OBJECTIVE:To establish a method for the contents determination of tetrahydropalmatine and rosmarinic acid in Xiaoheling capsule. METHODS:HPLC was performed on the column of Phenomenex C18 with mobile phase A of 0.1% phosphoric acid(adjusted pH to 6.0 with triethylamine)-methanol(45:55,V/V) and (adjusted pH to 6.0 with triethylamine) B of 0.1% phos-phoric acid-methanol(55:45,V/V)(gradient elution)at flow rate of 0.8 ml/min,detection wavelength was 282 nm(0-25 min)and 330 nm(25.01-60 min)and volume size was 10 μl. RESULTS:The linear range was 1.502-75.10 μg/ml for tetrahydropalmatine and 6.03-301.6 μg/ml for rosmarinic acid(r=0.999 8);RSDs of precision,reproducibility and stability tests were ≤1.36%;recoveries were 97.11-99.87%(RSD=0.95%,n=9)and 98.47-101.99%(RSD=1.08%,n=9),respectively. CONCLUSIONS:The method is simple,accurate and reliable,and can be used for contents determination of tetrahydropalmatine and rosmarinic acid in Xiaoheling capsule.

Objective To evaluate the diagnosis and treatment of pseudoaneurysm .Methods The clinical data of 17 cases of pseudoaneurysm from Jan .2007 to Dec .2011 were reviewed retrospectively ,10 cases were caused by iatrogenic ,6 cases were drug caused by injection of poison ,and 1 case was was caused by trauma .2 cases of brachial artery pseudoaneurysm ,and 15 cases of arte-ria femoralis pseudoaneurysm .1 case was treated by simple compressive therapy ,the other 16 underwent surgical treatment and the pseudoaneurysm was removed ,in which 3 cases were treated by artery patch repair ,7 were treated by bypass with prosthetic grafts , 2 were treated by bypass with saphenous vein ,4 were treated by bypass with superficial femoral vein branch .Results There was no death except 1 case died because her family gave up ,All patients with signs and symptoms were significantly alleviated ,and there was no severe ischemia limb leading to amputation in postoperative follow-up .Conclusion The revascularization of the artery in pseudoaneurysm patients can make a significant improvement in the long-term quality of life .

Objective To explore the etiology,clinical features,and imaging features of multiple osteochondroma.Methods A total of 21 patients of multiple osteochondroma with completely clinical and imaging data were brought into this study.All these patients accepted X ray DR examination, and 15 cases of them accepted CT scanning and 4 cases of them accepted MRI examination.The clinical and imaging data were analyzed retrospectively combined with the relative literature.Results All the 21 patients with multiple osteochondroma had family history,and it's likely to be found in the metaphysis of limb long bone.Patients with multiple osteochondroma usually had bony lump with cartilage cap in long bones' metaphysis.It often occured in double sides but not symmetry,and it was more likely to be found in the lower limbs.Bone around the knee-joint was the most common position of lesion,and the growth direction was usually deviating from the joints.Conclusion Multiple osteochondroma has the characteristics of internalization bone cartilage defects and osseous vegetations formation,and it's usually combined with bone bending,shortening deformity and adjacent joint activities obstacles.It can be diagnosed according to the typical clinical features,X-ray and CT imaging features.

Background & Objective: Sleep quality in neuromyelitis optica spectrum disorders (NMOSD) were investigated in two recent studies. However, factors affecting sleep quality have not been studied in NMOSD. This study aimed to investigate the prevalence of sleep disorders in Chinese outpatient clinics with NMOSD and its clinical correlates. Methods: We administered Chinese validated self-questionnaires on HRQOL (MSQOL-54), sleep (PSQI), pain (SF-MPQ-2), anxiety (HARS) and depression (HDRS) to 42 patients followed up in our outpatient department. We assessed the relationships between sleep quality with pain, anxiety, depression, gender, age, disability, disease duration, NMO-antibody status and explored the determinants of poor sleep quality. Results: Sixty four percent of NMOSD patients were poor sleepers. Significant correlations were found between duration, disability, pain, anxiety, depression and sleep quality. Disability, depression and the domain of affective descriptors of pain were the three main predictors of poor sleep in NMOSD. Conclusion: This study reveals that poor sleep in NMOSD is common and it decreases physical function of quality of life. It is worthwhile considering exploring adjuvant strategies aimed at controlling pain associated affect, and treatment of depression may help to improve sleep quality in NMOSD.
Neuromyelitis Optica

