) MPa, (51.6±2.08) MPa and (36.9±3.22) MPa, respectively. The absorption rate and degeneration rate were reduced by cross-linking. In the first day of drug release in vitro, the rate of gentamicin released from non-cross-linked 30%chitosan/hydroxyapatite-gentamicin drug-loaded artificial bone was 42.2%, while the rate was decreased to 33.6%after cross-linking. At the fol owing 9 days, the total release of the cross-linked material was lower than that of non-cross-linked material. Glutaraldehyde cross-linking could improve the stability, reduce the degradation rate, and significantly ameliorate the release of artificial bone.
2.Study on relationship between working pressure and mental health for nurses of transfusion room in paediatric department

Objective To learn the relationship between working pressure and mental health for nurse of transfusion room in pediatric department.Methods 120 nurses of transfusion room whose working years above half a year were investigated by questionnaire from January 2010 to December 2011.Results The maximal score of working pressure was working environment and resource (3.27±0.47),while the minimum was interpersonal relationship (1.80±0.68).The score of SCL-90 of the research group was higher than domestic model.More working pressures of transfusion room in pediatric department were positive correlated with SCL-90.Conclnsions In order to keep nurse sound in body and mind,the working pressure should be desolved in many ways.
3.Diagnosis value of soluble triggering receptor expressed on myeloid cells-1 in critically ill patients complicated with sepsis

Objective To investigate the diagnostic value of soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) in critically ill patients complicated with sepsis. Methods Fifty-six cases of systemic inflammatory response syndrome (SIRS) who admitted in intensive care unit (ICU) of the Second Hospital of Tianjin Medical University between May 2009 and June 2010 were recruited. The SIRS patients were divided into two groups: sepsis group (n= 32) and non-sepsis group (n= 24). The levels of procalcitonin (PCT), C-reactive protein (CRP) were measured within 24 hours of hospitalization.Levels of sTREM-1 were measured by enzyme linked immunoabsorbant assay (ELISA). Based on the receiver operating characteristic curve (ROC), cut-off value, sensitivity and specificity of sTREM-1 in diagnosis of sepsis were calculated. Comparison between groups was done by t test and Mann-Whitney U test. Count data were compared by chi square test. ResultsThe serum level of sTREM-1 were significantly higher in sepsis group than that in non-sepsis group (250.9 ng/L vs 103.6 ng/L, Z= 12. 864,P=0. 002). Areas under the curve (AUC) of sTREM-1, PCT and CRP were 0. 935, 0. 891 and 0. 602, respectively. With a cut-off value of 135.0 ng/L, the sensitivity and specificity of sTREM-1 in diagnosing sepsis were 93. 8% and 84. 7%, respectively. With a cut-off value of 2. 0 μg/L, the sensitivity and specificity of PCT were 84. 4%and 87. 8%, respectively.ConclusionSerum level of sTREM-1 could be used as an early indicator for diagnosing sepsis.

Leukotriene is a crucial inflammation medium in the process of asthma,involved in the inflammation and remodeling of airway.This article summarizes the origin,synthesis and biological activity of leukotriene in asthma.and discuss the implications of leukotriene receptor antagonists(Montelukast)for asthma in children.The randomized.double-blind and muhicenter clinical trials are needed to determine the efficacy of Montelukast in infants less than 2 years old with asthma.
5.Effect of systematic nursing intervention on life quality of heroin addicts during convalescence stage of drug detoxification period

Objective To explore systematic intervention methods on life quality of heroin addicts during convalescence stage of drug detoxification period in order to supply evidence for effective nursing intervention. Methods 156 patients accorded with standard were selected by subjective sampling method and were grouped randomly. 78 patients in the control group were given routine nursing,78 patients in the intervention group received systematic intervention including psychological and behavioral intervention and the therapy of physical symptoms. QOLDA was used to evaluate the life quality of all patients. Results The scores of the intervention group were significantly higher than those of the control group, and the intervention group had a higher body mass index (BMI). Conclusions Systematic nursing intervention can perfect the physiological function, mental function and social function, reduce the withdrawal syndrome and adverse effect of heroin addicts, and improve their quality of life.
6.Percutaneous vertebroplasty, percutaneous kyphoplasty and expansive pedicle screw fixation for repairing primary osteoporotic thoracolumbar fractures

BACKGROUND:Percutaneous vertebroplasty, percutaneous kyphoplasty and expandable pedicle screw fixation can treat primary osteoporotic thoracolumbar fractures. The three methods have their own advantages and disadvantages. OBJECTIVE:To investigate the methods and clinical effects of primary osteoporotic thoracolumbar fractures. METHODS:Clinical data of 61 patients with primary osteoporotic thoracolumbar fractures were col ected and retrospectively analyzed. Perioperative preparation must be done. Al patients were treated by percutaneous vertebroplasty, percutaneous kyphoplasty and expansive pedicle screw fixation. We recorded Visual Analogue Scale (VAS) and the Oswestry Disability Index (ODI) before treatment, 3 months after treatment, as wel as sagittal index (SI) and Cobb angle of vertebral fracture before treatment, 3 days and 3 months after treatment. RESULTS AND CONCLUSION:(1) Al cases were fol owed up for 12-18 months. (2) There was no significant difference in VAS scores, ODI, SI and Cobb angle of vertebral fracture among the three groups of patients preoperatively. (3) At 3 months after treatment, there were significant differences in VAS scores and ODI in the three groups as compared with that preoperation (P<0.05). However, no significant difference in VAS and ODI was determined among intergroup comparison (P>0.05). (4) SI and Cobb angle of vertebral fracture were significantly increased;the difference was statistical y significant (P<0.05). The efficacy was similar between the percutaneous kyphoplasty and expansive pedicle screw fixation groups (P>0.05), and was better than the percutaneous vertebroplasty group (P<0.05). (5) Three kinds of treatment can effectively restore the vertebral height and intensity, relieve pain and stabilize the spine, and no significant vertebral compression was found in the short term. However, restoration of postoperative vertebral height was better in percutaneous kyphoplasty and expansive pedicle screw fixation groups than in the percutaneous vertebroplasty group. In view of their respective indications, advantages and disadvantages, the key point of raising therapeutic effect was to choose appropriate surgical procedures.
7.Clinical Observation on Acupuncture Combined with Western Medicine in Treating Acquired Paralytic Strabismus

