1.Correlation between the Consumption of Antibacterial Drugs and the Bacterial Resistant in 70 Hospitals in Shanxi Province

OBJECTIVE:To investigate the relationship between the variation of bacterial-resistant and the consumption of antibacterial drugs. METHODS:The DDDs and its sequence of antibacterial drugs in 70 hospitals in Shanxi province over 3 years and the synchronic annual resistant rates of some pathogenic bacteria were analyzed statistically and the correlation between the variation of bacterial-resistant and the drug consumption was analyzed. RESULTS:In terms of variety,there was no obvious change for the top twenty antibacterial drugs. The antibacterial drugs with high DDDs thus ranked the lead were those kinds that with high bacterial drug resistance. At a value of over 50%,the bacterial drug resistance rate would not decrease obviously with the variation of the drug consumption. The drug resistance rate increased greatly with the increase of the consumption of the forth generation of cephalosporins cefepime. The consumption of vancomycin increased,but its resistance rate decreased. The antibacterial compound preparation showed satisfactory antibacterial activity even if its consumption had a marked increase. CONCLUSION:There is a direct relation between the consumption of antibacterials in large quantity and the variation of drug resistance. It is necessary to strengthen clinical management on antibacterials to raise rational drug use level.
2.Evaluation of application effect of nursing intervention on improvement of psychological behavior and quality of life for patients with Alzheimer disease

ObjectiveTo observe the effect of nursing interventions on improvement of psychological actions and quality of life of patients with Alzheimer disease. Methods50 patients with senile dementia diagnosed in our hospital were chosen.They were divided into the observation group and the control group with 25 patients in each group.All patients were taken with AD routine nursing care after admission.The observation group was taken with rehabilitation nursing interventions based on AD routine nursing care.The mini-mental state examination (MMSE)and the daily life ability scale (ADL)were used to evaluate the quality of life after nursing intervention. ResultsThe patients' MMSE score in the observation group was significantly higher than the control group,the difference was statistically significant.The ADL score of the observation group was significantly lower than the control group,the difference was statistically significant. ConclusionsThe use of integrated care intervention can significantly improve the cognitive function in AD patients,and improve daily living ability and quality of life significantly.
3.Advances in impacts of osteoprotegerin and insulin-like growth factor-1 on bone remodeling in osteoarthritis

Osteoarthritis (OA) is a kind of chronic and frequently-occurring disease in the elderly.It is also called degenerative disease.Pain,stiffness,swelling,deformity and dysfunction of joints are the main clinical manifestations,which seriously affect the quality of life.Kaschin-Beck Disease (KBD) is a kind of endemic diseases;its main feature is cartilage degeneration and necrosis.Although KBD becomes a typical OA in late stage,it is different from OA.Osteoprotegerin (OPG) and insulin-like growth factor (IGF)-1 participate in the bone growth,repair,and alteration.In this paper,we summarized clinical manifestations and pathological changes of OA,and reviewed the current situation and development of OPG and IGF-1 in OA bone remodeling.
4.Utilization of Narcotic Analgesics in Patients with Cancer in Our Hospital from 2005 to 2008

OBJECTIVE:To evaluate the status quo and the tendency of the use of narcotic analgesics in patients with cancer in our hospital.METHODS:The use of narcotic analgesics in the inpatients with cancer between 2005 and 2008 in our hospital was analyzed statistically in respect of the consumption sum,number of patients using this drug and the prescriptions etc.RESULTS:Morphine Sulfate Controlled release Tablets(10 mg) was not welcome by both physicians and patients; however,the consumption sum and DDDs of Fentanyl transdermal patches and Morphine Sulfate Controlled-release Tablets (30 mg) had been dominated the front places over the 4 years.The consumption quantity of common Morphine tablets kept steady increase yet no obvious change has been noted in average dose per prescription;however,the per capita consumption quantity has been increasing.Bucinnazine hydrochloride tables as an two step analgesics kept decreasing in consumption.The total consumption of Morphine injection and Bucinnazine injection were decreasing on the whole.CONCLUSION:The use of narcotic analgesics for patients with cancer in our hospital is in line with the principle of cancer pain three-step analgesic ladder recommended by WHO.
5.Ingredient Analysis and Lipid Peroxidation in Mice of Organic Pollutants of Polluted Irrigation Soil

