2.Autosolidifying calcium phosphate cement in the repair of bone defects due to different etiology among 94 cases

BACKGROUND: Due to good biocompatibility, autologous bone has been considered as the optimal graft, but its source is too limited to meet the clinical requirements. In addition, the harvesting of iliac bone is associated with significant morbidity, thus searching for a substitute material of autologous bone graft is a current topic of study.OBJECTIVE: To study the biocompatibility and osteogenesis capacity of autosolidifying calcium phosphate cement (CPC) in the repair of bone defects.DESIGN, TIME AND SETTING: A self-control observation was performed among 94 patients who were admitted at the Department of Orthopedics in Yiwu Central Hospital (Yiwu, Zhejiang, China) from February 2001 to September 2004.PARTICIPANTS: Ninety-four patients with bone defects were involved in this study. The underlying cause of bone defects was fracture collapse and reduction in 63 cases, osteomyelitis in 20 cases, bone cyst in 6 cases, fibrous dysplasia in 4 cases and enchondroma in 1 case. The area of bone defects ranged from 1cm×1cm to 4cm×20cm.METHODS: CPC was the product of Shanghai Rebone Biomaterials Co., Ltd (License N0. 2005-3460304, China). CPC powder was mixed with solidifying liquid according to the ratio of 3.0g: 1mL, and the filling dose of CPC was 3-42g. There were 74 cases implanted with pure CPC, including 38 cases with thoracolumbar fracture undergoing vertebroplasty, 25 cases with fracture undergoing open repair, and 11 cases underwent focal debridement of benign bone tumor. in another 20 cases of osteomyelitis, drug-loaded CPC was implanted.MAIN OUTCOME M[EASURES: After the CPC implantation, all patients were observed according to the following indexes: allergic or toxic reaction, rash or high fever, levels of serum calcium, phosphors and alkaline phosphatase. X-ray radiography at month 12 after implantation was employed to observe the osseointegration of the implanted CPC to host bone and the degradation of CPC.RESULTS: All 94 patients were followed up for 14 months, and 76 of them for 24 months, 47 of them for 36 months, and 36 of them for 48 months. CPC developed primary solidification in human body within 30 minutes. Neither allergic or toxic reaction nor rash or high fever were found in all patients. The levels of serum calcium, phosphors and alkaline phosphatase were noted to be normal. No case companied with the itching in incision. The radiological examination showed that. the implanted CPC was directly bonded to the bone at the interface, and the bone contour at the defect sites was completely or partly restored. Degeneration and new bone were formed in some of the patients. Incision oozing light yellow fluids occurred in 9 cases, the bacterial culture was detected as negative, and all wounds healed through dressing changes. In the 20 cases implanted with drug-loaded CPC, no cases experienced recurrence of osteomyelitis and CPC degeneration was not complete.CONCLUSION: With good biocompatibility, safety and few complications, CPC is a good substitute for autologous bone graft in the repair of bone defects, and drug-loaded CPC is a selective treatment for osteomyelitis.
3.Application of case method teaching to emergency talent training in environmental hygiene

Issues of necessity, feasibility,procedures and noticeable problems about application of case method teaching were dis-cussed.
4.Differential Diagnosis of Common Liver Lipid-containing Tumors Using Diffusion Weighted Imaging

Purpose To investigate the application value of diffusion weighted imaging (DWI) in differential diagnosis of common liver lipid-containing tumors.Materials and Methods Sixty patients with pathologically confirmed liver lipid-containing tumors from January 2011 to August 2015 were retrospectively analyzed,including 25 cases ofhepatocellular carcinoma (HCC),20 cases of hepatic adenoma (HA) and 15 cases of hepatic angiomyolipoma (HAML).Apparent diffusion coefficient (ADC) value and ratio of ADCtumor/ADCliver (rADC) of tumors were respectively measured.Routine MRI findings were analyzed.Results The MRI findings of these three common liver lipid-containing tumors were with their own characteristics.For HCC,HA and HAML,the average ADC value was (1.225±0.221)×10-3 mm2/s,(1.318±0.212)×10-3 mm2/s and (1.317±0.297)×103 mm2/s,respectively,(b=800 s/mm2);the average rADC value was 1.004± 0.151,0.984 ± 0.146 and 1.027±0.223,respectively.There was no significant difference in ADC and rADC among these three tumors (P>0.05).Conclusion ADC and rADC had no differential diagnosis value in common liver lipid-containing tumors.Routine MRI signals and enhanced imaging features are the main diagnostic basis.
5.A research on effects of rhubarb combined with glutamine on repair of gut mucosal barrier injury in rats

