1.Analysis of Rationality and Therapeutic Efficacy of Antiemetics in Cancer Patients of Our Hospital

OBJECTIVE:To provide reference for rational use of antiemetics in cancer patients. METHODS:Two thousand six hundred and sixteen pieces of medical orders of patients receiving antiemetics were collected from oncology department of our hospital via EMRS during Oct. 2015-Jun. 2016. RESULTS:Among 2 616 cases surveyed,1 301 cases conformed to inclusion and exclusion criteria,of which irrational use of drugs were found in 595 cases,760 times in total. Main manifestations were unsuitable drug selection(33.82%),irrational usage and dosage(25.26%),unsuitable drug combination(34.08%)and non-standard medication course(6.84%). The incidence of nausea in female was higher than male,with statistical sighificance(P=0.003),but gender had no significant effect on the incidence of nausea(P>0.05). Age had no significant effect on both nausea and vomiting(P>0.05). The control rate of nausea/vomiting in rational drug use group was higher than irrational drug use group. Average cost of antiemetic treatment of different emetic risk chemotherapy drugs in rational drug use group were all lower than irrational drug use group, indicating therapeutic efficacy could be guaranteed and economic burden was reduced in rational drug use group. CONCLUSIONS:The antiemetic plan should be selected rationally according to emetic risk degree of chemotherapy drugs. Patients receiving combined chemotherapy should select antiemetic plan according to highest emetic risk degree of chemotherapy drugs,and strictly master usage, dosage and duration. At the same time,treatment cost should be considered when selecting antiemetic plan to guarantee the safe, effective,economical and rational use of drugs.
2.Preparation of gastric floating sustained-release tablets of Triperygium wilfordii and its quality evaluation

Objective To prepare gastric floating sustained-release tablets of Triperygium wilfordii and study its behavior of floating and release characteristics. Methods The sustained-release tablets were prepared by direct powder compared to tablets technique. The floating and release of total diterpene-lactones were used as indicators to evaluate and optimize the formulation. Then the formulation was optimized by influential factors and orthogonal design test. Results The gastric floating sustained-release tablets which was taken orally twice one day, were prepared with HPMCK4M as matrix, cetyl alcohol as floating assistant, sodium bicarbonate as gas-producer, PVP as poremaking. The tablets released 30% in 2 h, 60% in 6 h, exceeding 90% in 12 h, the release behavior of the tablets was fitted to Higuchi equation,and it was properly characterized by the drug diffusion and bulk erosion mechanism. Conclusion Gastric floating sustained-release tablets of T. wilfordii prepared has good behavior of floating and release characteristics.
3.Evaluation on in vitro release of gastric floating sustained-release preparation of Tripterygium wilfordii by HPLC fingerprints

Objective To investigate the drug in vitro release behavior of gastric floating sustained-release preparation of Tripterygium wilfordii and establish their quality evaluating methods.Methods HPLC was Employed to gain the fingerprints of releasing medium of preparations.Triptolide was selected as marker to calculate the linear equation concerning peak area then the relative drug release and f2 simila-rity factor value were acquired.Results Various components in gastric floating sustained-release tablets and pellets of T.wilfordii could not release synchronously while sustained-releasing but all of them in the gastric floating sustained-release capsules of T.wilfordii prepared by using multiparticulate site-controlled release technology could release synchronously while sustained-releasing.Conclusion The quality and in vitro release behavior of gastric floating sustained-release preparation of T.wilfordii could be evaluated more scientifically and comprehensively by using HPLC fingerprints method.
4.The relationship between strategy to pseudomonas aeruginosa quorum sensing and drug resistance

This paper underlines the composition of OS(Quorum Sensing) systems in Pesudomonas aeruginosa,the resistance mechanism of Pesudomonas aeruginosa and the relevance between QS systems and the resistance mechanism.What's more,it introduces several pathways of the interfering of QS systems to weaken the resistance.These will show a new prospect to the control of bacterial infection.
5.Quantitative Analysis of Oleanolic Acid from the Fruits of Glossy Privet (Ligustrum lucidum) in Different Growing Periods

By means of TLC-densitomery, the contents of oleanolic acid extracted from the fruits of Ligustrum lucidum were determined to be 8.04% (g/g) in the unriped fruit collected in August and 2.73% (g/g) in riped fruit gathered in December. This result showed that the oleanolic acid content in fructus L. Iucidum decreases gradually with growing until the fruit is fully riped.
6.Carcinoma showing thymus-like element of the thyroid:a clinicopathological analysis

Objective To investigate the clinical-pathological feature of careinoma showing thymus-like dif- ferentiation(CASTLE) of the thyroid. Methods The clinical data, histopathologie changes and immunohistoche- mical findings were studied in 1 case of carcinoma showing thymus-like differentiation of the thyroid with review of the related literature. Results The age of the patient was 38 years old. The clinical manifestation displayed a pain- less mass in the thyroid. On ultrasound the tumor appeared hypoechoic. Histologically the tumor cells were arranged in nests and islands accompanied by desmoplasfie strorna. The architecture thus showed a superficial resemblance to the lobulation seen in thymic carcinomas. The tumor was characterized by squamoid or syheytial-appearing cells with lightly eosinophilic cytoplasm. Nuciec were oval, pale to versicular, and had small distinct nudeoli. Immunohistoche- mically the tumor cells were positive for CK, CD5, CD117 but neyative for TG, Calcitonin, SYN, NSE, TTF- 1, EBV. Conclusion CASTLE is a very rate a carcinoma thyroid with architectural resemblance to thymic epithelial tumors. The immunophenolype of CASTLE is identical with that of thymie carcinoma.
7.Analysis of 105 Docetaxel Related ADR Reports in Our Hospital

