1.Effect of High Intensity Focused Ultrasound on Immunity of Patients with Advanced Primary Liver Cancer

Objective To investigate the effect of high intensity focused ultrasound(HIFU) on the immunity of patients with advanced primary liver cancer(PLC).Methods Forty cases of PLC admitted to our institution from Mar.2003 to Dec.2003 were included in this study.Patients were divided into 2 groups and received either HIFU or radio-frequency ablation(RFA) treatment randomly.CD3,CD4,CD8,CD4/CD8,NK,IL-2,TNF were chosen to assess the immune status before and after treatment.The results were compared statistically.(Results The) survival rate after HIFU was 80.0%,61.1%,42.9%,33.3% at 3 months,6 months,9 months and 1 year respectively,which was similar to that after RFA treatment.The changes of immunity parameters of CD3,CD4,CD8,CD4/CD8,NK,IL-2 and TNF were not significant after HIFU treatment.In addition,the differences of those parameters between HIFU group and RFA group were insignificant.Conclusion There are no detrimental effects on immunity in the early period after HIFU treatment.
2.Endothelialization of Gore-Tex vascular graft by using cryopreserved human umbilical endothelial cells

Objective The feasibility of constructing endothelialized vascular graft by using cryopreserved HUVCs was studies. Methods Forty-two human umbilical cords were used in this study. HUVECs were isolated by means of filling umbilical veins with digestive enzyme solution. HUVECs were then cultured and observed. Endothelial cells were identified by von Willebrand factor immunofluorescence staining and scanning electron microscope examination. Endothelial cells were suspended in cryopreserving solution which contains 10% DMSO and 10% fetal bovine serum in M199 and cryopreserved in liquid nitrogen. Post-thawed cells and non-frozen cells proliferation was evaluated by measuring the metabolic activity of tetrazolium compound. The endothelial cell growth characteristics were determined by daily observation using phase contrast microscope. Post-thawed endothelial cells viability was determined by trypan blue staining test. Flow cytometry were applied to determine the apoptosis rate of post-thawed cells. Cryopreserved endothelial cells morphological examinations such as hematoxylin and eosins staining and scanning electron microscope examination were carried out in this study. After cell culture and amplification, cryopreserved HUVCs were seeded on the inner surface of Gore-Tex graft to construct tissue engineered vascular graft. Results Extreme high-purified endothelial cells could be isolated by infusing digestive solutions to the lumen of human umbilical veins. Compared with non-frozen endothelial cells, Post-thawed endothelial cells showed 95% of vitality. Post-thawed HUVEC growth curve was similar to non-frozen ones'. Post-thawed HUVEC apoptosis rate (5.85? 0.56) % was higher than non-frozen ones (5.34?0.49)%; however, the difference was not statistically different. Endothelialization of vascular graft was carried out successfully. Cryopreserved cells on Gore-Tex surface showed a good growth trend. Conclusion Cryopreserved HUVCs may be taken as a cell choice for tissue engineering. Enough high pure endothelial cells could be isolated by digestive solutions infusion of human umbilical veins. Post-thawed endothelial cells are proved to have high vitality and growth potential on Gore-Tex surface in vitro.
3.Comparison on rehabilitation effect and safety of laparoscopic herniorrhaphy through different approaches in the treatment of inguinal hernia

