1.Antibiotic Resistance and Nosocomial Infections Status of Staphylococcus aureus

OBJECTIVE To investigate the antibiotic resistance and nosocomial infections status of Staphylococcus aureus (SAU) and provide scientific evidence for diseasis diagnose and reasonable use of antibiotics. METHODS A retrospective review was performed to analyze the clinical distribution of 321 isolates of SAU. Antibiotic susceptibility test was performed by K-B method and drug-resistance results were analyzed. RESULTS SAU was found been resistant to the diverse antibiotics in different degree. However, the drug sensitivity rate to glycopeptide antibiotics was 100.0%, as well as to new type antibiotic of linezolid. The isolating rate of meticillin-resistant S. aureus (MRSA) reached 57.6%. CONCLUSIONS S. aureus shows multidrug resistance to the commonly used antibiotics. We should strengthen monitoring and controlling of it.
2.Differential Proteins of Liver Revealed with a Combined Technology of both Differential Centrifugation and Proteomics in Paralichthys Olivaceus Under Stress of Cadmium Salt

Both total proteome and differential proteins were effectively extracted, separated and selected by a combined approach of both differential centrifugation and 2D-PAGE in liver of Paralichtys olivaceus( POL) under the stress of cadmium chloride as an artificial pollution source. Approximately 800 spots for extraction of whole protein separated with 2D-PAGE were obtained by direct lysis in the POL. In addition, approximately 11 differential proteins in POL were also obtained under the stress of cadmium chloride. The differential centrifugal were used to prepare three sedimentation and a plasmolysis proteins, called POL component Ⅰ, POL component Ⅱ, POL component Ⅲ, and POL component Ⅳ(plasmolysis protein), respectively. Total protein spots for each gel were calculated to have 380, 550, 500, and 850, respectively, approximately 2280 spots in sum, while total spots are much higher than those by direct lysis approach. Using the comparison method, approximately 54 differential proteins in POL were obtained by a combined technology of both differential and two dimensional polyacylaminde gel electrophoresis(2D-PAGE) methods under the stress of cadmium chloride. In addition, these differential proteins can be further identified by peptide mass fingerprint(PMF). Here, these combined techniques can be effectively used to extract, separate and identify the whole proteins and the differential proteins including protein markers in the biological tissue.
3.Distribution and Drug-resistance of Pseudomonas aeruginosa in Nosocomial Infections

OBJECTIVE To investigate the distribution and drug-resistance of Pseudomonas aeruginosa(PAE) in nosocomial infections to provide the scientific evidence for the clinical use of the antibiotics.METHODS PAE was cultured and identified according to the National Rules of Procedures Clinical Laboratory.Drug resistance was analyzed by K-B methods.RESULTS A total of 318 strains of PAE were mainly isolated from intensive care unit(ICU),departments of cerebral surgery and chest surgery.Drug-resistance of PAE to 14 kinds of antibiotics showed that SXT(91.2%),cefotaxime(89.9%),aztreonam(70.1%),gentamicin and ciprofloxacin(69.8%),respectively.And the resistance to piperacillin,levofloxacin and cefepime was 50.0%.CONCLUSIONS Drug-resistance status of P.aeruginosa is very serious.Hospital should strengthen monitoring and controlling of it.
4.Clinical value of systemic vascular resistance index combined with procalcitonin in the early diagnosis of sepsis

