3.Features and Misdiagnosis of Benign Paroxysmal Positional Vertigo in the Elderly: 21 Cases Report

ObjectiveTo investigate the clinical features and the causes of misdiagnosis of benign paroxysmal positional vertigo (BPPV) in the elderly. Methods21 aged patients diagnosed with BPPV finally were analyzed retrospectively. ResultsBPPV occurred more in female and associated with the posterior semicircular canal. The symptoms of vertigo were obviously improved with the canalith repositioning. With suffering from chronic diseases such as hypertension and cervical spondylosis etc., BPPV in the aged was easily misdiagnosed. ConclusionThe main reason for the misdiagnosis of BPPV in the aged is deficiency of knowledge of vertigo. Canalith repositioning is a safe and effective treatment for the aged with BPPV.
4.Superiority of Multimedia in Ultrasound Diagnostics Teaching

This article introduces the way of making and using ultrasound multimedia courseware,which can improve teaching means,enrich teaching contents,activate classroom atmosphere and enhance the connection of basic and clinical knowledge.Meanwhile,the environment request of the demonstration courseware and interaction courseware as well as the necessity and significance of multimedia in ultrasound teaching are discussed.It is suggested that computer assisted instruction(CAI) plays an important role in ultrasound teaching.
5.Comparative study of methotrexate and uterine arterial embolization inβ-HCG, bleeding volume and success rate of women with cesarean scar pregnancy after cesarean section

Objective To explore comparison between methotrexate and uterine arterial embolization in β-HCG, bleeding volume and success rate of women with cesarean scar pregnancy after cesarean section.Methods 42 patients who were diagnosed with cesarean scar pregnancy after cesarean section were collected.All patients were randomly divided into uterine arterial embolization group and methotrexate group,21 cases in each group corresponding treatment were given respectively, after the treatment, the serum levels of β-HCG, bleeding volume and success rate were detected in all patients.Results After treatment, compared with methotrexate group, the serum level ofβ-HCG was lower in the uterine arterial embolization group,and the difference was statistically significant(P<0.05); the bleeding volume was lower in the uterine arterial embolization group(P<0.05); the success rate was higher in the uterine arterial embolization group(P <0.05).Conclusion Compared with methotrexate,the uterine arterial embolization can significantly reduce the serum level ofβ-HCG in patients with cesarean scar pregnancy after cesarean section,reduce the amount of bleeding, improve the success rate of treatment.
6.The effect of atorvastatin and rosuvastatin on endothelial function in diabetics with coronary heart disease

Objective To investigate the effect of atorvastatin and rosuvastatin on endothelial function in diabetics with coronary heart disease. Methods A total of 73 consecutive diabetics with coronary heart disease who were not receiving statins were randomized to receive atorvastatin 20 mg/d or rosuvastatin 10 mg/d. The levels of lipids, high-sensitivity C-reactive protein(hs-CRP), monoxide nitrogen(NO), endothelin-1 (ET-1) and endothclium-dependent relaxing function(EDF) were assessed before and after 3 months of the treatment. Results The treatment with statins significantly improved endothelial function in diabetics with coronary heart disease. For both rosuvastatin group and atorvastatin group in pro-treatment versus post-treatment, EDF was higher[(5.2±2.4)% vs. (7.9±3.1)%, P=0.008; (5.0±2.8)% vs. (7.6±3.6)%, P=0.024, respectively], NO was also higher((46.6±14.5) μmmol/L vs. (73.3±18.5) μmmol/L; (51.7±14.0) μmmol/L vs. (79.8±16.0) μmmol/L,both P<0.001], ET-1 was lower[(108.2±29.6) pg/L vs. (77.5±26.4) pg/L, P =0.005;(117.1±34.5) pg/L vs. (80.7±28.2) pg/L, P= 0.003, respectively],and hs-CRP was lower[3.17(1.33~6.32) mg/Lvs. 1.39(0.81~2.58) mg/L, P=0.006; 3.43(1.51~7.02) mg/L vs. 1.63(0.69~3.11) mg/L, P = 0.001, respectively]. There were no differences in these between rosuvastatin group and atorvastatin group. Significant negative correlation existed between EDF and hs-CRP. Conclusions A 3 months treatment with either atorvastatin 20 mg/d or rosuvastatin 10 mg/d is effective in improving endothelial function in diabetics with coronary heart disease.
7.The study on production of anti-thrombomodulin antibodies using genetic immunization