Objective:To explore the rehabilitation effect and safety of laparoscopic herniorrhaphy through different approaches in the treatment of inguinal hernia.Methods:The clinical data of 132 patients with inguinal hernia in Shushan Branch of Hefei First People′s Hospital from January 2018 to May 2020 were retrospectively analyzed. Laparoscopic herniorrhaphy was performed in all patients. The 46 patients who were treated with transabdominal preperitoneal patch (TAPP) repair were included in TAPP group, while another 86 patients who were treated with totally extraperitoneal patch implantation (TEP) were included in TEP group. The surgery-related indexes, influencing indexes of pneumoperitoneum on pathophysiology, inflammation indexes, pain score and postoperative complications were compared between 2 groups.Results:There were no significant differences in intraoperative blood loss, recovery time of gastrointestinal function, hospitalization time and non-surgery cost between 2 groups ( P>0.05). The operation time, leaving bed time and surgery cost in TEP group were significantly lower than those in TAPP group: (45.08 ± 4.92) min vs. (51.03 ± 5.62) min, (20.93 ± 2.64) h vs. (22.98 ± 2.01) h and (6 887 ± 603) yuan vs. (8 276 ± 813) yuan, and there were statistical differences ( P<0.01). Before pneumoperitoneum, at 10 min after pneumoperitoneum and after extubation, there were no significant differences in levels of total carbon dioxide (TCO 2), partial pressure of carbon dioxide (PaCO 2), partial pressure of oxygen (PaO 2) and bicarbonate radical between 2 groups ( P>0.05). Before and after surgery, there were no significant differences in interleukin (IL)-6, IL-10 and C-reactive protein (CRP) between 2 groups ( P>0.05). At 1, 3 and 7 d after surgery, there were no significant differences in visual analogue scale (VAS) between 2 groups ( P>0.05). There was no significant difference in the total incidence of complications between 2 groups ( P>0.05). Conclusions:Both TAPP and TEP are safe and effective in the treatment of inguinal hernia. The surgical cost of TEP is relatively lower, and operation time and leaving bed time are shorter. The choices of clinical surgical methods should base on surgeons′ experience and individualized features of patients.
4.Comparative analysis of immediate and selective interlocked intramedullary nailing for treatment of open tibial shaft fractures

Objective To compare the therapeutic effect and complication of immediate and selective interlocked intramedullary nailing in treatment of open tibial shaft fractures. Methods A total of 123 open tibial shaft fractures of 102 cases were treated with immediate fixation with interlocked intramedullary nailing after debridement (Group A) and with selective fixation with interlocking intramedullary nailing after debridement and closing the wound (Group B). Results The follow up for more than half a year showed that all fractures were healed, with excellent joint function of knee and ankle. Of all, 12 cases with skin necrosis and defect but without deep infection were healed after pedicle flap reconstruction. The result of Group A was much better than that of Group B in regard of total operation time, hospital stay, cost, amount of antibiotics used and partial weight bearing. No statistical difference in therapeutic effects and complications was found between two group. Conclusion Infection is not the contra-indication of interlocked intramedullary nailing in open tibial fractures. Immediate operation can be done after thorough debridement.
5.FRACTIONATION OF POLYSACCHARIDBS FROM CURCUMA ZEDOARIA ROSC AND ITS BIOACTIVITIES

Objective: To extract the polysaccharides of Curcuma zedoaria Rosc (PCZ) and study its bioactivities. Methods: The crude polysaccharides were extracted from herbal slice of Curcuma zedoaria Rosc with hot water ,and precipitated by ethanol and savage deproteinization and depigmentation with activated charcoal. The constituents were isolated by column chromatography and the total polysaccharides content was determined. Experiments of immune function,anti-oxidation and anti-tumor activity of KM mice and in viro were studied. Results: The content of total polysaccharides of Curcuma zedoaria Rosc was 33.12%. and the extraction rate was 1.938% . It could raise spleen cell proliferation and transformation rate of lymphocyte and SOD activity in blood of mice. A higher antitumor effect of the polysaccharides by injection was observed dose-dependently. Conclusion: PCZ could raise the immune and anti-oxidative activities and then had a higher antitumor effect.
6.The clinical efficacy of salvianolate on patients with chronic heart failure and plasma brain natriuretic peptide levels

