1.Clinical Evaluation of Netilmicin Sulfate in Treating Lower Respiratory Tract Infections

Objective: To observe the effectiveness of neltimicin sulfate in treating the infections of lower respiratory tract. Methods: 41 cases of lower respiratory tract infections served as observation subjects (treated with netilmicin sulfate 200 mg*d-1, iv drip) while the other 30 cases as control subjects (treated with penicillin 8×109 U*d-1 and ampicillin 8.0 g*d-1, iv drip). Results: The therapeutic effectiveness was significantly higher in the observation group than that in the control group (P<0.01). The sensitivity tests of isolates from the sputum showed that 71.1% of the isolates were highly sensitive to netilmicin sulfate, 28.9% moderately sensitive to netilmicin sulfate. The total effectiveness rate was 97.6%. Conclusion: Netilmicin sulfate is effective in treating the lower respiratory tract infections and causes less side effects.
2.Nitrates versus calcium antagonists in preventing cerebral infarction induced by hyperbaric oxygen

BACKGROUND: Hyperbaric oxygen may induce cerebral infarction. But what kind of medicine may prevent it and what is the preventive function of the medicine are not clear yet.OBJECTIVE: To compare the preventive function of nitrates with calcium antagonists in cerebral infarction induced by hyperbaric oxygen.DESIGN: Controlled retrospective observation based on patients.SETTING: Department of Hyperbaric Oxygen, Department of Neurology,Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA.PARTICIPANTS: Totally 192 patients receiving hyperbaric oxygen treatment in Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA, who were 127 males and 65 females, and aged 9 -78 years old. Among them, 6 patients developed into cerebral infarction.METHODS: Totally 192 patients receiving hyperbaric oxygen treatment were old, hypertensive disease, hyperlipemia, cerebral infarction(or cerebral hemorrhage), hyperviscosity in blood and diabetes as dangerous factors, prevention function of medicine was analyzed on the level of grouping different dangerous factors.INTERVENTIONS: Under the prerequisite condition of regular treatment medicine given to the 192 patients of the 3 groups, patients in nitrate group took isosorbide dinitrate or nitroglycerin orally, patients in calcium antagonist group took sibelium or Norvasc and so on orally, and patients in group without special medication did not take preventive drugs.MAIN OUTCOME MEASURES: Dangerous index was dangerous factorsto cerebral infarction.RESULTS: No one developed into cerebral infarction with grouping less than 4 dangerous factors. On the level of grouping 4 or more dangerous factors,nitrates had good preventive function, but in calcium antagonist group,33.3% patients developed into cerebral infarction. There was significant difference between them( P < 0.05).CONCLUSION: Nitrates have significant preventive functions, but calcium antagonists have no such functions.
3.Quantitative determination of LP(a)-C by agarose gel electrophoresis and its clinical application

Objective To evaluate an agarose gel electrophoresis for quantitative determination of Lipoprotein(a) cholesterol and its clinical application.Methods Quantification of LP(a)-C was performed by a new agarose gel electrophoresis method that allows the separation of LP(a) by Hellen REP system and the determination of LP(a)-C by enzymatic staining of cholesterol,The results of electrophoresis method were compared with the ones of INA and ITA. The specimens of 135 health men were assayed by electrophoresis for the reference range.Results The inter and intra CV of electrophoresis method at low, middle and high LP(a) concentration specimens were 4.7%~5.3% and 5.8%~6.4%. Objective To investigate the extended-spectrum beta-lactamases ( ESBLs) producing clinical isolates of gram negative bacilli and their antibiotic resistance in Shantou and to provide suggestions on empirical treatment against the bacteria.Methods A total of 1 445 strains of gram negative bacilli (895 strains of Escherichia coli and 550 strains of Klebsiella pneumonia) were collected and examined for the production of ESBLs and antibacterial susceptibility test by Vitek-2.Results There were 69.4% of escherichia coli and 33.6% of klebsiella pneumonia producing ESBLs.The resistance rate of the ESBLs-producing strains to penicillins,cephalosporins and monocyclic beta-lactam antibiotics were very high.The ESBLs-producing strains were multidrug resistant and the resistance rates were higher than that of the non-ESBLs-producing strains.Both ESBLs-producing and non-ESBLs-producing strains were susceptible to Imipenem.Conclusion The incidence of ESBLs-producing strains was high in gram negative bacilli in Shantou.The resistance rates of the ESBLs-producing strains were higher than that of the non-ESBLs-producing strains and they expressed multiple drug resistance phenotypes.Imipenem was the best drug in the treatment of infections caused by ESBLs-producing strains.
5.Role of SIRT1/Nrf2 signaling pathway in sleep deprivation-induced cognitive dysfunction in rats

