1.Pathogen Distribution and Resistance Analysis among Patients with Lower Respiratory Tract Infection

OBJECTIVE To investigate the distribution and resistance of pathogenic becteria isolated from inpatients with lower respiratory tract infection in Department of Gerontal Respiratory Medicine during from Aug 2006 to Aug 2007.METHODS All the clinical isolates from sputum were analyzed retrospectively.RESULTS There were 211(79.8%) strains of Gram-negative bacilli,40(15.2%) strains of Gram-positive bacteria and 12(6%) strains of fungi.The most important Gram-negative pathogens were Klebsiella pneumoniae.Pseudomonas aeruginosa,Enterobacter cloacae and Escherichia coli.Strains of Gram-negative bacteria were susceptible to imipenem,cefoperzone/sulbactam and amikacin,strains of Gram-positive pathogens were susceptible to vancomycin,nitrofurantoin,cefazolin and imipenem.CONCLUSIONS Gram-negative bacilli are the major pathogens in lower respiratory tract infection in Department of Gerontal Respiratory Medicine and the dominant is conditional pathogen.Rational use of antibiotics is important for the containment of bacterial resistance.
2.Analysis of surveillance results of iodine deficiency disorders in 2013 in Hainan Province

Objective To analyze the coverage of iodized salt and the focus population iodine nutrition after implementing new standard of iodized salt in Hainan Province.Methods Eighteen cities (counties,districts) were divided into eastern coastal,western coastal and inland areas according to coastline and terrain of Hainan Province in 2013.One township (town,street) was sampled from each city (county,district) in the east,the west,the south,the north and the center of these areas.Four villages (committees) and 15 households in each village (committee),and 40 children aged 8-10 in one primary school,20 pregnant women and lactating women were sampled from each township (town,street).Iodized salt samples collected from household were monitored by the method of direct titration;random urine samples of children aged 8-10,pregnant woman and lactating woman were tested by arsenic cerium catalytic spectrophotometry.Results Totally 6 310 edible salt samples were tested,and the median of salt iodine was 25.9 mg/kg.The coverage of iodized salt was 97.92% (6 179/6 310).Residents consumption rate of qualified iodized salt reached 95.47% (6 024/6 310),and the rate of non-iodized salt was 2.08% (131/6 310).About 4 444 urine samples of children aged 8-10 were examined,and the median of urinary iodine was 165.4 μg/L,in which suitable urinary iodine (100-199 μg/L) accounted 49.3% (2 194/4 444);about 1 286 urine samples of lactating woman were examined,the median of urinary iodine was 122.3 μg/L,≥ 100 μg/L was 60.6% (779/1 286);about 813 urine samples of pregnant woman were examined,the median of urinary iodine was 126.6 μg/L,59.6% (485/ 813) of this group were urinary iodine deficiency (< 150 μg/L).There were significant differences between the three groups in the east,the middle and the west areas of Hainan Province (children:176.7,147.7,158.0 μg/L,lactating women:148.7,108.7,106.7 μg/L,pregnant women:146.7,108.0,147.1 μg/L,H =34.70,26.76,35.95,all P < 0.01).Conclusion The results have showed that pregnant woman are iodine deficiency,they should be given more iodine.
3.Association of resting heart rate with dyslipidemia

Objective To investigate the association between resting heart rate(RHR)and dyslipidemia. Methods A total of 772 subjects(average age:40. 1 ± 11.8 years;male:74. 7%)were recruited by random sampling method from medical checkup population. The survey data included resting heart rate,blood lipid and other metabolic parameters. The subjects were divided into three groups according to the RHR,as RHR < 70 bpm,70 bpm ≤ RHR < 80 bpm,RHR ≥ 80 bpm groups. Results RHR were positively correlated with triglyceides (TG),as well as total cholesterol(TC)(r = 0. 316 and r = 0. 110 respectively,all P value < 0. 05). Low RHR had no significant correlation with HDL-C or LDL-C. The prevalences of hypertriglyceridemia and hypercholesterolemia increased significantly with RHR elevation(χ2 = 7. 214 and 5. 285 respectively,all P value < 0. 05). By logistic regression analysis,adjusted for BMI,gender,serum creatinine,serum uric acid,fasting blood glucose and age,we found that compared with RHR < 70 bpm group,the odds ratio(OR)values for hypertriglyceridemia was 1. 540 (95%CI:1. 086-2. 185,P=0. 016)in 70 bpm≤RHR <80 bpm group and 1.640(95% CI:1.067 -2.523,P =0. 024)in 80 bpm ≤ RHR group,and the OR values for hypercholesterolemia was 1.197(95 % CI: 0.749 - 1.913,P=0.453)in70 bpm≤RHR <80 bpm group and 1.814(95% CI:1. 036-3. 177,P =0.037)in 80 bpm≤RHR group. Conclusions RHR is independently and positively correlated with the prevalence of hypertriglyceridemia and hypercholesterolemia and the blood levels of both lipid. RHR has no significant correlation with HDL-C or LDL-C level.

