1.The effect of laminectomy spine fixation on restore function in treatment of patients with degenerative lumbar spinal stenosis

Objective To investigate the effect of laminectomy spine fixation on restore function in treatment of patients with degenerative lumbar spinal stenosis.Methods 60 patients with degenerative lumbar spinal stenosis were selected and divided into the observation group and control group with 30 patitents respectively.Patietns in the observation group were taken with laminectomy decompression of spinal fixation,while patients in the control were treated with part laminectomy decompression of spinal fixation.The treatment effect was observed.Results Compared with the preoperative data,postoperative neurologic recovery of the two groups were analyzed in A grade,B grade,D and E grade level terms after comparison analysis,the difference was statistically significant (χ2 =6.392,6.203, 7.213,7.382,all P <0.05).Compared with the patients of the control group,comparison analysis of A grade,B grade,D and E grade level terms in the patients of the observation group had a statistically significant difference (χ2 =6.012,6.102,7.124,7.203,all P <0.05).Compared with the preoperative data,the resuts of postoperative 3 months showed canal diameter of (17.2 ±3.1)mm,at last follow -up was (16.8 ±2.7)mm.After a comparative analysis,the differences were statistically significant (t =6.391,7.692,all P <0.05).Patients after three months,the diameter of the spinal canal comparative analysis of last follow -up,the difference was not statistically significant (t =0.592,P >0.05 ).Conclusion Surgery is the treatment of degenerative lumbar spinal stenosis disease the most effective treatment.Measures taken by posterior spinal decompression discectomy,again with pedicle fixation and interbody fusion approach,can make a good prognosis,and isworthy of clinical use.

3.Expression and function of TNF-αin dorsal root ganglion of rats with chronically compressed dorsal root ganglion

[ ABSTRACT] AIM: To investigate the role of TNF-αand NF-κB in the mechanism of neuropathic pain due to chronically compressed dorsal root ganglion ( CCD) .METHODS:Based on the CCD model, von Frey filaments were used to quantify behavior test.The expression changes of TNF-αand NF-κB were determined by Western blotting, and the cor-relation between the expression of TNF-αand the 50%paw withdrawal threshold was also analyzed.Moreover, the location of TNF-αin dorsal root ganglion ( DRG) was observed with immunofluorescence double staining.RESULTS: We found 50%paw withdrawal threshold of CCD decreased at the first day after operation.The mechanical allodynia was the most ob-vious at postoperative 7~14 d and lasted longer than 35 d.The expression of TNF-αand NF-κB increased significantly in DRG after operation (P<0.01), especially at 7~14 d, and then restored gradually.Moreover, there was a correlation between the protein expression of TNF-αand the changes of neuropathic behavior ( P<0.05 ) .CONCLUSION: TNF-αand NF-κB are involved in the mechanism of mechanical allodynia after chronically compressed DRG.
4.Quality Standard of Xiaochuan Oral Solution

OBJECTIVE: To establish xiaochuan oral oral solution quality control method.METHODS: Three kinds of main components' contents in xiaochuan oral solution were determined by HPLC.RESULTS: The prepared oral solution was brown liquid,with identification and tests all up to the standards specified in Chinese Pharmacopeia(2005 edition).The linear ranges of guaifenesin,ephedrine hydrochloride,and aminophylline(theophylline) were 7~13 ?g?mL-1(r=0.999 9),4.55~8.45 ?g?mL-1(r=0.999 8),and 10.64~19.76 ?g?mL-1(r=0.999 9),respectively,with the average recoveries at 99.35%(RSD=0.97%),100.40%(RSD=1.02%),and 100.57%(RSD=1.08%),respectively.CONCLUSION: The established quality control method could be used for quality control of it.
5.Therapeutic effects of low-dose prednisone combined with methotrexate and hydroxy-chloroquine in the treatment of rheumatoid arthritis

Objective To investigate the efficacy and safety of low-dose prednisone combined with methotrexate (MTX) and hydroxychloroquine (HCQ) in the treatment of rheumatoid arthritis (RA).Methods In this 12-week study,150 patients with active rheumatoid arthritis were randomly divided into two groups:prednisone group (70 cases who were received prednisone 5 ~ 10 mg/d + MTX 10 mg/w +HCQ 0.2 g/d) and control group (80 cases who were treated by Meloxicam 7.5 mg/d + MTX 10 mg/w +Leflunomide (LEF) 20 mg/d).The primary end-points were tender and swollen joint counts,visual analogue scales (VAS),and global physician and patients assessments of disease.The secondary end-points were morning stiffness time,C-reactive protein,erythrocyte sedimentation rate,the Health Assessment Questionnaire (HAQ),DAS28 and ACR20,ACR50.Results After 12 weeks,in terms of primary endpoints,tender and swollen joint counts,VAS and global physician assessments in the prednisone group were improved significantly [(4.5 ± 2.5),(3.2 ± 3.36),(21 ± 15),(24.2 ± 16.4),(20.2 ± 10.4) vs (6.4 ±5.84),(6.6±5.5),(46±14),(37.9±19.7),(34.1±12.4),P <0.05orP <0.01].In terms of secondary end-points,the prednisone group produced higher response rates [HAQ score (0.93 ± 0.52),CRP(10.2 ± 5.8) mg/L,ESR(30 ± 14) mm/h,morning stiffness time (32.0 ± 32.3) min,DAS 28 score (3.1±0.9) vs (1.22 ±0.81),(16.3±10.1)mg/L,(33±29)mm/h,(54.7±45.4)min,(4.9±1.9),P <0.05 orP <0.01].The incidence of adverse events was similar between two groups (43% vs 49%,P > 0.05).Conclusions Low-dose prednisone combined with MTX and HCQ produced rapid and relevant improvements in RA signs and symptoms.
6.Reconstruction of personalized three-dimensional entity model of hip bone based on reverse engineering and rapid prototyping technology