Objective To assess the clinical value of systemic vascular resistance index (SVRI) combined with serum procalcitonin (PCT) and sequential organ failure assessment (SOFA) score in the early diagnosis of sepsis. Methods A retrospective study was conducted. The data of critical patients admitted to Department of Critical Care Medicine of the Third People's Hospital of Hechi from November 2013 to April 2016 were enrolled. The clinical data were recorded as follows: gender, age, infection site, SOFA score, serum PCT level (enzyme linked fluorescence analysis) within 1 hour after intensive care unit (ICU) admission, hemodynamics parameters, including mean arterial pressure (MAP), central venous pressure (CVP), cardiac index (CI), SVRI, global end diastolic volume index (GEDVI), extravascular lung water index (EVLWI), which were monitored by pulse indicator continuous cardiac output (PiCCO) after ICU admission. The patients were divided into sepsis and non-sepsis groups according to the diagnostic criteria of sepsis. Septic patients were divided into low SVRI group, normal SVRI group, and high SVRI group according to SVRI normal value (170-240 kPa·s·L-1·m-2), and the differences in parameters among the three groups were compared. The correlations between SVRI and various parameters were analyzed by using Pearson correlation analysis. The receiver operating characteristic curve (ROC) was plotted to evaluate the diagnostic efficiency of each parameter. Results Totally 103 critical patients were enrolled, 55 in sepsis group, and 48 in non-sepsis group. Compared with non-sepsis group, SVRI in septic group was significantly lowered (kPa·s·L-1·m-2: 146.56±45.17 vs. 188.04±56.27), and serum PCT was significantly increased (μg/L: 10.43±6.17比0.32±0.11) with statistically significant differences (both P < 0.05). In 55 sepsis patients, there were 21 in low SVRI group, 19 in normal SVRI group, and 15 in high SVRI group. There were no statistically significant differences in gender, age and infection site among the three groups, indicating that the baseline data among all groups was balanced with comparability. SOFA score, PCT, and CI in the low SVRI group were significantly higher than those of normal SVRI and high SVRI groups [SOFA: 10.57±2.89 vs. 5.73±2.28, 5.73±2.15, PCT (μg/L): 24.15±12.43 vs. 7.18±5.05, 7.39±4.38, CI (mL·s-1·m-2): 71.01±9.67 vs. 62.01±8.34, 62.51±8.67, all P < 0.05], but no significant difference was found between the normal SVRI group and high SVRI group. There was no statistically significant difference in MAP, CVP, EVLWI, and GEDVI among the three groups. It was shown by Pearson correlation analysis that SVRI was negatively correlated with PCT, SOFA score, and CI (r value was -0.622, -0.598, -0.398, all P = 0.000). It was shown by ROC curve that area under ROC curve (AUC) of PCT combined with SVRI for diagnosis of sepsis was higher than that of PCT or SVRI alone (0.943 vs. 0.911, 0.884). When the cut-off value of PCT was 3.79 μg/L, and cut-off value of SVRI was 156.81 kPa·s·L-1·m-2, the sensitivity and specificity were 94.6% and 92.3% respectively. Conclusions For sepsis patients, SVRI is related to PCT and SOFA score. Combined monitoring of PCT, SVRI, SOFA score can accurately reflect the severity of sepsis patients, guide diagnosis and treatment, and estimate prognosis. The efficacy of PCT combined with SVRI in the early diagnosis of sepsis is better than that of the two alone.
5.Influence of tongxinluo on blood endothelial microparticles and MMP-9 in patients with acute myocardial infarction after percutaneous coronary intervention

Objective To study influence of tongxinluo on blood endothelial microparticles(EM Ps) and matrix metalloprotei‐nase‐9(MMP‐9) in patients with acute myocardial infarction after percutaneous coronary intervention(PCI) .Methods One hundred and twenty‐eight hospitalized patients with acute myocardial infarction after per‐cutaneous coronary intervention were recruited from January 2012 to December 2014 ,All patients were randomly divided into tongxinluo group (n=65) and control group (con‐ventional treatment ,n=63) .Tongxinluo group was on the basis of conventional treatment group with tongxinluo capsules 2 plus ,3 times a day .We detected the EMPs and MMP‐9 of two groups preoperatively and on the 7th postoperatively day .Results Com‐pared with the conventional treatment group ,blood EMPs and MMP‐9 in tongxinluo group were lower after 7 days treatment ,the difference was statistically significant (P<0 .05) .There was a positive correlation between the EMPs and MMP‐9(P<0 .05) .Con‐clusion For patients with acute myocardial infarction after percu‐taneous coronary intervention ,tongxinluo could further inhibit in‐flammatory reaction ,make the plaque stability and improve the function of endothelial cells on the basis of conventional treatment groups .
6.Comparison of the sedative and anti-anxiety effects of levo-tetrahydropalmatine and diazepam on rats with conditioned fear

Objective:To compare the sedative and anti-anxiety effects of levo-tetrahydropalmatine (L-THP) and diazepam on conditioned fear model rats.Methods:According to the random number table method, 32 adult male rats were divided into blank group, model group, diazepam group and L-THP group(with 8 rats in each group). The conditioned fear model was reproduced by the plantar electric shock method. Four days after the modeling, the rats in diazepam group and L-THP group were given diazepam (3.6 mg/kg) and L-THP (25 mg/kg) were respectively gavaged once a day for 10 days, the rats in blank group and model group were given the same volume of saline. After the administration, the elevated plus maze test and the open field test were used to measure the anxiety behavior of the rats, and the sleep energy monitoring system was used to detect changes in sleep and energy-related indicators. SPSS 23.0 and Graphpad Prism 7.0 softwares were used for data analysis, multiple samples between groups were compared by one-way ANOVA, and LSD test was used for pairwise comparison.Results:The results of the elevated plus maze experiment showed that compared with the model group, the percentage of open-arm entry times ((11.27±8.78)%, (30.11±14.59)%, P<0.05) and the percentage of open-arm residence time ((1.94±1.48)%, (17.53±8.21)%, P<0.05) in diazepam group were all significantly increased. Compared with the model group, the open arm residence time, the percentage of open arm residence time and the percentage of open arm entry times in L-THP group showed an upward trend, but there was no statistical significance (all P>0.05). The results of the open field experiment showed that compared with the model group, the time of entering the central grid ((2.99±1.83) s, (6.94±3.52) s, P<0.05) and the time of entering the peripheral field ((297.01±1.83) s, (293.30±3.52) s, P<0.05) in diazepam group both increased. Compared with the model group, there was no significant difference in the changes of various indexes in L-THP group (all P>0.05). The results of locomotor activities showed that the autonomic activity times of model group in nighttime was significantly lower than that of blank group((758.79±375.37)times/h, (1 101.93±525.96)times/h, P<0.05). Compared with the model group, the number of autonomous activities of rats in L-THP group in daytime ((820.57±364.60) times/min, (502.40±228.54)times/min, P<0.05) decreased, and the number of autonomous activities in the nighttime ((758.79±375.37) times/min, (1 146.85±309.69)times/min, P<0.05) increased, but there was no significant change in the number of autonomous activities in the whole day. Correlation analysis of energy metabolism related indexes and sleep time of rats in each group were analyzed. The experimental results showed that the daytime sleep time were negatively correlated with heat value ( r=-0.335, P<0.05), and the night sleep time was positively correlated with daytime heat value ( r=0.352, P<0.05). Conclusion:L-tetrahydropalmatine has no significant anti-anxiety effect in the concentration range used in this study, but its sedative and improving sleep activity rhythm are better than diazepam.
7.Application of Multi-element Fingerprints in the Study of Origin Traceability of Anemarrhenae Rhizoma