Objective To investigate the effects of rhubarb combined with glutamine on repair of gut mucosal barrier injury after gastrointestinal surgery in rats. Methods Fifty male healthy Sprague-Dawley(SD)rats were randomly divided into five groups:control(C group),enteral nutrition(EN group),rhubarb with EN(R-EN group), glutamine with EN(G-EN group)and rhubarb combined with glutamine and EN(R-G-EN group)groups(each n=10). The rats of C group received no treatment and were fed normally. Cecectomy and gastrostomy were performed in EN,R-EN,G-EN and R-G-EN groups,and in 24 hours after surgery,they were interfered with the use of amoxicillin 50 mg/d and metronidazole 20 mg twice a day. From the first day after operation,each group was treated with the same prescription of enteral isocaloric and isonitrogenous nutrition for consecutive 7 days. Seven days later, under anaesthesia,laparotomy was performed,and a specimen of 1 cm intestinal segment was taken from the terminal 5 cm long ileum for all the groups;under the light microscope,the morphological examination of intestinal mucosal epithelial tight junctions(including villus height and mucosal thickness)was carried out. For each group,enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of plasma citruline and D-lactate,and the serum concentration of TNF-αwas detected by utilizing double antibody ABC-ELISA. Results The levels of D-lactate and TNF-αin EN,R-EN,G-EN and R-G-EN groups were obviously higher than those in C group(all P<0.01). The level of plasma citruline(μmol/L:16.13±0.58 vs. 14.52±0.28)of R-G-EN group was remarkably higher than that in R-EN group(P<0.05),and its levels of serum D-lactate(mmol/L:3.12±0.56 vs. 3.62±0.52)and TNF-α(ng/L:2.82±0.62 vs. 3.54±0.49)were markedly lower than those of G-EN group(both P<0.05). The mucosal villus height(μm:126.50±6.31 vs. 175.67±11.91),mucosal thickness(μm:223.67±11.94 vs. 277.00±14.06)and the level of plasma citruline(μmol/L:13.25±0.36 vs. 15.67±0.55)in EN group were obviously lower than those of C group(all P<0.01);the above indexes of R-EN,G-EN and R-G-EN groups were higher than those of EN group (P<0.05 or P<0.01),but in the comparisons of the villus height and mucosal thickness among the three groups there were no statistical significant differences(all P>0.05). Conclusion The application of rhubarb combined with glutamine can protect gut mucosa,reduce the level of inflammatory reaction and elevate the level of plasma citruline, that is beneficial to the repair of gut mucosal barrier injury and its functional recovery after gastrointestinal surgery in rats.

7.Cystitis and obstructive uropathy in systemic lupus erythematosus: a case report and literature review

Objective To enhance the understanding of cystitis, obstructive uropathy (OU) and chronic intestinal pseudo-obstruction (CIPO) in systemic lupus erythematosus (SLE). Methods We presented a case of SLE complicated with cystitis, OU and CIPO, and reviewed relevant English and Chinese literature. Results The case was a 37 year-old female with a history of SLE for 4 years. She developed urinary irritant symptoms for 1 year and abdominal distention for 1 month before admission. Ultrasound and cystoscopic studies revealed a thickening of bladder wall and reduced bladder volume; cystoscopic study revealed inflammatory and edematours mucosa; Ultrasound study and X-ray image confirmed the existence of OU and CIPO. Symptoms of urinary irritant and CIPO were relieved after initiation of prednisone and cyclophosphamide (CTX), but reducing the dosage of prednisone and CTX had caused a relapse. Conclusion Cystitis is a rare clinical manifestation of SLE, it is likely to be misdiagnosed. The pathological change of cystitis complicated with SLE is interstitial cystitis and is the most common cause of OU in SLE. OU is closely related to CIPO and it is suggested that patients with these complications may be classified as a unique subgroup of SLE. Intestinal obstruction is a severe complication of SLE with high mortality rate, clinicians should be aware of this unique subgroup of SLE and treat it properly.
8.Effect of exercise training on amyloid-beta peptide and β-secretase in the hippocampus of the rats with vascular dementia

