1.Advancement of hybrid procedures in congenital heart disease

Hybrid procedure is an method that combines surgery and intervenfional catheterization. As collaboration between surgeon and interventionalist has improved, the procedures for complex congenital heart disease is progressing and developing remarkably. Combining echocardiography monitoring in hybrid surgical procedure of complex congenital heart disease not only reduces side effect of extracorporeal circulation and operation time, but also retrieves blind spot of interventional catheterization for infant and low weight baby. Currently it is applied in the field of atrial septal defect, muscular ventricular septal defect, pulmonary stenosis and post-operation residual shunts or stenosis.
2.Serum levels of uric acid in the population of health check-up

Total 11 008 subjects underwent routine health check-up at Tianjin General Hospital in 2008. The results showed that the rate of hyperuricemia was 11.56% ( 1272/11 008 ) in this population, 17. 17% ( 1216/7082 ) for male, and 1.43% (56/3926) for female (P <0. 05). The serum uric acid levels were positively correlates with body mass index ( r = 0. 346), waist circumference ( r = 0. 449), systolic blood pressure ( r = 0. 193 ), diastolic blood pressure ( r = 0. 240), total cholesterol levels ( r = 0. 240) and triglyceride ester levels ( r = 0. 284 ). The factors including overweight, hypertension, hyperlipidemia and hyperglycosemia were clustered in subjects with hyperuricemia.
3.Analysis of prognostic factors and index model in patients with brain metastases after whole-brain radiotherapy

Background and purpose: Radiation therapy is still the most primary treatment of brain metastases, and prognosis is affected by many factors. The aim of this study was to identify the prognostic factors and to establish a prognostic index model in patients with brain metastases after whole-brain radiotherapy (WBRT). Methods: We reviewed the clinical date of 140 patients with brain metastases radiotherapy in our hospital from Jan. 2008 to Jul. 2011. The signiifcance of prognostic variables in the survival was resulted from both univariate analysis and multivariate analysis. The prognostic index (PI) was established based on Cox regression analysis and subgrouping values. It was assessed whether recursive partitioning analysis classes (RPA), basic score for brain metastases (BS-BM) and the graded prognostic assessment index (GPA) were related to prognosis. Results:The median survival time was 222 days. The univariate analysis showed that the independent prognostic factors were KPS performance status, number of brain metastases, presence of extracranial metastases, primary tumor status, radiation dose, hemoglobin. The multivariate analysis showed that KPS performance status (P=0.002, Wald=9.700), presence of extracranial metastases (P=0.018, Wald=5.604) and primary tumor status (P=0.001, Wald=10.212) were signiifcantly correlated with overall survival. RPA, BS-BM and GPA were closely related to their prognosis by Log-rank test. In predicting 3 months and 6 months of survival for patients, PI was better than other modes. Conclusion:Our data suggest that the 3 indexes RPA,BS-BM and GPA are valid prognostic index models, but PI model is better.
4.The theoretical foundation and research progress for erlotinib combined with whole brain radiotherapy for the treatment for non-small cell lung cancer patients with brain metastases

The prognosis of patients with brain metastases from non-small cell lung cance (NSCLC) is dismal,and whole brain radiation therapy(WBRT) cannot simultaneously control the extracranial lesions.Study results of Tyrosine kinase inhibitor(TKI) for brain metastases in patients with lung cancer are encouraging,and treatment efficacy is related to EGFR mutation status.The sensitizing theoretical foundation exists in utilizing erlotinib combined with WBRT for the treatment in brain metastases patients with lung cancer.Currently,a small clinical trial data shows that erlotinib combined with WBRT has better efficacy compared to erlotinib alone,and the toxicity can be tolerated.

