Objective To compare the influence of two different managements of relief incision (exposed relaxation therapy or absorbable hemostatic gauze) in infants' palatoplasty. Methods A total of 118 infants with cleft palate between 5- to 8-month-old for the study were randomly divided into two groups: group A was exposed therapy group, and in group B both sides of relief incision were placed absorbable hemostatic gauze. The wound bleeding, body temperature, diet and incidence of fistula were observed and analyzed after operation. Results There was no significant difference between the two groups in postoperative temperature, diet and wound healing (P>0. 05). The postoperative wound bleeding was statistically significant differences between the two groups (P<0. 05). Conclusions Either exposed therapy or placed absorbable hemostatic gauze in relief incision, has minimal systemic and local reactions in the postoperative infant. The advantages of exposed treatment are economic and simple, and less irritant to the wound.

Objective To investigate the surgical correction of alar deformity and repairing function of unilateral cleft lip in infant. Methods Millards procedure was used to incise skin, muscle and mucosa, correct the nasal deformity through lip. At the normal side, ophthalmic scissors was applied to incise lips muscle and skin, and reach the deep level of the base of nasal pillor, cut the abnormal attachment between the orbicular muscle of mouth and anterior nasal spina, and sharp dissect cartilage and skin in nasal wing through deep level of nasal pillor. At the cleft side, the abnormal muscle branch was cut that attached at the down side of the base of nose and pyriform aperture, the tissue between nasal base and lip, maxilla relaxed, and the cartilage and skin in the nasal wing at the same side dissected. The deform cartilage was repairied as the same of normal side. After the rehabilitation of cartilage of nasal wing, the soft tissue at deep level was moved to normal location, but the deep level structure of nasal wing cartilage, nasal septum cartilage and nasal bone did not dissected. After the dissection of orbicular muscle of mouth, the muscle in nose and lip must be relaxed in illous side. The orbicular muscle of mouth was sewed under the condition of tensionless. The repairing of orbicular muscle of mouth will improve the balance of muscular strength around the nose. Thus, the deviation of nasal pillor was corrected and the base of nose repaired. Results Eighty cases of infant with unilateral cleft lip at the age of 3 months ～12 months, in which 11 cases were Ⅰ?,16 Ⅱ, and 3 Ⅲ?. Eighty cases were low grade alar deformity, and 13 middle grade deformity. After operation, 52 cases were better, 17 cases were good, and 11 cases were worse. The postoperational result was good. Conclusion While performing the cleft lip operation or Millard operation, the authors can correct the nasal deformity through lip. So, it is recommended that cleft lip and the alar deformity can be repaired at the same time.

AIM: IL-12 acts upon T lymphocytes and activates its receptor complexes of ?1/?2 ,and so IL-12 can regulate TH1/TH2 balance. Our study is aimed at IL-12-inducing apoptosis of T cells and expression and signal transduction of Fas/FasL during T cell apoptosis. METHODS: The apoptosis of T cells was detected by Annexin V staining cytometry and the expression of Fas/FasL under different inhibitors were detected by semi-quantitative PCR. RESULTS: IL-12 induced the human leukemic T cell line(TIB-152) and the human lymphoma T cell line(HTB-176) and the normal human T cells to undergo apoptosis. The FasL expression at 6 hours after treatment with IL-12 increased apparently, and reached the max at 24 hours, and FasL expression induced by IL-12 was inhibited by PKC inhibitor. But IL-12 did not influence Fas expression. CONCLUSIONS: IL-12 can induce T cells to undergo apoptosis which is characterized by early membrane changes, the inducing effect is correlated with the concentration of IL-12 and the maturation of T cells. FasL participates in the progression of T cell apoptosis as a apoptosis mediator, and the effect of IL-12 on FasL expression may be related with PKC pathway.