Objective To establish a method of nucleic acid extraction and enrichment based on magnetic nanoparticle as medium for elevating the analytical sensitivity of domestic HBV real-time PCR kit and detection of the trace amount HBV DNA. Methods After receiving antiviral treatment, the serum samples of 50 hepatitis B patients with HBV DNA concentration ≤1×104 IU/ml were collected. The WHO HBV DNA calibrator was used as the standard material. Nanometer magnetic beads were used to adsorb and enrich the HBV nucleic acid and increase the concentration of the extracted HBV nucleic acid template. Compared with Roche HBV DNA detection reagent and four domestic reagent with conventional nucleic acid extraction and detection method, the improvement effect of this method on domestic nucleic acid detection reagent was evaluated. Results After application of nanometer magnetic extraction method to domestic regent, the analytical sensitivities of the domestic reagent reached 10 and 50 IU/ml, respectively,which was about the same detection level to 12 IU/ml of the imported Roche reagent. The positive rates of the detection of serum trace amount HBV DNA of hepatitis B patients with four kinds of domestic extraction reagent were 64% ( 32 ), 56% ( 28 ), 62% ( 31 ) and 58% (29), respectively. There were significant statistical differences between Roche reagent and four domestic extraction reagent kits(x2 = 7. 895, 12. 698,9. 013 and 11. 416 ,P ＜0. 05 ). With nanometer magnetic extraction method combined with domestic reagent kits, the detection rates were 88% (44), 88% (44), 88% (44) and 86% (43) ,respectively. There was no significant difference compared with the imported Roche reagent (x2 = 0. 000, 0. 000, 0. 000 and 0. 088,P ＞0. 05). Moreover, when the HBV nucleic acid concentration was 101-103 IU/ml, the logarithm value of viral nucleic acid concentration was in reverse correlation to Ct value, but the correlation decreased in the concentration range of 103-106 IU/ml. Conclusions The nucleic acid extraction method based on magnetic nanoparticle as medium can significantly improve the analytical sensitivity of domestic HBV DNA detection reagent, which can be used to monitor the trace amounts HBV DNA in the sera of the hepatitis B patients.

In this study, a novel tetrapeptide hydrophilic interaction chromatography (HILIC) material was synthesized, and corresponding enrichment method for glycopeptides was developed.The tetrapeptide modified silica gel materials (denoted as Poly-DAPD) were synthesized by atom-transfer radical-polymerization (ATRP) and characterized by N2 adsorption desorption and thermometer, thermal gravimetric analyzer (TGA) and X-ray photoelectron spectrometer (XPS).The characterization results indicated that tetrapeptide had been successfully synthesized on silica gel.Poly-DAPD materials showed high enrichment selectivity toward fetuin glycopeptides under solid-phase extraction (SPE) mode.Comparing with commercialized ZIC-HILIC in glycopeptides enrichment of fetuin digest which mixed with 5 mole ratio of albumin bovine (BSA), the as-prepared materials showed higher selectivity in both aspects of the identified number of glycopeptides and anti-interference property.The SPE results demonstrated that the tetrapeptide-based HILIC materials could be a potential tool in large-scale glycosylation analysis.

Objective To explore and analyze the application effect of hospital quality monitoring system in medical quality management.Methods 800 patients from March 2021 to March 2022 were selected as the control group and routine medical quality management was adopted;and 800 patients from April 2022 to April 2023 were selected as the experimental group and the hospital quality monitoring system was adopted for medical quality management.Results The complication rate and re-admission rate after implementation were significantly lower than before implementation,with statistical significance(P＜0.05).The satisfaction of patients after implementation was significantly higher than before implementation,and the difference was statis-tically significant(P＜0.05).The score of medical service quality after implementation was significantly higher than before im-plementation,and the difference was statistically significant(P＜0.05).Conclusion The application effect of hospital quality monitoring system in medical quality management is remarkable,and it is worthy of promotion.