Objective:To evaluate the role of silent information regulator 1 (SIRT1)/nuclear factor E2 related factor 2 (Nrf2) signaling pathway in sleep deprivation-induced cognitive dysfunction in rats.Methods:Thirty-six adult male Sprague-Dawley rats, aged 54-56 weeks, weighing 600-750 g, were divided into 3 groups ( n=12 each) using a random number table method: control group (group C), sleep deprivation group (group SD) and sleep deprivation+ SIRT1 agonist Srt1720 group (group SD+ Srt). Sleep deprivation model was established by modified multi-platform water environment method.Srt1720 10 mg/kg was injected intraperitoneally every 12 h starting from 24 h before establishing the model in group SD+ Srt, while the equal volume of 0.9% normal saline was injected intraperitoneally in C and SD groups.After the end of sleep deprivation, Morris water maze test was performed to evaluate the cognitive function.Animals were then sacrificed, and their hippocampi were removed for determination of neuronal degeneration rate in hippocampal CA1 region (using HE staining), the apoptosis rate in hippocampal CA1 (using TUNEL assay ), the expression of SIRT1 and Nrf2 in hippocampal CA1 (by immunohistochemistry) and the contents of reactive oxygen species (ROS) and superoxide dismutase (SOD) (by microplate method). Results:Compared with group C, the escape latency was significantly prolonged, the time of staying at the platform quadrant was shortened, and the frequency of crossing the platform was decreased on 2-4 days, the apoptotic rate and neuronal degeneration rate were increased, the expression of SIRT1 and Nrf2 was down-regulated, ROC content was increased, and SOD content was decreased in SD and SD+ Srt groups ( P<0.05). Compared with group SD, the escape latency was significantly shortened, the time of staying at the platform quadrant was prolonged, and the frequency of crossing the platform was increased on 3 and 4 days, the apoptotic rate and neuronal degeneration rate were decreased, the expression of SIRT1 and Nrf2 was up-regulated, ROC content was decreased, and SOD content was increased in group SD+ Srt ( P<0.05). Conclusions:Sleep deprivation can induce oxidative stress response in hippocampus by inhibiting the activation of SIRT1/Nrf2 signaling pathway, leading to cognitive dysfunction in rats.
6.Selection and Identification of the Biological Characteristics of a Cold-adapted Genotype G1P8 ZTR-68 Rotavirus by Serial Cold-adapted Passaging.

We wished to select a cold-adapted genotype G1P[8] ZTR-68 rotavirus (China southwest strain) in MA104 cells for possible use as a live vaccine. ZTR-68 was recovered originally from children with diarrhea. The virus was cultivated at 37 degrees C at the first passage. Then, the cultivation temperature was decreased stepwise by 3 degrees C per eight passages. In total, the virus was passaged 32 times, and cultivation was terminated at 28 degrees C. Biological characteristics of the virus were analyzed during serial passages. There was no difference between the migration patterns of genomic dsRNA segments according to polyacrylamide gel electrophoresis of original and cold-adapted viruses. Infectious and red cell-agglutination titers of cold-adapted virus were lower than those of the parent virus. Also, the virus formed small-size plaques with irregular shapes at 31 degrees C and 28 degrees C. These results suggested that a genetically stable attenuated virus can be obtained through serial cold-adapted passages. Thus, an alternative strategy is provided by cold-adaption for development of attenuated live rotavirus vaccines.

7.131I therapy effect on medium and large goiter with goiter with hyperthyroidism

Objective To evaluate the effect of 131 iodine(131I) therapy in medium and large goiter with hyperthyroidism in order to investigate the influence of pretreatment with antithyroid drugs (ATD) methimazole(MMI) and propylthiouracide (PTU) on 131I therapy.Methods A total of 338 hyperthyroidism patients (136 cases for male,202 cases for female) with thyroid mass greater than 40 grams were treated with 131I in the People's Hospital of Sichuan Province.Thyroid function,thyroid 131I uptake ratio and thyroid imaging were measured before treatment were measured 1-3 months later after administration of 131I,and they were followed up for 6 months to 4 years.Results There were statistical difference between pretreatment and 3 months later of 131I therapy in terms of free triiodothyronine (FT3),(Free thyroxine) FT4.The level of FT3 decreased from (31.9 ± 16.2) pmol/L to (7.8 ±8.5) pmol/L(t =23.9,P =0.000) and level of FT4 decreased from (58.8 ± 22.2) pmol/L to (19.4 ± 16.9) pmol/L(t =25.4,P =0.000).Among 338 patients,109 patients (32.2%) were developed hypothyroidism,and 91 patients (26.9%) were cured,91patients (26.9%) with improved state,20 patients(5.9%) with ineffective and 27 patients(8.0%) with relapse after administration of 131I.The total effective rate was 94.1% (318/338).Thyroid weight,levels of FT3,FT4,thyroglobulin antibody(TGA),thyroid microsomal antibody(MCA) in effective group were (49.8 ± 9.97) g,(32.5 ± 16.3) pmol/L,(59.5 ± 22.2) pmol/L,(43.6 ± 35.3) %,(30.1 ± 22.6) % respectively,and were (56.9±15.7) g,(22.8 ± 12.8) pmol/L,(47.9 ±20.3) pmol/L,(22.8±30.0)%,(15.3 ±20.5)% respectively in ineffective group.There were statistically significant differences between the ineffective group and effective group (t =2.932,2.602,2.287,2.501,2.766 ; P =0.000,0.010,0.023,0.013,0.006).Logistic regression analysis was showed that weight of thyroid and serum FT3 were the most important factors in affecting 1131 therapy.Conclusion 131 I therapy for medium and large-sized goiter with hyperthyroidism is safe and effective.ATD may not reduce the effectiveness of subsequent 131I of hyperthyroidism.Thyroid weight and FT3 are the key influential factors in affecting 131I.
8.Application of Canadian Occupational Performance Measure on Survivors from Sichuan Earthquake