OBJECTIVE:To investigate the characteristics and influential factors of adverse drug reactions(ADRs)of docetax-el in our hospital,and to provide suggestions for clinical rational drug use and individualized administration. METHODS:Medical records of patients receiving anti-tumor chemotherapy were extracted from hospital information system(HIS)of our hospital during Apr. 2014 and Oct. 2015,and Docetaxel related ADR reports were collected and comprehensively analyzed by retrospective classi-fied statistics. RESULTS:Among 417 cases,105 cases of docetaxel related ADR were reported. There were 69 patients aged 50-69 with highest ADR frequency. The ADR incidence of female patients was higher than that of male patients (29.29% vs. 19.66%). The incidence of docetaxel related ADR and arrest of bone marrow increased as the increase of drug dosage;the incidence of breast cancer ADR was the highest (46.03%),followed by lung cancer (37.25%) and gastric cancer (25.00%). The incidence of docetaxel+platinum chemotherapy plan ADR was the highest (35.10%). Main organs/system involved in ADR was hematological system (55.65%). CONCLUSIONS:The individual difference of docetaxal related ADR are great and affected by several factors. Individual physical condition of patients should be taken into consideration as early as possible,which can reduce the incidence of ADR and promote clinical rational drug use.
8.Study on the Improving the Quality Standard for Compound Pediatric Anticough Oral Solution

OBJECTIVE:To optimize and improve the quality standard for Compound pediatric anticough oral solution. METH-ODS:TLC was adopted for the qualitative identification of Glycyrrhiza uralensis;chemical reaction was adopted for the qualitative identification of ipecac tincture,ammonium chloride;and HPLC was adopted for the content determination of ammonium glycyrrhi-zinate:the column was Agilent Zorbax XDB C18 with mobile phase of acetonitrile-0.2 mol/L ammonium acetate(20∶80,V/V)at a flow rate of 1.0 ml/min,the detection wavelength was 250 nm,column temperature was 30℃,and the injection volume was 10 μl. RESULTS:TLC spots of G. uralensis were clear and well-separated,negative control without interference. Differential response of ipecac tincture and ammonium chloride showed significant positive characteristics. The linear range of ammonium glycyrrhizinate was 6.08-72.96 μg/ml (r=0.999 9);RSDs of precision,stability and reproducibility tests were lower than 1.0%;recovery was 99.69%-100.28%(RSD=0.20%,n=6). CONCLUSIONS:Optimizing and improving the standard are helpful for the quality con-trol of Compound pediatric anticough oral solution.
9.Investigation and Analysis of Automatic Injection Drug Dispensing System in Inpatient Dispensary

OBJECTIVE:To optimize drug for injection dispensing model in order to supply reference for popularity of automatic injection drug dispensing system(AIDDS) in inpatient dispensary.METHODS:Two kinds of drug dispensing model were adopted in five departments.In 10 working days before and after application of AIDDS,the time of drug dispensing flow needed was recorded.The time of every drug dispensing step needed for single patient was recorded and analyzed to evaluate the benefit of system.RESULTS:After application of AIDDS,the drug dispensing time for single patient reduced from 1.5~2.1 min to 0.45~0.80 min.The drug dispensing efficiency was improved significantly,the extent of which was in positive correlation to amount of patient demanding drug dispensing.CONCLUSION:The application of AIDDS speedups drug for injection dispensing,shortens dispensing time and saves human resource.
10.Photoprotective effect of a self-made sunscreen cream: an evaluation in vitro and in vivo by using guinea pigs

[Objective] To determine the sun protection factor (SPF) and protection factor of ultraviolet A (PFA) of a self-made sunscreen cream.[Methods] The sample of a self-made sunscreen cream and a commercial sunscreen cream were painted to an artificial skin (3M film) at a dose of 2 μl/cm2,and a tester was used to evaluate their photoprotective effect.A total of 48 albinism guinea pigs were classified into 8 groups to remain unprotected or be protected by the self-made or commercial sunscreen cream.A solar ultraviolet light simulator (SUV-1000) was used to simulate the ultraviolet rays in sunlight to irradiate the guinea pigs,and the photoprotecfive effect of these sunscreen creams was determined.[Results] The in vitro evaluation revealed that the SPF value of the self-made sun-screen cream and commercial cream was 32.26 ± 2.42 and 30.87 ± 2.57respectively (n =5,t =0.94,P > 0.05),and the PFA value was 24.28 ± 2.44 and 17.53 ± 2.28 respectively (n =5,t =4.52,P< 0.01 ).As the in vivo experiment showed,the SPF value of the self-made sun-screen cream and commercial cream was 30.39 ± 6.65 and 28.79 ± 7.36,respectively (n =12,t =0.38,P> 0.05),and the PFA value was > 8.91 and > 8.93 respectively.[Conclusion] The photoprotective effect of the self-made sunscreen cream is similar to that of the commercial cream.