Objective To investigate the organic pollutants in the contaminated water irrigation area soil and to study the lipid peroxidation effect of the pollutants on mouse. Methods The organic pollutants of the polluted irrigation soil and control soil were extracted using the supersonic oscillator. The ingredients of the organic pollutants were measured by the GC-MS spectrum. The 40 sanitary Kunming mice were randomly divided into five groups: the dimethylsulfoxide control group,low dose group of control soil,high dose group of control soil,low dose group of polluted soil,high dose group of polluted soil. The administered doses of the low and high dose groups were 25.5 g dry soil/kg body weight per day by oral for two weeks. The activities of T-SOD,GSH-Px and the content of MDA in serum of mice were determined. Results Phenanthrene,yrene,diphenyl,fluorene,naphthalene,dimethyl-naphthalene,2,3,6-trimethyl-naphthalene,anthracene,dimethyl-anthracene,1,4-dimethyl-anthracene,fluoranthene,4-methyl-pyrene,dibenzothiophen,4-methyl-dibenzofuran,dibenzofuran,butylated-hydroxytoluene,dodecane,neopentyl-cyclohexane,trimethyl-heptane,1,2-dimethyl-cyclohexanimine,butethal were detected in the polluted irrigation soil and their levels were 14.3,2.1,1.3,1.1,0.8,1.3,0.4,2.9,7.2,2.3,9.1,0.2,0.4,1.4,1.7,0.37,0.9,0.11,0.02,0.02 and 0.13 mg/kg respectively in the polluted irrigation soil. Trimethyl-heptane,dimethyl-heptane and 1,2-dimethyl-cyclohexanimine were detected,and their levels were all 0.01 mg/kg in control soil. Compared with the solvent of dimethylsulfoxide group,the activities of T-SOD and GSH-Px of the high dose group of control soil and two dose groups of polluted soil all decreased,the differences were all significant(P0.05). Conclusion The soil has been contaminated with organic pollutants in the investigated area and the soil extract may produce a certain effect on lipid peroxidation in mice.
6.Optimizing Fermentation Conditions for Expression of Esterase B1 Fusion Protein in Recombinant Bacteria

Objective In order to research the best fermentation conditions of recombinant bacteria pThioHisA-B1/DH5? which contains gene of esterase B1.Methods Recombinant bacteria pThioHisA-B1/DH5? was induced in different culture media, induction temperatures, different inductor concentrations, different induction time in order to obtain the best expression effect.The expression products of different conditions were detected by SDS-PAGE and thin-layer gel scanning analysis.Results Based on the experiments, the optimized fermentation condition were that the culture medium was TB, inducing temperature was 28 ℃ and inducing time was 6 h,the concentration of IPTG was 0.4 mmol/L.The expression quantity of target protein attained 40.3% in total bacterial protein, and the soluble fusion protein accounted for 66.2% in total target proteins at the optimum conditions.Conclusion The optimized fermentation conditions of expressing fusion protein in recombinant bacteria have been found and in the conditions, soluble esterase B1 fusion protein of high yield can be obtained.
7.Expression and clinical significance of estrogen receptor and progesterone receptor in endometrial carcinoma in aged patients

Objective To investigate expression of elderly patients with endometrial cancer of estrogen receptor and progesterone receptor and its clinical significance.Methods 45 elderly patients with endometrial carcinoma from March 2012 to June 2014 were selected as the observation group, over the same period healthy women with normal endometrium as control group, using S-P immunohistochemical method, to detect the expression of receptor and progesterone receptor in two groups.Results In observation group, estrogen and progesterone receptor positive expression rates were 60.00%, 46.67%, compared with control group(93.33%, 86.67%), was significantly lower (P<0.05).Patients with low differentiation of estrogen and progesterone receptor, positive expression rate was 37.50%, 25.00%, was lower than other two groups, but which had no difference.Estrogen, progesterone receptor positive expression rates in progress stage were 35.71%, compared with patients ( 80.65%, 74.19%) in early stage, was significantly lower (P<0.05).The positive expression rate of estrogen receptor in patients with lymph node metastasis was 66.67%, compared with the patients with no metastasis, the difference was not statistically significant.The positive expression rate of progesterone receptor was 76.67%, compared with the non metastasis group (46.67%), and was significantly lower (P<0.05).Conclusion The expression of estrogen and progesterone can reflect the biological behavior and prognosis of endometrial carcinoma in aged patients, and have important significance for clinical diagnosis and treatment.
8.EFFECT OF 1?, 25-DIHYDROXY-VITAMIN D_3 ON DIFFERENTIATION AND PROLIFERATION OF A HUMAN MYELOID LEUKEMIA CELL LINE