BACKGROUND:Hip’s anatomy structure is complex, and the traditional two-dimensional imaging, such as X-ray film or CT scan, is susceptible to image overlap and other soft tissue. As a result, there are some limitations in the diagnosis of hip disease. The digital model is built based on reverse engineering and rapid prototyping. It is comprehensive, intuitive and accurate of the stereo configuration of the hip and the spatial relations of other parts’ anatomical structures. This study has a broad application prospect. OBJECTIVE:To reconstruct the personalized three-dimensional entity model of the hip bone with computer aided technology. METHODS: The human hips were scanned with a portable three-dimensional laser scanner, and then the point cloud data of the hips were gotten. The point cloud was processed and the three-dimensional model reconstruction was carried out by using the engineering software Geomagic and the computer aided software CimatronE. Three-dimensional print of hip was completed and the model was gotten, which was consistent with the geometry of hip bone, through the rapid prototyping machine. RESULTS AND CONCLUSION:A model with good geometric similarity and biomechanical characteristics was built by digital modeling method based on reverse engineering and rapid prototyping. This way of digital modeling provided the basis for digital manufacturing of artificial hip, virtual assembly, stress analysis and surgical simulation.

8.Value of ultrasound-guided fine needle aspiration biopsy in diagnosing thyroid nodules of TI-RADS 4-6 categories

Objective To explore the application value of ultrasound-guided fine needle aspiration biopsy(FNAB) in diagnosing thyroid nodules of TI-RADS 4-6 categories.Methods A retrospective analysis was made on the pathologic data of 494 patients (totally 501 nodules) who underwent TI-RADS ultrasonographic stratification and ultrasound-guided FNAB.168 thyroid nodules of TI-RADS 4-6 categories were selected from 163 patients and classified into three groups:group T,TI-RADS 4A categories;group Ⅱ,TI-RADS 4B category; group Ⅲ,TI-RADS 5-6 category(the pathologically confirmed cases in category 6 had been removed).The ultimate pathologic findings were taken as golden standards to calculate the accuracy,sensitivity,specificity,positive predictive value of ultrasound-guided FNAB in the three groups.ROC curve was drawn to evaluate the diagnostic value of FNAB.Results The accuracy,sensitivity,specificity,positive predictive value of the ultrasound-guided FNAB in the three groups were listed as follows:group Ⅰ 74.4%,73.3%,75.0%,64.7%;group Ⅱ 83.9%,87.5%,71.4%,91.3%;group Ⅲ 89.3%,91.7 %,75.0%,95.7 % respectively.The positive predictive values of the ultrasound-guided FNAB for group Ⅰ and group Ⅱ was higher than that of conventional TI-RADS classification criteria,so the difference was statistically significant; while the positive predictive values for group Ⅲ was close to the conventional ultrasound TI-RADS classification criteria,the difference was not statistically significant.Conclusions The combination application of TI-RADS classification criteria and FNAB can improve the accuracy of ultrasound diagnosis in group Ⅰ and group Ⅱ,and reduce the puncture rate of group Ⅲ.
9.Greater trochanteric transfer for the treatment of coxa brevis in children and adolescents

Objective To evaluate the effect of greater trochanteric transfer for the treatment of coxa brevis in children and adolescents.Methods From August 2005 to January 2011,twenty patients (22 hips)with coxa brevis underwent greater trochanteric transfer.Among them 18 patients (20 hips) were available for evaluation,including 4 boys and 14 girls,with an average age of 11.4 years (range,7.5-15.0 years) at operation.Five cases (6 hips) were caused by Perthes disease,and 13 cases (14 hips) were caused by developmental dysplasia of hip.Four patients (4 hips) had undergone greater trochanteric epiphyseodesis ever before.All patients were fixed with tension screw after the deformity was corrected during the operation.After operation,the human plaster spica was used for 6 weeks in all patients.Results All patients were followed up for 14 to 79 months (average,31 months).At the last follow-up,fatigue or pain in the hips disappeared or improved in 13 patients.Sixteen patients had limping and positive Trendelenburg sign preoperatively,at the last follow-up 9 patients got improvement.Twelve patients (13 hips) had limitation of abduction of the hip,the average range of abduction was 25.38°±1.20°,which was improved to 45.38°±1.05° at the last follow-up.The average articulotrochanteric distance and ratio of the distance from the greater trochanter tip to femoral head center and the radius of the femoral head at the last follow-up was (17.47+3.14)mm and 2.10±0.21,respectively,there were statistical differences compared with those before operation [(-2.89±4.62) mm and 1.59±0.22,respectiovely].The average leg-length discrepancy at the last follow-up was (0.78t±0.26) cm,which had on statistical differences compared with that [(0.83 ±0.33) cm]before operation.Conclusion Greater trochanteric transfer for the treatment of coxa brevis in children and adolescents could improve the clinical symptom,recover the normal anatomy of the proximal femoral,restore the hip biomechanics environment,but could uot improve the leg-length discrepancy.
