3.The supply and utilization of vision care in rural primary Health

Objective:To identify the vision care access in rural primary health institutions and the utilization of vision inspections among rural residents. Methods:Survey data was collected from primary health institutions ( town-ship health centers and village clinics) and households. The descriptive statistics and multivariate regression analysis were applied to analyze data. Results:The household survey data shows that 33. 2% of rural residents self-reported having poor vision, and 22. 1% of rural residents stated that they had ever used vision care (vision screening or vison examinations) . The health facilities survey data shows that 84% of township health centers and 44% of village clinics can provide vision care. The multivariate analysis shows that the vision care access in township health centers is sig-nificantly correlated with the probability of residents’ utilization of vision care, but there is no significant correlation between the provision of vision care in village clinics and its utilization. Conclusions:The vision care access is low in rural China though there is a huge demand therefore. Inadequate supply of primary vision care services in primary health institutions may result in low utilization among rural residents. It is suggested that the government further pro-mote the National Public Health Service Program and strengthen the capacity of primary health facilities to provide primary vision care. To do so, an increase in the utilization of vision care among rural residents can be expected, which would thereby reduce potential losses caused by further vision impairment.
4.Raised immune responses of HBV preS_2S DNA vaccines by administrating genetic adjuvant at different time

Objective:To study the different effect of fused gene IL-2/Fc administrated at different time on immune responses induced by HBV preS_2S genetic vaccine.Methods:BALB/c mice were immunized with purified recombinant plasmids at 0, 2, 4 weeks. Two groups were designed. One group were injected with pcDNA3.1S_2S at first, then pcDNA3.1IL-2/Fc was administrated after three days. While the other group were injected two plasmids at the same time. We compared the immune responses through detecting the anti-HBs titers, CTL cytotoxicity level, proliferation of lymphocytes and cytokines levels secreted by cultural splenocytes.Results:The data demonstrated that the humoral and cellular immune responses induced by pcDNA3.1IL-2/Fc as adjuvant and administrated after HBV preS_2S DNA vaccine 3 days mice were dominant higher than other groups.Conclusion:Genetic adjuvant administrated after Ag priming can enhance its effect.
5.Lack of association between ABCC2 polymorphisms and plasma carbamazepine concentrations or pharmacoresistance in Chinese patients with epilepsy

Multidrug resistance proteins (MRP2, ABCC2) may play a role in drug resistance in epilepsy by
limiting gastrointestinal absorption and brain access of antiepileptic drugs (AEDs). We sought to
investigate the effects of ABCC2 polymorphisms on plasma carbamazepine (CBZ) concentrations
and pharmacoresistance in Chinese patients with epilepsy. ABCC2 rs717620, rs2273697, rs3740066
polymorphisms were genotyped by polymerase chain reaction amplification followed by restriction
fragment length polymorphism analysis or direct automated DNA sequencing in 80 patients treated
with CBZ monotherapy. There were no differences in CBZ maintenance doses or adjusted plasma
CBZ concentrations among the ABCC2 rs717620, rs2273697 and rs3740066 genotypic groups.
No associations between all the studied genotypes and haplotypes involving the three SNPs of
ABCC2 and CBZ resistance were observed in this patient cohort. These results suggest that ABCC2
polymorphisms may not contribute to interindividual variabilities in CBZ daily maintenance doses,
plasma concentrations, and treatment efficacy.

6.Prevalence of plasmid-mediated quinolone resistance genes in Klebsiella pneumoniae strains in pediatrics in China

Objective To investigate the prevalence of plasmid-mediated quinolone resistance (PMQR) genes among clinical isolates of Klebsiella pneumoniae (K.pneumoniae) in pediatrics.MethodsA total of 131 non-duplicate clinical isolates of K.pneumoniae were collected in the Affiliated Children′s Hospital of Capital Institute of Pediatrics from 2010 to 2012.PMQR genes [qnrA, qnrB, qnrS, aac(6′)-Ⅰb-cr and qepA], mutations in the quinolone resistance-determining region (QRDR) and extended spectrum β-lactamases (ESBLs) genes in those strains were analyzed by PCR.Minimum inhibitory concentrations (MIC) of different antibiotics against those K.pneumoniae strains were determined by broth microdilution method and E-test according to the guidelines issued by the Clinical and Laboratory Standards Institute (CLSI).Transferability of the PMQR genes was examined by conjugation test with the sodiumazide-resistant Escherichia coli J53.Results Among the 131 isolates, 9.92% were resistant to quinolone and 30.5% were positive for PMQR genes, including 6.87% harboring qnrB gene, 22.9% harboring qnrS gene and 4.58% harboring aac(6′)-Ⅰb-cr gene.Neither qnrA-positive nor qepA-positive strain was detected.Among these PMQR genes-positive isolates, 90% were ESBLs-producing strains and two presented mutations in gyrA and parC genes.Conjugation test showed that these PMQR genes could be transferred horizontally and the ciprofloxacin resistance increased 2 to 32 folds in transconjugants.Conclusion This study indicates that the PMQR gene-carrying rate is high in K.pneumoniae strains isolated in paediatrics in China.Most of the PMQR gene-positive strains are also ESBLs-producing strains.The PMQR genes could be transferred horizontally in bacteria.
7.Pathology and the expression of Bcl- 2 and Bax in spinal cord of guinea pigs in experimental allergic encephalomyelitis