Abstract Objective:To study GST-? expression in breast cancer and axillary lymphnodes.Methods: The expression of GST-Tr proteinand mRNA was detected by means of immunohistochemistry and RT- PCR. Results: In normal breast tissue, cancer tissue , HE positive lymph-nodes and HE negative lymphnodes , GST-? protein expression rate is 0,60% ,77% ,43%; GST-? mRNA expression rate is 0,88% ,71%and 39% , respectively. Conclusion; GST-?gene whose expression is upregulated as tumor occurs and develops can be used as a tumor biomar-ker; GST-? expression in HE negative lymphnodes suggests the poor prognosis; drug-resistance response is more sensitive in lymphnode than inprimary cancer. The expression of GST-? gene may be regulated by RNA or before RNA transcription.

6.One-wayversusuniversal pedicle screw fixation for lumbar fractures:biomechanics

BACKGROUND:Previous studies have performed three-dimensional finite element analysis on lumbar fracture fixation. However, a few studies concerned three-dimensional finite element analysis of lumbar spinal pedicle screw fixation.
OBJECTIVE:To analyze the biomechanical characteristics of one-way screw and universal screw fixation for lumbar fractures using finite element method.
METHODS:Models of lumbar fractures treated with one-way screw and universal screw fixation were established. Three-dimensional finite element method was used to analyze and compare the maximum axial displacement, maximum bending degree, maximum stress on thepedicle screw fixation system and maximum stress of intervertebral disc in normal vertebral fracture model, one-way and universal pedicle screw fixation models.
RESULTS AND CONCLUSION:(1) The maximum axial displacement at anteflexion, posterior extension, left lateral curvature and right lateral curvature was significantly lower in the one-way fixation group and universal fixation group than in the normal group (P< 0.05-0.01). The maximum axial displacement at above directions was lower in the universalfixation group than in the one-way fixation group. (2) The maximum bending degree was significantly lower in the one-way and universal fixation groups than in the normal group (P< 0.05-0.01). The maximum bending degree at various directions was lower in the universal fixation group than in the one-way fixation group. (3) The maximum stress on the pedicle screw fixation system at various directions was lower in the universal fixation group than in the one-way fixation group (P< 0.05-0.01). (4) The maximumstress of intervertebral disc was significantly lower in the one-way and universal fixation groups than in the normal group (P< 0.05-0.01). The maximum stress of intervertebral disc at various directions was lower in the universal fixation group than in the one-way fixation group. (5) Results suggested that one-way and universal screw fixation for lumbar fracture can bear most of the stress, obviously decrease the maximum axial displacement, maximum bending degree, maximum stress on the pedicle screw fixation system and maximum stress of intervertebral disc. The stability of universal screw fixation was better than that of one-way screw fixation.
7.Association between airway obstruction and peripheralarterial stiffness in elderly patients with chronic obstructive pulmonary disease