Objective To investigate the effect of salvianolate on chronic heart failure in patients with cardiac function and plasma brain natriuretic peptide effect.Methods Sixty-eight cases with chronic heart failure patients were randomly divided into treatment group and control group (34 cases for each group).Patients in control group were given the conventional treatment,in treatment groups were given conventional treatment plan plus salvianolic acid at dose of 0.2 g added 5% glucose injection 250 ml (or 0.9% sodium chloride injection 250 ml),1 times a day for 12 weeks.The cardiac function was recorded and brain natriuretic peptide level was measured before and after treatment.Results After 12 weeks of treatment,the total efficiency in treatment group was 91.2% (31/34)) higher than that in control group(70.6% (24/34)),and the difference was statistically significant (x2 =9.399,P < 0.01).Before treatment,the left ventricular ejection fraction (LVEF),stroke volume(SV),cardiac output(CO) in treatment group were (38 ±6)%,(44.64 ± 11.03) ml,(4.81 ± 1.03) L/min respectively,differed from that after treatment ((51 ± 8) %,(63.21 ± 11.94) ml,(5.67 ± 1.17) L/min),and there were significant differences between before and after treatment (t =-7.580,-8.975,-3.233 respectively; P < 0.01).The levels of systolic blood pressure,diastolic blood pressure,heart rate,left ventricular end-diastolic internal diameter (Dd),the left ventricular diastolic wall thickness (PWT),diastolic interventricular septal thickness (IVST),left ventricular mass (LVMW),brain natriuretic peptide in treatment group before treatment were (131 ± 11) mmHg,(85 ± 7) mmHg,(116 ± 9) times/min,(55.1 ± 7.9) mm,(11.8 ± 2.4) mm,(11.4 ± 2.3) mm,(231 ± 112) g,(572.9 ± 183.6) ng/L respectively,significant differed from those of after treatment((104 ± 7) nmHg,(76 ± 8) mmHg,(75 ± 7) times/min,(48.8 ± 3.9) mm,(9.2±1.3) mm,(8.9± 1.1) mm) (172 ±57) g,(101.8 ± 18.5) ng/L respectively),and the differences were significant (t =12.075,4.937,20.961,4.169,5.556,5.721,2.738,14.886 ; P < 0.01).The levels of LVEF,SV in control group before treatment were (37 ±7)% and (44.87 ± 10.82) ml,differed from those of after treatment((42 ± 9)% and (56.70 ± 10.60) ml;t =-2.556,-4.554;P < 0.01).The systolic blood pressure,heart rate,Dd,IVST,plasma brain natriuretic peptide in control group before treatment were (130 ±12) mmHg,(114 ± 10) times/min,(54.8 ± 8.7) rmm,(11.3 ± 2.6) mm,(574.1 ± 181.4) ng/L respectively,significantly differed from those of after treatment ((115 ± 9) mmHg,(76 ± 8) times/min,(50.6 ±8.3) mm)(9.9±1.3) mm,(215.7 ±23.2) ng/L;t=5.830,17.304,2.037,2.806,11.427;P<0.01 or P < 0.05).The levels of systolic blood pressure,diastolic blood pressure,LVEF,SV,CO,PWT,IVST,plasma brain natriuretic peptide in treatment group were better than that in control group (t =-4.601,-3.093,4.358,3.253,2.802,-3.066,-3.425,-27.985,P<0.01).Conclusion Salvianolate is proved to be better drug on treating chronic heart failure curative with left ventricular reverse effect and less adverse reaction.
7.Proliferation of human adipose-derived stem cells with expended polytetrafluoroethylenee scaffold inplanted in subcutaneous tissue of rats

