1.Fulminate Stenotrophomonas maltophilia Infection in ICU:An Epidemiological Study

OBJECTIVE To find out fulminate epidemiological features of Stenotrophomonas maltophilia in ICU and the ways to prevent and treat this nosocomial infection.METHODS The case histories from 4 inpatients who developed S.maltophilia infection in the same ward from Feb 5 to 16,2006,were studied retrospectively to find out the reasons of its onset and its treatment based on the sputum culture results.RESULTS The fulminate S.maltophilia was found from the rail of the patient bed,the connection part of water container of 2 respiratory machines of exhalation valve assembly and the liquid of ultrasonic aeriation machine.CONCLUSIONS The infection is a localized one.The main reasons of the infection are unthorough disinfection of respiratory machine and the contamination of medical treatment environment.Whenever the infection is found in the ward,the thorough disinfection needs immediately,and no new patient admitted.
2.Correlation of ultrasonographic features with molecular subtypes of breast invasive ductal carcinoma

Background and purpose:Breast invasive ductal carcinoma (IDC) is the most common breast carci-noma, and the ultrasonographic features of its molecular subtypes has great clinical value. The purpose of this study was to discuss the correlation between ultrasonographic features of IDC and its molecular subtypes.Methods:Ultrasonographic features of 112 patients with preoperative integrated ultrasonographic information and pathologically confirmed IDC from Sep. 2012 to Feb. 2015 were retrospectively analyzed. Based on the immunohistochemistry results of ER (estrogen receptor), PR (progesterone receptor), HER-2 (human epidermal growth factor receptor-2) and Ki-67, these cases were categorized into 4 molecular subtypes: Luminal A group, Luminal B group, ERBB2 positive group and Basal-like group. Results:There were 14 cases (12.5%) in Luminal A group, 62 cases (55.4%) in Luminal B group, 21 cases (18.7%) in ERBB2 positive group and 15 cases (13.4%) in Basal-like group. The 4 molecular subtypes differed in tumor length, lymph node involvement, tumor boundary, tumor shape and internal blood flow on ultrasonography (P<0.05). There were no sig-nificant differences in tumor internal echo, posterior echo, micro-calcification and elastrography between subtypes (P>0.05).Luminal A group and Luminal B group had less lymph node involvement, more obscure boundary and irregular shape. More internal blood flow, bigger tumor size and lymph node involvement were observed in ERBB2 positive group of this study. Patients in Basal-like group were more likely to have clear tumor boundary, regular tumor shape, bigger tumor size and lymph node involvement on ultrasonogram.Conclusion:Correlation exists between ultrasonographic features and molec-ular subtypes of IDC. This has tremendous clinical significance in the early diagnosis of IDC, preoperative, intraoperative and prognosis evaluation of IDC patients.

4.Gab2 effects the invasion and metastasis of breast carcinoma through PI3 K/Akt/ARK5/MMP signal pathway

Aim To investigate the molecular mecha-nism of Gab2 in the invasion and metastasis of breast cancer and provide a theoretical basis for clinical pre-vention of breast cancer invasion and metastasis. Methods The Gab2 , MMP-2 and MMP-9 expressions in 80 cases of breast cancer were detected by immuno-histochemistry . Western blot was used to detect the ex-pression of Gab2 protein in MDA-MB-231 cells and MCF-7 cells. The siRNA plasmid was used to transfect MDA-MB-231 cells. Western blot was used to detect the proteins expression of Gab2 , MMP-2 and MMP-9 . Transwell in vitro experiment was used to detect the in-vasion ability of each group transfected MDA-MB-231 cells, Western blot was used to analyze phosphorylation of Akt and ARK5 induced by epithelial growth factor ( EGF ) in transfected cells ( SiGab2/MDA-MB-231 and Scr/MDA-MB-231 ) . Results The expression of Gab2 protein in invasive ductal carcinoma was signifi-cantly higher than in normal breast tissue ( P<0. 01 ) . The expression of Gab2 was dramatically correlated with lymph node metastasis, ER expression, tumor his-tological grade, MMP-2 and MMP-9 (P<0. 05). The expression of Gab2 protein in MDA-MB-231 cells was higher than in MCF-7 cells. The expression of Gab2, MMP-2 and MMP-9 decreased in SiGab2/MDA-MB-231 cells and the invasion ability of SiGab2/MDA-MB-231 cells was significantly decreased ( P<0. 05 ) , and after 5 minutes’ stimulating by EGF, the phosphoryla-tion of Akt and ARK5 was significantly reduced. Con-clusion Gab2 can promote the invasion and metasta-sis of breast cancer by effecting the expression of MMP-2 and MMP-9 through PI3 K/ Akt /ARK5 signal path-way.
5.Gut bacterial translocation contributes to microinflammationin experimental uremia

