1.Nursing of patient with second anastomotic leakage after radical gastric cancer surgery:a case re-port

Objective To conclude nursing experience of second anastomotic leakage after radical gastric cancer surgery. Methods The present study compared the differences of clinical symptoms between these two anastomotic leakages, and found a way to detect latent anastomotic leakage with an unobvious abdominal sign accurately and opportunely. The study also intervened in the patient′s observation, psychological condition, peritoneal irrigation and nutritional support after surgery. Results The patients were discharged from the hospital after cured with well controlled condition. Conclusions In order to make an early detection and treatment of the second anastomotic leakage, nurses should not only observe the abdominal sign, but also observe the systematic poisoning and concomitant symptoms.
2.Second-line Treatment with Gefitinib or Docetaxel for Advanced Non-small Cell Lung Cancer

Objective:To observe the efficacys and side effects of gefitinib(Iressa)and docetaxel(aisu)as the second-line treatment for advanced non-small cell lung cancer(NSCLC)patients.Methods:A total of 98 patients with advanced non-small cell lung cancer who had failed to previous fimt-line chemotherapy were randomly divided into two groups:gefitinib group and docetaxel group.In the gefitinib group,50 patients took Iressa 250 mg once daily until disease progression or intolerable toxicity occurred.In the docetaxel group,58 patients were treated with aisu 75mg/m~2 i.v.in 60 minutes in day 1.The regimen was repeated every 21 days for at least 2 consecutive cycles.They were assessed on the basis of RECIST evaluation standard of therapeutic effect for solid tumor.Results:In the gefitinib group,the response rate was 22.4%(11/49),the clinical benefit rate was 55.1%(27/49),median survival time was 7.1 months and 1-year survival rate was 35.9%.In the docetaxel group,the response rate,clinical benefit rate,median survival time and 1-year survival rate were 18.8%(9/48),50.0%(24/48),6.9months and 31.5%,respectively(P>0.05).The incidences of Ⅰ~Ⅳ degree and Ⅲ~Ⅳ degree rash were 51.0%and 10.2%in the gefitinib group,significantly lower than those in the docetaxel group (P=0.0000 and P=0.0296).The incidence of Ⅰ~Ⅳ degree diarrhea was significantly higher in the gefitinib group than that in the docetaxel group(P<0.01).The incidence of leukopenia was significantly higher in the docetaxel group than that in the gefitinib group(P=0.0000).The incidences of thrombocytopenia and anemia of Ⅰ~Ⅳ degree were also higher in the docetaxel group (P=0.0000 and P=0.0266).The improvement rate of quality of life was higher in the gefitinib group (P<0.05).Conclusion:Gefitinib has a similar anti-tumor effect as docetaxel on advanced NSCLC patients who have failed to previous fimt-line chemotherapy.Gefitnib can achieve higher improvement rate of quality of life in advanced NSCLC patients,with a lower incidence of toxicity.
3.Effects of Cl~-channels blockers on Ca~(2+) influx induced by ?_1-adrenoceptor subtypes

AIM To investigate the roles of Cl- channels in Ca2+ influx induced by activaion of al- adrenoceptor subtypes in transfected-CHO cells. METHODS The effects of drugs on ?1A、?1B and ?1D- AR-induced Ca2+ influx were investigated with Fura2 fluorescence technique. RESULTS The ?1A-AR- induced Ca2+ influx was inhibited by furosemide(2 .5 ~ 10 M?mol?L- 1 )and SK&F96365(5- 15 ?mol?L- 1 ) in a concentration- dependent manner respectively; The ?1B-AR-induced Ca2+ influx could also be inhibit inhibited by NFA(2. 5 ~ 10 ?mol? L-1 ), whereas the alD AR-induced Ca2+ influx was only suppressed by NFA. In ?1B-CHO cells, Adr-triggered Ca2+ influx could be further inhibited by NFA or furosemide after the maximal inhibition by SK&F96365;SK&F96365 could further inhibit Ca2+ influx which had been inhibited by NFA or furosemide. In ?1A-CHO cells, Adr-triggered Ca2+ influx could be further inhibited by SK&F96365 after had been inhibited by furosemide; furosemide could not further inhibite Ca2+ influx which had been inhibited by S&F96365. CONCLUSION There are different characteristics of CI- channels related to ?1A、 ?1B and ?1D-AR-induced Ca2+ influx.
4.Relationship between health-promoting lifestyle and work engagement in nurses of traditional Chinese medicine hospitals

Objective To explore the relationship between health-promoting lifestyle and work engagement in nurses of traditional Chinese medicine hospitals. Methods Totally 527 nurses from 3A level hospitals of the first, second, third affiliated to Zhejiang University of Traditional Chinese Medicine were investigated by the Chinese revision of Health-Promoting Lifestyle Scale, Chinese version of Utrecht Work Engagement Scale. Results M ultiple regression analysis showed that nurse health-promoting lifestyle had a significant correlation with work engagement, F = 8.987, P < 0.01. Among them, the self-realization dimension to the nurse work engagement most affected, t = 3.287, P < 0.01. Conclusions Improving nurses′health-promoting lifestyle can raise the level of work engagement and stabilize nursing staff and improve quality of care in traditional Chinese medicine hospitals.
5.A qualitative study on the training experience of tumor specialized nurses in Zhejiang