Objective To observe the growth of human adipose stem cells (hADSCs) co-culture with expanded polytetrafluoroethylenee (ePTFE) scaffold in vivo.Methods hADSCs were isolated from healthy human liposuction fat.The hADSCs were seeded onto the ePTFE scaffold as Group A,while the ePTFE scaffold without hADSCs as Group B,and the two groups were randomly implanted under the back skin of 10 SD rats.The transplant compound in two groups was harvested at 4 weeks after implantation.Wet weight of transplanted compound was measured.After HE and immunofluorescence CD31 staining,blood vessel density and fibrous proliferation were observed and evaluated.Results Primary cultured hADSCs were spindle-shaped cells.The histological study found that after cultured in vivo,hADSCs could grow in the space of the ePTFE scaffold,several cells were found at the edge of the scaffolds and parts of cells had grown into the inside even the whole layer of the scaffolds.The wet weight of transplanted compound in group A (511.62±32.82) mg was significantly higher than that in group B (363.56±34.74) mg,(P<0.05).Histological and immunofluorescence analysis showed the blood vessel density was (29.50±2.63)/HPL in group A,(12.00±2.26)/HPL in group B.Compared with group B,group A had significantly higher blood vessel density and lower fibrosis counts (P<0.05).Conclusions hADSCs have a good biocompatibility with ePTFE scaffold.ePTFE scaffold has no negative effect on the proliferation of hADSCs.
8.A comparative study of general practitioner training patterns in rural and remote areas of Australia and China

The manuscript introduced the overview, training objectives, policy advantages, training process,curriculum, examination of the Australian College in Rural and Remote Medicine and further contrasted that with China's domestics.The authors held that Australia's training is better targeted due to its colleges tailored to this end;the training duration for general practitioners of rural and remote areas is longer,and the training schedule is reasonable;the curriculum design and training content are more targeted;and the homogeneous training is better achieved as its examination is run by the college in a standardized manner.The authors therefore hold that China should develop detailed regulations for general practitioners from rural and remote areas and explore the feasibility of setting up second-level disciplines institutes for internal medicine, surgery, gynecology and obstetrics, pediatrics and general at national and provincial level.
9.Comparison of retinal nerve fiber layer thickness under low and moderate signal strength

Objective To investigate the difference of retinal nerve fiber layer thickness(RNFL)in opti-cal coherence tomography(OCT)under low and moderate signal strength. Methods Four hundred eyes of people aged 46~75 with clear fundus image,no obvious fundus diseases,and satisfactory optical coherence tomography were classified according to their ages and signal strength. The peripapillary and 4 quadrants of RNFL were detect-ed with OCT. Results In satisfactory OCT images ,the signal strength that reached 4/10 and 7/10 was 1.5% and 27% respectively. Under low and moderate signal strength ,the maximum thickness of RNFL was at the superior and inferior,and the minimum thickness of RNFL was at the nasal and temporal. There were no significant differ-ences in RNFL thickness under low or moderate signal strength. Conclusion RNFL results are reliable under low and moderate signal strength.
10.An updated review at molecular pharmacological level for the mechanism of anti-tumor, antioxidant and immunoregulatory action of silibinin.

Silibinin, from milk thistle (Silybum marianum), is a flavonolignan with anti-oxidative and anti-inflammatory properties. It has been therapeutically used for the treatment of hepatic diseases in China, Germany and Japan. Recently, increasing evidences prove that silibinin is also a potent antitumor agent, and the major anti-tumor mechanism for silibinin is the prominent inhibition of the activities of receptor tyrosine kinases (RTKs) and their downstream signal molecules in a variety of tumor cell lines, such as epidermal growth factor receptor 1 (EGFR) and insulin-like growth factor 1 receptor (IGF-1R) signaling pathways. Meanwhile, the evidences that silibinin selectively scavenges hydroxyl free radical (*OH) and specifically inhibits the action of nuclear factor kappaB (NF-kappaB) provide more complicated explanations for its antioxidant and anti-inflammatory effects. Some new findings such as that silibinin attenuating the cognitive deficits induced by amyloid beta protein (Abeta) peptide through its antioxidative and anti-inflammatory properties is valuable to broad the medical prospect of silibinin. In this review, we discuss the molecular pharmacological mechanisms of silibinin, focusing on its inhibition of tyrosine kinases, actions of antioxidation, free radical scavenging, immunoregulation and anti-inflammation.
