1.Analysis of T lymphocyte subsets in the peripheral blood of patients with ankylosing spondylitis

Objective To explore the phenotypic characteristics of T lymphocyte subsets in the peripheral blood (PB) of Patients with rheumatoid arthritis.Methods forty-seven patients with ankylosing spondylitis (AS) and 36 health control were selected as our subjects.The patients were divided into definite Diagnosis AS group and potential AS group according to symptom and X-ray,flow cytometry was applied to measure T lymphocyte subsets and HLA-B27 of peripheral blood.At the same time,the levels of ESR,CRP and IgA were also measured.The correlation between T lymphocyte subsets and CRP HLA-B27,IgA level was analyzed.Results There was no significant difference between The definite diagnosis AS patients and healthy controls regarding of the percentages of CD3 + cells in peripheral blood,However the percentage of CD4 + cells in definite diagnosis AS patients was significantly increased than that of healthy controls (P < 0.05).The ratio of CD4 +/CD8 + cells was 3.80 ± 0.28 in AS patients,higher than that of potential AS patients.T lymphocyte subsets were showed an increase tendency as the increase the level of CRP,IgA.Conclusion Lower CD8 + cell and higher CD4 + cell were showed in the peripheral blood of AS patients,which was correlated with the disease activity and suggesting T cells might be involved in the pathogenesis process of AS.Meanwhile our data indicated that the immune response might be a susceptible factor of AS and early detection of T lymphocyte subsets might provide a diagnosis and treatment basis for diagnosis of AS.Therefore the disorder of T lymphocyte subsets might play an important role in the development of rheumatoid arthritis.
2.Therapeutic effect of vacuumassisted closure combining surgical treatments on chronic osteomyelitis after severe open fractures

Objective To evaluate the effect of vacuumassisted closure (VAC) combining surgical treatments on chronic osteomyelitis after severe open fractures. Methods Fifty-eight patients with severe high energy open fractures were enrolled,35 patients were randomized to the experimental group and had identical treatment except that VAC was applied to the wounds between irrigation and debridement procedures, 23 patients were randomized to the control group and underwent initial irrigation and debridement followed by standard fine mesh gauze dressing. The infection rate, therapeutic effect and health condition were evaluated between the two groups. Results All patients were followed up for 14 - 36 (28.0 ± 7.3 ) months.Control group developed 7 cases ( 30.4%, 7/23 ) with deep infections, whereas experimental group developed 2 cases (5.7%,2/35) with deep infections. There was significant difference between the two groups (P =0.024). There were 24 cases (68.6%,24/35) grade A healing in experimental group,whereas there were 13cases (56.5%, 13/23) grade A in control group,the difference was very singificant (P<0.05). The health total score of experimental group was higher than that of control group at 3,9 months after operation (P =0.013,0.005). Conclusions Patients treated with VAC have a higher recovery rate,shorter length of stay and satisfactory curative effect compared with patients randomized to the traditional methods. VAC represents a promising new therapy for osteomyelitis after severe open fractures.
3.Detection and phylogenetic tree analysis of human parainfluenza virus type 3 in children with acute respiratory tract infection from a hospital in Shanghai

Objective To understand the epidemiologic feature of human parainfluenza virus type 3 (HPIV-3) in Shanghai,and to provide scientific evidence for formulating prevention and control measures in the future.Methods A total of 164 nasopharyngeal aspirates samples taken from children with acute respiratory infection (ARI) were collected from Xinhua Hospital Affiliated to Shanghai Jiaotong University and sent to Shanghai Public Health Clinical Center from June 2009 to June 2010.Samples were detected for HPIV-3 by reversed transcription-polymerase chain reaction (RT-PCR).Full-length hemagglutininneuraminidase (HN) gene (1 719 bp) of five positive samples were sequenced for phylogenetic analysis.Comparison between two groups was evaluated by the precise chi-square test (two sided).Results Of 164 samples,70 samples were infected with parainfluenza virus,and HPIV-3 was detected positive in 23 samples with the positive rate of 32.86%.HPIV-3 infections were most common in spring and summer,and most of infections were mainly found in 13-36 month-old infants.Five Shanghai isolates and 36 reference sequences from different countries and areas were divided by HN gene-based phylogenetic tree into three clusters (A,B and C).Five Shanghai isolates and five Beijing isolates belonged to C3a group.The homologies of nucleotide and amino acid sequences between five Shanghai isolates and five Beijing isolates were 99.0%-99.5% and 99.7%-100.0%,respectively.Conclusions HPIV-3 accounts for a high proportion in children with ARI in Shanghai.C3a group may be the main lineage of HPIV-3,which suggests that HPIV-3 may be of regionally correlation.
4.Catalytic Spectrophotometric Determination of Osmium (Ⅳ) Using Osmium (Ⅳ)-KIO4-Chlorophosphonazo-mA System