Objective:To study the origin traceability of anemarrhenae rhizoma from Bozhou and Hebei based on multi-element fingerprints technology , and establish a discrimination model .Methods:The contents of 48 elements were determined by using induc-tively coupled plasma mass spectrometry ( ICP-MS) for 44 samples of anemarrhenae rhizome from Bozhou and Hebei province .Princi-pal component analysis ( PCA) and orthogonal partial least squares discrimination analysis ( OPLS-DA) were applied in the data analy-sis to screen out the significant elements .And then Fisher linear discrimination analysis was used to determine the origin of anemarrhe-nae rhizoma and the discrimination models were developed .Results:Two discrimination models were developed by the discrimination a-nalysis of the whole model method with nine significant elements identified by PCA and OPLS -DA, and 100%correct classification and 95.5%cross validation were achieved by the models .Conclusion: It is a promising approach to classify the geographical origin of anemarrhenae rhizome based on multi-element fingerprints analysis combined with multivariate statistical analysis .The discrimination models are good enough to be applied in the origin traceability of anemarrhenae rhizome.
8.Oral multidisciplinary considerations for clinical strategies of endodontic microsurgery

Endodontic microsurgery is a vital treatment modality for teeth with persistent periradicular pathoses that have not responded to nonsurgical retreatment. The principle is to determine the reason for failure, completely eliminate the infection and promote periapical healing. Within recent years, endodontic microsurgery has evolved to become standardized and presents with a high success rate. However, its outcome is still influenced by many factors, including anatomy, periodontal condition, crown-to-root ratio, occlusion, the type of periradicular lesion, and prosthesis. Moreover, endodontists always concentrate on “the apex”, paying little attention to the general preoperative evaluation, accurate diagnosis, and comprehensive treatment plan. This article reviews the latest literature on these issues and the clinical experience of our research group and discusses the correlation between endodontic microsurgery and other oral disciplines, including periodontology, prosthodontics, oral implantology, oral and maxillofacial surgery and orthodontics. The oral interdisciplinary assessment should be made with comprehensive consideration of the root canal system, periradicular lesion, adjacent anatomical relationships, periodontal condition, occlusion, and esthetic rehabilitation. Based on these findings, the continuity of treatment will be optimized, and the best treatment plan will be proposed to provide clinical strategies for the diagnosis and treatment of complex periradicular diseases.
9.Relationship between eating out and overweight, obesity, and fatty liver disease in adult residents in a suburban area of Shanghai

ObjectiveTo investigate the prevalence of overweight, obesity and fatty liver disease (FLD) in adult residents in a suburban area of Shanghai, and to explore the relationship between these conditions and eating out of home (EOH). MethodsAdult residents from Zhongshan Street, Songjiang, Shanghai, were invited to report their frequency of EOH and undergo a physical examination using B-ultrasonography from May to September 2017. Binary logistic regression analyses were conducted to examine the relationship between EOH and conditions of overweight, obesity, and FLD. ResultsAmong the final sample size of 6 608 participants, the prevalence rates of overweight, obesity, and FLD were 41.65%, 14.71% and 38.29%, respectively. After adjusting for potential confounders, logistic analyses revealed that participants with an EOH frequency > once/week had a 1.44-fold higher prevalence of obesity (P=0.006), a 1.35-fold higher prevalence of overweight/obesity (P=0.005), and a 1.36-fold higher prevalence of FLD (P=0.008). ConclusionThe prevalence of overweight, obesity, and FLD is relatively high in Songjiang, Shanghai. EOH may be a risk factor for these diseases, and residents are recommended to reduce the frequency of EOH.