AIM: To elucidate the molecular mechanism of apoptosis in inflammatory cells occurring in the central nervous system during experimental allergic encephalomyelitis (EAE), the changes of pathology and the expression of apoptosis-related molecules Bcl-2 and Bax in the spinal cord of guinea pigs in EAE model were observed. METHODS: EAE model was induced in guinea pig. The cervical and lumbosacral enlargement of spinal cord was obmined during the 14 th and 28 th days after modeling. The H & E staining and ABC immunohistochemistry technique were used. RESULTS: HE staining: The lesion in the model was located in the inflammation of small veins, which were surrounded by inflammatory cells. It was specially noticed that inflammatory cells also infiltrated large veins. The lesions in the 28th day were more severe than that in the 14th day. ABC immunohistochemistry staining: The positive expression of Bcl-2 and Bax was obviously increased in the spinal cord of guinea pig with EAE. The number of Bcl-2 positive cells in the 28th day of model group was more than that of the 14th day. The number of Bax positive cells in the 28th day and 14th day of model group did not show obvious difference at un-lesion areas. CONCLUSION: These results suggest that Bcl-2 and Bax may play an important role in EAE.
8.Effect of Nasal Administration of Compound of Cholera Toxin B Subunit and Nerve Growth Factor on Spatial Learning and Memory Abilities in the Amnesic Mice

ObjectiveTo explore the effects of the conjugate prepared from the cholera toxin B subunit(CB) and nerve growth factor(NGF) on the spatial learning and memory abilities and cholinergic function.MethodsThe conjugate of CB-NGF was prepared by the improved sodium metaperiodate method and nasally administrated to the β-amyloid protein(Aβ25-35) induced amnesic mice for 7 days with 2 dosage (7-5 μg/d、15 μg/d). Spatial learning and memory abilities were evaluated by Morris water maze and cholinergic function was assessed with the choline acetyl transferase (ChAT) immunohistochemical methods.ResultsMorris water maze test showed that the escape latency in Aβ25-35-treated mice prolonged and the staying time reduced in the crossed first quadrant where the platform had been located, compared with the control mice (P<0-01). In addition, the number of ChAT positive neuron declined in the model mice(P<0-001). CB-NGF nasal administration significantly shortened the escape latency and elevated the staying time and number of ChAT positive neuron(P<0-01).ConclusionCB-NGF treatment can improve the spatial and memory performance which may involve the neuroprotection to cholinergic system.
9.Investigation of posttraumatic stress disorder and coping style in medical school undergraduates

Objective:To investigate the occurrence and characteristics of post-traumatic stress disorder (PTSD) in medical school undergraduates, in order to provide reference to promote their mental health.Methods:Adopting stratified and cluster sampling method, 883 undergraduates from Xi ′an Medical College were selected and surveyed on March 9-16, 2020 by general condition questionnaire, PTSD Checklist-Civilian Version (PCL-C) and Simplified Coping Style Questionnaire (SCSQ). Results:The total score of PCL-C was (24.34 ± 8.12) points, (7.19 ± 2.61) points for re-experiencing symptoms, (9.87 ± 3.49) points for avoidance symptoms, and (7.28 ± 2.85) points for arousal symptoms. The incidence of PTSD was 8.2% (72/883). The scores of PTSD symptoms were higher in grade four students and whose coping tendency was negative, (26.37 ± 9.26) and (26.49 ± 9.48), respectively ( F value was 3.065, P<0.05; t value was -9.357, P<0.001). Positive coping scores were negatively correlated to PTSD symptoms ( r value was -0.130, P<0.001), while negative coping scores were positively correlated to PTSD ( r value was 0.244, P<0.001). Medical students with strong professional attitude had lower PTSD scores (23.23 ± 6.92)( F value was 21.650, P<0.001) and lower negative coping scores (8.21 ± 4.65) ( F value was 6.567, P<0.001). Conclusions:The incidence of PTSD was 8.2% in medical school undergraduates, whose severity was related to grade and coping tendency. Appropriate crisis intervention should be taken among medical students to improve their positive coping style, strengthen their professional identification and promote their mental health.
10.Study on risk factors of screening myopia among primary and middle school students in Hainan Province in 2021

Objective:
To analyze the screening myopia status and risk assessment of influencing factors among primary and secondary school students in Hainan Province in 2021, so as to providea reference for formulating myopia prevention and control intervention strategies and measures of school in Hainan Province.
Methods:
According to the requirements of the national project monitoring plan, the stratified cluster random sampling method was used to investigate the myopia screening and related influencing factors in 5 monitoring points of common diseases monitoring of students in Hainan Province from September to December 2021, and 12 075 valid questionnaires were obtained for analysis. Using stratified random method, all samples were divided into training data set and test data set according to the ratio of 7∶3, and regression analysis was conducted to verify the robustness of the results.
Results:
In 2021, the screening myopia detection rate of primary and secondary school students in Hainan Province was 44.3%, among which the screening myopia detection rate of students in the middle and good districts was higher (53.5%), that of students in urban areas (52.6%) was higher than that of students in suburban counties (34.9%), and that of girls (51.7%) was higher than that of boys ( 37.3% ). The detection rate of Han students (47.7%) was higher than that of ethnic minorities, and the difference between groups was statistically significant( χ 2=152.71, 378.77, 167.81, 251.94, P <0.01); The detection rate of screening myopia increased with the increase of the school level( χ 2=1 421.66, P <0.01). Multivariate Logistic regression analysis showed that the results of the two sets of data were consistent, and being in a higher grade, daily frequency of eye exercises <2 times, poor habit of short distance use of eyes (lying or lying on the stomach reading or watching electronic screen), having one or both parents with myopia were positively correlated with screening myopia( P <0.05). For boys, time spent doing homework/reading and writing after school every day <2 h, time spent using mobile electronic devices per day <1 h, students with 1 h and daily sleep duration≥9 h were negatively correlated with the incidence of screening myopia( P <0.05).
Conclusion
The risk of screening myopia increases with the increase of school age in Hainan, and relevant departments should strengthen targeted intervention and prevention for students with high risk of screening myopia.
