1.Ventilation administration during fiberoptic bronchoscopy in neonates

Objective To assess ventilation administration during fiberoptic bronchoscopy (FB) in neonates. Methods Twenty-three neonates divided into two group (A group 12 neonates, B group 11 neonates) received FB. All were given pressure support ventilation (PSV)by a Y-like facility which connected to fiberoptic bronchoscope suction hole. In A group,after the tip of fiberoptic bronchoscope arrived at the carina, PSV was administrated. In B group, PSV was administrated in the entire process during FB, SpO2 and electrocardio were monitoring. Artery blood samples for blood gas analysis were obtained at four stages of just before FB,with the tip of the bronchoscope at the supralarynx,just before withdrawing bronchoscope out off trachea and within 20-30 minutes after FB. The arterial blood oxygen tension (PaO2), arterial blood carbon dioxide tension (PaCO2) and SpO2 just before FB served as baseline. The same indexes of other three stages were compared with the baseline. Results All 23 neonates were studied completely. When the tip of fiberoptic bronchoscope advanced from nostril to the supralarynx, SpO2, PaO2 and PaCO2 in two groups were similar to the baseline. In A group, when the tip below the glottis, cyanosis occurred, and SpO2 decreased significantly ( P<0. 01 ) in 11 cases (92%) by 25% ; When tip at the carina, after PSV, cyanosis disappeared, and SpO2 returned to the baseline level, PaO2 keep on the baseline just before withdrawing the bronchoscope out of the trachea. SpO2 ,PaO2 in all B group neonates keep on the baseline during FB. After the tip below the glottis,PaCO2 in all neonates of the two groups increased significantly ( P<0. 01 ), but returned to baseline within 20-30 minutes after FB. Conclusion FB can cause significant hypoxemia and hypercapnia in neonates. PSV through fiberoptic bronchoscope can be considered a safe and beneficial ventilation technique for maintaining oxygenation during FB.

3.Effect of Early Intervention on Expression of NB-3 in Neonatal Rats with Cerebral Injury Induced with Intrauterine Infection

Objective To investigate the effect of early intervention on adhesion molecule NB-3 expressing in cerebral injured neonatal rat induced with intrauterine infection. Methods Pregnant rats were consecutively injected with lipopolysaccharide (LPS) 380 μg/kg or saline 17 d after gestation. The offspring from the LPS group accepted early intervention (I group, n=40) or not (NI group, n=40), and those for saline group were as control group (n=40). They were assessed with suspension test 14 d and 28 d after born, and the expression of were NB-3 were detected with immunohistochemistry 1 d, 7 d, 14 d, 21 d, 28 d after born. Results The expression of NB-3 was different among these 3 groups, as well as the scores of suspension test (P<0.001). Conclusion Intrauterine infection increases the expression of NB-3 of neonatal rats with brain damage. The early intervention may increase persistently the expression of NB-3, and improve the motor function.
4.Aminoglycosides Modifying Enzyme Genes and intⅠ gene in Stenotrophomonas maltoplilia

OBJECTIVE To study the aminoglycosides modifying enzyme genes and intⅠ gene in Stenotrophomonas maltophilia in Chinese Armed Police Forces General Hospital.METHODS The samples of 27 multi-resistant S.maltophilia were collected from inpatiens from Jan 2006 to Oct 2007 in this Hospital.The sensitivity of the isolates to 14 antibacterial agents was determined using a broth induction method.The aminoglycosides modifying enzyme genes and intⅠ 1 gene were detected by PCR.RESULTS The multi-drug resistance of S.maltophilia was a serious problem.In 27 strains of S.maltophilia,the positive ant(2″)-Ⅰ were in 5 strains(18.5%),aac(3)-Ⅱ in 3 strains(11.1%)and aac(6')-Ⅱ in 1 strain(3.7%).The positive intⅠ gene was found in 11 strains(29.6%).CONCLUSIONS Multi-resistant S.maltophilia resistant to aminoglycosides mainly due to the presence of aminoglycoside modifying enzymes ant(2″)-Ⅰ,aac(3)-Ⅱ and aac(6')-Ⅱ.The aminoglycoside modifying enzymes ant(3″)-Ⅰ and aac(6)-ⅠZ were not detected carrying IntⅠ would be the reason of S.maltophilia resistant to aminoglycosides.
5.The Study of Relationship Between the CT Features and Blood GlucoseLevel in Cerebral Infarction of Diabetes Mellitus