Objective To evaluate the relationship between severity of airway obstruction and peripheral arterial stiffness in patients with chronic obstructive pulmonary disease (COPD).Methods 81 COPD patients [aged (78.32 ± 6.98)yrs,73 males,8 females] from Jan 2008 to Oct 2012 were enrolledin Geriatric Department of Peking University First Hospital.All patients underwent spirometry and Holter evaluation,while age,sex,BMI,smoking pack-years,medical history,blood levels of lipid profiles,glucose,high sensitivity C reactive protein (hsCRP)and arterial oxygen pressure (PaO2)were recorded.The severity of airway obstruction was evaluated by spirometry.The patients were divided into two groups:mild/moderate group [forced expiratory volume in one second (FEV1%) predicted ≥ 50 %,n=51] and severe group (FEV1% predicted < 50%,n=30).24-hour average heart rate (HR)was assessed by holter.Arterial stiffness was assessed by the brachial ankle pulse wave velocity (baPWV).The baPWV increment was considered to be a direct witness of arterial stiffness increase.Blood levels of lipid profiles,glucose,arterial oxygen pressure (PaO2)and hsCRP were compared between the two groups.Results Age,gender,smoking index,medical history of the two groups were matched.BMI was less in severe group than in mild/moderate group [(22.8± 4.2) kg/m2 vs.(25.3± 3.2) kg/m2,t=3.017,P<0.05].24-hour average HR was higher in severe group than in mild/moderate group [(77.4± 12.7)bpm vs.(70.8± 9.6)bpm,t=-2.602,P<0.05].The PaO2 was lower in severe group than in mild/moderate group [(74.6±13.0)mmHg vs.(82.4± 13.1)mmHg,t=2.456,P<0.05].There were no differences in blood lipid profile and glucose levels between the two groups.76 patients (93.4%)were detected with peripheral arterial stiffness by baPWV>14.00 m/s.baPWV was significantly increased in severe group as compared with mild/moderate group [(20.77 ± 3.71) m/s vs.(18.84 ± 1.88) m/s,t=-2.109,P<0.05].hsCRP was significantly higher in severe group than in mild/moderate group [[(6.51±5.66)mg/L vs.(3.27±3.34)mg/L,t=-2.658,P<0.01].Conclusions The morbidity of peripheral arterial stiffness is increased in patients with COPD.Progression of arterial stiffness is related to the severity of airway obstruction,which may be related to chronic inflammation,hypoxia,or excessive sympathetic activation.

9.The Role of Moxifloxacin in Treatment of MDR Pulmonary Tuberculosis:a Prospective Randomized Preliminary Study.

Objective To evaluate the effectiveness of Moxifloxaein in treatment of multidrug-resistant(MDR)pulmonary tuberculosis.Methods 65 MDR pulmonary Mycobacterium tuberculosis patients were randomized divided into two groups,separately receiving the regimens of contained Moxifloxacin0.4 qd or Levofloxacin 0.6 qd six drugs treatment accompanying with aminoglycosides,prothionamide,pasiniazid,pyrazinamide,ethambutol and clarithromyein from 2003—2005y in Guangzhou chest hospital.The treatment outcomes were evaluated when study executed one and half year later.Results 35 patients in moxifloxacin group had 27 cases cured,and 30 patients in levofloxacin group had 19 cases cured.The success rates were 77.1% versus 63.3%(P=0.222).The time to sputum culture conversion were (1.9?0.7)months and 3.0?1.8 months(P=0.035).Bacillary susceptible to levofloxacin,good adherence,radi- ographic extent less than one lung,and use of moxifloxacin were independent predictors of favorable outcome(odds ratios,7.3 to 21.4).Conclusion Moxifloxacin was found have a better bactericidal activity in vivo and less side effects. Its efficacy was higher than levofloxacin when incorporated into muhidrug regimens used for treatment of MDR tuberculosis.
10.TNF-αmediated activation of IDO pathway involved in mechanism of depression in rats induced by CUS

Aim To investigate the relationship be-tween the depression-like behaviors of rats induced by chronically unpredictable stress( CUS) and the protein expression of tumor necrosis factor alpha( TNF-α) , in-dole-2 , 3-dioxygenase ( IDO ) and 3-hydroxyl amino acid oxygenase( HAAO) . Methods Thirty male Spra-gue Dawley( SD) rats were randomly divided into con-trol group and model group. CUS plus solitary condi-tion were used to establish the depression model. The open field test ( OFT ) and the force swimming test ( FST ) were used to evaluate the depression-like behaviors of rats ;Western blot was used to determine the protein expressions of TNF-α, IDO and HAAO in the prefrontal cortex and hippocampus of rats. Results Model group rats showed obvious depression-like be-haviors with increasing immobile time in FST ( P <0. 01) and decreasing locomotive activity in OFT( P<0. 01 ) , and up-regulating the protein expression of TNF-α, IDO and HAAO in the prefrontal cortex and hippocampus significantly ( P <0. 05 ) , compared with control group rats. Conclusion The depression-like behaviors of rats induced by CUS may be associated with the activation of IDO-HAAO pathway mediated by TNF-α.
