1.One case of systemic mastocytosis with associated clonal hematological non-mast-cell lineage disease and literatures revivew

Objective To improve the understanding of systemic mastocytosis(SM) with associated clonal haematological non-mast-cell lineage disease (SM-AHNMD). Methods The clinical and laboratory features, treatment and prognosis of a rare case of SM-AHNMD was reported and the related literatures were reviewed. Results As the second most common subtype of SM, SM-AHNMD mainly has multifocal dense infiltrates of mast cells detected in sections of bone marrow and/or other extracutaneous organs, and some special cytology or biochemical markers(toluidine blue staining,tryptase,CD117,CD25 and/or CD2) results are positive. C-kit mutations are found in most adult patients. SM-AHNMD also has clinical characters of other clonal haematological non-mast-cell lineage disease (HNMD), such as myelodysplastic syndrome,myeloproliferative neoplasms, acute myeloid leukemia. Conclusion SM-AHNMD meets criteria for SM and other HNMD. Treatment for SM-AHNMD is mainly about the therapy of HNMD, and the prognosis greatly depends on the accompanying hematological disorder.There is no special treatment for SM,and the treatment should be directed at symptomatic control.

4.Simultaneous Determination of Six Flavonoid Ingredients in Shuxuening Injection by Quantitative Analysis of Multi-components by Single-marker

Objective:To simultaneous determine six flavonoids in Shuxuening injection by quantitative analysis of multi-compo-nents by single marker ( QAMS) and examine the durability and system suitability of the analysis. Methods:Shuxuening injection was the study object and rutin was used as the internal material, the relative correction factor ( f) between rutin and narcissus glycosides, quercetin-3-O-β-D-glucosyl (1-2)-α-L- rhamnoside, kaempferol-3-O-β-D-glucose group (1-2)-α-L-rhamnopyranoside, quercetin-3-O-[2-O-(1-O)-p-coumaric acid-β-D-glucose-yl]-α-L-rhamnoside, kaempferol-3-O-[2-O-(6-O)-on coumaroyl-β-D-glucose-based]-α-L-rhamnosides was respectively established. The result differences between the internal standard method and external standard method were compared, and the internal standard method was re-validated. Results: The contents of the six flavonoids in Shuxuening injection were calculated by the correction factors. Compared with those of the external standard method, the differences in the contents of the method were small( RSD<5%) . Conclusion:QAMS can be used as a simple and accurate quantitative quality evaluation meth-od for the determination of a variety of ingredients in Shuxuening injection, which provides reference for the subsequent study on quality standard.
5.Short effect and adverse reaction of dog days plaster for allergic rhinitis.

OBJECTIVETo evaluate the short effect and safety of the acupoint application of dog days for allergic rhinitis.
METHODSTwo hundred and forty-nine patients were randomly divided into an application group (166 cases) and a placebo group (83 cases). On the first day of the first dog days, the first day of the second of the three ten-day periods of the hot season and the first day of the last of the three ten-day periods of the hot season, acupoint application was adopted mainly at Dazhui (GV 14), Feishu (BL 13), Pishu (BL 20) and Shenshu (BL 23) in the application group. The plaster was made of Chinese herbs, including Baijiezi (semen brassicae), Xixin (asarum), Yanhusuo (corydalis tuber) and Gansui (euphorbia kansui). Buckwheat plaster without medical ingredient was used in the placebo group. The application was pasted for 2-6 h every time. On the first three days of all the three periods of dog days, treatment was used continuously. The whole period of dog days was made into one course. The changes of symptom and syndrome scores, visual analogue scale (VAS) for symptom before and after treatment and the adverse reaction with the condition disposed were observed in the two groups.
RESULTSAfter treatment, the symptom and syndrome scores and VAS for symptom were improved compared with those before treatment in the two groups (all P < 0.05), and the improvements of the application group were more obvious than those of the placebo group (all P < 0.05). The short effective rate of the application group was 68.1% (113/166), which was better than 48.2% (40/83) of the placebo group (P < 0.05). The adverse reaction rates were 4.2% (7/166), 2.4% (2/83) respectively, with no statistical significance between the two groups (P > 0.05).
CONCLUSIONThe dog days plaster achieves obvious effect with good safety for allergic rhinitis.

6.The Effect of changes in the Temporomandibular joint blood supply on glyco saminoglycans of the rabbit condylar cartilage and development of bone and joint disease

Objective The purpose of this study was to investigate the effect of blood supply decrease on glycosaminoglycans(GAG)of the cartilage in the temporomandibular joint(TMJ) . Method Four rabbits were used as controls, and 16 rabbits were injected 5% sodirm morrhumate through carotid artery unilaterlally. All specimens were examimined histochemically. Result A reduction in alcian(AB) blue staining was observed after operation, a gradual increase in AB staining with blood supply restoration of TMJ tissue was observed. Conclusion The result suggests that the decrease in blood supply of the temporomnandibular jorint leads to the loss of GAG in the condylar cartilage, which may play an important role in pathogenesis of osteoarthrosis.
7.Hypothermia combined with chemoradiotherapy in treatment of local advanced cervical cancer

