Objective:To explore influence of psychological intervention on mental state in patients With rheumatic heart dis-ease (RHD)after valve replacement surgery. Methods:A total of 60 RHD patients after valve replacement surgery Were randomly and equally divided into routine nursing group and psychological intervention group according to number table, the latter received additional psychological intervention besides routine nursing intervention.Symptom checklist 90 (SCL-90) Was used to assess mental state of tWo groups 1 d before and one Week after surgery. Results:There Were no significant difference in SCL-90 scores betWeen tWo groups before psychological intervention (P>0.05).Compared With routine nursing group one Week after operation, there Were significant reduction in SCL-90 scores of somatization [(1.31±0.47) scores vs.(0.55±0.68)scores],obsessive-compulsive symptom [(2.11±0.35)scores vs.(0.78±0.35)scores],inter-personal relationship sensitivity [(1.57±0.53)scores vs.(1.22±0.46)scores],depression [(1.63±0.29)scores vs. (1.00±0.41)scores],anxiety [(0.98±0.17)scores vs.(0.76±0.13)scores],hostility [(1.11±0.31)scores vs.(0.80 ±0.37)scores]and phobia anxiety [(1.39±0.26)scores vs.(0.53±0.19)scores]in psychological intervention group,P<0.01 or P<0.05. Conclusion:Psychological intervention can alleviate the adverse psychological state and improve quali-ty of life in patients With rheumatic heart disease.

Clinical Medicine of China 2000;0(12):-
Objective To evaluate the effects of percutaneous balloon mitral valvuloplasty (PBMV) on repeated rheumatism attacks after continual anti rheumatic treatment in vain over a long period of time. Methods 23 cases received treatment by PBMV and follow up visits were performed. Results After operation, patients' cardiac function and their physique were improved obviously and the activity of rheumatism ceased after routine anti rheumatic treatment and prevention of streptococcal infection.Follow up visits for 36~81 months showed that mitral re stenosis occurred to only 2 patients. Conclusion Patients with mitral stenosis whose rheumatism attacks repeatedly in spite of longterm anti rheumatic treatment may be treated with PBMV first and then given anti rheumatism. In this way,a better curative effect can be obtained.

Objective To analyse the clinical characteristics, gene mutation and enzymatic activity of αgalactosidase A(α-GalA)in a 15-year-old male patient with typical Fabry disease, whose mother was without any clinical manifestations. Methods Clinical features and laboratory data were collected from the patient and his mother.Genomie DNA was extracted from peripheral blood of the patient.his mother, and a healthy control subject.Seven exons of the GLA gene were amplified by PCR.PCR products were purified.cloned into T vector, and then sequenced. The enzymatic activity of α-GalA Was measured by fluorimetrie substrate assay. Results DNA sequencing results showed that a missense mutation of 10036-10038delAAG in exon 7 WaS identified in the patient, resulting in the replacement of 374 lysine and 375 glyeine by arginine, which Was not previously reported. The patient Was a hemizygote with gene mutation, his mother WaS a heterozygote carrying gene mutation, and the healthy control without mutation.α-GalA enzymatic activity assay showed that the enzymatic activity of the patient with GLA gene mutation was only 50%of the healthy control subject, while the enzymatic activity of the patient's mother Was about 70%of the heahhy control SObject.Conclusiolls Detecting GLA gene mutation and α-GalA enzymatic activity in patients with Fabry disease who have been clinically diagnosed seelns to be helpful in finding other patients in the family and in further understanding the molecular pathogenesis of that disease.

Objective To investigate the bettertherapeutic method and clinical efficacy of discoid meniscus injury under arthroscope. Methods Forty-one patients with discoid meniscus injury were selected as our subjects from Nov.2011 to Feb.2013,who were treated by arthroscopic surgery and performed with meniscus plasty.Thirt-eight patients were received ultra Fast-fix meniscal repair system.Joint function exercises were performed on bed within 1 month after operation.Incomplete weight bearing were performed after 1 month postoperation. After 3 month postoperation, they were permitted for full weight bearing. Results All patients were treated successfully with surgery and no postoperative complications occurred. The patients were followed up from 3.0 to 18.0 months and average was 10.5 months. The preoperative Lysholm scores were(68.28 ±8.04)) lower than that at the postoperation (88.17 ± 7.49)) and the difference were significant (t =11.59,P < 0.05).Furthermore,according to Ikeuchi criterion, the outcomes were 28 cases with excellent,10 cases with good and 3 cases with general rank. Conclusion Discoid meniscus plasty and suture under arthroscope is proved to be a better approach with less trauma, recovery quickly and fewer complications. It should be a preferred method for discoid meniscus injury.

With the increase of the incidence and the refinement of clinical treatment, the clear preoperative staging and the assessment of tumor recurrence and metastasis of cervical cancer is particular important.Different results may be explained in the same tumor applied different methods of imaging examination, which influences the clinical diagnosis and treatment. So imaging examination plays a more and more important role in the assessment of preoperative staging and tumor recurrence.

The feasibility of bone marrow stromal cells autologous transplantation for rabbit model of dilated cardiomyopathy induced by adriamycin was studied. Twenty rabbits received 2 mg/kg of adriamycin intravenously once a week for 8 weeks (total dose, 16 mg/kg) to induce the cardiomyopathy model with the monitoring of cardiac function by transthoracic echocardiography. Marrow stromal cells were isolated from cell-transplanted group rabbits and were culture-expanded on the 8th week. On the 10th week, cells were labeled with 4,6-diamidino-2-phenylindole (DAPI), and then injected into the myocardium of the same rabbits. The results showed that viable cells labeled with DAPI could be identified in myocardium at 2nd week after transplantation. Histological findings showed the injury of the myocardium around the injection site was relieved with less apoptosis and more expression of bcl-2. The echocardiography found the improvement of local tissue movement from (2.12+/-0.51) cm/s to (3.81+/-0.47) cm/s (P<0.05) around the inject site, but no improvement of heart function as whole. It was concluded bone marrow stromal cells transplantation for dilated cardiomyopathy was feasibe. The management of cells in vitro, the quantity and the pattern of the cells transplantation and the action mechanism still need further research.

SUMMARY Alport syndrome (AS) is a progressive renal disease characterized by hematuria and progressive renal failure. AS is a clinical and genetic heterogenous disease, X-linked dominant Alport syndrome(XLAS) is the major inheritance form. XLAS females have variable phenotypes, from microscopic hematuria to chronic renal failure. The effects of pregnancy course on pregnant woman and fetus were observed by follow-up study of an XLAS female. Results showed that pregnancy of the XLAS female was complicated by renal function deterioration, and fetal growth restriction. It suggests that the AS female with damaged renal function should be cautious in the choice of pregnancy.