Objective To investigate whether gut microbiome dysbiosis and translocation occurred in experimental uremia,and whether they consequently contribute to microinflammation.Methods Health male SD rats were randomly divided into uremic group and sham group.Uremic group were operated for 5/6 nephrectomy to establish uremic models,while sham group were only operated for nephrocapsulotomy.Postoperative blood,livers,spleens,and mesenteric lymph nodes (MLNs) were subjected to bacterial 16S ribosomal DNA amplification to determine if bacteria were present.Bacterial genomic DNA samples from the MLNs and colon were amplified with specific primers designed by the 16SrRNA sequence of the species obtained from blood,livers and spleens.Pyrosequencing was used to analyze the ileum and colonic microbio.me of each subject.Intestinal permeability to 99mTc-DTPA,plasma hs-CRP,and IL-6 were measured.Results Bacterial DNA in extraiutestinal sites and altered colonic microbiomes at the phylum,family,and genus levels were detected in some rats in the uremic group.Bacterial genomic DNA in MLNs and colon were obtained by primers specific for bacterial species observed from blood,livers,and spleens of identical individuals.Intestinal permeability,plasma hs-CRP,and IL-6 levels were statistically higher in the uremic group compared with that in sham group(all P < 0.05).Conclusion Gut microbiome dysbiosis occurs and presumably bacteria translocate to the systemic and lymph circulation,thereby contributing to microinflammation in experimental uremia.
6.Nir1 promotes invasion of glioma cells by binding to chemokine (C-C motif) ligand 18

Background and purpose:Nir1 is a transmembrane receptor for chemokine (C-C motif) ligand 18. CCL18 speciifcally binds to Nir1 at the cellular membrane of breast cancer cells to exert its invasion and metastasis. However, the speciifc mechanism of Nir1 is not clear in glioma. This study probed the effect and mechanism of Nir1 in the invasion of glioma cells.Methods:Western blot was used to detect the expression of Nir1 in glioma cells. siRNA plasmid was used to transfect U251 cells. Western blot was used to analyze the expression of Nir1 and protein phosphorylation of Akt in the cells transfected by Nir1 plasmid.In vitro Matrigel invasion assay was used to detect the invasive ability in the cells that were transfected. F-actin polymerization assay was used to detect F-actin recognition ability in cells.Results:The expression of Nir1 was higher in all glioma cells. After transfection, the invasion of siNir1/U251 was obviously decreased than the SCR/U251, F-actin content was reduced compared to the control group. Akt phosphorylation experiment result showed that the protein phosphorylation of Akt was enhanced in control group cells CCL18 following stimulation. However, the existence of CCL18 would affect the phosphorylation of Akt in siNir1/U251.Conclusion:Nir1 is high expression in glioma cells, and Nir1 binding to chemokine CCL18 promotes glioma cells invasion and metastasis through regulation the phosphorylation of Akt and F-actin polymerization .
7.Contribution of decreased expression of Ku70 to enhanced radiosensitivity by sodium butyrate in glioblastoma cell line (U251).

The present study investigated the enhanced radiosensitivity of U-251 cells induced by sodium butyrate (NaB) and its possible mechanisms. Increased radiosensitivity of U251 cells was examined by clonogenic cell survival assays. The expression of Ku70 mRNA and protein was detected by using RT-PCR and Western blotting respectively. γ-H2AX foci were measured at different time points after ionizing irradiation alone or combined with NaB treatment. The results showed that cell survival rate was significantly reduced, both D0 and Dq values were decreased (D0: 1.43 Gy vs. 1.76 Gy; Dq: 1.22 Gy vs. 2.05 Gy) after the combined treatment as compared with irradiation alone, and sensitivity enhancing ratio (SER) reached 1.23. The average number of γ-H2AX foci per cell receiving the combined treatment was significantly increased at different time points, and the expression levels of Ku70 mRNA and protein were suppressed by NaB in a dose-dependent manner. It was concluded that enhanced radiosensitivity induced by NaB involves an inhibited expression of Ku70 and an increase in γ-H2AX foci, which suggests decreased ability in DSB repair.