Objective To investigate the effects of Wortmannin combined with mitomycin C (MMC) on the proliferation,apoptosis and expressions of related genes Cyclin D1 and Bad in liver cancer cell line HepG-2. Methods HepG-2 cells (104/ml) were treated with 0,25,50,100,200 or 400 nmol/L Wortmannin,0,1.75,3.75,7.5,15 or 30 nmol/L MMC,or combined concentrations (0,26.875,53.75,107.5,215 or 430 nmol/L) respectively. Cell suppression rate were detected by MTT assay. Nucleus apoptosis of Hep-G-2 cells were examined by Hochest 33258 fluorescent staining. Change of cell cycle and apoptosis in HepG-2 cells were detected by flow cytometry (FCM). Expressions of PI3K/Akt signaling transduction pathway related genes and proteins were tested by reverse transcription-polymerase chain reaction (RT-PCR) and Western blot analysis. Aim To investigate the effects of monocrotaline pyrrole on production of nitric oxide and expression of eNOS protein of cultured pulmonary artery endothelial cells and on contractility of cultured pulmonary artery smooth muscle cells.Methods DAF-2 fluorescence technique was used to determine NO level,Western blot analysis was performed to determine the level of eNOS protein,and collagen gel contraction system was adopted to analyze muscle contractility.Results NO production induced by ACh and expression of eNOS protein were obviously inhibited by monocrotaline pyrrole compared with those of control group and gel contraction area in MCTP-treated cells induced by Thapsigargin obviously decreased.Conclusions monocrotaline pyrrole could inhibit the level of the ACh-induced production of NO and expression of eNOS protein,and enhance the contractility of pulmonary artery smooth muscle cells,which may be one of the possible mechanisms of MCTP-induced pulmonary artery hypertension.
7.The expression and antibacterial function of neutrophil extracellular traps in systemic lupus erythematosus

Objective To investigate the expression and antibacterial function of Neutrophil extracellular traps (NETs) in systemic lupus erythematosus (SLE) patients.Methods Thirty stable SLE patients and thirty normal controls (NC) were recruited to this study.The acitivity of SLE was assessed by SLE Disease Activity Index (SLEDAI).Density gradient centrifugation was uesd to isolate neutrophilic granulocytes.Picogreen assay was used to quantify NETs formation.Colony counting method was used to compare the antibacterial rate of NETs between two groups.Variation of quantity and activities of neutrophil elastase (NE) and myeloperoxidase (MPO) on induced NETs was tested by enzyme linked immunosorbent assay (ELISA).Results There was no significant difference in the number of NETs between SLE and NC.Compared to the control group,more NETs were induced from neutrophils in SLE,the antibacterial rate of induced NETs was significantly lower,with the activities of antibacterial protein MPO decreased.Conclusions Stable SLE patients are more easily to induce NETs,and the antibacterial function of induced NETs are truly defective,may be related to the decreased activity of the antibacterial protein MPO.
8.ZNF330 promotes erythroid differentiation

Objective To explore the effects of ZNF330 on erythroid differentiation of K562 cells and underlying mechanism .Methods Realtime PCR was performed to detect the expression of ZNF 330 in K562 cells induced by hemin .After CD34 +cells being infected by the recombination lentivirus ZNF 330-RNAi, Realtime PCR was applied to detect the expression of CD235a and γ-globin.The luciferase report assay was performed to examine if ZNF 330 could act as a trans-acting factor in 293T/17 cells.Co-Immunoprecipitation (Co-IP) was applied in 293T/17 cells to detect the interaction between ZNF 330 and ZNF408 which was involved in mRNA degradation .Results The ex-pression of ZNF330 was up-regulated after hemin treatment .The expression of CD235a andγ-globin decreased after inhibition expression of ZNF 330 had no effect on report gene .Co-Ip in two ways confirmed the direct binding be-tween ZNF330 and ZNF408 .Conclusions ZNF330 can promote erythroid differentiation , and a possible mecha-nism is that ZNF330 inhibits the function of ZNF 408 , a factor that is involved in mRNA degradation , through the interaction between the two proteins .
9.Effect of three different modes of non-invasive positive pressure ventilation on cardiac indices of premature infant with respiratory distress syndrome: a randomized control study