Objective To describe the true feelings and needs in oncology professional nurses training, so as to provide evidences for improving training content and patterns, orientating the career development. Methods Phenomenological methodology of qualitative study was adopted in this research. In-depth interview were conducted on 1-5 session specialized nurse in Zhejiang tumor base. Data was analyzed by Colaizzi analysis. Results Oncology professional nurses teaching effect was good, there were quite abundant harvest. Nurses′ professional attitudes were positive. Their training pressure was bigger, but there were good external support ways to cope. They had increased theory class time, improve the effect of theory course, refine the training content, and strengthening clinical practice teaching needs. Conclusions The training of tumor specialized nurse has certain necessity, and the nurses′ability has great advanced. Respondents were optimistic to the career prospects. However, the training patterns and content should be continuously perfected and updated, in order to improve the training effect.
6.Study on the HPLC Fingerprint of Blumea balsamifera and Its Fake B. riparia

OBJECTIVE:To establish the HPLC fingerprint for Blumea balsamifera and its fake B. riparia. METHODS:HPLC was performed on the column of Uitimate-C18 with mobile phase of acetonitrile-0.05% phosphoric acid(gradient elution)at a flow rate of 0.6 mL/min,detection wavelength was 270 nm,column temperature was 25 ℃,and injection volume was 7 μL. Using quercetin as a reference,Similarity Evaluation Software for Chromatographic Fingerprint of Traditional Chinese Medicine(2004 A edition)was used for the common peaks identification and similarity analysis of 16 batches of B. balsamifera and 5 batches of B. ri-paria. RESULTS:There were 61 common peaks in the 16 batches of B. balsamifera,similarity degree was 0.931-0.995,which was higher than the similarity degree of 5 batches of B. riparia. CONCLUSIONS:The established fingerprint can provide reference for the identification and quality evaluation of B. balsamifera.
7.Post-operation nursing for patients with autologous transplantation of the microvascular submandibular gland

Objective:To summarize the complications' prevention and nursing experience for patients after autologous transplantation of the microvascular submandibular gland for severe keratoconjunctivis sicca.Methods:Retrospective analysis of 141 patients(151 sides) after autologous microvascular submandibular gland transfer operation was undertaken to record the timing and incidence of major complications and to summarize the nursing experience.Results: The main complications after the transplantation were: vascular crisis,catheter obstruction,salivary fistula and temporary paralysis of hypoglossal nerve.Related nursing measures in response to these complications were discussed.Conclusion:Targeted nur-sing interventions for patients with autologous transplantation the submandibular gland have a positive effect on the prevention of the surgical complications.

9.Cost-benefit Analysis of Three Kinds of Packing Materials

OBJECTIVE To appraise the bacteriostatic and blocking effect and cost construction of different kinds of materials.METHODS To divide all items into three parts at random:cotton fabric,paper-plastic package,and medical grade wrinkle paper and to study the bacteriostatic and blocking effect and cost construction of them.RESULTS Paper-plastic package and medical grade wrinkle paper had better bacteriostatic and blocking properties than cotton fabric.Comparing with cotton fabric,paper-plastic package and medical grade wrinkle paper could reduce the cost separately by 27% and 50%;the cost of wrinkle paper was lower than paper-plastic package by 32%.CONCLUSIONS Paper-plastic package and wrinkle paper have better effect on blocking bacteria and less cost than cotton fabric,which deserve more extensive application in clinic.
10.Distribution and Antibacterial Resistance of Pathogens in Intensive Care Unit

OBJECTIVE To investigate the tendency of bacterial distribution and drug resistance of pathogens in ICU,then instruct clinical application of antibiotics reasonably.METHODS The identification of bacteria,isolated from patients in ICU from Jan 2005 to Dec 2006,and antibiotic susceptibility by ATB expression were used.RESULTS A total of 1197 pathogens strains were isolated,Gram-negative bacilli,Gram-postive cocci and fungi accounted for 64.2%,19.6% and 15.8%,respectively.The most common pathogens of them were Pseudomonas aeruginosa(22.1%).Except Escherichia coli(40%),the rate of the ESBL-producing strains of Serratia marcescens,Klebsiella pneumoniae,Acinetobacter baumannii and Enterobacter cloacae were higher than 70%.The most common pathogens of Gram-postive cocci were Staphylococcus.Meticillin-resistant strains of Staphylococcus aureus accounted for 93.8%.Mainly pathogenic bacteria were multi-resistant to some antibiotics.There were 189 strains fungi.And all the fungi were sensitive to amphotericin B,fluconazole and ketoconazole.CONCLUSIONS The infection and multi-drug resistance of pathogens are serious in ICU.It′s important and urgent to carry out surveillance of bacterial resistance for appropriately using antibiotics.