9.Study on the causes of drug shortage in Shanxi province and the construction of forewarning model

Objective:To construct the forewarning model of drug shortage in Shanxi province, so as to realize the early prediction of drug shortage.Methods:According to the drug shortage of data of each monitoring station in Shanxi province, from the four dimensions of drug factors, policy attributes, supplier factors and demander factors, 14 factors were selected for grey correlation analysis, for example whether they were urgent drugs for women and children, shortage types, etc. The main factors affecting the degree of drug shortage were selected, and the early warning analysis model based on two-step cluster analysis method was established.Results:A total of six factors with the highest correlation with the degree of drug shortage were determined in this study, in order: whether they were urgent drugs for women and children, shortage types, low-price drugs, the number of moving annual total monitoring enterprises in sale in the current month, shortage reasons, and whether they were basic drugs. Based on the two-step cluster analysis, a model of drug shortage forewarning in Shanxi Province was established, which was divided into four optimal clusters and the warning level was determined.Conclusions:This study establishes the early warning model of drug shortage in Shanxi province, which is helpful to find the risk of drug shortage as soon as possible, objectively predict the risk level, and assist the drug regulatory department to realize layered implementation and collaborative response.
10.Role of Nrf2/NLRP3 signaling pathway in propofol postconditioning-induced reduction of hippocampal neuron injury in a rat model of oxygen-glucose deprivation and restoration

Objective:To evaluate the role of nuclear factor erythroid 2-related factor 2 (Nrf2)/nucleotide-binding oligomerization domain-like receptor family pyrin domain containing 3 (NLRP3) signaling pathway in propofol postconditioning-induced reduction of hippocampal neuron injury in a rat model of oxygen-glucose deprivation and restoration (OGD/R).Methods:The hippocampal neurons were isolated from fetal rats of Wistar rats at 16-18 days of gestation and primarily cultured for 7 days and then divided into 4 groups ( n=42 each) using a random number table method: control group (group C), OGD/R group (group O), propofol post-conditioning group (group P) and Nrf2 siRNA(-) transfection group (group N). The cells were routinely cultured in group C. The cells were subjected to oxygen-glucose deprivation for 1 h followed by oxygen and glucose supply in group O. Propofol (final concentration 1.2 μg/ml) was added immediately after oxygen and glucose supply, the cells were then cultured for 2 h, and the culture medium was replaced with the normal culture medium in group P. The primarily cultured neurons were transfected with Nrf2 gene knockout lentivirus on 3rd day of culture, 24 h later the cells were then routinely cultured, and the model was prepared and propofol conditioning was performed on 7th day. Cells were collected at 24 h of incubation for determination of the cell apoptosis (by flow cytometry), expression of Nrf2 and NLRP3 mRNA and protein (using quantitative real-time polymerase chain reaction or Western blot), concentrations of tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and IL-1β, and activities of glutathione (GSH), superoxide dismutase (SOD) and catalase (CAT) (kit method). Results:Compared with group C, the apoptosis rate of neurons was significantly increased, concentrations of TNF-α, IL-6 and IL-1β were increased, the levels of GSH, SOD and CAT were decreased, the expression of Nrf2 and NLRP3 protein and mRNA was up-regulated, and the nuclear/plasma ratio of Nrf2 was increased in O and P groups ( P<0.05). Compared with group O, the apoptosis rate of neurons was significantly decreased, the concentrations of TNF-α, IL-6 and IL-1 β were decreased, the levels of GSH, SOD and CAT were increased, the expression of Nrf2 protein and mRNA was up-regulated, the nuclear/plasma ratio of Nrf2 was increased, and the expression of NLRP3 protein and mRNA was down-regulated in group P ( P<0.05). Compared with group P, the apoptosis rate of neurons was significantly increased, concentrations of TNF-α, IL-6 and IL-1β were increased, the levels of GSH, SOD and CAT were decreased, the expression of Nrf2 protein and mRNA was down-regulated, the nuclear/plasma ratio of Nrf2 was decreased, and the expression of NLRP3 protein and mRNA was up-regulated in group N ( P<0.05). Conclusions:Nrf2/NLRP3 signaling pathway is involved in propofol postconditioning-induced reduction of hippocampal neuron injury in a rat model of OGD/R.