A catalytic spectrophotometric method for the determination of micro amounts of osmium has been established based on the catalytic action of Os(Ⅳ) on the oxidation fading reaction of chlorophosphonazo-mA with KIO4 in alkaline-medium. The reaction conditions were optimized by orthogonal experimental design. The detection limit for osmium was 2.0 μg/L.Beer's law was obeyed in the range from 7.0 to 25.0 μg/L for Os(Ⅳ). The method has been applied to the determination of micro amounts of Os(Ⅳ) in concentrate of noble metals and secondary alloy,both of the relative errors were 0.9%. Recoveries varied from 95.38% to 106.0%.
5.Case study of pharmaceutical direct supply at Wuhan Pu Ai Hospital

Wuhan Pu Ai Hospital implemented the pharmaceutical direct supply mode since October 2006 as an experiment.The pilot proved the mode as successful in reducing drug costs proportion,promoting rational use of drugs,and raising business volume of the hospital.Authors also recommend to further efforts in drug supply price supervision mechanism,government subsidy mechanism,direct supply catalogue and drug supply mode among others.Other recommendations include innovation in directly supply mode,hospital internal reform,packaged performance appraisal and incentive mechanism for medical workers,and building a ceiling mechanism for total medical expense control.
6.Analysis of Drug Treatment Strategy for One Case of ARDS after Fracture Operation

Objective: To explore the entry points and monitoring points of individualized treatment for fracture surgery patients with ARDS.Methods: Clinical pharmacists comprehensively assessed the patient's conditions and looked up lots of literatures.Some suggestions on the anti-infection treatment efforts, glucocorticoid use dispute and rational use of ambroxol were offered.The efficacy and adverse reactions of drugs in the treatment process were monitored using some subjective and objective indicators.Results: The consensus about the pros and cons, dosages, as well as the possible dosage decrease of glucocorticoid therapy, the dosages of antimicrobial agents, and the dosages and treatment course of ambroxol was achieved between clinical pharmacists and doctors.As a result, the maximum benefits were obtained for the patient.Conclusion: Clinical pharmacists can explore the treatment and monitoring for specific diseases and find out intervention entry points of clinicians and patients to play a certain role in clinics, and accumulate treatment experience gradually.
7.Study of sunitinib-liposome-loaded microbubbles for growth Inhibition and apoptosis induction of renal carcinoma cell strain GRC-1

Objective To investigate the effect of growth inhibition and apoptosis induction of sunitinib-liposome-loaded microbubbles on renal carcinoma cell strain.Methods GRC-1 cell strain was cultured in vitro,and was divided into 6 groups:blank control group,pure microbubbles group,pure lipsomes group,sunitinib group,sunitinib-liposome-loaded microbubbles without ultrasound treat group,sunitinib liposome-loaded microbubbles with ultrasound treat group.Growth inhibition in different groups was observed at different time with MTT assay,apoptosis induction with Sigma-FlTC technology and transmission electron microscope.Results The growth inhibition and apoptosis promotion of GRC-1 cell were significantly increased in sunitinib-liposome-loaded microbubbles with ultrasound treat group compared to the other groups.Conclusions Microbubble guided sunitinih delivery can increase the effect of the growth inhibition and apoptosis induction of GRC-1 cells,which may provide an more effective approach for cancer treatment.
8.Unilateral versus bilateral cerebral perfusion in deep hypothermia circulatory arrest surgery