Objective To study the effect of three different modes of non-invasive positive pressure ventilation on cardiac indices of premature infants with respiratory distress syndrome (RDS).Method From January 2014 to October 2015,preterm infants who had RDS received intubation-pulmonary surfactantextubation in the neonatal intensive care unit of the Hospital were randomly assigned (by random number table) to three groups based on the primary mode of ventilation:nasal continuous positive airway pressure (NCPAP),bi-level positive airway pressure (BiPAP),and synchronized bi-level positive airway pressure (SBiPAP).The mean airway pressure (MAP) were about 6 cmH2O in the three groups.The level of plasma B-type natriuretic peptide (BNP),cardiac troponin Ⅰ (cTnI),and correct QT intervals dispersion (QTcd) were monitored before and 42-54 h after non-invasive ventilation.Result There were 173 cases in our study,59 of which in NCPAP group,56 in BiPAP group,and 58 in SBiPAP group.The plasma BNP level at 42-54 h after non-invasive ventilation in the three groups were all higher than that before non-invasive ventilation [NCPAP group:(247.9 ± 137.9) ng/L vs.(182.5 ± 1 10.7) ng/L,P =0.007;BiPAP group:(258.5 ± 131.2) ng/L vs.(182.6 ± 105.0) ng/L,P < 0.001;and SBiPAP group:(260.9 ± 159.7) ng/L vs.(177.5 ± 101.5) ng/L,P =0.002].After 42-54 h non-invasive ventilation,there were no significant changes of plasma cTnI level and QTcd in all the three groups (all P > 0.05).The level of plasma BNP,cTnI,and QTcd among the three groups before and after 42-54 h non-invasive ventilation all showed no significant differences statistically (all P > 0.05).Conclusion Longer duration (42-54 h) of non-invasive positive pressure ventilation (MAP:6 cmH2O) in preterm infants with RDS may lead to increased plasma BNP level,and may affect their cardiac function.However,it may not lead to serious myocardial damage and abnormality of ventricular repolarization.There were no significant differences in cardiac indices of premature infant with RDS among NCPAP,BiPAP,and SBiPAP group with the same MAP.
10.The value of D-dimer in assessing severity and predicting long-term prognosis in patients with community acquired pneumonia

Objective To evaluate the value of D-dimer in assessing severity and predicting longterm prognosis in patients with community acquired pneumonia (CAP).Methods From June 2009 to December 2010,a total of 189 patients with CAP were enrolled.After admission,D-dimer,procalcitonin (PCT) and C-reactive protein (CRP) were measured,and Pneumonia Severity Index (PSI) was calculated.They were assigned into two groups according to their D-dimer levels:high D-dimer levels group (D-dimer levels≥500 μg/L) and normal D-dimer levels group (D-dimer levels < 500 μg/L).The followup time was one year.A Kaplan-Meier survive curve was constructed to assess the 1-year mortality,and multivariate logistic regression analysis were used to assess the value of D-dimer for predicting long-term prognosis.Results D-dimer levels increased with increasing PSI class [class Ⅰ-Ⅲ:378.37 μg/L (216.74,649.50) μg/L; class Ⅳ:673.41 μg/L (544.77,866.85) μg/L; class Ⅴ:831.58 μg/L (591.78,1066.39) μg/L,x2 =56.58,P < 0.01].The Kaplan-Meier survival curve showed that 1-year mortality rate of high D-dimer levels group was higher than normal D-dimer levels group (log-rank test,x2 =52.51,P < 0.01).The multivariate logistic regression analysis showed an independent relationship between higher D-dimer levels and long-term mortality (OR =2.05,95% CI:1.48-2.61,P < 0.01).Conclusion D-dimer is an independent predictor of severity and long-term prognosis in patients with CAP.
