1.Observation of effect of Jidesheng snake medicine on patients with bites by unknown poisonous inSects

ObjectiveTo observe the effect of Jidesheng snake medicine on patients with bites by unknown poisonous insects. Methods69 cases with bites by unknown poisonous insects were randomly divided into the experimental group (35 cases)and the control group (34 cases).The experimental group used Jidesheng snake medicine for external application,the control group used povidone iodine.The treatment effect within one week was compared,including cure rate,total effective rate,satisfaction degree of patients and incidence rate of complication. ResultsAfter treatment,the cure rate and total effective rate of the experimental group was 71.43% and 97.14%,higher than 61.76% and 85.29% of the control group.The incidence rate of complications was 2.86% in the experimental group,lower than 8.82% of the control group.The satisfaction degree of patients in the experimental group was 91.43%,significantly higher than 85.29% of the control group. ConclusionsApplication of Jidesheng snake medicine in treatment of bites by unknown poisonous insects is more effective,and the incidence rate of complication was lower.It is worthy of clinical application.

The DSM-5 cross-cutting symptom measures were developed by the DSM-5 Task Force and Work Groups to serve as a review of mental systems in each patient who presents for mental health evaluation and treatment.The cross-cutting symptom measures have two levels.Level 1 questions are a brief survey for adult patients and for child and adolescent patients.Level 2 questions provide a more in depth assessment of certain domains.The comprehensive symptoms could be evaluated through the objective assessment other than symptoms fit nearly into the diagnostic criteria.The following are briefly introduced.
3.Situation Changes of DNA Demethylation Analysis of FOXP3 TSDR at the Beginning of Patients with Secondary Pulmonary Tuberculosis

Objective To analyze changes of DNA demethylation analysis of FOXP3 TSDR at the beginning of the secondary pulmonary tuberculosis patientsby utilizing real-time PCR technology.Methods To select 47 patients of secondary pulmona-ry tuberculosis as a research group from June 2014 to May 2015 and 40 healthy donors as a control group.The peripheral blood mononuclear cells (PBMC)of research group and control group were isolated.CD4+CD25+T cells were isolated from PBMC.Genomic DNA was isolated from CD4+CD25+T cells.PCR was performed in a final reaction volume containing dem-ethylation-specific primers.Plasmid standard was generated by PCR products were enzyme digestion,TOPO TA cloning,and recycling and purification.A real-time PCR system was established by quantitatively analyzing the specificity of FOXP3 TS-DR demethylation to treg (regulatory T-cell).Treg numbers of control group at week 0 and research group treated at week 0,week 2,week 4 and week 8 by using real-time PCR assay of the FOXP3 TSDR demethylation.The experimental data was analyzed by using SPSS 1 6.0 software.Results The M.tuberculosis in sputum of research group were positive by smear mi-croscopy,however the results of control group were negative.The treg frequency of control group,2+ group and 3+ group respectively was 1.63%±0.70%,1.96%±0.10% and 0.86%±0.21%,respectively.The difference between the treg fre-quency of control group and that of 2+ group by smear microscopy had not statistical significance,however which of 3+group was opposite.The average treg frequency of research group treated at week 0,2,4 and 8 respectively was at 1.05%, 2.04%,3.44% and 2.79%,range of which respectively was 0.32%~2.03%,0.95%~3.95%,2.35%~4.95% and 1.02%~4.27%,95% confidence interval of which respectively was (0.93%,1.18%),(1.85%,2.24%),(3.27%,3.61%) and (2.60%,2.98%).The treg frequency of difference between control group and research group at week 0 had statistical significance (t=4.669,P<0.05).The treg frequency was influenced by time of therapy,using One-Way ANOVA analysis (F=347.2,P<0.001,df=3,within-subjects Contrasts:F=407.4,P<0.001,df=3).Test of the treatment time and group interaction effect was linear (F=678.2,P<0.001,df=1).Conclusion DNA demethylation analysis of FOXP3 TSDR was high sensitivity and specificity in monitoring changes of treg at the beginning of the secondary pulmonary tuberculosis pa-tients.
4.Progress of Caveolin and Its Role in Brain