Objective To produce anti-thrombomodulin antibodies.Methods Using genetic immunization: Eukaryotic expression plasmid pcDNA3.1/TM(LEO),encoding all the extracellular domain of human thrombomodulin and signal peptides but lacking the transmembrance and cytoplasmic domains was constructed, which recombinant thrombomodulin was secreted soluble product. The plasmid was isolated from large-scale bacterial cultures by treatment with alkali and SDS, purified by precipitation with polyethylene Glycol (PEG). Recombinant plasmid was injected into tibial muscle of BALB/c mice. The productions of TM and anti-TM have been detected. Results The positive of RT-PCR and expressed TM identified the function of the recombinant plasmid. The pcDNA3.1/TM(LEO) induced higher titer of anti-TM. The antibody titer peaked between the 5th and 7th injection with a titer of 1∶8 000 detected by cell-ELISA coated with EVC-304. Specificity has been identified by western blot and immunohistochemistry.Conclusion The production of antibody through genetic immunization was a feasible method due to the difficulties in obtaining and purification of natural thrombomodulin.
8.Effects of tacrolimus on the proliferation of HepG2.2.15 cells and the replication of hepatitis B virus in vitro

BACKGROUND: The relationship between liver cancer recurrence and hepatitis B virus recurrence remains poorly understood and it is considered to be related to application of immunosuppressive agent after liver transplantation. OBJECTIVE: To investigate the effects of tacrolimus (FK506) on the proliferation of HepG2.2.15 cells and the replication of hepatitis B virus in vitro. METHODS: HepG2.2.15 cells were in vitro cultured. After passage 3 HepG2.2.15 cells were cultured for 24 hours, they were interfered with different concentrations of FK506. 0 g/L FK506-interfered group served as control group, 50 g/L FK506-interfered group as low-concentration FK506 group, 100 g/L or 500 g/L FK506-interfered group as medium-concentration FK506 group, and 1 000 g/L or 3 000 g/L FK506-interfered group as high-concentration FK506 group. RESULTS AND CONCLUSION: Moderate- and high-concentration FK506 exhibited inhibitory effects on the proliferation of HepG2.2.15 cells, while low-concentration FK506 exhibited no inhibitory effects with correlation. High-concentration FK506 made HepG2.2.15 cells arresting at G0/G1 stage. FK506 decreased CyclinA expression in HepG2.2.15 cells in a dose-dependent manner. Higher concentration of FK506 leaded to lower expression of CyclinA. FK506 did not produce effects on the replication of hepatitis B virus in HepG2.2.15 cells. These results indicate that FK506 inhibits the proliferation of HepG2.2.15 cells in vitro, which occurs possibly due to Cyclin A, but it would not affect the replication of hepatitis B virus in vitro.
9.Study on quality assurance system of grade 3 laminar operation room

ObjectiveTo explore the features of the cleanliness change with time during the use of laminar operating rooms in order to establish reliable quality assurance system. MethodsTwo grade 3 lami-nar operation rooms were chosen to be tested after one and a half year and two and a half years, laser counter was used to detect the static particulates after cleaning for 30 minutes and 45 minutes. ResultsAfter the laminar operation room was used for one and a half years the amount of 0.5 μm particulates in indoor air was less than 350 per liter, and after two and a half years it's above 350 per liter after cleaning for 30 minutes, an obvious difference than the previous one. The anaount of 1 μm and 2 pan particulates also increase obviously, but ff cleaning for 45 minutes the amount of 0.5 μm particulates in indoor air will fulfill the qualification. Al-though the amount of 0.5 μm, 1 pan and 2 μm particulates after 2 years using was higher than the one and a half yesrs'using after cleaning for 45 minutes, but the difference had no statistic meaning. CondusionsThe elesnliness of laminar operation rooms will drop as time prolonged, Amount of particulates in indoor air will in-crease too but the cleanliness can be ensured by extending cleaning time. After the quality assurance sys-tem was established the amount of smile particulates is still within the standard level
10.Mutation of X region nucleotide sequence of hepatitis B virus DNA in patients with HBV reinfection after Hver transplantation

Objective To analyze the mutation of HBV X region nucleotide sequence in patients with HBV reinfection after liver transplantation. Methods In this study 320 patients received liver transplantation due to HBV-related end stage hver diseases between June 2002 and Dec 2003.Postoperatively polymerase chsin reaction was used to aInplify their serum HBV DNA fragments for direct sequence analysis.Patients that were followed-up for 1.5~3 years were enlisted for analysis.Results All the 11 reinfection recipients showed nucleotide mutations in X region mng4ng from 5 to 39 sites after transplantation.An A to T mutation at nt1762 and G to A mutation at nt1764 were found in 6 cases.The mutations at nucleotide fnt)1636~1741 were found in all 11 cases.Condusions The results indicated that mutations of nt 1762 and nt 1764 are very common and immunosuppressants cannot change the mutations in patients with HBV reinfection after liver transplantation.