Objective To study the applicability of unilateral and bilateral antegrade cerebral perfusion during deep hy-potbermia circulatory arrest (DHCT) surgery and to determine a better method of brain protection. Methods 60 patients un-derwent aortic arch surgery were random divided into either unilateral cerebral perfusion group with a cannula in the innominate artery (n = 30 ) , or bilateral cerebral peffusion gToup with an additional cannula in the left carotid artery ( n = 30 ). S-100β,neuron specific enolase (NSE) were assayed at the following time points: the beginning of cardiopulmonary bypass (T_1), the beginning of circulatory arrcst( T_2 ), antegradc cerebral perfusion (ACP) 25 min( T_3 ), the end of ACP( T_4 ), the end of car-diopulmonary bypass ( T_5 ) , 1h( T_6 ) , 6h ( T_7 ) and 24h( T_8 ) after operation. Neurological deficit incidence and mortality were also obtained. Results There is no statistical significance for S-100β and NSE plasma concentration among time points of T_1、T_2 and T_3. But every time point among T_4、T_5、T_6、T_7 ,Ts do have statistical significances. The incidence of central nervous system dysfunction in group unilateral ACP was higher than that in group bilateral ACP ( 33.33% vs. 10.00%, P = 0.028 ).But there is no significant difference between the two groups in mortality( 3.33% vs. 6.66% ,P = 1. 000 ). Conclusion There is no significant difference between unilateral ACP and bilateral ACP in 25 min during DHCA. But as the DHCA time extends, the effect of bilateral ACP will be better.
9.Correlation Study of the Expression of XRCC1 to the Effect of Radiotherapy in Gliomas

Objective To explore the relationship between the expression of XRCC1 and glioma. Methods Total of 26 samples of glioma were divided into 4 groups:gradeⅠ,gradeⅡ,gradeⅢand gradeⅣ. The expression of XRCC1 in 26 Gliomas tissues were examined using SP immunohistochemical staining.Results The positive staining of XRCC1 protein was localized in nucleus of tumor cells in Glioma. There was no correlation among them. The difference of XRCC1 expression among gradeⅠ~Ⅳ was not significant ( >0.05) .Conclusion The difference of XRCC1 expression among gradeⅠ~Ⅳ was not significant. The expression of XRCC1 was closely correlated with the effect of radiotherapy.
10.Diagnosis and treatment of Charcot spinal arthropathy

Charcot Spinal Arthropathy (CSA) is a rare and progressive serious degenerative spinal disease. The clinical manifestations of CSA are concealed and atypical, which could lead to missed misdiagnosis, disease prognosis, and a huge burden on patients. However, there is no systematic review of CSA in China. The causes of CSA are mainly divided into spinal cord injury and non-injury neuropathy. The risk factors for CSA caused by spinal cord injury include long-segment fixation, scoliosis, laminectomy, overload spinal exercise and obesity. CSA usually occurs in the lower thoracic or lumbar spine. The symptoms of CSA include spinal deformity, unbalanced sitting posture and local pain. The CSA can be diagnosed after excluding non-specific chronic inflammation in histology and other inflammatory diseases or tumor based on the following items, damage to proprioception, pain and temperature perception, bone destruction, absorption and new bone formation on imaging. Conservative treatment can be considered for patients with CSA who have good stability without infections, stable nerve function, skin fistulas, balanced sitting posture, and autonomic dysfunction. Surgery is recommended for patients with symptoms lasting for more than 6 months with spinal instability, skin fistulas or complicated infections. Before surgery, it is recommended to evaluate the heterotopic ossification or rigidity of both hip joints. During operation, more attention should be paid to the adequate removal of necrotic tissue and inflammatory tissue in the lesion and sufficient bone grafting. Spinal fusion is recommended at the sacrum or pelvis. Postoperative complications include failure of internal fixation, new Charcot joint formation, difficulty in wound healing and infection. The authors emphasize that the overall thoracolumbar spine should be followed up for patients with spinal cord injury and paraplegia for the long-term. The typical symptoms of CSA are helpful for early diagnosis and selection of appropriate interventions.