Caveolins are a family of plasmalemmal vesicles caveolae-associated integral membrane proteins and a marker protein of caveolae involved in the formation and localization that associated with vesicular transport, cellular cholesterol homeostasis and signal transduction. Recent years, strong experimental evidences indicated that caveolins play a pivotal role in the brain function such as neural development, synaptic plasticity and neurodegenerative diseases. Recent progress on studies of the structure and functions of caveolins was simply summarized. The regulatory role of caveolins in the brain functions has been reviewed and expected.
5.Effects of Vitaimin D on Insulin Sensitivity and the Adipokine Factors APN,Visfatin, RBP4,TNF-αin Patients with Gestational Dellitus Mellitus

Objective To investigate the relationship and mechanism of the serum vitamin D levels with insulin sensitivity and adipokines APN,RBP4,visfatin and TNF-α in patients with gestational diabetes mellitus(GDM). Methods A total of 101 patients with gestational diabetes( GDM group) and 50 pregnant women with normal blood sugar( control group) were recruited to detect the serum levels of correlative biochemical indexes,APN,RBP4,visfatin,TNF-α and 25OHD3 using an HPLC method. Twenty-four patients in the GDM group diagnosed with vitamin D deficiency(<25 nmol·L-1 ) were randomized to receive either 1,25(OH) 2D3(0. 25 μg·d-1) orally or no treatment. Serum levels of correlative biochemical indexes were detected after 1 month. Results GDM patients had 25OHD3 levels significantly lower than the control group(P<0. 05),APN levels lower than the control group,and RBP4,visfatin,TNF-α levels significantly higher than the control group(all P<0. 05). After 1 month of 1,25(OH) 2D3 treatment,HOMA-IR increased,while HOMA-β decreased. APN levels was positively correlated(r= 0. 526) with 25OHD3;RBP4,visfatin,TNF-α decreased and were negatively correlated with 25OHD3( r values were -0. 272,-0. 153,-0. 072). Conclusion Vitamin D can reduce the adipokines RBP4,visfatin,TNF-α,increase APN,and thus play a protective role for gestational diabetes.
6.Application of optic coherent tomography in assessing visual performance of idiopathic macular epiretinal membrane patients

Objective To investigate the correlation between retinal deformation degree and retina thickness (RT) detected by optic coherent tomography (OTC) in patients with idiopathic macular epiretinal membrane (IMEM). Methods The 66 eyes (56 patients) with IMEM diagnosed by OCT were retrospectively analyzed in this study. After best corrected visual acuity (BCVA), ocular fundus and OCT examination, patients were divided into three groups (mild, medium and severe) according to retinal deformation degrees. The RT value was measured manually. Results There were significant differences in RT values between mild, moderate and severe groups:(311.95 ±51.78) μm, (447.13±41.95) μm and (560.00± 58.23) μm (P<0.05). The values of BCVA were 0.78±0.16, 0.38±0.12 and 0.27±0.14 for mild, moderate and severe groups, there were significant differences between them (P<0.05). There was no significant correlation between RT and BCVA in mild group (r=-0.352,P=0.128). There was negative correlation between RT and BCVA in medium group and serious group (r=-0.768 and-0.482,P<0.05). Conclusion The retinal deformation degree and RT are two objective indicators to assess visual performance in patients of IMEM. When RT is more than 400 μm, it can be used as objective criteria for surgical intervention.
7.Application of cosmetic suture technique for reparation and reconstruction in facial emergency surgery

Objective To investigate the clinical effect of cosmetic suture technique applied in organs of facial emergency surgery. Methods There were 25 patients who were admitted in our hospital from August 2009 to December 2013. After anatomical reduction and functional restoration, they were given surgery with cosmetic suture technique on the base of debridement. Results All of the patients a-chieved good surgical result, the satisfaction is 100%. After the first stage of operation, 23 patients of them were of no obvious scar and good function recovery. The other 2 patiens also got good effects after secondary surgery. Conclusion Cosmetic suture technique pay attaintion to anatomical and functional restoration. It is of equisite technique, slight injury, less scar after operation and it can maximumly get close to the normal tissue structures and achieve the objective of beauty.
8.Effect of Acupuncture on Expression of IL-6 mRNA of Acute Cerebral Ischemia Following Reperfusion in Rats