1 ? 25-dihydroxy-vitamin D3(1? 25 (OH)2 D3)induced differentiation of human promyelocytic leukaemia cells ( HL-60 ) into mature myeloid cells in vitro. The ratio of nuclei to cytoplasma decreased; Their nucleoli reduced; Cells became irrgular and extended the pseudopods. 1 ? 25 (OH ) 2 D3 caused significant increase of nitroblue tetrazolium ( NBT ) reduction and ?-non-specific acid esterase (?-NAE ) and acid phosphatase (ACP ) activities. These data and morphological characteristics suggest that HL-60 cells were conclusively identified as monocytes/macrophages. The histogram of DNA distribution ahylyzed by flow cytometry demonstrated G0 + G1 phase increased and S phase increased and S phase decreased remarkably after treatment with l?, 25 ( OH ) 2 D3 as compared with untreated cells.
9.Early prediction,diagnosis and treatment of thoracic injury complicated by adult respiratory distress syndrome

Objective To investigate the early prediction,diagnosis and treatment of thoracic injury complicated by adult respiratory distress syndrome (ARDS).Methods The blood gas,occurrence index of traumatic ARDS,pathogenic bacteria and drug sensitivity in ventilator associated pneumonia (VAP) were analyzed retrospectively in 18 cases of thoracic injury.Results The initial PaCO 2 was (32.1?5.2) mm Hg,PaO 2 (57.7?10.1) mm Hg,the occurrence index of traumatic ARDS (-29.00?11.50) and the mechanical ventilation time 6.4 days on average.Of the infective bacterium of VAP,gram negative bacterium took up 70.6%, gram positive bacterium 17.10%,fungus 12.30% and the mixed infection 31.60% with the total mortality of 16.6%.Conclusion The occurrence index of traumatic ARDS can be used as a parameter of the early prediction and outcome.Meanwhile multiple injuries and shock should be treated correctly,ventilator applied earlier and the mechanical ventilation time shortened.It suggests that more attention should be paid to the prevention of VAP and the reasonable use of antibiotic drugs.
10.The changes of HMGB-1 in patients with sepsis and the effect of carbachol on prevention of HMGB-1

Objective To observe the changes of HMGB-1 of burned patient with sepsis and to investigate the effect of carbaehol on production of HMGB-1 from human monocytes stimulated by lipopolysaccharide(LPS) and its mechanism. Methods The peripheral blood samples of burned patients with sepsis and healthy donors were taken for isolation of monoeyte. The subjects were divided into six groups: the menocytes group were added only with 1640 culture medium,LPS group were added only with LPS;nicotine group and carbachol group were with carbachol first or nicotine for 5 rain and then with LPS. The levels of HMGB-1 were tested by EIJSA. α-bungarotoxin + carba-chol group were added with atropine or α-bungarotoxin for 5 min and then were given carbachol and 5 min later were stimulated with LPS. 48 h-incubation later supemate were collected for the detection of HMGB-1 by ELISA. Results HMGB-1 levels of burned patients with sepsis were ( 12.94±6.54)μ/L,which were much higher than that of healthy donors( [ (2.01±0.03 )μ/L), P <0.01 ]. When monoeytes were stimulated by LPS alone, concentrations of HMGB-1 were (9.39±1.37 ) μ/L, which were obviously higher than that of controls [( 1.48±0.69 ) μ/L), P < 0.01]. Mter pretreated by carbaehol or nicotine,concentrations of HMGB-1 were (3.52±1.64)μ/L and (4.01± 1.56) μ/L respectively, which were obviously decreased compared with that of LPS stimulation alone group ( P < 0.01). When pretreatedby atropine before addition of carbachol, there was no significant changes in concentrations of HMGB-1 [ (3.87±2.01 )μ/L]. When pretreated with α-Bungaretoxin before carbachol adminitration, inhibitive effect of carbachol on production of HMGB-1 were blocked [(8.97±1.97 )μ/L ]. Conclusion The release of HMGB-1 in burned patients with sepsis is increased. Carbachol could obviously reduce the production of HMGB-1, which may affect through activating α 7 subunit of cholinergic N receptor.