Objective ToexploretheprognosticvalueoftroponinT(cTnT)concentrationinpatientswithacuterespiratorydis-tress syndrome(ARDS) .Methods 82 patients from January 2008 to June 2011 were enrolled ,and admission blood samples were obtained for cTnT measurement .A variety of clinical and laboratory variables were recorded .With 28 days as end point ,the cTnT concentration ,acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) score ,and Murray lung injury score(LIS) were compared respectively between survivors and non-survivors .The association between the concentration of cTnT and 28-day mortali-ty was assessed .Results 43 patients died during the 28-day observation period among 82 patients .cTnT concentration among non-survivors were significantly higher than those of survivors ,(0 .048 ± 0 .03)ng/mL vs .(0 .027 ± 0 .014)ng/mL(P<0 .01) .The con-centration of cTnT showed positive correlation with APACHE Ⅱ score and LIS(r1 =0 .509 ,r2 =0 .314 ,both P<0 .01) .cTnT con-centration had an area under the receiver operating characteristic (ROC) curve(AUC) of 0 .734 for predicting 28-day mortality ,with 95% confidence interval(95% CI) 0 .627 -0 .841 ;the optimal cTnT cut point for predicting 28-day mortality was 0 .034 ng/mL . cTnT concentration higher than 0 .034 ng/mL ,APACHE Ⅱ score higher than 17 .5 and LIS higher than 1 .65 were the independent risk factor of 28-day mortality .Conclusion cTnT concentration is an independent predictor of 28-day mortality in patients with ARDS .cTnT concentration shows a high predictive value in patients with ARDS ,similar to APACHE Ⅱ score and LIS .

Objective The purpose of this study was to evaluate the outcome of autogenous bone grafts in unilateral cleft lip and palate patients following early orthodontic tooth movement,and to determine the volume of new bone formation in the bone grafted region with spiral computed tomography.Methods Computed tomography scans of 12 patients were taken immediately preoperatively and at 6 months postoperatively.The patients underwent bone grafting between 9 and 13 years of age were divided into two groups based on whether postoperative orthodontic tooth movement were initiated or not.Three-dimensional models were created in each period,and the defect of alveolar cleft and volume of the newly formed bone were calculated in each patient.The roots of the moved teeth and their positions to the alveolar bone were also observed.Results The preoperative cleft width and cleft volume were not significantly different between both groups.The volume of the newly formed bone in group A was (0.98±0.23) mm3,significantly higher than that in group B,which was (0.73± 0.15) mm3.The rate of newly formed bone in group A was (72.5 ± 11.9)％,significantly higher than that in group B,which was (53.2±9.7)％.The cleft adjacent teeth could move smoothly into the bone grated area,with no root resorption observed in the computed tomography scans.Conclusions Early orthodontic tooth movement can reduce bone resorption in autogenous bone grafted unilateral cleft lip and palate patients through the observation of spiral computed tomography.It plays an active role in the bone remolding process after bone grafting.

Objective To investigate the present condition of unique copy of TCM ancient books, and analyze the literature value. Methods Bibliographic research was underwent first, then investigation table was made, investigation team for spot investigation was organized, the bibliography was sorted, and on this basis analysis of the literatures was performed. Results 59 collection units at home and abroad and 1370 unique copy TCM ancient books were investigated, which including 57 books published by photocopy and 37 books published by proof reading. Conclusion The unique copy TCM ancient books have important academic value and can be effectively saved and sorted through in-depth investigation and analysis.