Objective To apply Canadian Occupational Performance Measure (COPM) on the patients injured in Sichuan Earthquake. Methods COPM was applied to evaluate 51 patients in hospital before and 1 months after treatment. The first evaluation confirmed the problems on occupational activities for them. Then the plan was made to solve the problems. The second evaluation was to assess the effect of the treatment.Results The problem of self-care activity is more than that of productive activities and leisure activities(P<0.01). Total scores of performance of occupational activity and satisfaction improved(P<0.01, P<0.01)Conclusion COPM is helpful to confirm the problems of occupational activities and contribute to develope the primary goals for rehabilitation and treatment programs.
9.Impact of Cardiac Resynchronization Therapy on Ventricular Remodeling in Patients With III°Atrio-ventricular Block Combining Systolic Dysfunction

Objective: To observe the impact of cardiac resynchronization therapy (CRT) on ventricular remodeling in patients with III°atrio-ventricular block (AVB) combining systolic dysfunction.
Methods: A total of 49 III °AVB patients received CRT in our hospital from 2009-01 to 2014-10 were studied. Echocardiography was conducted at pre-operation and 6, 12 months post-operation to measure left ventricular ejection fraction (LVEF), left ventricular end-systolic volume (LVESV), left ventricular end-diastolic volume (LVEDV), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) and mitral regurgitation (MR) grade in order to observe the changes of cardiac structure and function in relevant patients.
Results: Compared with pre-operative condition, at 6 months post-operation, LVEF was increased (4.92±5.24)%and at 12 months post-operation, it was further increased (5.02±6.52)%, both P<0.05;at 6 months post-operation, LVESV reduced (25.02±17.95) ml and at 12 months post-operation, it was further reduced (24.79±22.49) ml, both P<0.05. Compared with pre-operative condition, at 6 months post-operation, LVEDV dropped (25.61±24.24) ml, LVEDD dropped (3.22±2.91) mm, LVESD dropped (4.43±2.86) mm and MR grade dropped 0.49±0.76, all P<0.05. Compared with 6 months post-operation, at 12 months post-operation, LVEDV declined (28.18±22.36) ml, LVEDD declined (4.17±3.14) mm, both P<0.05, LVESD declined (4.92±4.40) mm, P<0.01 and MR grade declined (0.22±0.55), P<0.05.
Conclusion:CRT may reverse ventricular remodeling and improve cardiac function in patients with III°AVB combining systolic dysfunction.
10.The evaluation of immune effects on bivalent rotavirus vaccine

Objective To evaluate the immune effects of bivalent inactivated rotavirus vaccine (IRV) and investigate the viability of development of bivalent IRV.Methods Firstly,bivalent IRV was prepared by mixing G1 IRV and G3 IRV with equal amount,G1 IRV and G3 IRV as monovalent control,PBS as negative control.Secondly,those vaccines were vaccinated to the mice by intramuscular injection.Then,to evaluate the immune effects of bivalent IRV,the levels of serum or fecal rotavirus specific IgG and IgA were assessed by ELISA,the levels of serum neutralized antibody were measured by microneutralization assay,the number of IFN-γ or IL-4 secreting cells were analyzed by ELISPOT assay.Results Compared to negative control group,bivalent IRV induced the higher levels of serum and fecal G1 and G3 rotavirus specific antibody.It was found that there were no significant differences for the levels of serum IgG and IgA,fecal IgG and IgA,serum neutralized antibody between induced by bivalent IRV and induced by G1 type monovalent vaccines ; but there were significantly increase for the levels of serum IgG (t =2.691,P<0.05) and serum neutralized antibody (t =2.561,P<0.05) between induced by bivalent IRV and induced by G3 monovalent vaccines,there were no significant differences for other antibodies between induced by bivalent IRV and induced by G3 monovalent vaccines.At the same time,compared to negative control group,bivalent IRV induced significantly increase in the number of IFN-γ or IL-4 secreting cells in spleen lymphocytes.It was found that there were no significant differences for the number of IFN-γ or IL-4 secreting cells stimulated by G1 rotavirus between bivalent IRV and G1 monovalent vaccines; but there were significantly increase for the number of IL-4 secreting cells (t =2.327,P<0.05) stimulated by G3 rotavirus between bivalent IRV and G3 monovalent vaccines,there were no significant differences for the number of IFN-γ secreting cells stimulated by G3 rotavirus between bivalent IRV and G3 type monovalent vaccines.Conclusion The bivalent IRV can induce effective immune response,in which there were no inhibitory interference between the components of bivalent IRV,which provided the experimental basis for the development of bivalent IRV.