Objective To observe clinical curative effect of acupuncture combined with western medicine in treating acquired paralytic strabismus.Methods Randomly divided 60 patients with acquired paralytic strabismus into a treatment group and a control group,with 30 cases in each group.The control group accepted compound anisodine injection for subcutaneous injection beside superficial temporal artery and oral administration of neurotrophic drugs.The treatment group accepted acupuncture treatment in addition to the treatment of the control group.Both groups were treated for 4 weeks.Before and after treatment,measured the ocular symptom scores,including eye position deviation,eyeball movement obstacle,diplopia,the first squint angle being greater than the second,compensatory head posture and evaluated the clinical curative effect.Results The clinical curative rate in the treatment group was 40.0% and the total effective rate was 93.4%,which in the control group were 10.0% and 66.7% respectively.The total effective rate and curative rate in the treatment group were significantly higher than those in the control group (P< 0.05).After treatment,the improvement in the total ocular symptom score and diplopia,deviation of eye position,eyeball movement disorder scores were superior to those in the control group (P < 0.05).Conclusion Acupuncture combined with western medicine was superior to simple western medicine in treating acquired paralytic strabismus,and could significantly improve the eye symptom.
8.One case of human nasal myiasis.

Human myiasis is caused by the parasitic maggots of flies. We report a case of nasal myiasis in a 37-year-old woman in Henan. The patient presented with nasal foreign-body sensation, nansal itching sneezing, snuffle and such allergic symptoms in nasal cavity. Based on maggots that were bowed from the right nasal cavity, a detailed endoscopic examination, sinus CT scan and identification of the maggot's species done by parapsychologists of the parasitology teaching and reseach section of the Zhengzhou University School of Medicine, the diagnosis of human nasal infestation by oestrus ovis was definited.

9.Analysis of six cases concerning clinical features of rhinolith.

To analyze six cases concerning clinical features, the treatment process and curative effect of Rhinolith. The six patients had unilateral nasal obstruction, three of them were accompanied with purulent nasal discharge, two had blood with them, one patient had repeated hemorrhage of nasal cavity and two patients had developed headache. By using endoscopic, four irregular stones which are yellow, gray and brown were found in the middle and back end. Because of the severe nasal septum deviation and inferior turbinate polypoid, the stone could not be seen clearly but could be touched by aspirator. In sinus CT examination of 6 cases, it showed incomplete irregular shapes and had high density unilateral nasal cavity in the posterior segment. 4 patients had nasal deviation, 3 patients had ipsilateral sinusitis. 4 patients removed their stone by using endoscopy, 2 patients took the stone away after correction of nasal septum. Pathologic diagnosis: rhinolith. Two patients also had exogenous foreign matters which were considered as pseudo nasal stones.

10.The effects of interleukin-10 and dexamethasone on expression of TNF-? mRNA, IL-1? mRNA, IL-1ra mRNA in acute lung injury in rats

Objective To evaluate the effects of interleukin-10 and dexamethasone on expression of TNF-? mRNA, IL-1? mRNA, and IL-1ra mRNA in acute lung injury (ALI). Methods A rat model of ALI was reproduced by intratracheal instillation of endotoxin (LPS) with a dose of 10mg/kg. 72 male S-D rats were randomly divided into control group, LPS group, LPS+IL-10 group, and LPS+Dex group, 18 rats in each group (6 rats at 2h, 6h, and 24h respectively). RT-PCR method was used to determine the expression of TNF-? mRNA, IL-1? mRNA, and IL-1ra mRNA in lung tissue. Results In LPS group TNF-? mRNA expression peaked at 2h, then decreased sharply; IL-1? mRNA expression increased markedly at 2h, peaked at 6h, then decreased; IL-1ra mRNA expression increased and peaked at 6h, and remained higher than control group at 24h. Both IL-10 and Dex inhibited TNF-? mRNA and IL-1? mRNA expression, but with no effect on IL-1ra mRNA expression. Pathological examination revealed that IL-10 or Dex attenuated injury to lung parenchyma. Conclusions The up-regulation of expression of both TNF-? mRNA and IL-1? mRNA was much earlier than that of the expression of IL-1ra mRNA, suggesting that there was an imbalance of inflammatory and anti-inflammatory mediators in the early phase of ALI. Both IL-10 and Dex could inhibit the expression of inflammatory mediators and exert no effect on the expression of anti-inflammatory mediators, thus contributing a balance effect between them and relieving ALI in rats.
