Objective To study the role of Th1/Th2 cytokine profile in the pathogenesis of recurrent genital herpes (RGH), and to find out the relationship between Th1/Th2 cytokines. Methods A two-colour immunofluorescent staining of cell surface antigen and intracellular cytokines (IL-2、 IL-10、 IL-12、 IFN-? and TNF-? ) in CD3+ T-lymphocytes of 20 patients with RGH with flow cytometric analysis were performed. Results Compared to controls, patients with RGH showed a decreased number of CD3+ T cells( P0.05) . Conclusions RGH patients seem to have a predominance of Th2 cytokine profile, which may play an important role in the pathogenesis of RGH.

Objective To investigate Shuxuening injection combined with deproteinized extract of calf blood on serum IGF-1, IL-1 and ICAM-1 level in patients with cerebral infarction recovery stage.Methods 120 patients with cerebral infarction were collected.According to the different drug treatment, 60 cases in each group were given corresponding drug treatment, on the basis of control group, the calf blood extract injection was given in experimental group.After the end of treatment, all patients blood rheology, IGF-1, IL-1 and ICAM-1 levels were tested.Results Compared with control group, the indexes of the patients in experimental group improved more significantly, whole blood viscosity, red blood cell pressure volume, platelet aggregation rate decreased significantly(P<0.05); serum IGF-1 levels in experimental group decreased significantly(P<0.05).The levels of serum IL-1, ICAM-1 in experimental group decreased significantly(P<0.05).Conclusion Shuxuening injection combined with deproteinized extract of calf blood can significantly reduce serum IGF-1, IL-1 and ICAM-1 levels in patients with cerebral infarction recovery stage, improve blood flow, reduce blood viscosity.
5.Detection of Th1/Th2Cytokines in the Peripheral Blood Lymphocytes of Patients with Condyloma Acuminatum

Objective To detect cytokine levels secreted by CD3 + T cells and monitor the Th cell im-balance in the patients with condyloma acuminatum(CA).Methods The peripheral blood lymphocytes were isolated and stimulated prior to analysis.Cytokine detection was performed using monoclonal antibodies,which were fluorochrome-conjugated for easier direct manipulation in flow cytometry analysis.The results were pre-sented as cytograms showing dual staining of lymphocytes.Results The levels of IL-2,IL-12,IFN-?and TNF-?produced by Th1cells decreased,while the level of IL-10produced by Th2cells had no significant change in CA patients in comparison with healthy controls.Conclusions There is Th1/Th2imbalance in CA patients with a decline of Th1type cytokines.The Th1/Th2imbalance might be the mechanism that HPVs es-cape from immunosurveillance in CA patients.
6.Conversion from cyclosporine to tacrolimus for the treatment of chronic allograft nephropathy

Objective To assess the effect of immunosuppression conversion from cyclosporine(CsA) to tacrolimus(FK506) on progression of chronic allograft nephropathy(CAN).Methods A retrospective study was performed in 36 cyclosporine treated renal transplant recipients.Patients were included if they were biopsy-proven CAN and they were converted from cyclosporine to tacrolimus due to CAN.The FK506 dose was 1/50 to 1/100 of the cyclosporine's and the dose was adjusted according to the trough level of tacrolimus,body weight of the patient and the situation of CAN.Other immunosuppressive agents kept unchanging.Serum creatinine and FK506 trough level were monitored after conversion.Fasting blood triglyceride,total cholesterol and glucose were determined at baseline and 6th month after conversion to tacrolimus.Results After switching over to FK506 for 6 months,there was a significant improvement in function of renal allograf(P
7.Diagnosis and treatment of severe pulmonary infection in patients after kidney transplantation

Objective To study the diagnosis and treatment of the severe pulmonary infection in the patients after kidney transplantation. Methods The clinical data of 26 patients with severe pulmonary infection following kidney transplantation were analyzed retrospectively. Results Microorganisms were isolated and identified in 22 patients out of 26 kidney transplantation patients with severe pulmonary infection. The main etiological pathogens according to their frequency and type were: bacteria (15 cases, including Escherichia coli, Aerobacter cloacae, Klebsiella fredlanderi, Enterococcus faecalis, Staphylococcus epidermidis, etc.), fungi (12 cases, Fermentum, Blastomyces albicans, Candida tropicalis, Aspergillus, etc.), and cytomegalovirus (10 cases). 46.15% (12/26) of patients were infected with one kind of microorganism, and 53.85% (14/26) of patients were mixed infection. In 73.1% (19/26) of patients the pulmonary infection occurred during 1-6 months after renal transplantation. Among 26 patients, 12 developed ARDS, and 4 patients gave up therapies due to high expenses. With energetic treatment, 18 patients (81.82%) were cured and 4 died. Conclusions Intensive care and active measures should be given in the treatment of severe pulmonary infection after kidney transplantation. Early diagnosis, administration of broad-spectrum and combined use of antibiotics, the early identification of pathogens, enforcement in systemic support, including correction of immunosuppression, the timely use of mechanical ventilation to correct hypoxia, are the key treatment strategies for a successful result.
8.Relationship between preoperative breast specific gamma imaging tumor/normal take-up ratio and the prognosis of breast cancer