Objective To observe the expression of IL-6 mRNA and study the mechanism of acupuncture in focal cerebral ischemia.Methods In situ hybridization was used to detect the expression of IL-6 mRNA in the sham operation group, control group and acupuncture group 2 h, 6 h, 12 h, 24 h after reperfusion in the brain of middle cerebral artery occlusion rats, meanwhile and also observe ultrastructure in cortex of ischemic brain tissue 24 h after reperfusion.Results The sham operation group showed a basic expression of IL-6 mRNA in cortex and striate body. The cells expressing IL-6 mRNA were increased 2 h after ischemic reperfusion in cortex and striate body and peaked 24 h after reperfusion in control group and acupuncture group. The cells expressing IL-6 mRNA in cortex of ischemic area were stronger in acupuncture group than in control group at every research time point besides 2 h after ischemic reperfusion. The cells expressing IL-6 mRNA striate body of ischemic area were stronger in acupuncture group than in control group at every research time point. Acupuncture can lighten the degeneration of cell nucleolus and necrosis of cell, resume the integrity of the nucleolus, improve the function of mitochondrial and endoplasmic, and have significant difference with control group. Conclusion Acupuncture can increase the cells expressing of IL-6 mRNA.
9.Choice of optimal phase for liver angiography and multi-phase scanning with multi-slice spiral CT

Objective To evaluate the effieaey of test bolus technique with multi-slice spiral CT (MSCT) for determining the optimal scan delay time in CT Hepatic artery (HA)-portal vein (PV) angiography and multi-phase scanning.Methods MSCT liver angiography and multi-phase scanning were performed in 187 patients divided randomly into two groups.In group A (n =59),the scan delay time was set according to the subjective experiences of operators; in group B (n=128),the scan delay time was determined by test bolus technique.Abdominal aorta and superior mesenteric,vein were selected as target blood vessels,and 50 HU was set as enhancement threshold value.20 ml contrast agent was injected intravenously and time-density curve of target blood vessels were obtained,then HA-PV scanning delay time were calculated respectively.The quality of CTA images obtained by using these 2 methods were compared and statistically analysed using Chi-square criterion.Resuits For hepatic artery phase,the images of group A are:excellent in 34(58%),good in 17(29%),and poor in 8 (13%),while those of group B are excellent in 128( 100%),good in 0(0%),and poor in 0(0%).For portal vein phase,the images of group Aare:excellent in 23(39%),good in 27(46%),and poor in 9(15%),while those of group B are excellent in 96 (75%),good in 28 (22%),and poor in 4 (3%) respectively.There was statistically significant difference between the ratios of image quality in group A and group B (X2=14.97,9.18,P < 0.05).Conclusion Accurate scan delay time was best determined by using test bolus technique,which can improve the image quality of liver angingraphy and multi-phase scanning.
10.Effects of Immunoglobulin G on TNF-α Secretion of Human Peripheral Blood Mono-nuclear Cells in Type 2 Diabetes Mellitus Complicated with Alzheimer's Disease

Objective To explore the effects of immunoglobulin G on tumor necrosis factor (TNF-α) secretion of human peripheral blood mononuclear cells(PBMCs) in type 2 diabetes mellitus complicated with Alzheimer's disease(AD). Methods A total of 20 cases were chosen in each group from patients with type 2 diabetes mellitus complicated with Alzheimer' s disease and patients with type 2 diabetes mellitusm.PBMCs were isolated using density gradient centrifugation method and incu-bated with phytohemagglutinin (PHA) as well as immunoglobulin G extracted from blood serum of the patients with type 2 diabe-tes mellitus.TNF-α concentration in supernatant was measured by enzyme linked immunosorbent assay (ELISA). Results TNF-α concentration in supernatant of PBMCs was significantly increased in type 2 diabetes mellitus complicated with AD com-pared with that in type 2 diabetes mellitus (P<0.01).IgG further increased TNF-α concentration in type 2 diabetes mellitus and type 2 diabetes mellitus complicated with AD(P<0.01). Conclusion IgG extracted from the serum of type 2 diabetes mellitus patients can promote TNF-α secretion in PBMCs of type 2 diabetes mellitus complicated with AD.Immunity injury could play a role in the development of type 2 diabetes mellitus complicated with AD by promoting inflammatory reaction.
