BACKGROUND:Platelet-rich plasma has benefit in diabetic wound healing;however, the effect of al ogeneic platelet-rich plasma is stil unclear. OBJECTIVE:To observe the effect of al ogeneic platelet-rich plasma on col agen synthesis during diabetic wound healing. METHODS:Streptozocin-induced diabetic rats were randomly divided into al ogeneic platelet-rich plasma group and saline control group. Each group had 15 rats. A 1 cm2 ful-thickness skin defect on the rat dorsum was excised. Platelet-rich plasma or saline was applied. Platelet-rich plasma was prepared from the whole blood of al ogeneic healthy rats. Animals of each group were sacrificed on 3, 7, 14 days post operation. The differences of the wound closure rate, morphological character, hydroxyproline content and the relative mRNA expression of the col agen were observed. RESULTS AND CONCLUSION:The wound closure rates were lower in control group on day 3, 7, 14 post operation (P<0.05). Masson staining showed a decreased, disorder, loose col agen fibers distribution in diabetic control wound tissue. The results of the hydroxyproline test showed hydroxyproline content was significantly higher in platelet-rich plasma group at each time point (P<0.01). The relative mRNA expression of type I and III col agen presented a higher expression in platelet-rich plasma treated wound tissue at each time point (P<0.05). And the ratio between type I and III col agen was higher in platelet-rich plasma group at each time point (P<0.05). In summary, al ogeneic platelet-rich plasma can promote diabetic wounds healing, which may attribute to an enhanced col agen synthesis.

Objective To summarize our experiences in the diagnosis and treatment of giant colonic lipoma, and enhance awareness of the disease and avoid misdiagnosis. Methods We retrospectively analyzed the clinical data of 5 cases of giant colonic lipoma that were mistaken for colon cancer, and evaluate the possible reasons of misdiagnosis. Results Colonic lipoma is an uncommon benign tumor. Most cases with larger tumor size were misdiagnosed as colon cancer when they suffered from the symptoms of abdominal pain, bloody stool, abdominal mass, even bowel obstruction and intussusception.Barium enema and colonoscopy are important diagnostic means.Contrast enhanced CT scan can provide definitive topographical information identifying a colonic lipoma.Surgical resection was performed in all the 5 cases. Conclusion Giant colonic lipoma with intussusception can mimic a colonic carcinoma leading to misdiagnosis.

AIM: To determine the role of the glycocalyx in microvascular permeability. METHODS: Dextran was intravenously injected and quatitatively examined in the rat transient cerebral ischemic model. At the same time, endothelial glycocalyx (anionic sites) was labelled with the probe cationic gold colloid (CGC) using post-embedding technique and examined with electron microscope. RESULTS: The labeling of CGC decreased significantly following ischemia, meanwhile, microvascular permeability to dextran increased. CONCLUSION:Endothelial glycocalyx is very sensitive to ischemia or anoxia. Its disruption may be the initiator of the dysfunction of endothelium and the determinant of increased permeability.

Journal of Medical Postgraduates 2014;(10):1033-1037
Objective Knowledge about the infiltration of macrophages in diabetic wounds can help to figure out the pathogene -sis of poor healing of diabetic wounds .The aim of this study was to observe the macrophage infiltration and the expression of relative in-flammatory factors during the wound healing of diabetic rats and explore the relationship between macrophages and diabetic wound heal -ing. Methods Male Sprague Dawley rats were randomly divided into STZ induced diabetic group and normal control group .Each group had 15 rats.A 1 cm2 full-thickness skin defect was created on the rat dorsum .Wound samples and was excised .On post injury day (PID) 3, 7, and 14, rats of each group were sacrificed after wound samples and tissues around the wound edge were obtained .The differences of the wound closure rate , macrophage infiltration and the relative mRNA expression of the inflammatory factors from macro-phages were observed . Results The wound closure rate was lower in diabetic group on PID 3, 7, and 14 ([29.5 ±5.4]%vs [45.9 ± 12.8]%, [71.6 ±3.1]%vs [80.1 ±6.9]%, [93.9 ±2.8]%vs [99.4 ±1.4]%, P<0.05).HE staining showed inflammatory cells infil-diabetic wound tissue on PID 3 (P<0.05) and a higher expression on PID 14 (P<0.05).The CCR7 fluorescence staining showed more positive staining cells stayed in diabetic wound on PID 14. Conclusion Macrophage infiltration decreases in the early phase of diabetic wound healing and sustains in wound tissue in advanced stage accompanied by the expression change of related inflammatory factors, which could contribute to the difficult wound healing of diabetic rats .

Objective To discuss the diagnosis and surgical treatment of multiple endocrine neoplasia 1 (MEN1). Methods The clinical data of MEN1 from 1974 to 2005 were retrospectively analyzed. Results There were 17 cases of MEN1 in our group. Among them, 11 cases were from 4 families. MEN1 gene mutation was detected in 14 cases admitted after 1997. There were 11 cases of pHPT, 6 cases of insulinoma, 8 cases of pituitary adenoma, 6 cases of adrenal adenoma, 2 cases of thymic carcinoid and 2 cases of collagenoma. Two patients had respectively 4 glands involved, 3 patients had 3, 7 patients had 2 kinds of gland involved respectively. Four patients had only one kind of gland involved. A 12 year old girl had no clinical symptom and biochemical change. Six patients presented with nephrolithasis, 6 patients had impaired pancreatic endocrine function. Subtotal parathyroidectomy was performed in 4 patients of pHPT, and one patient received parathyroid adenoma enucleation. Insulinomas were enucleated in 4 patients. Two patients underwent thymus tumor extirpation. Conclusions MEN1 varies in symptoms, even those from the same family. Cermline mutation test helps establish diagnosis. Operations should be aiming at tumor resection as well as the improvement of life quality.

Objective To discuss the diagnosis and surgical treatment of multiple endocrine neoplasia 2 (MEN2). Methods The clinical data of 28 MEN2 cases from Jun 1997 to Jun 2006 were retrospectively analyzed. Results There were 25 cases of MEN2a and 3 cases of MEN2b. Among the patients of MEN2a, 23 patients were from 7 families with mutation of codon 634, exon 11 of RET, 3 patients of MEN2b had mutation of codon 918, exon 16 of RET and no family history. Twenty-two cases of MEN2a had thyroid masses with elevated calcitonin level, in which 17 were pathologically diagnosed as medullary thyroid carcinoma (MTC) ,12 patients had pheochromocytomas. Among them, 5 were of multiple foci and 2 were malignant. Five patients presented hyperparathyroidism and 3 patients were asymptomatic without biochemical alterations. Three MEN2b patients had MTC and mucosal ganglioneuromatosis with Marfanoid, one patient had bilateral pheochromocytoma. Total thyroidectomy with bilateral dissection of regional lymph nodes was performed in 12 patients of MEN2a, and nodule enucleations was done in other 5 patients followed by persistent elevated calcitonin level. Nine MEN2a patients underwent pheochromocytoma enucleation including bilateral adrenal resection in 3 cases. Three MEN2b patients underwent total thyroidectomy with bilateral lymph node dissection. Conclusions MTC is the most often complications of MEN2. Germline mutation test helps to make early diagnosis. Radical total thyroidectomy in young patients may prevent MTC.