Objective To study the CT features of cerebral infarction and the relation to blood glucose level in diabetes mellitus.Methods The CT features and relation to the level of blood glucose were retrosepectively analyzed in 41 cases of cerebral infarction diabetes mellitus.The diagnosis of diabetes mellitus was based on the WHO criteria in all cases.Results The focus of infarction located at the area of base ganglion,brain stem and thalamencephalon in 38 cases(92.7%).Multiple focus was found in 30 cases(73.2%)and there was positive correlation between focus number and the level of blood glucose.Lacuna cerebral infarction was observed in 38 cases(92.7%) and large patchy cerebral infarction in 3 cases.Brain white matter demyelination was complicated in 24 caese(58.5%).Conclusion Cerebral infarction of diabetes mellitus mainly manifestes as multiple lacuna cerebral infarction and has close relation to the level of blood glucose.
6.Determination of Selenium Species in Burdock and Panax Notoginseng Using Ultrasonic Assistant Extraction Combined with High Performance Liquid Chromatography-Inductively Coupled Plasma Mass Spectrometry

A high performance liquid chromatography-inductively coupled plasma mass spectrometric ( HPLC-ICP-MS) method for analysis of selenocystine (SeCys2), selenite (SeⅣ), selenomethionine (SeMet) and selenate ( SeⅥ) was described. Chromatographic separation was performed using 8 mmol/L citric acid solution (pH 5). The potentially interfering 40Ar40Ar+ at selenium masses of m/z 80 was reduced using CH4 as a reactive cell gas in the dynamic reaction cell. The selenium species was extracted using ultrasonic extraction system with a mixture of protease K and lipase. The limits of detection for SeCys2 , SeⅣ, SeMet, and SeⅥwere observed as 0. 088, 0. 033, 0. 30, and 0. 17 ng/mL, respectively. The correlation coefficients were greater than 0 . 9995 . The recoveries were observed in the range of 95 . 1%-114 . 6%. The developed method was successfully applied for the determination of selenium species in Burdock and Panax notoginseng.
7.Therapeutic Observation of Comprehensive Muscle-region Therapy in Chuang Medicine for Cervical Spondylosis of Neck Type

Objective To observe the clinical efficacy of comprehensive muscle-region therapy based on Chuang Medicine in treating cervical spondylosis of neck type.Method A hundred patients with cervical spondylosis of neck type were randomized into a treatment group and a control group, 50 cases in each group. The treatment group was intervened by the comprehensive muscle-region therapy based on Chuang medicine (muscle-region tuina, fire-needle acupuncture of Chuang medicine, and cupping), while the control group was by regular acupuncture treatment. The Visual Analogue Scale (VAS) score and symptoms and signs scores were observed before and after intervention, and the clinical efficacies were compared.Result The VAS scores respectively after 1 and 2 treatment courses were significantly different from that before intervention in the treatment group (P<0.05,P<0.01). The VAS score after 2 treatment courses was significantly different from that before intervention in the control group (P<0.05). After 2 treatment courses, the VAS score in the treatment group was significantly different from that in the control group (P<0.05). After treatment, the symptoms and signs scores (pain intensity, pain-affected area, number of tender points, limited motion, and general score) were significantly different from that before intervention in the treatment group (P<0.01, P<0.05). In the control group, the pain intensity, limited motion and general score after treatment were significantly different from that before intervention (P<0.05,P<0.01). After treatment, the symptoms and signs scores in the treatment group were significantly different from that in the control group (P<0.05,P<0.01). The total effective rate was 100.0% in the treatment group versus 84.0% in the control group, and the difference was statistically significant (P<0.05).Conclusion Comprehensive muscle- region therapy in Chuang medicine is an effective approach in treating cervical spondylosis of neck type.
8.Effects of nursing intervention on the beliefs and behavior of early detection of breast cancer among Shanghai women