Objective:To investigate the relationship between T/N take-up value and the clinic feature of breast cancer, and explore the predictive ability of T/N take-up value for breast cancer prognosis.Methods:Breast cancer patients treated at Zhongshan Hospital, Fudan University and undergoing BSGI examination before surgery from Jan 2014 to Nov 2018 were collected. The relationship between T/N take-up value and the clinical features of breast cancer and DFS was analyzed.Results:A total of 419 breast cancer patients were collected. The median follow-up time after operation was 34 months. At the end of the follow-up, 43 cases relapsed and 376 cases did not. The average T/N values of CC position and MLO position before surgery were 2.74 and 2.58. The T/N value of CC is related to tumor stage (χ 2=22.077, P<0.05), lymph node metastasis ( Z=2.138, P<0.05) and degree of invasion ( Z=3.371, P<0.05). The T/N value of MLO is related to tumor stage (χ 2=23.091, P<0.05), lymph node metastasis ( Z=2.531, P<0.05) and degree of invasion ( Z=2.99, P<0.05). The best cut-off values of CC position and MLO position calculated by ROC curve are 2.59 and 2.97. Univariate analysis showed that tumor staging ( HR: 2.039, 95% CI: 1.404-2.962, P=0.001), T/N value at CC position ( HR: 4.349, 95% CI: 2.141-8.883, P<0.001) and T/N value at MLO position ( HR: 2.767, 95% CI: 1.520-5.039, P<0.001) is an independent risk factor for the prognosis of breast cancer. Multivariate COX regression analysis showed that tumor stage ( HR: 1.959, 95% CI: 1.302-2.946, P=0.001) and T/N value at MLO position ( HR: 3.498, 95% CI: 1.531-7.992, P=0.003) are independent risk factor of breast cancer prognosis. Conclusion:The BSGI T/N value has a certain correlation with breast cancer DFS.
9.CT image features of urinary tract inflammatory myofibroblastic tumor

Objective To evaluate the CT image features of urinary tract inflammatory myofibroblastic tumor (IMT) for use in clinical and differential diagnosis.Methods Eight pathologically proven IMT cases were retrospectively analyzed in this study.There were six female patients and two male patients.Patient average age was 35 (13-62) years.The main clinical manifestations were hematuria and urination pain.Tumors were found in the bladder in 5 cases,in ureter in 2 cases and in urethra in 1 case.All the 8 cases accepted CT scan five of which had contrast enhanced CT scan.The imaging features were then analyzed.Results Of the IMT in bladder,CT scan showed a cauliflower like bladder tumor or round soft tissue mass with even or uneven density of 11.5-36.0 HU.Three tunors were accompanied with necrosis.Four cases had associated adjacent bladder wall thickening and two cases were found with fuzzy surrounding fat space.Ureteral lesions in two cases showed a solid mass with smooth edges and even density of 40.3 HU compared to the surrounding muscle.The urethral lesions had clear mass realm of 17.5-22.6 HU accompanied with calcification.Enhanced CT scan showed that homogeneous or heterogeneous mild to moderate enhancement was found in the tumor.Delayed phase showed consistant enhancement of 102.7-118.6 HU.Conclusion Tumor CT imaging features,particularly the significant and consistant enhancement in delayed phase,provide important information for diagnosis and differential diagnosis of the urinary tract IMT.
10.Epidemiological study of polyomavirus type BK infection in renal transplant recipients——single center and prospective study

Objective To investigate the epidemiological characteristics of polyomavirus type BK infection in renal transplant recipients.Method We systematically screened for active BKV infection preoperation and at 0.5,1,3,6,9,12 and 15 months after transplantation in 116 renal transplant recipients.The screening tests included urine cytology (by the Papanicolaou method) and BKV DNA PCR (the kit for testing the BK virus) assay of both urine and plasma,and the results were recorded.Renal biopsy was performed if the graft function was deteriorated gradually or the loads of BKV replication were very high.Routine histopathological examination and immunohistochemistry were performed on renal tissues from partial patients who received the tests of renal biopsy.Result Throughout the follow-up of 15 months,urinary decoy cells (median 8/10 HPF,[1~ 48/10 HPF]),BKV viruria (median 2.63 × 105 copies/mL,[1.78 × 103 ~ 8.54 × 109 copies/mL]),BKV viremia (median 2.70 × 104 copies/mL,[1.95 × 103 ~6.31 × 106 copies/mL]),and BKVAN (4 patients) occurred in 53.46%,24.17%,20.72% and 3.45% of renal-transplant recipients,respectively.The positive rate of the decoy cell and BKV DNA in urine reached the peak at the third month to the ninth month after transplantation,and the peak time of the BK viremia was the fifth month post-transplantation throughout the follow-up period.The change in BKV DNA level remained constant in blood and urine throughout the follow-up period.Conclusion The peak time of BKV infection was apparently three to nine months after transplantation,suggesting the importance of monitoring urine cytology and BKV DNA loads in post-transplantation patients closely during this period in order to reduce BKVAN after transplantation.