Objectives To investigate the effects of military stress and psychological resilience on job burnout and mental health in military personnel and construct the relationship among resilience,job burnout and mental health during military stress.Methods By cluster random sampling,520 personnel in army taking part in the war-game were selected as the experimental group,and another group of 520 who not taking part in the wargame were selected as the control group.After one week of the war-game,the experimental group and control group were investigated by Military Personnel Mental Disorder Prediction Scale,Military Job Burnout Scale and Military Personnel Resilience Scale.The acquired datum were analyzed by t-test,correlation analysis,regression analysis and structural equation modeling analysis.Results (1) The total scores of mental disorder prediction and factor scores of psychosis,depression,mania,neurosis,personality disorder were((15.85±9.30),(0.36±0.91),(1.10±1.53),(2.66±1.78),(1.16± 1.57),(2.04± 1.83)) and the total scores of job burnout and its all factor scores were((23.05±9.40),(5.13±2.47),(3.19±2.18),(5.16±3.01),(1.71 ± 1.66),(3.29±2.21)) of experimental group were significantly higher than control group((11.20±6.24),(0.11 ±0.53),(0.55± 1.10),(1.89± 1.50),(0.51± 1.02),(1.10±1.13),(17.92±9.64),(4.28±2.92),(2.64±2.06),(3.84±2.46),(1.26± 1.37),(2.77±2.43),P=0.000).(2)Psychological resilience negatively correlated with job burnout and mental health(r=-0.52～-0.09,P＜0.05),and job burnout positively correlated with mental health (r=0.12 ～ 0.57,P＜0.01).(3) Under the military stress,psychological resilience was the accommodation to effect the mental health and job burnout of military personnel.(4)The analysis of structural equation modeling showed that psychological resilience had a direct effect on job burnout(the path coefficient was-0.56,P＜0.01),but it had no significant effect on mental health (the path coefficient was-0.06,P=0.302).Job burnout had a direct effect on mental health (the path coefficient was 0.73,P＜0.01),and the structural equation modeling fitted the data very well(x2/ df =3.138,GFI =0.943,AGFI =0.911,NFI =0.930,RFI =0.905,IFI =0.952,TLI =0.933,CFI =0.951,RMSEA =0.065).Conclusion Military stress can lead to job burnout and mental disorders that could be moderated by psychological resilience.Job burnout can fully mediate the relation between psychological resilience and mental health.Psychological resilience effects mental health by job burnout indirectly.

Objective To compare the three kinds of methods for in vitro primary culturing of rheumatoid arthritis synovial fibroblast-like cells (RASFs), in order to get fast and effective culture methods. Methods Synovial tissue from RA synovial arthroscopic resection were treated by collagenase digestion method, modified tissue culture method, double enzyme digestion method respectively. By using an inverted phase contrast microscope, cell morphology and growth characteristics were observed and identified with vimentin staining. Trypan blue was used to count the number of living cells after culturing for 14d. Results The three primary methods could successfully isolate and culture RASFs, and RASFs met the morphological characteristics of vimentin-positive cells>95%, namely, the proportion of RASFs cell confluence was 70% after 16-20days by the collagenase digestion method,whose cell confluence proportion reached 95%after 4 weeks;and the cell confluence proportion was above 70%after 10-14days by modified tissue culture method,and the cell confluence proportion reached 85%after 4 weeks by the double enzyme digestion method. The comparison of the viable cells number cultured same number of synovial tissue by the three methods show the viable cells number cultured by the modified tissue culture method were (1.60±0.08) ×106, those by the collagenase digestion method were (1.41±0.08) ×106, those by the double enzyme digestion method were (1.19 ±0.05) ×106, which were with significant difference among them (P<0.05) .The comparison of incubation time of RASFs primary cells showed it took (267.50±16.58) mins by the collagenase digestion method, (183.75 ±11.08) mins by the double enzyme digestion method, and 149.10 ±13.71mins by the modified tissue culture method, with significant differences (P<0.05) .Conclusion Modified tissue culture for RASFs is an efficient and fast culture method, the number and purity of RASFs can meet the requirements for biology experiments.

OBJECTIVE:To provide reference for rational drug use in respiratory department. METHODS:ADR/ADE report-ed by respiratory department of a class three grade A hospital during Jan. 2011 to Dec. 2015 from national ADR monitoring system were collected. 2011-2015 ADR/ADE database of respiratory department in a hospital was established and analyzed statistically. RE-SULTS:202 records were gotten,among which 89 cases were male(44.06%)and 113 cases were female(55.94%);the patients’ age approximated normal distribution,the incidence of ADR/ADE was the highest in 61-80 age group (55.94%);the occurrence time of ADR/ADE was mostly lower than 1 h (47.21%);the main route of administration was intravenous drip (71.29%);22 kinds of drugs were involved,and antibiotics took up the lead(43.11%);84 patients(41.58%)used 2 and more kinds of drugs before the occarrence of ADR/ADE;organs/systems involved in ADR/ADE were skin and its appendants (32.86%) and digestive system(26.19%). Most of the ADR/ADE could be cured or recovered after related treatment. CONCLUSIONS:The occurrence of ADR/ADE in respiratory department is related to various factors,of which advanced age,intravenous administration and antibiotics use are important ones. Great importance should be attached to rational clinical drug use as well as ADR/ADE monitoring and re-porting. Clinical also should pay attention to the safety of drug use in elderly patients.