Objective To evaluate the effects of nursing intervention on the beliefs and behavior of early detection of breast cancer. Methods Four communities in Shanghai were selected by convenient sampling process. Then these four communities were randomly assigned to intervention group and control group. One hundred and eighty women who met the criteria of this study were selected from each group by convenient sampling process.Altogether 739 women were recruited. In intervention group,a one-year nursing intervention guided by health belief model was implemented,such as health education seminars,telephone follow in the third,sixth,and ninth months,and flyers of breast health care in the sixth month. Control group only received conventional education. Results After intervention women became aware of breast cancer (B=0.210,P<0.01),perceived more benefits from attending early breast cancer detection (B=0.105,P<0.01),and less difficulties in attending the activities of early detection of breast cancer (B=-0.086,P<0.05). The number of women who performed breast self-examination once per month increased and more women received clinical breast examination (OR=3.0946) and mammogram after intervention (OR=2.746). Conclusion The nursing intervention,guided by health belief model,is helpful to improve women's beliefs and behaviors of breast cancer detection.
9.Clinical Observation of Medicinal Thread Moxibustion plus Chinese Medicinal Washing for Acute Eczema

ObjectiveTo observe the efficacy and safety of medicinal thread moxibustion plus Chinese medicinal washing in treating acute eczema.MethodEighty patients with acute eczema were randomized into a treatment group and a control group, 40 in each group. The control group was externally treated with 3% boric acid solution, while the treatment group was by medicinal thread moxibustion at acupoints from Zhuang Medicine, including Changzi point, Kuihua point, and Jumei point, plus Quchi (LI11), Shousanli (LI10), and Xuehai (SP10), in association with Chinese medicinal washing. The clinical efficacies were then analyzed. ResultAfter 2 treatment courses, the total effective rate was 100.0% in the treatment group versus 77.5% in the control group, and the difference was statistically significant (P<0.05), indicating that the total effective rate of the treatment group washigher than that of the control group; besides, there was no adverse events happened in the treatment group during the whole treatment. ConclusionMedicinal thread moxibustion plus Chinese medicinal washing is efficient and safe in treating eczema.
10.CT Features and Misdiagnosis of Lung Cancer in Young Patients

Objective To describe CT features and misdiagnosis of lung cancer in young patients.Methods The clinical data and CT findings of primary bronchogenic carcinoma in 33 young patients proved pathologically were reviewed.Results Masses,nodules andpulmonary consolidation were found in 13,8 and 7 cases on chest CT respectively.The soft nodule in bronchus with obstructive pulmonary atelectasis was found in 3 cases,the thickening of bronchial wall and bronchiarctia with obstructive pulmonary infection,plumonary cavity and multicenter lesion were found in 1 respectively.There were thoracic effusion or with nodulous pleurae in 8 cases,nodules in lungs in 7 cases,enlarged lymph nodes in 6 cases,destruction of thoracic skeleton in 2 cases and mass involving mediastinum or large vessels in 4 cases.The chest metastatic rate was 81.8%.Pulmonary inflammation and tuberculosis were firstly diagnosed in 6 and 15 cases,the misdiagnostic rate was 63.6%.Conclusion Masses,nodules and pulmonary consolidation are the common CT features of lung carcinoma in young patients.Most cases are advanced stage of lung carcinoma and misdiagnosed as pulmonary tuberculosis in initial examination.