Objective To evaluate the efficacy and toxicity of hyperthermia combined with chemoradiotherapy for local advanced cervical cancer. Methods Sixty-eight patients with local advanced cervical cancer were treated. Each patient was given cisplatin at the dose of 20 mg ivgtt dl -2 and 5 -fluorouracil 0.75 ivgtt dl -2, repeating every week, total 3 weeks. Thirty -eight patients received chemoradiotherapy alone. Thirty patients received hyperthermia with deeply focus technology, the temperature was increased and maintained 42.5 ℃ for 30 min at least, within 2 hours following irridiation. Hyperthermia was given twice per week with 7 or 12 sessions, as the same chemoradiation methods as the reference group.After 40 Gy therapy to estimate by cervical examination and ultrasonography, the patients with down stage were for operation, and the others continued to irridiation and additional brachytherapy with californium 252until radical dose. Results All patients were tolerated, fifteen patients of hyperthermia combined group were operated and ten patients of the reference group were operated, the difference was significance (50.0 % vs.26.3 %, P <0.05). The response rate of the combined group was 90.0 %, and the reference group was 84.2 %.The incidence of radiotherapy toxicity of the combined group was not higher than the reference group.Conclusion Hyperthermia combined with chemoradiotherapy can elevate the resection rate and the local control rate, not increase the radiotherapy toxicity and increase quality of life for cervical cancer patients.
8.Comparison of efficacy of video-assisted thoracoscopic surgery and chest tube drainage combined with urokinase for the treatment of empyema after fibropurulent pneumonia

Objective To compare the clinical effect of video-assisted thoracoscopic surgery (VATS) and chest tube drainage combined with urokinase for the treatment of empyema after fibropurulent pneumonia, and to determine the best therapy for patients. Methods Patients were randomly divided into two groups according to inclusion and exclusion crite?ria:55 patients were selected as group VATS performed VATS;46 patients were selected as drainage group performed 12F catheter chest closed drainage by Seldinger technique under B ultrasound guidance. The 100 000 unit urokinase was inject?ed into chest, and open after 4-hour closed, once a day for three days. Values of white blood cell (WBC), pH, glucose, lactate dehydrogenase (LDH) and C reactive protein (CRP) of pleural effusion before treatment were recorded, and the duration of fe?ver after treatment, duration of chest-tube placement, antibiotic use, hospital stay, cure rate, complication and hospitaliza?tion expenses were also recorded. Results The duration of fever after treatment,, duration of chest-tube placement, antibiot?ic use and hospital stay were significantly lower in VATS group than those in drainage group. The cure rate was significantly higher in VATS group than that of drainage group (P<0.05). There were no significant differences in complication rates and hospitalization expenses between two groups. There was no death in both two groups. Conclusion VATS is more suitable for the treatment of empyema after fibropurulent pneumonia than chest tube drainage combined with urokinase, and which is worthy of clinical promotion for not adding complication rates and hospitalization expenses.
9.Comparison of dexmedetomidine versus midazolam administered intranasally for preoperative sedation in pediatric patients undergoing neurosurgical procedures

Objective To compare the dexmedetomidine and midazolam administered intranasally for preoperative sedation in the pediatric patients undergoing neurosurgical procedures.Methods Forty pediatric patients of both sexes, aged 2-7 yr, weighing 1 1-26 kg, of American Society of Anesthesiology Anesthesiologists physical status Ⅱ, were equally and randomly assigned into either midazolam group (group M) and dexmedetomidine group (group D).Group M received midazolam 0.2 mg/kg administered intranasally, and group D received dexmedetomidine 2 μg/kg administered intranasally in the room for preoperative preparation.The pediatric patients were separated from their patients at 30 min after administration.Oxygen was inhaled by mask after admission to the operating room, and venipuncture was performed.The responses to nasal mucous membrane irritation and separation from their parents were recorded.Ramsay sedation scores were recorded when the patients were separated from their parents.The occurrence of bradycardia, hypotension, hypertension and hyoxemia was recorded from the end of administration until venipuncture.Results The pediatric patients were successfully separated from their patients in the two groups.Compared with group M, the incidence of responses to nasal mucous membrane irritation was significantly decreased, and Ramsay sedation scores were increased in group D (P<0.05).No pediatric patients developed adverse reactions such as bradycardia, hypotension, hypertension and hyoxemia in the two groups.Conclusion Dexmedetomidine administered intranasally provides better efficacy than midazolam when used for preoperative sedation in the pediatric patients undergoing neurosurgical procedures.
10.The demand analysis and plan design for the maintenance management system of medical equipment

Objective:To analyze the demands of maintenance management system of medical equipment, and realize the scientization and normalization of plan design for maintenance management system of medical equipment.Methods: On the basis of current status of maintenance management system of medical equipment, the demands of various department during management process were analyzed, and then a systematic maintenance management plan was designed to face to all of medical equipment in hospital.Results: The preponderances of this system were discussed from three aspects: equipment maintenance, equipment archive and equipment benefit.Conclusion: The application of this system can accelerate the progress of maintenance for medical equipment, enhance maintenance awareness of clinical medical staffs for medical equipment, improve the transparency of equipment scrapping and disposal, and provide effective references for demonstration and purchase of medical equipment.