ObjectiveTo study the effect of exercise training on β-amyloid polypeptide (Aβ) and β-secretase(BACE) in the hippocampus of the rats with vascular dementia (VD).Methods 30 Sprague-Dawley (SD) rats were carried out to an exercise group (n =10 ),a model group (n =10 ),and a sham-operation group ( n =10 ).VD rat models were made by the ligation of bilateral common carotid arteries.Morris water maze test were carried out 4 weeks after the operation to assess the ability in learning and memory of the rats and Aβ and β-secretase (BACE)expression was detected in the hippocampus of the rats using immunohistochemical techniques.ResultsIn the Morris water maze test,the model group showed reduction in the learning and memorizing ability,with obvious longer escape latencies ( ( 101.34 ± 19.67 ) s,(95.42 ± 23.89 ) s,( 89.39 ± 22.67 ) s,( 90.12 ± 19.77 ) s,respective-ly) than that of sham-operation group ( ( 62.13 ± 11.38 ) s,( 24.84 ± 13.69 ) s,( 16.98 ± 12.51 )s,( 11.41 ± 8.93 ) s,correspond-dingly) (P < 0.05 ),and the exercise group was improved in the learning and memorizing ability ( corresponding to ( 80.15 ± 21.56 ) s,( 51.24 ± 20.91 ) s,( 43.78 ± 22.36) s,( 45.67 ± 20.87 ) s ),compared with the model group(P<0.05).The grey values of Aβ in the hippocampus of the rats for the exercise group was ( 130.12 ± 19.01 ),( 116.77 ± 23.67 ) for the model group and ( 148.44 ± 17.67 ) for the sham-operation group(P< 0.05).The grey values of BACE in the hippocampus of the ratsfor the exercise group were( 131.21± 25.25 ),( 120.53± 10.21 ) for the model group(P< 0.05 ) and ( 162.38 ± 28.11 ) for the sham-operation group (P < 0.05).ConclusionExercise training can lower the expression of BACE and Aβ in the hippocampus of rats with VD,therefore improving the learning and memory ability of rats with VD.
9.Local moderate hypothermia therapy for acute cerebral hemorrhage

Objective To evaluate local moderate hypothermia technology on acute brain hemorrhage cases.Methods 100 patients of cerebral hemorrhage were randomly divided into two groups(moderate hypothermia groupand control group). The moderate hypothermia group was 50 cases treated with moderate hypothermia after routine treatment,the control group was 50 cases with routine treatment. The two groups were assessed after treatment for 21 days. Results The group treated via moderate hypothermia therapy showed the nerve function damages relatively lower than the control group(P < 0.05), and the surviving probability and curative ratio were significant higher than the control group(P< 0.05 ). Conclusion The local moderate hypothermia therapy for acute cerebral hemorrhage patient in clinical can significantly improve the never cell function and reduce mortality and disablement.
10.Treatment of sacroiliac joint dislocation with percutaneous sacroiliac screw internal fixation

Objective To analyze the clinical effect of percutaneous sacroiliac screw internal fixation in treatment of sacroiliac joint dislocation. Methods From June 2002 to August 2006,16 patients with sacroiliac joint dislocation were treated with percutaneous sacroiliac screw internal fixation under C-arm X-ray tomography.There were 10 males and 6 females at age range of 10-58 years(mean 34.3 years).Results The operation lasted for 30-90 minutes(average 50.5 minutes).All patients were followed up for 12-36 months(average 18.3 months).The results of postoperative normotopia,lateral,ingate and egress Xray and CT scanning showed that all the screws were located within S1 and S2 of all,14 patients obtained satisfactory result of reduction,with no infection,nerve injuries,loosening or breakage of the screw fixation.The function and the sensation of the sacroiliac ioint and low extremities recovered to normal. Conclusions Percutaneous sacroiliac screw intemal fixation is an ideally safe and effective way to treat sacroiliac joint dislocation,for it has many advantages such as minimal invasion,reliable fixation,less complication and quick recovery.It is also very necessary to take caudad and cephalad view under an image intensifier during the operation to assure the accuracy of implantation.
